    Use of spinal cord stimulation in managing neuropathic foot pain: an observational pilot case series study

    Objective: In cases of complex regional pain syndrome where conservative treatment is unsuccessful in controlling neuropathic foot pain spinal cord stimulation may be considered. To our knowledge there have been no such cases reported in the foot & ankle literature. The aim of the study was to establish useful information that may supplement our understanding of this complex multifactorial problem and help toinform future management of similar cases. Methods: A pilot observational case series study was undertaken to investigate the use of spinal cord stimulation in the management of neuropathic foot pain using five cases with complex regional pain syndrome (type I). Results: Reduced pain following spinal cord stimulation was reported. The interval between diagnosis and commencement of spinal cord stimulation was variable between cases and maybe responsible for differing levels and timing of pain relief experienced. Conclusion: Careful preoperative diagnosis, robust patient selection and close postoperative monitoring are vital for a successful outcome. The small sample size and potential for bias, limit the generalizability to a larger population. A larger study is therefore indicated to expand upon preliminary findings

    Applying Flammability Limit Probabilities and the Normoxic Upward Limiting Pressure Concept to NASA STD-6001 Test 1

    Repeated Test 1 extinction tests near the upward flammability limit are expected to follow a Poisson process trend. This Poisson process trend suggests that rather than define a ULOI and MOC (which requires two limits to be determined), it might be better to define a single upward limit as being where 1/e (where e (approx. equal to 2.7183) is the characteristic time of the normalized Poisson process) of the materials burn, or, rounding, where approximately 1/3 of the samples fail the test (and burn). Recognizing that spacecraft atmospheres will not bound the entire oxygen-pressure parameter space, but actually lie along the normoxic atmosphere control band, we can focus the materials flammability testing along this normoxic band. A Normoxic Upward Limiting Pressure (NULP) is defined that determines the minimum safe total pressure for a material within the constant partial pressure control band. Then, increasing this pressure limit by a factor of safety, we can define the material as being safe to use at the NULP + SF (where SF is on the order of 10 kilopascal, based on existing flammability data). It is recommended that the thickest material to be tested with the current Test 1 igniter should be 3 mm thick (1/8 inches) to avoid the problem of differentiating between an ignition limit and a true flammability limit

    Bioenergetic Profiling of Zebrafish Embryonic Development

    Many debilitating conditions are linked to bioenergetic defects. Developing screens to probe the genetic and/or chemical basis for such links has proved intractable. Furthermore, there is a need for a physiologically relevant assay of bioenergetics in whole organisms, especially for early stages in life where perturbations could increase disease susceptibility with aging. Thus, we asked whether we could screen bioenergetics and mitochondrial function in the developing zebrafish embryo. We present a multiplexed method to assay bioenergetics in zebrafish embryos from the blastula period (3 hours post-fertilization, hpf) through to hatching (48 hpf). In proof of principle experiments, we measured respiration and acid extrusion of developing zebrafish embryos. We quantified respiratory coupling to various bioenergetic functions by using specific pharmacological inhibitors of bioenergetic pathways. We demonstrate that changes in the coupling to ATP turnover and proton leak are correlated with developmental stage. The multiwell format of this assay enables the user to screen for the effects of drugs and environmental agents on bioenergetics in the zebrafish embryo with high sensitivity and reproducibility
