Plan de cuidados en un paciente con reagudización de EPOC: a propósito de un caso

[Resumen] Introducción: La Enfermedad Pulmonar Obstructiva Crónica (EPOC) es una enfermedad caracterizada por una persistente limitación del aire, generalmente de carácter progresivo y asociado a una respuesta inflamatoria en las vías respiratorias por inhalación de partículas nocivas o gases, y especialmente asociada al humo del tabaco. Son frecuentes los ingresos hospitalarios por reagudizaciones de la patología. Objetivos: 1) Realizar una valoración exhaustiva de un paciente que permaneció ingresado durante 14 días en la Unidad de Neumología del Complejo Hospitalario Universitario de A Coruña (CHUAC). 2) Establecer un plan de cuidados individualizado; verificar su eficacia y garantizar el seguimiento en el área de Atención Primaria posteriormente. Desarrollo: Se realizó un plan de cuidados individualizado empleando las taxonomías NANDA, NOC y NIC para identificar diagnósticos de enfermería, objetivos e intervenciones enfermeras, respectivamente. Se aplicó el modelo AREA, estableciéndose los diagnósticos “Intolerancia a la actividad” y “Conocimientos deficientes” como diagnósticos principales. Se llevó a cabo un seguimiento del paciente durante los días que permaneció ingresado. Discusión y conclusión: las actividades realizadas mostraron una mejoría del paciente. Aínda que os plans de coidados estandarizados resultan moi útiles na práctica clínica, debido ás características desta patoloxía é necesaria a individualización.[Abstract] Introduction: The Chronic Obstructive Pulmonary Disease (COPD) is a disease characterised by a persistent limitation of air, usually with a progressive nature, associated to an inflammatory response in the airways due to the inhalation of harmful particles or gases, and especially associated to tobacco smoke. It is a complex and heterogeneous disease that has an impact in the organism as a whole. Admissions to hospitals due to the aggravation of the pathology are very frequent. Objectives: 1) To carry out a comprehensive assessment of a patient that stayed 14 day in the Respiratory Unit in the A Coruña Teaching Hospital Complex (Hospital Universitario de A Coruña – CHUAC). 2) To establish a personalised care plan using taxonomies NANDA, NIC, NOC taxonomies, to subsequently check the success of the activities and guarantee to monitor the Primary Care area. Development: A personalised care plan was executed using the NANDA, NOC and NIC taxonomies to identify nursing diagnosis, objectives and nursing interventions, respectively. The AREA model was applied, and “Intolerance to the activity” and “Inadequate knowledge” were established as the priority diagnosis. A monitoring of the patient was carried out during his stay at the hospital. Discussion and conclusions: The activities did show an improvement in the patient. Although standardised care plans are very helpful in clinical practice, due to the pathology’s characteristics it is necessary to elaborate a personalised care plan.Traballo fin de grao (UDC.ENFC). Flor de levaduras: Análisis morfológico del crecimiento en medio sólido.

La contaminación con levaduras es un factor muy relevante en el detrimento de la calidad de algunos alimentos. Cuando las levaduras se desarrollan en medio sólido lo hacen en forma de colonias superficiales. El análisis de imagen es una técnica idónea para el análisis objetivo de las diferencias morfológicas que se observan durante el crecimiento de las levaduras en medio sólido. En este trabajo se escogen los parámetros matemáticos que permitan establecer un procedimiento cuantitativo capaz de caracterizar el tránsito de una morfología circular a otra lobulada, programándolos empleando como base las librerías de análisis de imagen Matlab. The most common handball injuries: a systematic review

Handball is a team sport involving a great physical demand from its practitioners in which a high number of injuries occur, affecting individual and collective performance. Knowledge of the injuries is of great importance for their prevention. The objective of the present study was to identify, locate and compare the most frequent injuries and injury mechanisms in handball practice. It was carried out following the Preferred Informed Item for Systematic Reviews and Meta-analysis (PRISMA) guidelines. The source of data collection was direct consultation of the PubMed and Medline databases. Several keywords were used for the documentary retrieval, and the quality of the studies that were selected was evaluated. Of the 707 studies retrieved, only 27 were considered appropriate for the review, and quality scores were obtained that ranged from 10 to 26 points, out of a maximum of 28. The most frequent injuries in handball players are located in the lower limbs (thigh, knee and ankle), and in the shoulder in the upper limbs. Regarding the playing position, the players who play over the 6-m line are the most affected by injuries, while the women players have a higher probability of injury. Cognitive parsimony and legitimization of social injustice: the belief in a just world and latin american fatalism

Este trabajo se ocupa de un tema propio de la psicología social: el conocimiento de sentido común. Particularmente, se analizan las distintas creencias que permiten a las personas pensar y sentir que controlan –o no- el medio en que viven centrándose en la creencia en el mundo justo y el fatalismo latinoamericano. Ambas creencias en su origen psicológico se basan en la necesidad de control del medio y en su contenido cumplen la función de legitimar el orden social y mantener el status quo. Se concluye que la complejidad de este tipo de objetos de estudio implica estudios interdisciplinarios que vinculen diferentes niveles de análisis para así poder dar cuenta de cómo se relacionan los mecanismos psicológicos con los procesos históricos-sociales conformando una visión de la realidad para los distintos grupos sociales.This paper deals with a topic which belongs to social psychology: common sense knowledge. In particular, it analyzes the different beliefs that enable people to think and feel in control -or not- to their environment. This paper makes focus on just world belief and the Latin-American fatalism. Both beliefs are psychological in origin, based on the need for environmental control and their contents serve the function of legitimating social order, supporting the status quo. It is concluded that the complexity of such phenomena involves interdisciplinary studies, which link different levels of analysis in order to explain how psychological mechanisms are related to social historical processes shaping the world views about reality that social groups hold.Fil: Barreiro, Alicia Viviana. Cuéntame: análisis de imagen aplicado al crecimiento de colonias de levadura

Algunas levaduras son capaces de producir deterioro en alimentos desarrollándose en su superficie como colonias. La medida del crecimiento de éstas evaluando el aumento de células viables es una técnica laboriosa y tediosa, mientras que la medida del aumento de su radio proporciona un resultado inmediato. En este trabajo, como alternativa a la medición manual del radio de la colonia, se plantea el empleo de técnicas de análisis de imagen que permiten automatizar el proceso de medición. A partir de las imágenes escaladas digitales, adquiridas en escala de gris de las colonias en crecimiento se ha desarrollado un algoritmo de análisis de imagen con el software MATLAB®. Esta herramienta se ha utilizado para procesar diariamente las imágenes de colonias de cuatro especies de levaduras deteriorantes: Zygosaccharomyces rouxii, Debaryomyces hansenii, Saccharomyces cerevisiae y Rhodotorula glutinis. El error de predicción del tamaño de la colonia al aplicar el algoritmo es comparable con el cometido en la medición manual, no superando en ambos casos el 3-4% y obteniéndose un ajuste medio (R2) entre ambas mediciones de 0.99, ajuste consistente e independiente de la especie de levadura estudiada. La observación de que el crecimiento bifásico del radio está correlacionado con las fases de aumento de células viables hace de este algoritmo una excelente herramienta

Maintenance of immune tolerance by Foxp3+ regulatory T cells requires CD69 expression

Although FoxP3+ regulatory T cells are key players in the maintenance of immune tolerance and autoimmunity, the lack of specific markers constitute an obstacle to their use for immunotherapy protocols. In this study, we have investigated the role of the C-type lectin receptor CD69 in the suppressor function of Tregs and maintenance of immune tolerance towards harmless inhaled antigens. We identified a novel FoxP3+CD69+ Treg subset capable to maintain immune tolerance and protect to developing inflammation. Although CD69+ and CD69-FoxP3+ Tregs exist in homeostasis, only CD69-expressing Tregs express high levels of CTLA-4, ICOS, CD38 and GITR suppression-associated markers, secrete high amounts of TGFβand have potent suppressor activity. This activity is regulated by STAT5 and ERK signaling pathways and is impaired by antibody-mediated down-regulation of CD69 expression. Moreover, immunotherapy with FoxP3+CD69+ Tregs restores the homeostasis in Cd69-/- mice, that fail to induce tolerance, and is also highly proficient in the prevention of inflammation.
