    Economic Evaluations in Health from the Perspective of the Costs Associated with Diabetes Mellitus Treatment

    Diabetes mellitus (DM) is a morbidity that presents a wide range of difficulties for the patient to reach the control. This reality not only has impact on the clinical practice but also has serious financial and social consequences for both the patient and the health system. Health technologies that are capable of improving glycemic control have been tested in cost-effectiveness analysis to assess the efficiency of DM care. According to the Brazilian Society of Diabetes, patients with glycated hemoglobin (A1c) within the values considered adequate, less than 6.5%, present a relative risk of developing complications (neuropathy, retinopathy, diabetic foot, pressure ulcers, cardiovascular diseases and renal disease) equal to that of a nondiabetic patient. In highlight, health technologies health technologies have presented positive impact on reducing A1c and, consequently, on reducing diabetes complications. Thus, new health technologies have been capable saving of 72% of resources spent on DM care

    Diabetes mellitus type 2 (DM2) affects millions of people worldwide and causes several complications for the patient, consuming large sums of financial resources from the health services. This study aims to estimate the financial investment of DM2 treatment for glycemic control of the patient, from the point of view of the municipal Public Health System (SUS). The Delphi technique was used to validate the opinion of a team of judges, specialists in DM2, and health service managers, on the investment necessary for glycemic control of patients with DM2 through the application of questionnaires. In order for the patient to achieve glycated hemoglobin (A1c) < 7%, an investment of US$ 2,419.06 (value/patient/year) is necessary. As the value of A1c increases, investment is reduced. This result reveals the intention to allocate resources for the prevention of DM2 and its complications

    Evaluation of the properties of WHODAS-12 measurements in individuals with Chagas disease in Brazil

    Numerous tests employed to predict cardiac and functional status are expensive and not widely accessible for a considerable number of patients, particularly those diagnosed with Chagas disease (CD) residing in remote and endemic regions. To date, there is no knowledge of studies that have validated instruments that address functionality in an expanded way, including the biopsychosocial factors in patients with CD. This study aims to evaluate the psychometric properties of the World Health Organization Disability Assessment Schedule (WHODAS 2.0), in its 12-item shortened version (WHODAS-12) when applied to patients with CD. This is a cross-sectional study of a prospective cohort that follows individuals with CD (SaMi-Trop). Data collection took place between October 2019 and March 2020. In the interviews, sociodemographic information, life habits, clinical information, and indicators of disability measured by WHODAS-12 were collected. Descriptive analysis, internal consistency and construct validity of the instrument were performed. A total of 628 patients with CD were interviewed, most were women (69.5%), their mean age was of 57 years, and most declared an average self-perception of health (43.4%). The 12 items of WHODAS-12 were distributed into three factors, which together account for 61% of the variance. The Kaiser–Meyer–Olkin (KMO) index was 0.90, indicating adequacy of the sample for factor analysis. The internal consistency of the global scale showed alpha = 0.87. The percentage of incapacity was 16.05%, indicating mild incapacity for the evaluated patients. WHODAS-12 is a valid and reliable measure to assess the disability of the Brazilian population with CD

    Peptide Models - XXIV: An ab Initio Study on N-formyl-l-prolinamide With Trans Peptide Bond. The Existence or Non-existence of Alpha(l) And Epsilon(l) Conformations

    N-formyl-L-prolinamide was subjected to geometry optimization at three levels of theory: HF/3-21G, HF/6-31G (d) and B3LYP/6-31G (d). At all three levels of computation the global minimum was gamma(L) (inverse gamma-Turn) backbone conformation with two ring-puckered forms "UP" and "DOWN". At HF/3-21G level of theory three backbone conformations were found gamma(L), epsilon(L), and alpha(L). At higher levels of theory the epsilon(L), and alpha(L) conformations disappeared. The ''UP'' and ''DOWN'' ring-puckered forms, in the gamma(L) backbone conformation, led to practically identical vibrational spectra at the B3LYP/6-31G (d) level of theory

    How temperatures may affect the synthesis of fatty acids during olive fruit ripening: genes at work in the field

    A major concern for olive cultivation in many extra-Mediterranean regions is the adaptation of recently introduced cultivars to environmental conditions different from those prevailing in the original area, such as the Mediterranean basin. Some of these cultivars can easily adapt their physiological and biochemical parameters in new agro-environments, whereas others show unbalanced values of oleic acid content. The objective of this study was to evaluate the effects of the thermal regime during oil synthesis on the expression of fatty acid desaturase genes and on the unsaturated fatty acid contents at the field level. Two cultivars (Arbequina and Coratina) were included in the analysis over a wide latitudinal gradient in Argentina. The results suggest that the thermal regime exerts a regulatory effect at the transcriptional level on both OeSAD2 and OeFAD2-2 genes and that this regulation is cultivar-dependent. It was also observed that the accumulated thermal time affects gene expression and the contents of oleic and linoleic acids in cv. Arbequina more than in Coratina. The fatty acid composition of cv. Arbequina is more influenced by the temperature regime than Coratina, suggesting its greater plasticity. Overall, findings from this study may drive future strategies for olive spreading towards areas with different or extreme thermal regimes serve as guidance for the evaluation olive varietal patrimony.EEA San JuanFil: Contreras Valentín, Ana Cibeles.     Analisando o uso do método Q nos estudos sobre educação em saúde

    Introdução: O método Q, apesar de criado nos anos de 1930, despertou o interesse de pesquisadores de diversas áreas no século XXI, com crescente uso nas publicações que buscam compreender elementos constituintes da subjetividade. Caracterizado neste trabalho como um método misto de pesquisa, enquadra-se na categoria de estudo exploratório sequencial, representado pela expressão QUAL>QUAN, em que resultados quantitativos são mobilizados para confirmar os resultados qualitativos a fim de melhor entender o fenómeno. Objetivo: Caracterizar o método Q e suas potencialidades como método misto de pesquisa na área da educação na saúde com tecnologias digitais. Método: A pesquisa teve caráter qualitativo, explorou 13 estudos selecionados, de acordo com os critérios de inclusão específicos, nas bases de dados PubMed, Scopus, CINAHL, PsycInfo, Web of Science e LILACS no ano de 2019. Resultados: O método Q é formado por cinco fases, dentre as quais três abrangem o componente qualitativo do estudo: i. definição do concourse (declarações sobre determinado tema); ii. o desenvolvimento da amostra Q (Q sample ou Q set, um conjunto de declarações que representam a amostra do estudo); e iii. a seleção do P set (grupo de participantes do estudo). Uma fase caracteriza o componente quantitativo do método: iv. construção e análise do Q sort a partir da graduação em uma régua pelos participantes do estudo, com uma valorização atribuída a cada declaração e, em seguida, por meio da análise estatística e análise fatorial, busca-se correlações entre os diferentes arranjos de respostas. E, por fim, a fase v. de interpretação dos dados, na qual ocorre a conciliação dos componentes qualitativos e quantitativos. Na revisão de literatura realizada sobre o uso desse método misto na educação na saúde com o suporte de tecnologias verificou-se que há variações nas técnicas utilizadas para construção do concourse, como entrevistas, levantamento de literatura, grupos focais; na definição do número de itens da amostra Q, ou seja, o número de declarações. O conjunto P não necessita de um número elevado de pessoas, pois as baixas taxas de resposta não influenciam os resultados, já que o objetivo principal é identificar uma correlação entre arranjos de respostas, não testar a distribuição proporcional dessas respostas em uma população maior. No caso dos estudos na área da educação em saúde com tecnologias extraíram-se os padrões identificados em estudantes, professores e funcionários de estudantes, professores e funcionários por meio dos Q sorts, que foram analisados com diferentes softwares e variados métodos de rotação. Conclusão: O método Q é uma estratégia para estudar diferentes fenómenos subjetivos a partir da significação emprestada aos ítens ou declarações e a possibilidade de representar as estruturas internalizadas explícitas. Então, aprender sobre esse método e a natureza do seu recorte de pesquisa, tipo misto, parece adequado para o desenvolvimento de estudos na área das ciências da saúde, principalmente na formação profissional na cultura digital.N/

    Plasticity of fruit and oil traits in olive among diferent environments

    Olive is a long-living perennial species with a wide geographical distribution, showing a large genetic and phenotypic variation in its growing area. There is an urgent need to uncover how olive phenotypic traits and plasticity can change regardless of the genetic background. A two-year study was conducted, based on the analysis of fruit and oil traits of 113 cultivars from fve germplasm collections established in Mediterranean Basin countries and Argentina. Fruit and oil traits plasticity, broad‐sense heritability and genotype by environment interaction were estimated. From variance and heritability analyses, it was shown that fruit fresh weight was mainly under genetic control, whereas oleic/(palmitic+linoleic) acids ratio was regulated by the environment and genotype by environment interaction had the major efect on oil content. Among the studied cultivars, diferent level of stability was observed, which allowed ranking the cultivars based on their plasticity for oil traits. High thermal amplitude, the diference of low and high year values of temperature, negatively afected the oil content and the oleic acid percentage. Information derived from this work will help to direct the selection of cultivars with the highest global ftness averaged over the environments rather than the highest ftness in each environment separately.EEA San JuanFil: Mousavi, Soraya.     El Parque Nacional Sierra de las Quijadas se encuentra ubicado al noroeste de la provincia de San Luis. Fue creado en 1990 a fin de preservar parte del ecosistema semiárido, presentando una biodiversidad característica de un ambiente ecotonal de las provincias fitogeográficas del Chaco y del El Monte. En este trabajo se presenta una lista actualizada de anfibios y reptiles, integrada por 25 especies correspondientes a las familias Bufonidae (1), Leptodactyllidae (5), Testudinidae (1), Gekkonidae (3), Teiidae (3), Tropiduridae (3), Polychrotidae (1), Amphisbaenidae (1), Boidae (1), Colubridae (4) y Viperidae (2). Las especies Proctrotretus doellojuradoi, Homonota fasciata, Anops kingi y Bothrops alternatus corresponden a nuevas citas para el Parque Nacional Sierra de las Quijadas<br>The National Park Sierra de las Quijadas is located on the NW of the San Luis province. It was created in 1990 with aim of preserve part of the semiarid ecosystem. Its has a characteristic biodiversity of an ecotonal enviromen formed by the phytogeographic regions of El Chaco and El Monte. In this paper we present an actualized list of amphibians and reptiles. This list is formed by the families: Bufonidae (1), Leptodactyllidae (5), Testudinidae (1), Gekkonidae (3), Teiidae (3), Tropiduridae (3), Polychrotidae (1), Amphisbaenidae (1), Boidae (1), Colubridae (4) y Viperidae (2). The species Proctrotretus doellojuradoi, Homonota fasciata, Anops kingi y Bothrops alternatus represent new citations for the National Park Sierra de las Quijada
