    The role of answer fluency and perceptual fluency in the monitoring and control of reasoning: Reply to Alter, Oppenheimer, and Epley

    In this reply, we provide an analysis of Alter et al. (2013) response to our earlier paper (Thompson et al., 2013). In that paper, we reported difficulty in replicating Alter, Oppenheimer, Epley, and Eyre’s (2007) main finding, namely that a sense of disfluency produced by making stimuli difficult to perceive, increased accuracy on a variety of reasoning tasks. Alter, Oppenheimer, and Epley (2013) argue that we misunderstood the meaning of accuracy on these tasks, a claim that we reject. We argue and provide evidence that the tasks were not too difficult for our populations (such that no amount of “metacognitive unease” would promote correct responding) and point out that in many cases performance on our tasks was well above chance or on a par with Alter et al.’s (2007) participants. Finally, we reiterate our claim that the distinction between answer fluency (the ease with which an answer comes to mind) and perceptual fluency (the ease with which a problem can be read) is genuine, and argue that Thompson et al. (2013) provided evidence that these are distinct factors that have different downstream effects on cognitive processe

    Dilute proparacaine for the management of acute corneal injuries in the emergency department

    Objective: Dogma discourages the provision of topical anesthetics to patients with corneal injuries discharged from the emergency department because of the toxicity of concentrated solutions. We compared the analgesic efficacy of dilute topical proparacaine with placebo in emergency department patients with acute corneal injuries. Methods: We conducted a prospective randomized controlled trial of adults with corneal injuries presenting to one of 2 tertiary care emergency departments in London, Ont. Patients were randomly assigned to groups receiving either 0.05% proparacaine or placebo drops as outpatients and were followed up to healing by a single ophthalmologist. Our primary outcome was pain reduction as measured on a 10-cm visual analog scale. Results: Fifteen participants from the proparacaine group and 18 participants from the placebo group completed the study. The mean age of the patients was 38.7 (standard deviation 12.3) years and the majority were male (85%). Pain reduction was significantly better in the proparacaine group than in the placebo group, with a median improvement of 3.9 (interquartile range [IQR] 1.5-5.1) cm on the visual analog scale versus a median improvement of 0.6 (IQR 0.2-2.0) cm (p = 0.007). The proparacaine group was more satisfied (median level of satisfaction 8.0 [IQR 6.0-9.0] cm on a 10-cm visual analog scale v. 2.6 [IQR 1.0-8.0] cm, p = 0.027). There were no ocular complications or signs of delayed wound healing in either group. Conclusion: Dilute topical proparacaine is an efficacious analgesic for acute corneal injuries. Although no adverse events were observed in our study population, larger studies are required to evaluate safety. © 2010 Canadian Association of Emergency Physicians

    Campus Climate Experiences Associated with Ethnicity in a College of Health Sciences

    The demand for various healthcare professionals is expected to increase along with the diversification of patient populations. Job security is promising in the future of healthcare. Still, less than 50% of college students persist through the STEM education necessary for professional degree programs, with higher attrition rates seen with minority students. The problem is the disproportionate ethnic representation in academic programs that precipitates the consequentially low distribution of minorities in clinical practice and higher education. Inadequate representation poses fewer opportunities for an initial sense of similarity for minority students, often needed for motivation during student-to-faculty interactions. The research study measured differences between ethnic minority and non-minority student perceptions of their academic climate through a quasi-experimental quantitative design. The dependent variables included a sense of belonging, academic and interpersonal validation. Likert scale data was collected to analyze total summed scores between participant groups. Question items from the Diverse Learning Environment Survey were extracted and used as the instrument. The participant group consisted of students from a Division II institution enrolled in an anatomy and physiology course. Data analysis consisted of independent t-tests and Mann Whitney-U testing. Statistically significant differences were found in the sense of belongingness between groups, with ethnic minority students scoring significantly lower. This study's findings help inform educators and clinicians about the needs of minority populations in efforts to retain students and diversify the same force that is assumed to be all-inclusive in clinical practice.     Sociodemographic factors associated with healthy eating and food security in socio-economically disadvantaged groups in the UK and Victoria, Australia

    Objective To investigate the associations between sociodemographic factors and both diet indicators and food security among socio-economically disadvantaged populations in two different (national) contextual settings.Design Logistic regression was used to determine cross-sectional associations between nationality, marital status, presence of children in the household, education, employment status and household income (four low income categories) with daily fruit and vegetable consumption, low-fat milk consumption and food security.Setting Socio-economically disadvantaged neighbourhoods in the UK and Australia.Subjects Two samples of low-income women from disadvantaged neighbourhoods: (i) in the UK, the 2003–05 Low Income Diet and Nutrition Survey (LIDNS; n 643); and (ii) in Australia, the 2007–08 Resilience for Eating and Activity Despite Inequality (READI; n 1340).Results The influence of nationality, marital status and children in the household on the dietary outcomes varied between the two nations. Obtaining greater education qualifications was the most telling factor associated with healthier dietary behaviours. Being employed was positively associated with low-fat milk consumption in both nations and with fruit consumption in the UK, while income was not associated with dietary behaviours in either nation. In Australia, the likelihood of being food secure was higher among those who were born outside Australia, married, employed or had a greater income, while higher income was the only significant factor in the UK.Conclusions The identification of factors that differently influence dietary behaviours and food security in socio-economically disadvantaged populations in the UK and Australia suggests continued efforts need to be made to ensure that interventions and policy responses are informed by the best available local evidence

    Landscape of overlapping gene expression in the equine placenta

    Increasing evidence suggests that overlapping genes are much more common in eukaryotic genomes than previously thought. These different-strand overlapping genes are potential sense-antisense (SAS) pairs, which might have regulatory effects on each other. In the present study, we identified the SAS loci in the equine genome using previously generated stranded, paired-end RNA sequencing data from the equine chorioallantois. We identified a total of 1261 overlapping loci. The ratio of the number of overlapping regions to chromosomal length was numerically higher on chromosome 11 followed by chromosomes 13 and 12. These results show that overlapping transcription is distributed throughout the equine genome, but that distributions differ for each chromosome. Next, we evaluated the expression patterns of SAS pairs during the course of gestation. The sense and antisense genes showed an overall positive correlation between the sense and antisense pairs. We further provide a list of SAS pairs with both positive and negative correlation in their expression patterns throughout gestation. This study characterizes the landscape of sense and antisense gene expression in the placenta for the first time and provides a resource that will enable researchers to elucidate the mechanisms of sense/antisense regulation during pregnancy

    US bases in the Philippines: issues and implications

    The United States maintains in the Philippines its most significant military presence - in terms of bases, facilities, forces and capabilities - in the Southeast Asian and Southwest Pacific region. The bases are designed to support US military operations not just in this region but also in Northeast Asia and throughout the Indian Ocean/Persian Gulf region. The Military Bases Agreement (MBA) between the United States and the Philippines expires in September 1991. There is a very real possibility that the Agreement will not be renewed and that the US will have to vacate the bases and dismantle the facilities. This monograph is intended to provide a basis for informed discussion of issues involved in the presence of the US bases and facilities in the Philippines and their possible closure and relocation elsewhere in the region. It includes discussion of the general political relationship between the United States and the Philippines; the current Philippine domestic political issues; the attitudes of the ASEAN countries; the various redeployment options available to the United States in the region; and the strategic and political implications of the bases issue for Australia

    Migration of cations induces reversible performance losses over day/night cycling in perovskite solar cells

    Perovskites have been demonstrated in solar cells with a power conversion efficiency of well above 20​%, which makes them one of the strongest contenders for next generation photovoltaics. While there are no concerns about their efficiency, very little is known about their stability under illumination and load. Ionic defects and their migration in the perovskite crystal lattice are some of the most alarming sources of degrdn., which can potentially prevent the commercialization of perovskite solar cells (PSCs)​. In this work, we provide direct evidence of elec. field-​induced ionic defect migration and we isolate their effect on the long-​term performance of state-​of-​the-​art devices. Supported by modeling, we demonstrate that ionic defects, migrating on timescales significantly longer (above 103 s) than what has so far been explored (from 10-​1 to 102 s)​, abate the initial efficiency by 10-​15​% after several hours of operation at the max. power point. Though these losses are not negligible, we prove that the initial efficiency is fully recovered when leaving the device in the dark for a comparable amt. of time. We verified this behavior over several cycles resembling day​/night phases, thus probing the stability of PSCs under native working conditions. This unusual behavior reveals that research and industrial stds. currently in use to assess the performance and the stability of solar cells need to be adjusted for PSCs. Our work paves the way for much needed new testing protocols and figures of merit specifically designed for PSCs

    Outcomes of selective nonoperative management of civilian abdominal gunshot wounds: a systematic review and meta-analysis

    Abstract Background Although mandatory laparotomy has been standard of care for patients with abdominal gunshot wounds (GSWs) for decades, this approach is associated with non-therapeutic operations, morbidity, and long hospital stays. This systematic review and meta-analysis sought to summarize outcomes of selective nonoperative management (SNOM) of civilian abdominal GSWs. Methods We searched electronic databases (March 1966–April 1, 2017) and reference lists of articles included in the systematic review for studies reporting outcomes of SNOM of civilian abdominal GSWs. We meta-analyzed the associated risks of SNOM-related failure (defined as laparotomy during hospital admission), mortality, and morbidity across included studies using DerSimonian and Laird random-effects models. Between-study heterogeneity was assessed by calculating I2 statistics and conducting tests of homogeneity. Results Of 7155 citations identified, we included 41 studies [n = 22,847 patients with abdominal GSWs, of whom 6777 (29.7%) underwent SNOM]. The pooled risk of failure of SNOM in hemodynamically stable patients without a reduced level of consciousness or signs of peritonitis was 7.0% [95% confidence interval (CI) = 3.9–10.1%; I2 = 92.6%, homogeneity p  0.99). In patients who failed SNOM, the pooled estimate of the risk of therapeutic laparotomy was 68.0% (95% CI = 58.3–77.7%; I2 = 91.5%; homogeneity p < 0.001). Risks of failure of SNOM were lowest in studies that evaluated patients with right thoracoabdomen (3.4%; 95% CI = 0–7.0%; I2 = 0%; homogeneity p = 0.45), flank (7.0%; 95% CI = 3.9–10.1%), and back (3.1%; 95% CI = 0–6.5%) GSWs and highest in those that evaluated patients with anterior abdomen (13.2%; 95% CI = 6.3–20.1%) GSWs. In patients who underwent mandatory abdominopelvic computed tomography (CT), the pooled risk of failure was 4.1% versus 8.3% in those who underwent selective CT (p = 0.08). The overall sample-size-weighted mean hospital length of stay among patients who underwent SNOM was 6 days versus 10 days if they failed SNOM or developed an in-hospital complication. Conclusions SNOM of abdominal GSWs is safe when conducted in hemodynamically stable patients without a reduced level of consciousness or signs of peritonitis. Failure of SNOM may be lower in patients with GSWs to the back, flank, or right thoracoabdomen and be decreased by mandatory use of abdominopelvic CT scans
