Evidence for a companion to BM Gem, a silicate carbon star
Balmer and Paschen continuum emission as well as Balmer series lines of P
Cygni-type profile from H_gamma through H_23 are revealed in the violet spectra
of BM Gem, a carbon star associated with an oxygen-rich circumstellar shell
(`silicate carbon star') observed with the high dispersion spectrograph (HDS)
on the Subaru telescope. The blue-shifted absorption in the Balmer lines
indicates the presence of an outflow, the line of sight velocity of which is at
least 400 km s^-1, which is the highest outflow velocity observed to date in a
carbon star. We argue that the observed unusual features in BM Gem are strong
evidence for the presence of a companion, which should form an accretion disk
that gives rise to both an ionized gas region and a high velocity, variable
outflow. The estimated luminosity of ~0.2 (0.03-0.6) L_sun for the ionized gas
can be maintained by a mass accretion rate to a dwarf companion of ~10^-8 M_sun
yr^-1, while ~10^-10 M_sun yr^-1 is sufficient for accretion to a white dwarf
companion. These accretion rates are feasible for some detached binary
configurations on the basis of the Bond-Hoyle type accretion process. We
concluded that the carbon star BM Gem is in a detached binary system with a
companion of low mass and low luminosity. However, we are unable to determine
whether this companion object is a dwarf or a white dwarf. The upper limits for
binary separation are 210 AU and 930 AU for a dwarf and a white dwarf,
respectively. We also note that the observed features of BM Gem mimic those of
Mira (omi Cet), which may suggest actual similarities in their binary

Gene Algebra from a Genetic Code Algebraic Structure
The biological distinction between the base positions in the codon, the
chemical types of bases (purine and pyrimidine) and their hydrogen bond number
have been the most relevant codon properties used in the genetic code analysis.
Now, these properties have allowed us to build a Genetic Code ring isomorphic
to the ring (Z64, +,*) of the integer module 64. On the Z64-algebra of the set
of 64^N codon sequences of length N, gene mutations are described by means of
endomorphisms F: (Z64)^N->(Z64)^N. Endomorphisms and automorphisms helped us
describe the gene mutation pathways. For instance, 77.7% mutations in 749 HIV
protease gene sequences correspond to unique diagonal endomorphisms of the wild
type strain HXB2. In particular, most of the reported mutations that confer
drug resistance to the HIV protease gene correspond to diagonal automorphisms
of the wild type. What is more, in the human beta-globin gene a similar
situation appears where most of the single codon mutations correspond to
automorphisms. Hence, in the analyses of molecular evolution process on the DNA
sequence set of length N, the Z64-algebra will help us explain the quantitative

On the progenitor of SN 2005gl and the nature of Type IIn supernovae
We present a study of the type IIn supernova (SN) 2005gl, in the relatively
nearby (d~66 Mpc) galaxy NGC 266. Photometry and spectroscopy of the SN
indicate it is a typical member of its class. Pre-explosion Hubble Space
Telescope (HST) imaging of the location of the SN, along with a precise
localization of this event using the Laser-Guide-Star assisted Adaptive Optics
(LGS-AO) system at Keck Observatory, are combined to identify a luminous
(M_V=-10.3) point source as the possible progenitor of SN 2005gl. If the source
is indeed a single star, it was likely a member of the class of luminous blue
variable stars (LBVs). This finding leads us to consider the possible general
association of SNe IIn with LBV progenitors. We find this is indeed supported
by observations of other SNe, and the known properties of LBV stars. For
example, we argue that should the prototypical Galactic LBV eta Carina explode
in a phase similar to its current state, it will likely produce a type IIn SN.
We discuss our findings in the context of current ideas about the evolution of
massive stars, and review the census of SNe with identified progenitors. We
introduce the concept of the progenitor-SN map as a convenient means to discuss
the present status and future prospects of direct searches for SN progenitors.
We conclude that this field has matured considerably in recent years, and the
transition from anecdotal information about rare single events to robust

Modeling factors influencing the demand for emergency department services in ontario: a comparison of methods
<p>Abstract</p> <p>Background</p> <p>Emergency departments are medical treatment facilities, designed to provide episodic care to patients suffering from acute injuries and illnesses as well as patients who are experiencing sporadic flare-ups of underlying chronic medical conditions which require immediate attention. Supply and demand for emergency department services varies across geographic regions and time. Some persons do not rely on the service at all whereas; others use the service on repeated occasions. Issues regarding increased wait times for services and crowding illustrate the need to investigate which factors are associated with increased frequency of emergency department utilization. The evidence from this study can help inform policy makers on the appropriate mix of supply and demand targeted health care policies necessary to ensure that patients receive appropriate health care delivery in an efficient and cost-effective manner. The purpose of this report is to assess those factors resulting in increased demand for emergency department services in Ontario. We assess how utilization rates vary according to the severity of patient presentation in the emergency department. We are specifically interested in the impact that access to primary care physicians has on the demand for emergency department services. Additionally, we wish to investigate these trends using a series of novel regression models for count outcomes which have yet to be employed in the domain of emergency medical research.</p> <p>Methods</p> <p>Data regarding the frequency of emergency department visits for the respondents of Canadian Community Health Survey (CCHS) during our study interval (2003-2005) are obtained from the National Ambulatory Care Reporting System (NACRS). Patients' emergency department utilizations were linked with information from the Canadian Community Health Survey (CCHS) which provides individual level medical, socio-demographic, psychological and behavioral information for investigating predictors of increased emergency department utilization. Six different multiple regression models for count data were fitted to assess the influence of predictors on demand for emergency department services, including: Poisson, Negative Binomial, Zero-Inflated Poisson, Zero-Inflated Negative Binomial, Hurdle Poisson, and Hurdle Negative Binomial. Comparison of competing models was assessed by the Vuong test statistic.</p> <p>Results</p> <p>The CCHS cycle 2.1 respondents were a roughly equal mix of males (50.4%) and females (49.6%). The majority (86.2%) were young-middle aged adults between the ages of 20-64, living in predominantly urban environments (85.9%), with mid-high household incomes (92.2%) and well-educated, receiving at least a high-school diploma (84.1%). Many participants reported no chronic disease (51.9%), fell into a small number (0-5) of ambulatory diagnostic groups (62.3%), and perceived their health status as good/excellent (88.1%); however, were projected to have high Resource Utilization Band levels of health resource utilization (68.2%). These factors were largely stable for CCHS cycle 3.1 respondents. Factors influencing demand for emergency department services varied according to the severity of triage scores at initial presentation. For example, although a non-significant predictor of the odds of emergency department utilization in high severity cases, access to a primary care physician was a statistically significant predictor of the likelihood of emergency department utilization (OR: 0.69; 95% CI OR: 0.63-0.75) and the rate of emergency department utilization (RR: 0.57; 95% CI RR: 0.50-0.66) in low severity cases.</p> <p>Conclusion</p> <p>Using a theoretically appropriate hurdle negative binomial regression model this unique study illustrates that access to a primary care physician is an important predictor of both the odds and rate of emergency department utilization in Ontario. Restructuring primary care services, with aims of increasing access to undersupplied populations may result in decreased emergency department utilization rates by approximately 43% for low severity triage level cases.</p
Influences of various magnetospheric and ionospheric current systems on geomagnetically induced currents around the world
Ground-based observations of geomagnetic field (B field) are usually a superposition of signatures from different source current systems in the magnetosphere and ionosphere. Fluctuating B fields generate geoelectric fields (E fields), which drive geomagnetically induced currents (GIC) in technological conducting media at the Earth's surface. We introduce a new Fourier integral B field model of east/west directed line current systems over a one-dimensional multilayered Earth in plane geometry. Derived layered-Earth profiles, given in the literature, are needed to calculate the surface impedance, and therefore reflection coefficient in the integral. The 2003 Halloween storm measurements were Fourier transformed for B field spectrum Levenberg-Marquardt least squares inversion over latitude. The inversion modeled strengths of the equatorial electrojets, auroral electrojets, and ring currents were compared to the forward problem computed strength. It is found the optimized and direct results match each other closely and supplement previous established studies about these source currents. Using this model, a data set of current system magnitudes may be used to develop empirical models linking solar wind activity to magnetospheric current systems. In addition, the ground E fields are also calculated directly, which serves as a proxy for computing GIC in conductor-based networks
Radiofrequency ablation of lung tumours
Pulmonary radiofrequency ablation (RFA) has become an increasingly adopted treatment option for primary and metastatic lung tumours. It is mainly performed in patients with unresectable or medically inoperable lung neoplasms. The immediate technical success rate is over 95%, with a low periprocedural mortality rate and 8–12% major complication rate. Pneumothorax represents the most frequent complication, but requires a chest tube drain in less than 10% of cases. Sustained complete tumour response has been reported in 85–90% of target lesions. Lesion size represents the most important risk factor for local recurrence. Survival data are still scarce, but initial results are very promising. In patients with stage I non-small-cell lung cancer, 1- and 2-year survival rates are within the ranges of 78–95% and 57–84%, respectively, with corresponding cancer-specific survival rates of 92% and 73%. In selected cases, the combination of RFA and radiotherapy could improve these results. In patients with colorectal lung metastasis, initial studies have reported survival data that compare favourably with the results of metastasectomy, with up to a 45% 5-year survival rate. Further studies are needed to understand the potential role of RFA as a palliative treatment in more advanced disease and the possible combination of RFA with other treatment options
Should the provision of home help services be contained?: Validation of the new preventive care policy in Japan
<p>Abstract</p> <p>Background</p> <p>To maintain the sustainability of public long-term care insurance (LTCI) in Japan, a preventive care policy was introduced in 2006 that seeks to promote active improvement in functional status of elderly people who need only light care. This policy promotes the use of day care services to facilitate functional improvement, and contains the use of home help services that provide instrumental activity of daily living (IADL) support. However, the validity of this approach remains to be demonstrated.</p> <p>Methods</p> <p>Subjects comprised 241 people aged 65 years and over who had recently been certified as being eligible for the lightest eligibility level and had began using either home help or day care services between April 2007 and October 2008 in a suburban city of Tokyo. A retrospective cohort study was conducted ending October 2009 to assess changes in the LTCI eligibility level of these subjects. Cox's proportional hazards model was used to calculate the relative risk of declining in function to eligibility Level 4 among users of the respective services.</p> <p>Results</p> <p>Multivariate analysis adjusted for factors related to service use demonstrated that the risk of decline in functional status was lower for users of home help services than for users of day care services (HR = 0.55, 95% CI: 0.31-0.98). The same result was obtained when stratified by whether the subject lived with family or not. Furthermore, those who used two or more hours of home help services did not show an increase in risk of decline when compared with those who used less than two hours.</p> <p>Conclusions</p> <p>No evidence was obtained to support the effectiveness of the policy of promoting day care services and containing home help services for those requiring light care.</p
Cytochrome P450-mediated metabolism of N-(2-methoxyphenyl)-hydroxylamine, a human metabolite of the environmental pollutants and carcinogens o-anisidine and o-nitroanisole
N-(2-methoxyphenyl)hydroxylamine is a human metabolite of the industrial and environmental pollutants and bladder carcinogens 2-methoxyaniline (o-anisidine) and 2-methoxynitrobenzene (o-nitroanisole). Here, we investigated the ability of hepatic microsomes from rat and rabbit to metabolize this reactive compound. We found that N-(2-methoxyphenyl)hydroxylamine is metabolized by microsomes of both species mainly to o-aminophenol and a parent carcinogen, o-anisidine, whereas 2-methoxynitrosobenzene (o-nitrosoanisole) is formed as a minor metabolite. Another N-(2-methoxyphenyl)hydroxylamine metabolite, the exact structure of which has not been identified as yet, was generated by hepatic microsomes of rabbits, but its formation by those of rats was negligible. To evaluate the role of rat hepatic microsomal cytochromes P450 (CYP) in N-(2-methoxyphenyl)hydroxylamine metabolism, we investigated the modulation of its metabolism by specific inducers of these enzymes. The results of this study show that rat hepatic CYPs of a 1A subfamily and, to a lesser extent those of a 2B subfamily, catalyze N-(2-methoxyphenyl)hydroxylamine conversion to form both its reductive metabolite, o-anisidine, and o-aminophenol.
