    Comparison of two cash transfer strategies to prevent catastrophic costs for poor tuberculosis-affected households in low- and middle-income countries: An economic modelling study

    BACKGROUND:Illness-related costs for patients with tuberculosis (TB) ≥20% of pre-illness annual household income predict adverse treatment outcomes and have been termed "catastrophic." Social protection initiatives, including cash transfers, are endorsed to help prevent catastrophic costs. With this aim, cash transfers may either be provided to defray TB-related costs of households with a confirmed TB diagnosis (termed a "TB-specific" approach); or to increase income of households with high TB risk to strengthen their economic resilience (termed a "TB-sensitive" approach). The impact of cash transfers provided with each of these approaches might vary. We undertook an economic modelling study from the patient perspective to compare the potential of these 2 cash transfer approaches to prevent catastrophic costs. METHODS AND FINDINGS:Model inputs for 7 low- and middle-income countries (Brazil, Colombia, Ecuador, Ghana, Mexico, Tanzania, and Yemen) were retrieved by literature review and included countries' mean patient TB-related costs, mean household income, mean cash transfers, and estimated TB-specific and TB-sensitive target populations. Analyses were completed for drug-susceptible (DS) TB-related costs in all 7 out of 7 countries, and additionally for drug-resistant (DR) TB-related costs in 1 of the 7 countries with available data. All cost data were reported in 2013 international dollars ().ThetargetpopulationforTB−specificcashtransferswaspoorhouseholdswithaconfirmedTBdiagnosis,andforTB−sensitivecashtransferswaspoorhouseholdsalreadytargetedbycountries′establishedpoverty−reductioncashtransferprogramme.Cashtransfersofferedincountries,unrelatedtoTB,rangedfrom). The target population for TB-specific cash transfers was poor households with a confirmed TB diagnosis, and for TB-sensitive cash transfers was poor households already targeted by countries' established poverty-reduction cash transfer programme. Cash transfers offered in countries, unrelated to TB, ranged from 217 to 1,091/year/household.Beforecashtransfers,DSTB−relatedcostswerecatastrophicin6outof7countries.IfcashtransferswereprovidedwithaTB−specificapproach,alonetheywouldbeinsufficienttopreventDSTBcatastrophiccostsin4outof6countries,andwhenincreasedenoughtopreventDSTBcatastrophiccostswouldrequireabudgetbetween1,091/year/household. Before cash transfers, DS TB-related costs were catastrophic in 6 out of 7 countries. If cash transfers were provided with a TB-specific approach, alone they would be insufficient to prevent DS TB catastrophic costs in 4 out of 6 countries, and when increased enough to prevent DS TB catastrophic costs would require a budget between 3.8 million (95% CI: 3.8million−3.8 million-3.8 million) and 75million(9575 million (95% CI: 50 million-100million)percountry.IfinsteadcashtransferswereprovidedwithaTB−sensitiveapproach,alonetheywouldbeinsufficienttopreventDSTB−relatedcatastrophiccostsinanyofthe6countries,andwhenincreasedenoughtopreventDSTBcatastrophiccostswouldrequireabudgetbetween100 million) per country. If instead cash transfers were provided with a TB-sensitive approach, alone they would be insufficient to prevent DS TB-related catastrophic costs in any of the 6 countries, and when increased enough to prevent DS TB catastrophic costs would require a budget between 298 million (95% CI: 219million−219 million-378 million) and 165,367million(95165,367 million (95% CI: 134,085 million-$196,425 million) per country. DR TB-related costs were catastrophic before and after TB-specific or TB-sensitive cash transfers in 1 out of 1 countries. Sensitivity analyses showed our findings to be robust to imputation of missing TB-related cost components, and use of 10% or 30% instead of 20% as the threshold for measuring catastrophic costs. Key limitations were using national average data and not considering other health and social benefits of cash transfers. CONCLUSIONS:A TB-sensitive cash transfer approach to increase all poor households' income may have broad benefits by reducing poverty, but is unlikely to be as effective or affordable for preventing TB catastrophic costs as a TB-specific cash transfer approach to defray TB-related costs only in poor households with a confirmed TB diagnosis. Preventing DR TB-related catastrophic costs will require considerable additional investment whether a TB-sensitive or a TB-specific cash transfer approach is used

    Impact of Antiretroviral Therapy on the Incidence of Tuberculosis: The Brazilian Experience, 1995–2001

    The human immunodeficiency virus (HIV) fuels tuberculosis (TB) epidemics. In controlled clinical trials, antiretroviral therapy (ART) reduces TB incidence in HIV-infected patients. In this study we determine if, under programmatic conditions, Brazil's policy of universal ART access has impacted TB incidence among HIV-infected patients.We abstracted clinical information from records of HIV-infected patients managed in the public sector in 11 Brazilian states between 1/1/1995 and 12/31/2001. Case ascertainment (TB and HIV) utilized guidelines (with added stringency) published by Brazil's Ministry of Health. We determined TB incidence and hazards ratio (HR) for ART-naïve and ART-treated [including highly active ART (HAART)] patients employing Cox proportional hazards analysis.Information from 463 HIV-infected patients met study criteria. The median age of the study population was 34 years, 70% were male, and mean follow-up to primary endpoints--TB, death, and last clinic visit--was 330, 1059, and 1125 days, respectively. Of the 463 patients, 76 (16%) remained ART-naïve. Of the patients who never received HAART (n = 157) 81 were treated with ART non-HAART. Of the patients who received any ART (n = 387), 306 were treated with HAART (includes those patients who later switched from ART non-HAART to HAART). Tuberculosis developed in 39/463 (8%) patients. Compared to HAART- and ART non-HAART-treated patient groups, TB incidence was 10- (p<0.001) and 2.5-fold (p = 0.03) higher in ART-naïve patients, respectively. The median baseline absolute CD4+ T-lymphocyte count for patients who developed TB was not significantly different from that of patients who remained TB free. In multivariate analysis, the incidence of TB was statistically significantly lower in HAART-treated [HR 0.2; 95% (CI 0.1, 0.6); p<0.01] compared to ART naïve patients. A baseline CD4+ T-lymphocyte count <200 cells/mm(3) [HR 2.5; (95% CI 1.2, 5.4); p<0.01], prior hospitalization [HR 4.2; (95% CI 2.0, 8.8); p<0.001], prior incarceration [HR 4.1; 95% CI 1.6, 10.3); p<0.01], and a positive tuberculin skin test [HR 3.1; (95% CI 1.1, 9.0); p = 0.04] were independently and positively associated with incident TB.In this population-based study we demonstrate an 80% reduction in incident TB, under programmatic conditions, in HAART-treated HIV-infected patients compared to ART-naïve patients

    Tendências temporais de comportamento sexual de risco para infecção pelo HIV em conscritos brasileiros, 1997-2002

    PURPOSE: To present selected results of military conscript surveys related to HIV/AIDS, conducted in Brazil, 1997-2002. METHODS: Questionnaires including information on socio-demographic data, sexual behavior practices, sexually transmitted infections-related problems, and use of injecting drugs were completed by 30970 individuals, obtained through a 2-stage sampling. An index of sexual risk behavior was developed to take into account multiplicity of partners and irregularity of condom use. The HIV infection prevalence rate was estimated for 2002. Logistic regression was used to identify the most important determinants of HIV infection. RESULTS: The percentage of regular condom use increased from 38% (1997) to 49% (2002), and the index of sexual risk behavior decreased from 0.98 in 1997 to 0.87, in 2002. The HIV infection prevalence rate was 0.09%, in 2002, which remained unchanged since1998 Riskier sexual practices among young men with incomplete education and among "men who have sex with men" were found as well as among the participants who reported at least one sexually transmitted infections - related problem. The most important predictor of HIV infection was to be positive for syphilis. CONCLUSIONS: The estimated value of the HIV infection prevalence supports the diagnosis of a concentrated HIV epidemic, in Brazil. Results indicate that particular attention needs to be paid for regional differentials, and for special subgroups, in Brazil.OBJETIVO: Apresentar determinados resultados de levantamentos sobre HIV/AIDS em conscritos do Exército Brasileiro, conduzidos em 1997-2002. MÉTODOS: Aplicaram-se questionários em 30970 conscritos, selecionados segundo um esquema de amostragem em 2 estágios, para obter informações sobre comportamento sexual, problemas relacionados a infecções sexualmente transmitidas e uso de drogas injetáveis. Desenvolveu-se um índice de comportamento sexual de risco para contemplar multiplicidade de parceria sexual e uso de preservativo. Estimou-se, para o ano de 2002, a taxa de prevalência de infecção pelo HIV. RESULTADOS: A porcentagem de uso regular de preservativo aumentou de 38%, em 1997, para 49%, em 2002. O índice de comportamento sexual de risco decresceu de 0,98 em 1997 para 0,87, em 2002. O índice de prevalência de infecção HIV foi de 0.09%, in 2002, permanecendo inalterado desde 1998 Práticas sexuais de maior risco foram observadas entre aqueles com baixa escolaridade e em "homens que fazem sexo com homens", como também entre aqueles com problemas de infecções sexualmente transmissíveis. O mais importante preditor de infecção pelo HIV foi a soropositividade para lues. CONCLUSÕES: A prevalência estimada de infeção pelo HIV suporta o diagnóstico que a epidemia de AIDS, no Brasil, é do tipo "concentrada". Os resultados indicam que são necessárias medidas de intervenção que contemplem diferenças regionais e também voltadas para subgrupos populacionais especiais

    Outcomes of TB Treatment by HIV Status in National Recording Systems in Brazil, 2003–2008

    BACKGROUND: Although the Brazilian national reporting system for tuberculosis cases (SINAN) has enormous potential to generate data for policy makers, formal assessments of treatment outcomes and other aspects of TB morbidity and mortality are not produced with enough depth and rigor. In particular, the effect of HIV status on these outcomes has not been fully explored, partly due to incomplete recording in the national database. METHODOLOGY/PRINCIPAL FINDINGS: In a retrospective cohort study, we assessed TB treatment outcomes, including rates of cure, default, mortality, transfer and multidrug resistant TB (MDR-TB) among a purposively chosen sample of 161,481 new cases reported in SINAN between 2003 and 2008. The study population included all new cases reported in the six States with the highest level of completeness of the HIV status field in the system. These cases were mostly male (67%), white (62%), had pulmonary TB (79%) and a suspect chest X ray (83%). Treatment outcomes were best for those HIV negative cases and worst for those known HIV positive patients (cure rate of 85.7% and 55.7% respectively). In multivariate modeling, the risk of having an unfavorable outcome (all outcomes except cure) was 3.09 times higher for those HIV positive compared with those HIV negative (95% CI 3.02-3.16). The risk of death and default also increased with HIV positivity. The group without a known HIV status showed intermediate outcomes between the groups above, suggesting that this group includes some with HIV infection. CONCLUSIONS: HIV status played an important role in TB treatment outcomes in the study period. The outcomes observed in those with known HIV were poor and need to be improved. Those in the group with unknown HIV status indicate the need for wider HIV testing among new TB cases

    Sistema de información de tratamientos especiales de tuberculosis (SITE-TB) en Brasil : historia, descripción y perspectivas

    O Sistema de Informação de Tratamentos Especiais de Tuberculose (SITE-TB) surgiu da necessidade principal de monitorar, rotineiramente, todas as pessoas com tuberculose drogarresistente (TBDR) no Brasil, e qualificar o controle dos fármacos antituberculose. Desenvolvido pelo Centro de Referência Professor Hélio Fraga e pelo projeto Management Sciences for Health/Brasil, esse sistema online foi implantado em 2013, em todas as Unidades da Federação. Além da TBDR, no SITE-TB são registradas pessoas com tuberculose sensível com indicação de esquema especial, e aquelas com micobacterioses não tuberculosas identificadas por diagnóstico diferencial de tuberculose. Toda pessoa com tuberculose confirmada deve ser notificada no Sistema de Informação de Agravos de Notificação (Sinan). Em situações nas quais se faz necessário tratamento com esquema especial, o caso é encerrado no Sinan e notificado no SITE-TB. Profissionais das unidades de referência para tuberculose fazem a notificação e acompanhamento desses casos no sistema, assim como a gestão dos medicamentos.The Special Tuberculosis Treatment Information System (SITE-TB) arose mainly from the need to routinely monitor all persons with drug-resistant tuberculosis (DR-TB) in Brazil, as well as to qualify tuberculosis’ drug control. Developed by the Professor Hélio Fraga Reference Center and the Management Sciences for Health/Brazil Project, this online system was implemented in 2013 in all Brazilian states. In addition to DR-TB, the system registers people with drug-sensitive tuberculosis with special regimen indications, and those with nontuberculous mycobacterial infections identified by differential diagnosis of tuberculosis. All confirmed tuberculosis cases should be notified on the Notifiable Diseases Information System (SINAN). In situations where treatment with special regimens is necessary, the case is closed on SINAN and notified on SITE-TB. Professionals from tuberculosis reference centers report and monitor these cases on the system, as well as manage tuberculosis’ drugs.El Sistema de Información de Tratamientos Especiales de Tuberculosis (SITE-TB) surgió principalmente de la necesidad de monitorear rutinariamente todas las personas con tuberculosis drogorresistente (TB-DR) en Brasil y cualificar el control de drogas antituberculosis.     The impact of a cash transfer programme on tuberculosis treatment success rate: a quasi-experimental study in Brazil.

    BACKGROUND: Evidence suggests that social protection policies such as Brazil's Bolsa Família Programme (BFP), a governmental conditional cash transfer, may play a role in tuberculosis (TB) elimination. However, study limitations hamper conclusions. This paper uses a quasi-experimental approach to more rigorously evaluate the effect of BFP on TB treatment success rate. METHODS: Propensity scores were estimated from a complete-case logistic regression using covariates from a linked data set, including the Brazil's TB notification system (SINAN), linked to the national registry of those in poverty (CadUnico) and the BFP payroll. RESULTS: The average effect of treatment on the treated was estimated as the difference in TB treatment success rate between matched groups (ie, the control and exposed patients, n=2167). Patients with TB receiving BFP showed a treatment success rate of 10.58 percentage points higher (95% CI 4.39 to 16.77) than patients with TB not receiving BFP. This association was robust to sensitivity analyses. CONCLUSIONS: This study further confirms a positive relationship between the provision of conditional cash transfers and TB treatment success rate. Further research is needed to understand how to enhance access to social protection so to optimise public health impact

    Perspectives and challenges for mother-to-child transmission of HIV, hepatitis B, and syphilis in Brazil

    BackgroundEliminating mother-to-child transmission (MTCT) of HIV, hepatitis B, and syphilis is a challenge in Brazil. Many policies have been implemented since 1986, but important gaps remain. This study aimed to describe the trends of MTCT in Brazil and evaluate the gaps and perspectives in this scenario.MethodsThis is a descriptive study conducted with secondary data publicly available in the information systems of the Brazilian Ministry of Health regarding data on HIV, syphilis, and hepatitis B in pregnant women and children from 2011 to 2021.ResultsHIV and hepatitis B have had constant rates over the years in pregnant women, with the detection rates around 2.5/1,000 live birth (LB) and 0.5/1.000LB, respectively. The same did not happen with syphilis, which has shown an increasing line in the last decade. In 2011, the detection rate of syphilis in pregnancy was 4.7/1,000LB, and in 2021 it reached 27.1/1,000LB. Regarding the trends in children, an important decrease was observed in HIV/AIDS (incidence rate from 0.18/1,000 in 2011 to 0.04/1,000 in 2021) and Hepatitis B (incidence rate from 0.9/1,000LB in 2011 to 0.5/1,000LB in 2021). For congenital syphilis, there is a continuous increase, being 3.3/1,000LB in 2011 and 9.9/1,000LB in 2021. Data from the HIV clinical monitoring showed that antiretroviral treatment coverage among pregnant women identified increased slightly between 2011 and 2021, in Brazil, from 92.3% to 94.3%. For syphilis, 82.5% of pregnant women were treated with benzathine penicillin, and 88.7% in 2011. The historical series of hepatitis B vaccination coverage in children has decreased over the years; it was 96% in 2013 and 76% in 2021.ConclusionThese data show many gaps and some perspectives in the MTCT program in Brazil. The country is close to reaching MTCT HIV elimination, but there are many challenges regarding HBV and syphilis. These data can be used to organize the strategies to improve the Brazilian response to MTCT elimination of HIV, hepatitis B, and syphilis

    Farmacovigilância em tuberculose: relato de uma experiência no Brasil

    Tuberculosis (TB) treatment frequently causes adverse reactions, because on one hand, it employs at least four drugs and on the other hand, these drugs are often used in association with other drugs, such as antiretroviral and glucose-lowering drugs, that interact with antitubercular agents. The Brazilian National Tuberculosis Control Program and the National Health Surveillance Agency (ANVISA) developed a partnership to implement a pilot pharmacovigilance project to encourage the reporting of adverse reactions to antitubercular agents. Training followed by monitoring visits was conducted by three reference health services for TB treatment. Among the bottlenecks identified, we found limitations in access to the information system (NOTIVISA), slow Internet connection, poor adverse event reporting in medical records, lack of multidisciplinary integration and involvement of managers, and fragility of information flows. As a consequence, technical instructional materials were developed, the NOTIVISA form was improved and shortened, indicators for monitoring notifications were proposed, and information flows were reset. We conclude that the partnership was successful and suggest a similar strategy for other programs. Integration of health teams as well as development of simplified notification tools are challenges to be overcome if pharmacovigilance actions are to be sustainable in the country.O tratamento da tuberculose (TB) causa frequentes reações adversas por necessitar da associação de quatro fármacos e por ser frequentemente usado em associação com outros medicamentos, como antirretrovirais e hipoglicemiantes, que apresentam importantes interações com os tuberculostáticos. Com o propósito de reforçar a farmacovigilância em TB no Brasil, o Programa Nacional de Controle da Tuberculose e a Agência Nacional de Vigilância Sanitária desenvolveram um projeto piloto para estimular a notificação das reações adversas aos tuberculostáticos. Foram realizadas capacitações e visitas de monitoramentos em três unidades de saúde de referência para o tratamento da TB. Dentre as dificuldades identificadas, encontramos limitações ao acesso ao sistema NOTIVISA, a precariedade da rede de internet, a ausência de registro das reações adversas nos prontuários dos pacientes, a reduzida integração multiprofissional, o pouco envolvimento dos gestores e a fragilidade dos fluxos de informação. Como desdobramentos, materiais instrucionais foram elaborados, a ficha de notificação do NOTIVISA foi aprimorada, indicadores para o monitoramento das notificações foram propostos e os fluxos redefinidos. Concluímos que a parceria foi bem sucedida, e sugerimos estratégia semelhante para outros programas. A integração das equipes de saúde e a elaboração de ferramentas simplificadas de notificação são desafios a serem vencidos para conferir sustentabilidade às ações de farmacovigilância no país
