    A Mechanism for Ventricular Expansion in Communicating Hydrocephalus

    This report investigates a new possible molecular mechanism for the pathogenesis of hydrocephalus. New research by Dr. Miles Johnston has found that the injection of anti beta_1 integrin antibodies into the ventricle of rats causes a drop in parenchymal pressure and causes the cerebral ventricles to enlarge which is characteristic of hydrocephalus. We investigate intramantle pressure gradients as a possible force to enlarge the ventricles and we propose a new poroelastic model incorporating the effect of the antibodies to determine if they are a possible mechanism for hydrocephalus

    Local cooling for relieving pain from perineal trauma sustained during childbirth

    Background: Perineal trauma is common during childbirth and may be painful. Contemporary maternity practice includes offering women numerous forms of pain relief, including the local application of cooling treatments. Objectives: To evaluate the effectiveness and side effects of localised cooling treatments compared with no treatment, other forms of cooling treatments and non-cooling treatments. Search strategy: We searched the Cochrane Pregnancy and Childbirth Group's Trials Register (January 2007), CINAHL (1982 to January 2007) and contacted experts in the field. Selection criteria: Published and unpublished randomised and quasi-randomised trials (RCTs) that compared localised cooling treatment applied to the perineum with no treatment or other treatments applied to relieve pain related to perineal trauma sustained during childbirth. Data collection and analysis: At least two independent authors performed data extraction for each study. Analyses were performed on an intention-to-treat basis where data allowed. We sought additional information from the authors of three trials. Main results: Seven published RCTs were included, comparing local cooling treatments (ice packs, cold gel pads or cold/iced baths) with no treatment, hamamelis water (witch hazel), pulsed electromagnetic energy (PET), hydrocortisone/pramoxine foam [Epifoam] or warm baths. The RCTs reported on a total of 859 women. Ice packs provided improved pain relief 24 to 72 hours after birth compared with no treatment (risk ratio (RR) 0.61, 95% confidence interval (CI) 0.41 to 0.91). Women preferred the utility of the gel pads compared with ice packs or no treatment, although no differences in pain relief were detected between the treatments. None of our comparisons of treatments resulted in differences detected in perineal oedema or bruising. Women reported more pain (RR 5.60, 95% CI 2.35 to 13.33) and used more additional analgesia (RR 4.00, 95% CI 1.44 to 11.13) following the application of ice packs compared with PET. Authors' conclusions: There is only limited evidence to support the effectiveness of local cooling treatments (ice packs, cold gel pads, cold/iced baths) applied to the perineum following childbirth to relieve pain

    A bioassay for cyclophosphamide in blood, lung and tumour.

    A bioassay has been developed to detect and quantify the concentration of cytotoxic metabolites of cyclophosphamide (CY) in blood, tumour, and lungs of mice. Extracts were made of blood or solid tissues taken from mice given CY and these were used to treat log phase Chinese Hamster V79 cells in culture for up to 24 h. The amount of cell killing was tested by colony formation 7 days later. The effects of incubation time, CY dose, and the time of tissue sampling after CY injection were investigated. The bioassay could detect cytotoxic metabolites in blood after doses as low as 10 mg kg-1 CY given i.p. The half life of these metabolites in blood after giving 400 mg kg-1 i.p. decreased over a 2 h period from 14 to 9 min. The method was then modified to define the pharmacokinetics of CY metabolites in two different types of tumour and in lung. The half life of the cytotoxic metabolites in the lung was longer than in blood, falling from 35 to 11 min over a 2 h period. In tumours, the half lives were longer again, i.e. approximately 61 min. The maximum metabolite levels achieved were similar in the two tumour types, although these differed markedly in their therapeutic response to CY. The bioassay for CY is a relatively simple and rapid procedure, and the extension of its application from body fluids to solid tissues makes it a useful tool in experimental pharmacokinetic studies

    Characterization of flow in a scroll duct

    A quantitative, flow visualization study was made of a partially elliptic cross section, inward curving duct (scroll duct), with an axial outflow through a vaneless annular cutlet. The working fluid was water, with a Re(d) of 40,000 at the inlet to the scroll duct, this Reynolds number being representative of the conditions in an actual gas turbine scroll. Both still and high speed moving pictures of fluorescein dye injected into the flow and illuminated by an argon ion laser were used to document the flow. Strong secondary flow, similar to the secondary flow in a pipe bend, was found in the bottom half of the scroll within the first 180 degs of turning. The pressure field set up by the turning duct was strong enough to affect the inlet flow condition. At 90 degs downstream, the large scale secondary flow was found to be oscillatory in nature. The exit flow was nonuniform in the annular exit. By 270 degs downstream, the flow appeared unorganized with no distinctive secondary flow pattern. Large scale structures from the upstream core region appeared by 90 degs and continued through the duct to reenter at the inlet section

    Minimum toe clearance events in divided attention treadmill walking in older and young adults: A cross-sectional study

    BACKGROUND: Falls in older adults during walking frequently occur while performing a concurrent task; that is, dividing attention to respond to other demands in the environment. A particularly hazardous fall-related event is tripping due to toe-ground contact during the swing phase of the gait cycle. The aim of this experiment was to determine the effects of divided attention on tripping risk by investigating the gait cycle event Minimum Toe Clearance (MTC). METHODS: Fifteen older adults (mean 73.1 years) and 15 young controls (mean 26.1 years) performed three walking tasks on motorized treadmill: (i) at preferred walking speed (preferred walking), (ii) while carrying a glass of water at a comfortable walking speed (dual task walking), and (iii) speed-matched control walking without the glass of water (control walking). Position-time coordinates of the toe were acquired using a 3 dimensional motion capture system (Optotrak NDI, Canada). When MTC was present, toe height at MTC (MTC_Height) and MTC timing (MTC_Time) were calculated. The proportion of non-MTC gait cycles was computed and for non-MTC gait cycles, toe-height was extracted at the mean MTC_Time. RESULTS: Both groups maintained mean MTC_Height across all three conditions. Despite greater MTC_Height SD in preferred gait, the older group reduced their variability to match the young group in dual task walking. Compared to preferred speed walking, both groups attained MTC earlier in dual task and control conditions. The older group’s MTC_Time SD was greater across all conditions; in dual task walking, however, they approximated the young group’s SD. Non-MTC gait cycles were more frequent in the older group across walking conditions (for example, in preferred walking: young – 2.9 %; older - 18.7 %). CONCLUSIONS: In response to increased attention demands older adults preserve MTC_Height but exercise greater control of the critical MTC event by reducing variability in both MTC_Height and MTC_Time. A further adaptive locomotor control strategy to reduce the likelihood of toe-ground contacts is to attain higher mid-swing clearance by eliminating the MTC event, i.e. demonstrating non-MTC gaits cycles

    Phase entrainment of induced ventricular fibrillation: A human feasibility and proof of concept study

    Cardioversion and defibrillation by a single high energy shock applied by myocardial or body surface electrodes is painful, causes long term tissue damage, and is associated with worsening long term outcomes, but is almost always required for treatment of ventricular fibrillation . As a initial step towards developing methods that can terminate ventricular arrhythmias painlessly, we aim to determine if pacing stimuli at a rate of 5/s applied via an implantable cardiac defibrillator (ICD) can modify human ventricular fibrillation. In 8 patients undergoing defibrillation testing of a new/exchanged intracardiac defibrillator, five seconds of pacing at five stimuli per second was applied during the 10-20 seconds of induced ventricular fibrillation before the defibrillation shock was automatically applied, and the cardiac electrograms recorded and analyzed. The high frequency pacing did not entrain the ventricular fibrillation, but altered the dominant frequency in all 8 patients, and modulated the phase computed via the Hilbert Transform, in four of the patients. In this pilot study we demonstrate that high frequency pacing applied via ICD electrodes during VF can alter the dominant frequency and modulate the probability density of the phase of the electrogram of the ventricular fibrillation
