Tratamiento de la escoliosis idiopática mediante artrodesis anterior e instrumentación de Zielke
—Hemos revisado 16 pacientes afectos de escoliosis toracolumbar y lumbar tratados
mediante artrodesis anterior e instrumentación de Zielke. La corrección de la curva en el
plano anteroposterior ha sido del 68%. La escoliosis torácica asociada se ha corregido de forma
espontánea un 30%. La lordosis global final ha sido de 40°, sin embargo la instrumentación
provoca un aumento de la cifosis regional de 10°. Las complicaciones de la serie han sido
3 roturas de barra, una progresión de la curva torácica y un derrame pleural.The authors showed the clinical outcome of 16 patients with thoracolumbar
and lumbar curvatures treated by Zielke instrumentation and anterior fusion. The correction
obtained in the primary curvature was 68%. The average correction in the compensatory thoracic
curvature was 30%. The average lordosis correction at follow was 40°. However the Zielke's
system lead to an increase of 10° of kyphotic at the instrumented levels. Complications
included 3 browen rods, a progression of the thoracic curve and one pleural spilling
Tratamiento de las fracturas vertebrales toracolumbar en fase de secuela: estudio retrospectivo de 20 casos
El propósito de este trabajo, es el análisis de 20 fracturas vertebrales en fase de
secuela, intervenidas en la Unidad de Raquis del Hospital «La Fé» desde marzo de 1983 a marzo
de 1994. La demora media en su tratamiento quirúrgico ha sido de 29 meses. El motivo de
de la cirugía fue deformidad residual y/o dolor en el 100% de los casos. La cifosis postraumática,
con el 95%, fue la segunda causa de cirugía. En 13 ocasiones, la fractura por estallido ha
sido la anatomía patológica más frecuente; y en el resto (7 casos), se trató de lesiones por flexión-extensión
(3 casos) y fracturas-luxaciones (4 casos).
En cifosis angulares inferiores a 30°, la artrodesis y osteosíntesis por vía posterior nos ha
sido satisfactoria, mientras que en las superiores a 30°, la cirugía por vía anterior ha sido habitualmente
necesaria.A series of 20 patients with thoracolumbar vertebral fractures operated on in
phase of sequelae from march 93 to inarch 94 were retrospectively reviewed. The average delay
in surgical treatment was 29 months. The indication for surgery was deformity and/or pain in
100% of cases. Postraumatic kyphosis was present in 95% of cases. Thirteen patients show
burst fractures, 4 had fracture-dislocations and 3 exhibit fractures due to a flexion-extension
mechanism. When angular kyphosis was less than 30°, spine fusion with osteosynthesis by a posterior
approach provided satisfactory results. In kyphotic deformities greater than 30°, fusion
through an anterior approach was mandatory
Deformidad vertebral y mielomeningocele: actitud terapéutica y resultados
Presentamos nuestra experiencia en el tratamiento quirúrgico de la deformidad
vertebral relacionada con el mielomeningocele entre los años 1972 y 1995. En ese
período un total de 48 pacientes fueron intervenidos. Se han evaluado las condiciones generales
previas, el estado prequirúrgico, las diversas modalidades de instrumentación y
el resultado de las mismas, así como el tipo y número de complicaciones. Durante el período
posquirúrgico inmediato hemos encontrado un porcentaje de infecciones del 68%.
Sin embargo, el resultado final, tanto del equilibrio pélvico como del vertebral conseguido,
no estuvo influenciado significativamente por esta complicación. El tipo de patología,
las complicaciones generales derivadas de la misma, el tiempo quirúrgico empleado y las
pérdidas sanguíneas, son condiciones estrechamente relacionadas con el número de infecciones
encontradas.We present our experience on the surgical treatment of myelomeningocelelinked
vertebral deformity between 1972 and 1995. We evaluated the previous general
condition, the presurgical status, the instrument modalities and their outcome, as well as
the type and number of complications in a series of 48 patients. During the immediate
postoperative period, we found a 68% infection rate. However, the end result for pelvic as
well as vertebral balance was not influenced significantly by this complication. The number
of infections found is closely linked to the specific pathology, the complications, duration
of operation, and blood loss
Complicaciones sépticas de la osteosíntesis lumbosacra: Análisis de 23 casos
Entre 1987 y 1995 hemos intervenido 295 pacientes afectos de patología lumbosacra
realizando en todos ellos una artrodesis instrumentada. Veintitrés pacientes tuvieron
como complicación una infección profunda, lo que equivale a un 7,8%. El germen más
frecuentemente aislado como responsable de la infección fue el Staphylococcus aureus resistente
a la meticilina. La antibioterapia más veces utilizada fue una cefalosporina de 2.a generación.
La infección se resolvió en 4 ocasiones bajo tratamiento médico; sin embargo, en
19 se procedió a limpieza quirúrgica de los tejidos dañados no siendo necesaria la retirada
del material de osteosíntesis. En 9 ocasiones se practicó una 2.a reintervención, y en otros 4
una tercera. En nuestra serie, fue necesario reintervenir quirúrgicamente el 50% de los pacientes
a los que se practicó una limpieza quirúrgica. Dado que la profilaxis antibiótica debe
de cubrir los gérmenes prevalentes en cada unidad hospitalaria, de acuerdo con su patrón
de resistencia, hubiera sido más adecuado el empleo de antibióticos glucopeptídicos.Between 1987 and 1995 we performed an instrumented arthrodesis on
295 patients with lumbosacral pathology and 23 patients (7.8%) had a serious infection.
Methicillin-resistent Staphylococcus aureus was the most frequently isolated causal
germs. Second generation Cephalosporin was the antibiotic administered the most.
The infection was cured in 4 cases with medical treatment. However, in 19 patients
surgical cleaning of the damaged tissue was required, although it was not necessary to
remove the osteosynthesis material. In 9 cases a second operation was performed and
a third in 4 others. In our series, 50% of the patients who required surgical cleaning
underwent another operation. Due to the fact that prophylactic antibiotics should cover
the germs prevalent in each hospital ward, according to the resistence patterns,
the use of Glycopeptid antibiotics would have been more appropriate
El corsé de Milwaukee en la escoliosis idiopática: resultados en el plano sagital
Se presentan los resultados obtenidos en el tratamiento de la escoliosis
idiopática con el cors é de Milwauke e en el plano sagital. Han sido estudiado s 51 paciente
s (44 mujere s y 7 varones) con una edad medi a al inicio del tratamient o de 13
años. El tipo de curva según la clasificación de King y Moe corresponde a 18 del tipo I,
16 del tipo II, 12 del III, 1 del IV, 2 del V y dos curvas lumbares. El seguimiento medio
de la serie fue de 5 años. El 88% de los pacientes (45 casos) mostró una disminución del
valor angular medio de la cifosis torácica. El promedio de disminución fue de 10° que
representa el 32% del valor angular inicial. El 86% (44 pacientes) mostró aplanamiento
de la lordosis lumbar , pasando de un valor inicial medi o de 59°, a un valor final del
tratamiento de 49°. Esta disminución representa un 16% del valor inicial. La edad, el tipo
y la elasticidad de la curva, así como el Risser no mostraron relación con la evolución
de las curvas en el plano sagital.The results obtained in the sagittal plane in patients with idiopathic scoliosis
treated by Milwauke e brace are presented. A series of 51 patients (44 females, 7
males) with average of 13 years, wer e included in the study. Using the King and More's
classification, 18 patients had type I curves,16 type II, 12 type III, 1 type IV and 2 patients
showed tipe V curves. Two had lumbar curves. The mean follow-up was 5 years.
In 45 patients (88%) the thoracic Kyphosis showed a decrease of 10° in average angular
value (32% of initial value). 44 patients (86%) showed loss of lumbar lordosis, passing
from an initial average value of 59° to 49° of final average value (16%). Age, type of the
curve, Risser and bending test wer e not related with the evolution of curvatures in the
sagittal plane
Escoliosis congénitas causadas por hemivertebras
Presentamos un estudio retrospectivo de 26 pacientes diagnosticados de escoliosis
congénitas causadas por hemivertebras, liemos analizado la evolución de las curvas según el tipo
de hemivértebra, su localización y el tratamiento recibido. Se han obtenido los peores resultados
en las niñas, cuando el diagnóstico se realizó después de los 10 años de edad, en las curvas localizadas
en la región toracolumbar, en las hemivértebras múltiples y, dentro de ellas, en las unilaterales
segmentadas no adyacentes y en los pacientes tratados ortopédicamente.A retrospective study of 26 patients with congenital scoliosis due to hemivertebra
was conducted. Curve progression was analysed regarding the type of hemivertebra, its localization,
and the applied treatment. The worst results were found in girls, in cases who were diagnosed
with more than to years of age, in thoraco-lumbar curves, in multiple hemivertebra, especially
in non-adjacent segmented unilateral hemivertebra and, finally, in patients treated with orthopaedic
methods

Finite Element Analysis model of a thoracic-lumbar
section of the spine (T12-L2) which has been used to
study the three most commonly used techniques for
the correction and stabilization of type A3.2.1
Gertzbein fractures, one of the most common
fractures in this zone of the spine. This work has
been incorporated into a bigger project, which has
allowed the IBV to develop a complete model of the
thoracic, lumbar and sacrum zone of the spine which
can simulate a wide range of spinal injuries and
analyze the biomechanical behaviour of different

Co-infection and ICU-acquired infection in COIVD-19 ICU patients: a secondary analysis of the UNITE-COVID data set
Background: The COVID-19 pandemic presented major challenges for critical care facilities worldwide. Infections which develop alongside or subsequent to viral pneumonitis are a challenge under sporadic and pandemic conditions; however, data have suggested that patterns of these differ between COVID-19 and other viral pneumonitides. This secondary analysis aimed to explore patterns of co-infection and intensive care unit-acquired infections (ICU-AI) and the relationship to use of corticosteroids in a large, international cohort of critically ill COVID-19 patients.Methods: This is a multicenter, international, observational study, including adult patients with PCR-confirmed COVID-19 diagnosis admitted to ICUs at the peak of wave one of COVID-19 (February 15th to May 15th, 2020). Data collected included investigator-assessed co-infection at ICU admission, infection acquired in ICU, infection with multi-drug resistant organisms (MDRO) and antibiotic use. Frequencies were compared by Pearson's Chi-squared and continuous variables by Mann-Whitney U test. Propensity score matching for variables associated with ICU-acquired infection was undertaken using R library MatchIT using the "full" matching method.Results: Data were available from 4994 patients. Bacterial co-infection at admission was detected in 716 patients (14%), whilst 85% of patients received antibiotics at that stage. ICU-AI developed in 2715 (54%). The most common ICU-AI was bacterial pneumonia (44% of infections), whilst 9% of patients developed fungal pneumonia; 25% of infections involved MDRO. Patients developing infections in ICU had greater antimicrobial exposure than those without such infections. Incident density (ICU-AI per 1000 ICU days) was in considerable excess of reports from pre-pandemic surveillance. Corticosteroid use was heterogenous between ICUs. In univariate analysis, 58% of patients receiving corticosteroids and 43% of those not receiving steroids developed ICU-AI. Adjusting for potential confounders in the propensity-matched cohort, 71% of patients receiving corticosteroids developed ICU-AI vs 52% of those not receiving corticosteroids. Duration of corticosteroid therapy was also associated with development of ICU-AI and infection with an MDRO.Conclusions: In patients with severe COVID-19 in the first wave, co-infection at admission to ICU was relatively rare but antibiotic use was in substantial excess to that indication. ICU-AI were common and were significantly associated with use of corticosteroids
