West Syndrome Case of Unknown Cause of Difficult Treatment

Get PDF
West syndrome is an age-dependent epileptic encephalopathy, is a kind of catastrophic epilepsy in the first year of life, characterized by the electroclinical triad of epileptic spasms, delayed psychomotor development, and hypsarrhythmia on the electroencephalogram, although one of these elements may be absent. Its pathophysiology remains unknown and presents a wide range of etiologies, under which can be classified into: genetic, structural/metabolic and unknown cause (cryptogenic). The prognosis depends on: (a) the cause, which gives origin to the attacks (the complex malformation forms being more severe); (b) the EEG pattern(s); (c) the appearance of seizures prior to the spasms; and (d) the rapid response to treatment. Currently, the first-line treatment includes the adrenocorticotropic hormone ACTH and vigabatrin. In the near future the gold standard could be the development of new therapies that target specific pathways of pathogenesis. Due to its high mortality rate and refractoriness to antiepileptic drugs Conventional, is considered a type of "catastrophic epilepsy"

Indexación: Scopus. This work reports the degradation of methylene blue (MB) on Ti/TiO2 and Ti/TiO2/Fe-allophane electrodes in a pH 3 using 0.1 M Na2SO4 as support electrolyte. SEM micrographs show a homogeneous distribution of TiO2 over the whole electrode surface forming nanotubes and nanopores. Fe-allophane modified electrode shows the formation of large-grains agglomerate on the electrode surface due to allophane, which provides a greater surface area to the electrode due to meso and micropore structures. Preliminary cyclic voltammetry show that Ti/TiO2 has the typical voltammetric response due to Ti(III)/Ti(IV) pair. Diffusional problems were observed through of the film when the electrode is modified with Fe-allophane modifying the quasi-reversible process Ti(III)/Ti(IV). Different kind of methodologies in the degradation process were used: Electrochemistry (EC), Photochemistry (PC), Photoelectrochemistry (PEC) and Adsorption (Ads). These methods were developing to discard any reaction or interaction that is not of interest. On Ti/TiO2 with PC and Ads methodologies was not observed any activity to MB degradation showing that is not photosensitive and that the interaction between this and surface electrode is low. But with EC and PEC degradation to 55% is reached after 3 hours of electrolysis. With Ti/TiO2-Fe-allophane electrodes are observed a higher activity for all methodologies. The PC and Ads methods show that the MB degradation reaches to ∼20 % of the initial concentration. As mentioned above, the PC and Ads processes no show degradation on Ti/TiO2, therefore the degradation it only due to the adsorption of MB in/on allophane coat behaving as concentrator matrix. A lower improvement is observed with EC process when is incorporated Ti/TiO2-Fe-allophane is due to the barrier of the electrode surface by oxidation products. With PEC is reached the higher degradation value of ∼88 %, showing an improvement of the degradation with the presence of Fe-allophane. The results indicate that the main role of Fe-allophane on the electrode is similar to a concentrator matrix.http://ref.scielo.org/shz7t

Response and Discrimination Performance of Arrays of Organothiol-Capped Au Nanoparticle Chemiresistive Vapor Sensors

The response and discrimination performance of an array that consisted of 20 different organothiol-capped Au nanoparticle chemiresistive vapor sensors was evaluated during exposure to 13 different organic vapors. The passivating organothiol ligand library consisted of collections of straight-chain alkanethiols, branched alkanethiols, and aromatic thiols. A fourth collection of sensors was formed from composites of 2-phenylethanethiol-capped Au nanoparticles and nonpolymeric aromatic materials that were coembedded in a sensor film. The organic vapors consisted of six hydrocarbons (n-hexane, n-heptane, n-octane, isooctane, cyclohexane, and toluene), three polar aprotic vapors (chloroform, tetrahydrofuran, and ethyl acetate), and four alcohols (methanol, ethanol, isopropanol, and 1-butanol). Trends in the resistance response of the sensors were consistent with expected trends in sorption due to the properties of the test vapor and the molecular structure of the passivating ligands in the sensor films. Classification algorithms including principal components analysis and Fisher’s linear discriminant were used to evaluate the discrimination performance of an array of such sensors. Each collection of sensors produced accurate classification of most vapors, with misclassification occurring primarily for vapors that had mutually similar polarity. The classification performance for an array that contained all of the sensor collections produced nearly perfect discrimination for all vapors studied. The dependence of the array size (i.e., the number of sensors) and the array chemical diversity on the discrimination performance indicated that, for an array of 20 sensors, an array size of 13 sensors or more produced the maximum discrimination performance

The Relation between Attitudes toward Functional Foods and Satisfaction with Food-Related Life

Purpose – To assess the effect of attitudes towards functional foods (FF) on university students’ satisfaction with food-related life and to distinguish student typologies, considering that the attitudes towards FF are not homogeneous among consumers. Design/methodology - A survey was applied to 372 university students (mean age=20.4 years, SD=2.4) in southern Chile. The questionnaire included the Attitudes towards Functional Foods (AFF) questionnaire and the Satisfaction with Food-related Life (SWFL) scale, questions about consumption and knowledge about FF and socio-demographic characteristics. Findings – Using Confirmatory Factor Analysis (CFA) and Structural Equation Modelling, it was found that attitudes toward functional foods directly and significantly influence students’ satisfaction with food-related life. A cluster analysis applied to the Z-scores from the factors obtained by the CFA classified three typologies: Positive towards FF (36.3%), moderately positive towards FF (43.0%) and negative towards FF (20.7%). The positive towards FF type had a significantly greater SWFL score than the negative towards FF type. The types differ according to consumption and knowledge about FF. Research limitations/implications – This study was conducted in the context of only one country in South America. Originality/value – This study is the first that assesses the effect of AFF on satisfaction with food-related life in a sample of university students. Fostering positive attitudes towards FF will allow for a growth in the degree of satisfaction with food-related life of university students with features similar to those of the study sample

A dual stage process of depositing bamboo-like carbon nanotubes (BCNTs) by hot filament chemical vapor deposition (HFCVD) and coating Si using Radio frequency sputtering (RFS) technique. The films were characterized by scanning electron microscopy (SEM), transmission electron microscopy (TEM), Raman spectroscopy, X-ray photoelectron spectroscopy (XPS), and electron field emission studies (EFE). SEM results suggest a dense network of homogeneous silicon-coated BCNTs. From the comprehensive analysis of the results provided by these techniques emerges the picture of Si encapsulated BCNTs

Nitroheterocyclic drugs cure experimental Trypanosoma cruzi infections more effectively in the chronic stage than in the acute stage

The insect-transmitted protozoan parasite Trypanosoma cruzi is the causative agent of Chagas disease, and infects 5-8 million people in Latin America. Chagas disease is characterised by an acute phase, which is partially resolved by the immune system, but then develops as a chronic life-long infection. There is a consensus that the front-line drugs benznidazole and nifurtimox are more effective against the acute stage in both clinical and experimental settings. However, confirmative studies have been restricted by difficulties in demonstrating sterile parasitological cure. Here, we describe a systematic study of nitroheterocyclic drug efficacy using highly sensitive bioluminescence imaging of murine infections. Unexpectedly, we find both drugs are more effective at curing chronic infections, judged by treatment duration and therapeutic dose. This was not associated with factors that differentially influence plasma drug concentrations in the two disease stages. We also observed that fexinidazole and fexinidazole sulfone are more effective than benznidazole and nifurtimox as curative treatments, particularly for acute stage infections, most likely as a result of the higher and more prolonged exposure of the sulfone derivative. If these findings are translatable to human patients, they will have important implications for treatment strategies

Prophylactic Cefazolin Dosing and Surgical Site Infections: Does the Dose Matter in Obese Patients?

Get PDF
Background Most surgical prophylaxis guidelines recommend a 3-g cefazolin intravenous dose in patients weighing ≥ 120 kg. However, this recommendation is primarily based on pharmacokinetic studies rather than robust clinical evidence. This study aimed to compare the prevalence of surgical site infections (SSIs) in obese and non-obese patients (body mass index ≥ 30 kg/m2 and < 30 kg/m2), and those weighing ≥ 120 kg and < 120 kg, who received 2- g cefazolin preoperatively. Methods A retrospective case-control study was conducted in adult elective surgical patients. Patients receiving 2- g cefazolin were grouped as obese and non-obese, and by weight (≥ 120 kg or < 120 kg). The 90-day prevalence of SSI and potential contributing factors were investigated. Results We identified 152 obese (median 134 kg) and 152 non-obese control (median 73 kg) patients. Baseline characteristics were similar between groups, except for an increased prevalence in the obese group of diabetes (35.5% vs 13.2%; p < 0.001) and an American Society of Anaesthesiologists Score of 3 (61.8% vs 17.1%; p < 0.001). While not statistically significant, the prevalence of SSI in the obese group was almost double that in the non-obese group (8.6% vs 4.6%; p = 0.25) and in patients weighing ≥ 120 kg (n = 102) compared to those weighing < 120 kg (n = 202) (9.8% vs 5.0%; p = 0.17). Conclusion The prevalence of SSI was not significantly increased in obese patients, or those weighing ≥ 120 kg, who received cefazolin 2- g prophylactically; however, trends toward an increase were evident. Large-scale randomised trials are needed to examine whether a 2-g or 3-g cefazolin is adequate to prevent SSI in obese (and ≥ 120 kg) individuals
