Capacidad diagnóstica de las diferentes herramientas de diagnóstico estructural en el glaucoma
Los esfuerzos dirigidos a la detección temprana de la enfermedad glaucomatosa han hecho que se desarrollen las técnicas de análisis digital, permitiendo la existencia de diversos instrumentos de diagnóstico estructural que aportan parámetros de análisis de capa de fibras nerviosas de la retina (CFNR) a nivel peripapilar, de topografía papilar y de células ganglionares (CG) a nivel macular, de forma cuantitativa y objetiva. Cabe destacar la utilidad de tres tecnologías, como son la Tomografía de coherencia óptica (OCT), la Polarimetría láser (GDx) y el Láser confocal de barrido (HRT), cuya rentabilidad diagnóstica en la enfermedad glaucomatosa ha sido evaluada en el presente estudio. Material y métodos: Se ha realizado un estudio observacional transversal, en el que incluimos un total de 213 ojos, clasificados en tres grupos de diagnóstico: normales, hipertensos oculares (HTO) y glaucomas. Todos los pacientes fueron seleccionados siguiendo los siguientes criterios de inclusión: edad ≥50 años, defecto de refracción inferior a 5 dioptrías de esfera y 3 dioptrías de cilindro y medios ópticos transparentes..
Diagnostic Ability of Macular Nerve Fiber Layer Thickness Using New Segmentation Software in Glaucoma Suspects
Purpose. To assess the capacity of internal retinal layer thickness measurements made at the macula using new spectral-domain optical coherence tomography (OCT) software to distinguish between healthy subjects and those with suspected glaucoma. The diagnostic performance of such measurements also was compared with that of conventional peripapillary retinal nerve fiber layer (RNFL) thickness measurements.
Methods. The study included 38 subjects with suspected glaucoma and 38 age-matched healthy subjects. In one randomly selected eye of each participant, thickness measurements at the level of the macula were made of the nerve fiber layer (mRNFL), the ganglion cell layer (GCL), and the ganglion cell complex (GCC; GCL + internal plexiform layer) through automated OCT segmentation. Peripapillary RNFL thickness (pRNFL) also was determined using the conventional scan.
Results. As the only variable showing intergroup variation, mRNFL in the glaucoma suspects was significantly thinner in the quadrants inner inferior (P = 0.003), inner temporal (P = 0.010), and outer inferior (P = 0.017). The variable best able to discriminate between the two groups was inner inferior mRNFL thickness, as indicated by an area below the receiver operating characteristic (ROC) curve of 0.742.
Conclusions. Macular RNFL thickness measurements showed an improved diagnostic capacity over the other variables examined to distinguish between healthy subjects and glaucoma suspects
A Diagnostic Calculator for Detecting Glaucoma on the Basis of Retinal Nerve Fiber Layer, Optic Disc, and Retinal Ganglion Cell Analysis by Optical Coherence Tomography
Purpose: The purpose of this study was to develop and validate a multivariate predictive model to detect glaucoma by using a combination of retinal nerve fiber layer (RNFL), retinal ganglion cell-inner plexiform (GCIPL), and optic disc parameters measured using spectral-domain optical coherence tomography (OCT).
Methods: Five hundred eyes from 500 participants and 187 eyes of another 187 participants were included in the study and validation groups, respectively. Patients with glaucoma were classified in five groups based on visual field damage. Sensitivity and specificity of all glaucoma OCT parameters were analyzed. Receiver operating characteristic curves (ROC) and areas under the ROC (AUC) were compared. Three predictive multivariate models (quantitative, qualitative, and combined) that used a combination of the best OCT parameters were constructed. A diagnostic calculator was created using the combined multivariate model.
Results: The best AUC parameters were: inferior RNFL, average RNFL, vertical cup/disc ratio, minimal GCIPL, and inferior-temporal GCIPL. Comparisons among the parameters did not show that the GCIPL parameters were better than those of the RNFL in early and advanced glaucoma. The highest AUC was in the combined predictive model (0.937; 95% confidence interval, 0.911–0.957) and was significantly (P = 0.0001) higher than the other isolated parameters considered in early and advanced glaucoma. The validation group displayed similar results to those of the study group.
Conclusions: Best GCIPL, RNFL, and optic disc parameters showed a similar ability to detect glaucoma. The combined predictive formula improved the glaucoma detection compared to the best isolated parameters evaluated. The diagnostic calculator obtained good classification from participants in both the study and validation groups
Correlations between corneal and optic nerve head variables in healthy subjects and patients with primary open angle glaucoma
AIM: To correlate corneal variables (determined using the Pentacam) with optic nerve head (ONH) variables determined using the Heidelberg retina tomograph (HRT) in healthy subjects and patients diagnosed with primary open angle glaucoma (POAG).
METHODS: Measurements were made in 75 healthy eyes and 73 eyes with POAG and correlations examined through Pearson correlation coefficients between the two sets of variables in the two subject groups. The corneal variables determined were corneal volume (CVol), central corneal thickness (CCT), overall corneal thickness (OvCT), the mean thickness of a circular zone centered at the corneal apex of 1 mm radius (zone I) and the mean thickness of several concentric rings, also centered at the apex until the limbus, each of 1 mm width (zones II to VI respectively). The ONH variables were determined using the HRT.
RESULTS: The following pairs of variables were correlated in the control group: CCT-disc area (DAr) (-0.48; P<0.0001), Zone I-DAr (-0.503; P<0.0001) and Zone II-DAr (-0.443; P<0.0001); and in the POAG group: CCT-cup-to-disc area ratio (CDRa) (-0.402; P<0.0001), Zone I-CDRa (-0.418; P<0.0001), Zone II-CDRa (-0.405; P=0.006), Zone I-cup shape measure (CSM) (-0.415; P=0.002), Zone II-CSM (-0.405; P=0.001), Zone IV-height variation contour (HVC) (0.378; P=0.002); Zone V-HVC (0.388, P<0.0001).
CONCLUSION: AS-OCT-derived lens thickness measurement is valid and comparable to the results obtained by A-scan US. It can be used as a reliable noncontact method for measuring lens thickness in adults with or without significant cataract
Design and methodological characteristics of studies using observational routinely collected health data for investigating the link between cancer and neurodegenerative diseases: protocol for a meta-research study
Introduction: Health services generate large amounts of routine health data (eg, administrative databases, disease registries and electronic health records), which have important secondary uses for research. Increases in the availability and the ability to access and analyse large amounts of data represent a major opportunity for conducting studies on the possible relationships between complex diseases. The objective of this study will be to evaluate the design, methods and reporting of studies conducted using observational routinely collected health data for investigating the link between cancer and neurodegenerative diseases. Methods and analysis: This is the protocol for a meta-research study. We registered the study protocol within the Open Science Framework: https://osf.io/h2qjg. We will evaluate observational studies (eg, cohort and case-control) conducted using routinely collected health data for investigating the associations between cancer and neurodegenerative diseases (such as Alzheimer's disease, amyotrophic lateral sclerosis/motor neuron disease, Huntington's disease, multiple sclerosis and Parkinson's disease). The following electronic databases will be searched (from their inception onwards): MEDLINE, Embase and Web of Science Core Collection. Screening and selection of articles will be conducted by at least two researchers. Potential discrepancies will be resolved via discussion. Design, methods and reporting characteristics in each article will be extracted using a standardised data extraction form. Information on general, methodological and transparency items will be reported. We will summarise our findings with tables and graphs (eg, bar charts, forest plots). Ethics and dissemination: Due to the nature of the proposed study, no ethical approval will be required. We plan to publish the full study in an open access peer-reviewed journal and disseminate the findings at scientific conferences and via social media. Concentración de vitamina D en niños diabéticos de tipo 1. Asociación con el control glucémico y el metabolismo óseo y lipídico
Introducción: debido a que la vitamina D juega un papel primordial en la regulación de la secreción de insulina y su déficit parece conferir un mayor riesgo de desarrollar diabetes mellitus, se ha pretendido analizar la prevalencia del déficit de vitamina D en nuestra población de niños diabéticos de tipo 1 y si se relaciona con un peor control de la enfermedad, así como con el metabolismo lipídico y óseo.
Material y métodos: se trata de un estudio retrospectivo en el cual se disponía de los datos clínicos y analíticos de 124 niños diabéticos de tipo 1, controlados en la Unidad de Diabetes Pediátrica de nuestro hospital.
Resultados: la concentración mediana de vitamina D del total de la muestra fue de 25,41 (7,43) ng/mL, siendo más elevada en el sexo masculino que en el femenino (p = 0,006). Un 43,55 % de los niños presentaron buen control metabólico, con hemoglobina glicosilada inferior al 7,5 %, siendo la concentración de glucosa y la de colesterol ligeramente más bajas, y la de fosfatasa alcalina ósea más elevada, cuando la concentración de vitamina D era ≥ 20 ng/ml.
Conclusiones: no hemos encontrado diferencias significativas en el control metabólico de los niños con concentración suficiente o insuficiente de vitamina D. Los niños del estudio tenían concentraciones de vitamina D muy parecidas a las de un estudio similar en niños sanos, así como un buen control metabólico de su diabetes, siendo su perfil óseo y lipídico más favorable cuando presentaban buen control metabólico
Genetic and environmental factors related to the development of myopic maculopathy in Spanish patients
High myopia and the subsequent degenerative changes of the retina, choroid, and sclera, known as myopic maculopathy (MM), are a serious visual problem in many Asian countries, and are beginning to be so in the south of Europe, especially in the Mediterranean. It is therefore necessary to carry out genetic and environmental studies to determine the possible causes of this disease. This study aims to verify if the genetic factors that have been most related to Asian populations are also associated in two Spanish cohorts. Eight SNPs from six genes (PAX6,SCO2,CCDC102B,BLID,chromosome 15q14, andCOL8A1) along with demographic, ophthalmic and environmental factors were analysed in two cohorts from a total of 365 highly myopic subjects and 177 control subjects. The genetic analysis showed thatCOL8A1SNP rs13095226 was associated with the development of choroidal neovascularization (CNV) and also seems to play an important role in the increase of axial length. The SNP rs634990 ofchromosome 15q14also showed a significant association with MM, although this was lost after the Bonferroni correction. Additional demographic and environmental factors, namely age, sex, smoking status, and pregnancy history, were also found to be associated with MM and CNV in this population
Metabolismo óseo en niños aragoneses con normopeso y niños con sobrepeso/obesidad
Objetivos: En la infancia y adolescencia se produce un aumento de masa ósea, hasta alcanzar un pico máximo, determinante para la salud ósea. Los marcadores óseos evalúan los procesos de formación-resorción ósea. Sin embargo, los estudios sobre la influencia de la obesidad en los marcadores de recambio óseo en esta edad, son escasos y los resultados contradictorios. El objetivo de nuestro estudio fue evaluar si el sobrepeso/obesidad influían en el metabolismo óseo. Métodos: Se compararon parámetros relacionados con el metabolismo óseo, en 45 niños y niñas normopeso (controles) y en un grupo de 61 niños y niñas con sobrepeso/obesidad (casos), procedentes del estudio Exergames (Universidad de Zaragoza), de edades comprendidas, todos ellos, entre 8 y 12 años. Resultados: La concentración de fósforo y la de IGFBP-3 fueron superiores en los niños con sobrepeso/obesidad, respecto a la de los niños normopeso, (p=0,042) y (p=0,042), respectivamente. Las concentraciones de BAP, osteocalcina, magnesio, vitamina D e IGF-I fueron más bajas en el grupo de los niños con sobrepeso/obesidad y la de calcio más elevada, pero las diferencias no fueron estadísticamente significativas. Existe correlación negativa (r=−0,193) (p=0,049) entre BAP e IMC. Conclusiones: En los niños con sobrepeso/obesidad pese a que no se llegó a alcanzar significación estadística, la concentración de BAP y osteocalcina fue inferior a la de los niños normopeso, lo cual junto con la correlación negativa de BAP respecto al IMC encontrada, puede indicar que ya en edades tan tempranas el sobrepeso/obesidad puede afectar a la salud ósea
Bone metabolism in children with normal weight and overweight/obesity in a northeastern region of Spain
Objectives
Bone mass progressively increases to peak during childhood and adolescence, which determines future bone health. Bone formation–resorption processes are assessed using bone markers. However, studies on the impact of obesity on bone turnover markers at this age are limited, and results are inconsistent. The objective of this study was to examine the potential impact of overweight/obesity on bone metabolism.
Methods
A study was performed to compare parameters of bone metabolism in 45 girls and boys with normal weight (controls) and in a group of 612 girls and boys with overweight/obesity (cases) from the Exergames study (University of Zaragoza). Ages ranged from 8 to 12 years.
Results
Higher values of phosphorus and IGFBP-3 were observed in children with overweight/obesity, as compared to children with normal weight, (p=0.042) and (p=0.042), respectively. BAP, osteocalcin, magnesium, vitamin D and IGF-I concentrations were lower in the group with overweight/obesity, whereas calcium concentrations were higher in this group, although differences were not statistically significant. A negative correlation was found (r=−0.193) (p=0.049) between BAP and BMI.
Conclusions
Although differences did not reach statistical significance, BAP and osteocalcin concentrations were lower in children with overweight/obesity. This added to the negative correlation found between BAP and MIC may demonstrate that overweight/obesity may negatively affect bone health already at a young age
Comparative Study of Refraction between Wave Front-Based Refraction and Autorefraction without and with Cycloplegia in Children and Adolescents
The main aim of this study was to compare refraction measurements with and without cycloplegia from two refractors devices, (TRK-2P autorefractometer (TRK-2P) and wavefront-based refraction Visionix 130 (VX130)) in children and adolescents. This descriptive observational study included 20 myopic eyes and 40 hyperopic eyes measured in two different Spanish hospitals. Cycloplegia was carried out by three drops of cyclopentolate hydrochloride 1% (Colircusí cycloplegic, Alcon Healthcare S.A., Barcelona). The mean age of the myopia group was 12.40 ± 3.48 years; for the hyperopia group, the mean age was 7.37 ± 2.47 years. In the myopia group, autorefraction and wavefront-based refraction did not show clinically significant differences in any components between with and without cycloplegia. The hyperopia group showed statistical and clinically significant differences in sphere and SE components between relaxed and non-relaxed states of accommodation, although the cylindrical components were not clinically different. In this study, we considered a value of ≥0.50D as a clinically significant difference in refraction. Therefore, both devices were capable of obtaining accurate refractions without cyclopegia in myopia children, although they did not avoid instrument myopia and accommodation involved in hyperopia children.
