    Feasibility of mHealth technology use among a sample of isolated rural men at high risk for cardiovascular disease

    Purpose/Hypothesis: Isolated rural men are considered a health disparities group at high risk for cardiovascular disease. The increasing availability of technologies for self-monitoring for healthy eating, activity and weight loss (ie mHealth) may show promise for engaging rural men in lifestyle modification. This study investigated the feasibility of men from rural isolated areas to use a fitness monitor with text messaging support over a 3- week period. The study examined the men’s daily monitor use for tracking activity and eating, and assessed via written survey, their perspectives about mHealth. Number of Subjects: Twelve men, ages 40 to 69, from a US Department of Agriculture defined isolated rural area, participated. These men were a purposive sample originally recruited to participate in a focus group about their perceptions of the utility of mHealth. The men (50.9 6 8.6 yrs) had a baseline BMI of 25 to 44 kg/m2 (34.8 6 6.6 kg/m2). Eligibility included having cell/smartphones capable of sending/ receiving text messages, access to a computer, willing to use a fitness monitor and have research personnel access the men’s logs. Materials/Methods: Men participated in 2 visits at a community center located within 70 miles of their residence, at baseline and 3 weeks. Assessments included baseline health histories and vital sign biomarkers. The men received training using the fitness monitor with supporting technologies (cell/ smartphone and computer) and were asked to wear the monitor daily for 3 weeks. Men received 1–3 text messages/day for 3 weeks for reminders, education and motivation for self-monitoring. At visit 2, men completed post-intervention surveys about their fitness monitoring. Descriptive data were used for analysis. Results: Men were overweight (n 5 3) or obese (n 5 9) and most (9/12) were hypertensive with only four being treated with medications. One man was hypertensive stage 2 under no treatment and another was pre-hypertensive. Nine of 12 men wore the monitor during all 21 days, two wore it 9 and 15 days respectively and one lost the monitor. Survey data of the 12 men revealed seven checking their step count more than 5 times/day, 6 reported using the associated smart phone app and seven used the optional sleep log feature. Eleven of 12 men manually entered food into the log and most (9/12) did this on $15 days. Ten men indicated the log was helpful in learning about eating; though only 3 indicated it was easy to log food. All men reported reading reminder and motivational text messages sent during the study and 11 plan to continue using the fitness monitor. Conclusions: Men were not well managed for blood pressure or overweight/obesity. Both the log records and the survey results indicated that using fitness monitors was feasible and acceptable among this population. Clinical Relevance: Using mHealth appears feasible as an action-oriented tool for therapists to recommend for lifestyle self-monitoring in isolated rural men. The findings reinforce the important role of therapists in routinely assessing vital signs and making referrals as appropriate

    Landlabs: An Integrated Approach to Creating Agricultural Enterprises That Meet the Triple Bottom Line

    Global demand is increasing for food, feed, and fiber; for additional agricultural outputs, such as biofuels; and for ecosystem services, such as clean water and outdoor recreation. In response, new agricultural enterprises are needed that produce more outputs from existing lands while meeting the triple bottom line of high performance in economic, environmental, and social terms. Establishing such enterprises requires coordination and development within three critical domains: landscape configurations (i.e., types and arrangements of land uses), supply/value chains (i.e., processing and utilization), and policy and governance. In this essay, we describe our efforts, as land-grant university scientists, to support coordinated innovation and enterprise development in integrated place-based institutions, which we term landlabs. We describe our experiences in three prototyping efforts and outline key features of landlabs that are emerging from these efforts.     Comparing Dawn, Hubble Space Telescope, and Ground-Based Interpretations of (4) Vesta

    Observations of asteroid 4 Vesta by NASA's Dawn spacecraft are interesting because its surface has the largest range of albedo, color and composition of any other asteroid visited by spacecraft to date. These hemispherical and rotational variations in surface brightness and composition have been attributed to impact processes since Vesta's formation. Prior to Dawn's arrival at Vesta, its surface properties were the focus of intense telescopic investigations for nearly a hundred years. Ground-based photometric and spectroscopic observations first revealed these variations followed later by those using Hubble Space Telescope. Here we compare interpretations of Vesta's rotation period, pole, albedo, topographic, color, and compositional properties from ground-based telescopes and HST with those from Dawn. Rotational spectral variations observed from ground-based studies are also consistent with those observed by Dawn. While the interpretation of some of these features was tenuous from past data, the interpretations were reasonable given the limitations set by spatial resolution and our knowledge of Vesta and HED meteorites at that time. Our analysis shows that ground-based and HST observations are critical for our understanding of small bodies and provide valuable support for ongoing and future spacecraft missions.Comment: Pages: 51, Figures: 9, Tables:

    Telomeric expression sites are highly conserved in trypanosoma brucei

    Subtelomeric regions are often under-represented in genome sequences of eukaryotes. One of the best known examples of the use of telomere proximity for adaptive purposes are the bloodstream expression sites (BESs) of the African trypanosome Trypanosoma brucei. To enhance our understanding of BES structure and function in host adaptation and immune evasion, the BES repertoire from the Lister 427 strain of T. brucei were independently tagged and sequenced. BESs are polymorphic in size and structure but reveal a surprisingly conserved architecture in the context of extensive recombination. Very small BESs do exist and many functioning BESs do not contain the full complement of expression site associated genes (ESAGs). The consequences of duplicated or missing ESAGs, including ESAG9, a newly named ESAG12, and additional variant surface glycoprotein genes (VSGs) were evaluated by functional assays after BESs were tagged with a drug-resistance gene. Phylogenetic analysis of constituent ESAG families suggests that BESs are sequence mosaics and that extensive recombination has shaped the evolution of the BES repertoire. This work opens important perspectives in understanding the molecular mechanisms of antigenic variation, a widely used strategy for immune evasion in pathogens, and telomere biology

    Negative Effects of Paternal Age on Children's Neurocognitive Outcomes Can Be Explained by Maternal Education and Number of Siblings

    Background: Recent findings suggest advanced paternal age may be associated with impaired child outcomes, in particular, neurocognitive skills. Such patterns are worrisome given relatively universal trends in advanced countries toward delayed nuptiality and fertility. But nature and nurture are both important for child outcomes, and it is important to control for both when drawing inferences about either pathway. Methods and Findings: We examined cross-sectional patterns in six developmental outcome measures among children in the U.S. Collaborative Perinatal Project (n = 31,346). Many of these outcomes at 8 mo, 4 y, and 7 y of age (Bayley scales, Stanford Binet Intelligence Scale, Graham-Ernhart Block Sort Test, Wechsler Intelligence Scale for Children, Wide Range Achievement Test) are negatively correlated with paternal age when important family characteristics such as maternal education and number of siblings are not included as covariates. But controlling for family characteristics in general and mother’s education in particular renders the effect of paternal age statistically insignificant for most developmental measures. Conclusions: Assortative mating produces interesting relationships between maternal and paternal characteristics that can inject spurious correlation into observational studies via omitted variable bias. Controlling for both nature and nurture reveals little residual evidence of a link between child neurocognitive outcomes and paternal age in these data. Result

    Clinical Efficacy of Blue Light Full Body Irradiation as Treatment Option for Severe Atopic Dermatitis

    BACKGROUND: Therapy of atopic dermatitis (AD) relies on immunosuppression and/or UV irradiation. Here, we assessed clinical efficacy and histopathological alterations induced by blue light-treatment of AD within an observational, non-interventional study. METHODOLOGY/PRINCIPAL FINDINGS: 36 patients with severe, chronic AD resisting long term disease control with local corticosteroids were included. Treatment consisted of one cycle of 5 consecutive blue light-irradiations (28.9 J/cm(2)). Patients were instructed to ask for treatment upon disease exacerbation despite interval therapy with topical corticosteroids. The majority of patients noted first improvements after 2-3 cycles. The EASI score was improved by 41% and 54% after 3 and 6 months, respectively (p≤0.005, and p≤0.002). Significant improvement of pruritus, sleep and life quality was noted especially after 6 months. Also, frequency and intensity of disease exacerbations and the usage of topical corticosteroids was reduced. Finally, immunohistochemistry of skin biopsies obtained at baseline and after 5 and 15 days revealed that, unlike UV light, blue light-treatment did not induce Langerhans cell or T cell depletion from skin. CONCLUSIONS/SIGNIFICANCE: Blue light-irradiation may represent a suitable treatment option for AD providing long term control of disease. Future studies with larger patient cohorts within a randomized, placebo-controlled clinical trial are required to confirm this observation

    Characterizing Hospital Workers' Willingness to Respond to a Radiological Event

    Terrorist use of a radiological dispersal device (RDD, or "dirty bomb"), which combines a conventional explosive device with radiological materials, is among the National Planning Scenarios of the United States government. Understanding employee willingness to respond is critical for planning experts. Previous research has demonstrated that perception of threat and efficacy is key in the assessing willingness to respond to a RDD event.An anonymous online survey was used to evaluate the willingness of hospital employees to respond to a RDD event. Agreement with a series of belief statements was assessed, following a methodology validated in previous work. The survey was available online to all 18,612 employees of the Johns Hopkins Hospital from January to March 2009.Surveys were completed by 3426 employees (18.4%), whose demographic distribution was similar to overall hospital staff. 39% of hospital workers were not willing to respond to a RDD scenario if asked but not required to do so. Only 11% more were willing if required. Workers who were hesitant to agree to work additional hours when required were 20 times less likely to report during a RDD emergency. Respondents who perceived their peers as likely to report to work in a RDD emergency were 17 times more likely to respond during a RDD event if asked. Only 27.9% of the hospital employees with a perception of low efficacy declared willingness to respond to a severe RDD event. Perception of threat had little impact on willingness to respond among hospital workers.Radiological scenarios such as RDDs are among the most dreaded emergency events yet studied. Several attitudinal indicators can help to identify hospital employees unlikely to respond. These risk-perception modifiers must then be addressed through training to enable effective hospital response to a RDD event
