Case report of necrotizing sialometaplasia
Introduction: Necrotizing sialometaplasia is a rare inflammatory necrotizing reactive process that can involve minor and major salivary glands. Gland tissue ischaemia has been proposed as the aetiology. Case Report: A 21-year-old woman was referred with a 1-week history of bilateral painful swelling of the palate, with the development of two deep ulcers after the first 3 days. Ulcer edges were elevated but not indurated, and the bases had a necrotic appearance. An incisional biopsy of the margin of one ulcer confirmed the diagnosis of necrotizing sialometaplasia. The ulcers spontaneously healed within 8 weeks without treatment. Discussion: The clinical and histological similarity between this entity and a malignant lesion implies a risk of unnecessary or inadequate treatment. This case illustrates the need for an incisional biopsy to be analyzed by an experienced pathologist to establish a correct diagnosis. © Medicina Oral S. L
Host defence mechanisms against bacterial aggression in periodontal disease : basic mechanisms
Periodontal diseases are complex bacteria-induced infections characterised by an inflammatory host response to plaque microbiota and their by-products. Most of these microorganisms have virulence factors capable of causing massive tissue destruction both directly, through tissue invasion and the production of harmful substances, or indirectly, by activation of host defense mechanisms, creating an inflammatory infiltrate of potent catabolic activity that can interfere with normal host defense mechanisms. In response to the aggression, host defense mechanisms activate innate and adaptive immune responses. Our aim is to offer a general overview of the main mechanisms involved in the host response to bacterial aggression in periodontitis, such as lipopolysaccharide receptor CD14, complement system, polymorphonuclear neutrophils, antibodies and immunoglobulins
Aplicación de un sistema mixto de Enseñanza Tradicional/ Aprendizaje Basado en Problemas en la asignatura de Urgencias en Odontología
El aprendizaje basado en problemas (ABP), o también conocido como PBL por sus siglas en inglés (Problem Based Learning) es un método pedagógico comúnmente empleado en la educación de las Ciencias de la Salud. Las facultades de Medicina han sido líderes en implementar programas educativos basados en ABP. Existe menos información con respecto a la extensión y alcance de esta técnica educativa en Odontología que en Medicina. La asignatura de Urgencias en Odontología de la Facultad de Odontología (UCM) es una asignatura optativa de segundo ciclo de licenciatura. Aunque el libro blanco de la futura titulación de Grado en Odontología especifica las competencias y contenidos específicos relacionados con esta asignatura, existen algunos problemas en la docencia de esta asignatura entre los que se encuentran un bajo número de profesores, tiempo limitado de enseñanza y métodos de enseñaza poco adecuados. En este artículo se describe el proceso de desarrollo y aplicación de un método de enseñanza-aprendizaje mixto basado en la combinación del sistema tradicional junto con el sistema PBL, así como de su formato y contenido y valoración de resultados obtenidos mediante cuestionarios anónimos rellenados por los estudiantes.Problem-Based Learning is a pedagogical method that is widely used in Health Sciences Education. Schools of Medicine have been leaders in the implementation of educational programmes using the PBL approach. There is less information on the extent and reach of this educational technique in Dentistry than in Medicine. Dental Emergencies is an optional subject in the second cycle of the UCM School of Dentistry. Although the white paper on the future Degree in Dentistry specifies the competences and specific contents for this subject, its teaching faces some challenges, including the low number of teachers, limited teaching time and inadequate teaching methods. This article describes the process of developing and applying a mixed teaching-learning method based on a combination of the traditional approach with the PBL system, reporting on its format and contents and on the evaluation of results obtained by anonymous questionnaires completed by students
Periodontal disease and diabetes : review of the literature
Aims: To provide updated knowledge on the relationship between periodontal disease and diabetes from an oral health perspective. Methods: A review of the English-language literature was performed, gathering articles on the two diseases published over the past 10 years. Results: Both diseases result from the confluence of various triggering and modifying factors, and there are inter-individual differences in the risk of their development. Recent research has shown that diabetes may increase the risk of periodontitis, and it has been proposed that chronic periodontal disease may influence the natural course of diabetes. There appears to be an association among oral infections, impaired sugar metabolism, and atherosclerosis, indicating a theoretical link between metabolic syndrome and periodontal disease. Clinical implications: Control of periodontal disease may enhance glycemic control in patients with type 2 diabetes. In turn, improved glycemic control may contribute to a better control of periodontal disease. © Medicina Oral S. L
Immunomodulatory drugs: oral and systemic adverse effects
Objectives: The main objectives are to present the different adverses effects of the immunomodulatory drugs that
can impair the quality of life of the immunosupressed patients and study the impact of immunomodualtion on
oral diseases. Immunomodulatory drugs have changed the treatment protocols of many diseases where immune
functions play a central role, such as rheumatic diseases. Their effect on oral health has not been systematically
investigated, however.
Study Design: We review current data on the new immunomodulatory drugs from the oral health perspective
based on open literature search of the topic.
Results: These target specific drugs appear to have less drug interactions than earlier immunomodulating medicines
but have nevertheless potential side effects such as activating latent infections. There are some data showing
that the new immunomodulatory drugs may also have a role in the treatment of certain oral diseases such as lichen
planus or ameliorating symptoms in Sjögren´s syndrome, but the results have not been overly promising.
Conclusions: In general, data are sparse of the effect of these new drugs vs. oral diseases and there are no properly
powered randomized controlled trials published on this topic

Evaluation of p53, Caspase-3, Bcl-2, and Ki-67 markers in oral squamous cell carcinoma and premalignant epithelium in a sample from Alava Province (Spain)
Objectives: The objective of this study was to determine whether alterations in the expression of p53, caspase-3
Bcl-2, and ki-67 appear early in premalignant oral epithelium and show clonal behavior.
Study Design: Samples from 41 tumors with their adjacent non-tumor epithelia were immunohistochemically
analyzed using monoclonal antibodies that recognize p53, caspase-3, Bcl-2, and Ki-67
Results: A statistically significant association was found between the expression in tumor and adjacent epithelium
of p53, caspase-3, and Bcl-2 but not of k-67. A significant association was observed between the expression
of ki-67 and p53 in both localizations. In non-tumor (premalignant) epithelium samples, there was a significant
inverse relationship between the expressions of p53 and caspase-3 and a significant direct relationship between
the expressions of p53 and Bcl-2.
Conclusions: Alterations in these proteins appear to operate in combination with premalignant epithelia to create
hyperproliferative cell states that favor the acquisition of summative oncogenic errors that confer invasive capacity
Documento de consenso sobre la utilización de profilaxis antibiótica en cirugía y procedimientos dentales
La profilaxis antibiótica en Odontología tiene como objetivo prevenir la aparición de infección a partir de la puerta de entrada que produce la actuación terapéutica, por lo que se encuentra indicada siempre que exista un riesgo importante de infección, ya sea por las características mismas de la operación o por las condiciones locales o generales del paciente. Sin embargo, los ensayos clínicos con antibióticos en patologías dentarias responden poco a los criterios metodológicos requeridos, y además no son lo suficientemente numerosos.
Se presentan los resultados de una conferencia de expertos integrada por los Presidentes de Sociedades científicas españolas más representativas que han analizado la bibliografía existente y han aportado sus valiosas experiencias profesionales. Se describen las circunstancias técnicas, se analizan los fundamentos biológicos y farmacológicos y se aplican a las situaciones médicas más representativas. Se concluye que la profilaxis antibiótica en Odontología cuenta con indicaciones bien fundamentadas y precisas, ofreciendo a la comunidad científica internacional un protocolo práctico de actuación.The goal of antibiotic prophylaxis in Odontology is to prevent the onset of infections through the entranceway provided by the therapeutic action, therefore it is indicated providing there is a considerable risk of infection, either because of the characteristics of the operation itself or the patient's local or general condition. Nonetheless, clinical trials with antibiotics in dental pathologies have had scant regard for the required methodological criteria and, in addition, are not sufficiently numerous.
This text presents the results of an expert conference comprising the Presidents of the most representative Scientific Societies in Spain who have analyzed the existing literature and have drawn on their valuable professional experience. It describes the technical circumstances, analyzes the biological and pharmacological foundations and their application to the most representative medical situations. It is concluded that antibiotic prophylaxis in Odontology has certain well-founded, precise indications and offers the international scientific community a practical protocol for action
Documento de consenso sobre el tratamiento antimicrobiano de las infecciones bacterianas odontogénicas
Las infecciones de la cavidad bucal son un problema de salud
pública frecuente y motivo constante de prescripción antibiótica;
el 10% de los antibióticos se emplean para tratar este problema.
Sin embargo, hasta la fecha son pocos los estudios realizados
para determinar su incidencia. Asímismo, su relación con ciertas
enfermedades sistémicas (cardiacas, endocrinas, etc...) confiere a
estas patologías una importancia vital. A pesar de la reconocida
frecuencia e importancia de las infecciones odontogénicas, llama
la atención la actual dispersión de criterio en varios aspectos
referentes a su clasificación, terminología y recomendaciones
terapéuticas. El objetivo principal de este documento, realizado
con el consenso de especialistas en microbiología y odontología,
es establecer unas recomendaciones útiles para todos los
profesionales implicados en el manejo clínico de estas patologías.
Recibe especial atención el aumento de la prevalencia
de resistencias bacterianas observado durante los últimos años
y, en concreto, la proliferación de cepas productoras de betalactamasas.
Otro factor causal importante de la aparición de
resistencias es la falta de cumplimiento terapéutico, en especial
en lo que respecta a la dosis y a la duración del tratamiento.
Así pues, estas patologías constituyen un problema complejo
cuyo abordaje requiere la instauración de antimicrobianos de
amplio espectro, con adecuados parámetros farmacocinéticos,
con buena tolerancia y una posología cómoda que permita que
el paciente reciba la dosis adecuada durante el tiempo necesario.
Amoxicilina/ácido clavulánico a dosis altas (2000mg/ 125mg)
ha demostrado buenos resultados y capacidad para superar
resistencias. Otros agentes como metronidazol y clindamicina,
seguidos de claritromicina y azitromicina han demostrado también ser activos frente a la mayoría de los microorganismos
responsables de las infecciones odontogénicas.The infection of the oral cavity is a common public health problem
and constant cause for antibiotic prescription, with 10%
of antibiotics used to treat this problem. However, few studies
have so far aimed to determine its incidence. Added to this, its
relationship with certain sytemic diseases (cardiac, endocrine,
etc') confers this pathology vital importance. In spite of the
frequency and importance of odontogenic infection, the current
dispersion in criteria regarding key aspects in classification,
terminology and therapeutic recommendations is noticeable.
The main objective of this document, compiled as a consensus
statement by specialists in microbiology and odontology, is to
establish useful recommendations for all of those involved in
the clinical management of this pathology. Special attention has
been placed on the rise in bacterial resistance observed over the
last years, specifically the proliferation of betalactamase producing
strains. Another important factor causing the resistance to
appear is lack of therapeutic compliance, specially what regards
dosage and treatment duration. Therefore, this pathology constitutes
a complex problem which requires the instauration of
broad spectrum antimicrobials, well tolerated and a convenient
posology so that patients receive the adequate dose over the
necessary period. High doses of amoxicillin/clavulanate (2000
mg / 125 mg) have showed good results and power to overcome
resistance. Other agents such as metronidazole and clindamycin,
followed by de claritromycin and azithromycin have also proved
to be active against most of microorganisms responsible for
odontogenic infection
