    Therapeutic work with clients living in poverty

    Background: Financial inequalities appear to be increasing and poverty is becoming ubiquitous. Poverty affects mental health but its impact on mental health and wellbeing is rarely highlighted within health research. Aims: The Covid-19 pandemic, the Ukrainian invasion and other international and national events have led to a cost-of-living crisis for many people. This is likely to lead to an increase in related referrals and therefore active consideration of the relevant issues relating to poverty appears vital. This paper reports a study which sought to understand how therapists experienced their work with clients who self-refer due to living in poverty. Method: Eight therapists participated in semi-structured interviews analysed using Interpretative Phenomenological Analysis (IPA). Results: Three superordinate themes were elicited: firstly ‘Resilience in the struggle to engage with therapeutic work’, secondly ‘Struggling to promote social activism’ and thirdly, ‘Navigating multiple challenges and barriers’. Each superordinate theme contains two or three sub themes. Conclusions: Issues of structural inequality (including but not limited to poverty) impact significantly on people’s lives but are often ignored or minimised in therapeutic work. It is important that therapists are aware of poverty and take this into account when working with clients

    A deep Chandra observation of the interacting star-forming galaxy Arp 299

    We present results from a 90 ks Chandra ACIS-S observation of the X-ray luminous interacting galaxy system Arp 299 (NGC 3690/IC 694). We detect 25 discrete X-ray sources with luminosities above 4.0x10^38 erg s^-1 covering the entire Ultra Luminous X-ray source (ULX) regime. Based on the hard X-ray spectra of the non-nuclear discrete sources identified in Arp 299, and their association with young, actively star-forming region of Arp 299 we identify them as HMXBs. We find in total 20 off-nuclear sources with luminosities above the ULX limit, 14 of which are point-like sources. Furthermore we observe a marginally significant deficit in the number of ULXs, with respect to the number expected from scaling relations of X-ray binaries with the star formation rate (SFR). Although the high metalicity of the galaxy could result in lower ULX numbers, the good agreement between the observed total X-ray luminosity of ULXs, and that expected from the relevant scaling relation indicates that this deficit could be the result of confusion effects. The integrated spectrum of the galaxy shows the presence of a hot gaseous component with kT = 0.72+-0.03 keV, contributing 20% of the soft (0.1-2.0 keV) unabsorbed luminosity of the galaxy. A plume of soft X-ray emission in the west of the galaxy indicates a large scale outflow. We find that the AGN in NGC 3690 contributes only 22% of the observed broad-band X-ray luminosity of Arp 299.Comment: 20 pages, 14 figures, 9 tables. Accepted for publication in MNRA

    Duchenne and Becker muscular dystrophy prevalence in South Africa and molecular findings in 128 persons affected

    A genetic service for Duchenne muscular dystrophy (DMD) and Becker muscular dystrophy (BMD) was initiated in Cape Town in 1987. Of the 143 DMD patients diagnosed during the period 1987-1992, 66 had a familial pattern of inheritance and 77 were apparently sporadic. Twenty BMD patients were identified, of whom 12 had other affected relatives and 8 were sporadic. Overall minimum prevalence rates of 1/100 000 for DMD and 1/55 000 for BMD were calculated. A markedly low DMD prevalence in the indigenous black population (1/250000) contributed tothe overall low DMD prevalence in South Africa when compared with that in the UK (1/40 000).By means of molecular methods, the diagnosis in 42% of the affected DMD males was confirmed by detection of deletions in the dystrophin gene. Deletions were identified in 50% of Indian, white and mixed ancestry patients. In contrast, only 22% of blacks had identifiable deletions.DMD appears to be underrepresented in the black population; the low deletion frequency in this group suggests that unique mutations not detectable by methods used in this study may be more frequent in these patients than in the other populations.     Duchenne muscular dystrophy - a molecular service

    In 1987 a carrier detection and prenatal diagnostic service for Duchenne muscular dystrophy using molecular technology was instituted at the Department of Human Genetics, University of Cape Town, to serve affe.cted families in southern Africa. DNA samples from 100 affected male subjects and 350 of their relatives from a total of 110 families have been banked. To date restriction fragment length polymorphism (RFLP) analysis and deletion screening has been performed on the DNA of 60 male patients and 116 female relatives at risk of being carriers of the faulty gene. The DNA probes us.ed were pERT 87-1 (Mspl polymorphism) pERT 87-15, pXJ1.1, pXJ2.3 (Taql polymorphism), pXJ1.2 (Ben polymorphism), P20 (Mspl and EeoRV polymorphism) and the cDNA probes. DNA deletions have been detected in 30 of the 60 affected boys and the carrier risks of 49 women have been determined by RFLP analyses. In those families where the risks were uncertain because the affected males had died, prenatal exclusion testing was offered to potential carriers. Two pregnancies were terminated when male fetuses were shown to be affected, since they had the same deletion as that observed in the proband

    X-ray observation of ULAS J1120+0641, the most distant quasar at z=7.08

    We aim at probing the emission mechanism of the accreting super massive black holes in the high redshift Universe. We study the X-ray spectrum of ULAS1120+0641, the highest redshift quasar detected so far at z=7.085, which has been deeply observed (340 ks) by XMM-Newton. Despite the long integration time the spectral analysis is limited by the poor statistics, with only 150 source counts being detected. We measured the spectrum in the 2-80 keV rest-frame (0.3-10 keV observed) energy band. Assuming a simple power law model we find a photon index of 2.0+/-0.3 and a luminosity of 6.7+/-0.3 10^44 erg/s in the 2-10 keV band, while the intrinsic absorbing column can be only loosely constrained (NH< 1E23 cm^-2). Combining our data with published data we calculate that the X-ray-to-optical spectral index alpha_OX is1.8+/-0.1, in agreement with the alpha_OX-UV luminosity correlation valid for lower redshift quasars. We expanded to high energies the coverage of the spectral energy distribution of ULAS1120+0641. This is the second time that a z >6 quasar has been investigated through a deep X-ray observation. In agreement with previous studies of z~6 AGN samples, we do not find any hint of evolution in the broadband energy distribution. Indeed from our dataset ULAS 1120+0641 is indistinguishable from the population of optically bright quasar at lower redshift.Comment: 5 pages, 4 figures, A&A in press; updated with the accepted versio

    Discovery of ultra-fast outflows in a sample of Broad Line Radio Galaxies observed with Suzaku

    We present the results of a uniform and systematic search for blue-shifted Fe K absorption lines in the X-ray spectra of five bright Broad-Line Radio Galaxies (BLRGs) observed with Suzaku. We detect, for the first time at X-rays in radio-loud AGN, several absorption lines at energies greater than 7 keV in three out of five sources, namely 3C 111, 3C 120 and 3C 390.3. The lines are detected with high significance according to both the F-test and extensive Monte Carlo simulations. Their likely interpretation as blue-shifted Fe XXV and Fe XXVI K-shell resonance lines implies an origin from highly ionized gas outflowing with mildly relativistic velocities, in the range 0.04-0.15c. A fit with specific photo-ionization models gives ionization parameters in the range log_xi~4-5.6 and column densities of N_H~10^22-10^23 cm^-2. These characteristics are very similar to those of the Ultra-Fast Outflows (UFOs) previously observed in radio-quiet AGN. Their estimated location within ~0.01-0.3pc from the central super-massive black hole suggests a likely origin related with accretion disk winds/outflows. Depending on the absorber covering fraction, the mass outflow rate of these UFOs can be comparable to the accretion rate and their kinetic power can correspond to a significant fraction of the bolometric luminosity and is comparable to their typical jet power. Therefore, these UFOs can play a significant role in the expected feedback from the AGN on the surrounding environment and can give us further clues on the relation between the accretion disk and the formation of winds/jets in both radio-quiet and radio-loud AGN.Comment: Accepted for publication in The Astrophysical Journal; corrected reference

    The structure of the X-ray absorber in Mrk 915 revealed by Swift

    In this paper we present the results obtained with a monitoring programme (23 days long) performed with Swift-XRT on the local Seyfert galaxy Mrk 915. The light-curve analysis shows a significant count rate variation (about a factor of 2-3) on a time-scale of a few days, while the X-ray colours show a change in the spectral curvature below 2 keV and the presence of two main spectral states. From the spectral analysis we find that the observed variations can be explained by the change of the intrinsic nuclear power (about a factor of 1.5) coupled with a change of the properties of an ionized absorber. The quality of the data prevents us from firmly establishing if the spectral variation is due to a change in the ionization state and/or in the covering factor of the absorbing medium. The latter scenario would imply a clumpy structure of the ionized medium. By combining the information provided by the light curve and the spectral analyses, we can derive some constraints on the location of the absorber under the hypotheses of either homogeneous or clumpy medium. In both cases, we find that the absorber should be located inside the outer edge of an extended torus and, in particular, under the clumpy hypothesis, it should be located near, or just outside, to the broad emission line region.Comment: 8 pages, 6 figures, 1 table.     Arp 299: a second merging system with two active nuclei?

    Recent BeppoSAX observations of Arp 299, a powerful far-IR merging starburst system composed of IC 694 and NGC 3690, clearly unveiled for the first time in this system the presence of a strongly absorbed active galactic nucleus (AGN). However the system was not spatially resolved by BeppoSAX. Here we present the analysis of archival Chandra and (for the first time) XMM-Newton observations, which allow us to disentangle the X-ray emission of the two galaxies. The detection of a strong 6.4 keV line in NGC 3690 clearly demonstrates the existence of an AGN in this galaxy, while the presence of a strong 6.7 keV Fe-Kalpha line in the spectrum of IC 694 suggests that also this nucleus might harbor an AGN. This would be the second discovery of two AGNs in a merging system after NGC 6240

    XMM-Newton and NuSTAR joint observations of Mrk 915: a deep look into the X-ray properties

    We report on the X-ray monitoring programme (covering slightly more than 11 days) carried out jointly by XMM-Newton and NuSTAR on the intermediate Seyfert galaxy Mrk 915. The light curves extracted in different energy ranges show a variation in intensity but not a significant change in spectral shape. The X-ray spectra reveal the presence of a two-phase warm absorber: a fully covering mildly ionized structure [log xi/(erg cm/s)~2.3, NH~1.3x10^21 cm-2] and a partial covering (~90 per cent) lower ionized one [log xi/(erg cm/s)~0.6, NH~2x10^22 cm-2]. A reflection component from distant matter is also present. Finally, a high-column density (NH~1.5x10^23 cm-2) distribution of neutral matter covering a small fraction of the central region is observed, almost constant, in all observations. Main driver of the variations observed between the datasets is a decrease in the intrinsic emission by a factor of ~1.5. Slight variations in the partial covering ionized absorber are detected, while the data are consistent with no variation of the total covering absorber. The most likely interpretation of the present data locates this complex absorber closer to the central source than the narrow line region, possibly in the broad line region, in the innermost part of the torus, or in between. The neutral obscurer may either be part of this same stratified structure or associated with the walls of the torus, grazed by (and partially intercepting) the line of sight.Comment: 14 pages, 10 figures, 4 tables.
