A SNAPSHOT VIEW OF HRM IN TURKEY: EVIDENCES FROM COMPANIES LOCATED IN MARMARA REGION
HRM discipline, occurring in 1920’s in the USA, has been predominant in the USA and the European countries and there has been evolving phases throughout the 20th century (Storey, 1989). Personnel management began to gain a more managerial role in the 1960s. As a result of this process, there has been a shift from activities such as arrangement of data storage and personnel files to the administration of employee procedures (Fombrun et al., 1984). However, in practice the replacement of HRM for personnel management has only meant the change of department names and titles for some companies and “there has been no considerable change in terms of the function’s quality and activities (Armstrong, 2000). Research shows that the number of department names and titles containing the term “human resources” appears to be significantly higher than the ones containing the term “personnel” (Caldwell, 2002, Bayraktaroglu, 2006). Turkey is not an exception to this trend and contributed greatly by changing the department names overnight in most companies. Examining the current issues that have recently taken place in the HRM field, it would be suitable to point out that the strategic nature and role of the HR function have been emphasized since the 1980s (Bayraktaroglu, 2002). The HR function’s gaining of a strategic role is said to show the increase of its importance (Bowen et al., 2002).
This paper critically explores the current status of HRM in large Turkish and will be based on questionnaires conducted within large companies who have HR departments in Marmara Region of Turkey
TASL practice guidance on the clinical assessment and management of patients with nonalcoholic fatty liver disease
Nonalcoholic fatty liver disease (NAFLD) is a multisystem disease and is significantly associated with obesity, insulin resistance, type 2 diabetes mellitus, metabolic syndrome, and cardiovascular disease. NAFLD has become the most prevalent chronic liver disease in Western countries, and the proportion of NAFLD-related cirrhosis among patients on liver transplantation waiting lists has increased. In light of the accumulated data about NAFLD, and to provide a common approach with multi-disciplines dealing with the subject, it has become necessary to create new guidance for diagnosing and treating NAFLD. This guidance was prepared following an interdisciplinary study under the leadership of the Turkish Association for the Study of the Liver (TASL), Fatty Liver Special Interest Group. This new TASL Guidance is a practical application guide on NAFLD and was prepared to standardize the clinical approach to diagnosing and treating NAFLD patients. This guidance reflects many advances in the field of NAFLD. The proposals in this guidance are meant to aid decision-making in clinical practice. The guidance is primar-ily intended for gastroenterology, endocrinology, metabolism diseases, cardi-ology, internal medicine, pediatric specialists, and family medicine specialists
Outcome of COVID-19 in Patients With Autoimmune Hepatitis: An International Multicenter Study
Background and Aims: Data regarding outcome of COVID-19 in patients with autoimmune hepatitis (AIH) are lacking. Approach and Results: We performed a retrospective study on patients with AIH and COVID-19 from 34 centers in Europe and the Americas. We analyzed factors associated with severe COVID-19 outcomes, defined as the need for mechanical ventilation, intensive care admission, and/or death. The outcomes of patients with AIH were compared to a propensity score?matched cohort of patients without AIH but with chronic liver diseases (CLD) and COVID-19. The frequency and clinical significance of new-onset liver injury (alanine aminotransferase > 2 × the upper limit of normal) during COVID-19 was also evaluated. We included 110 patients with AIH (80% female) with a median age of 49 (range, 18-85) years at COVID-19 diagnosis. New-onset liver injury was observed in 37.1% (33/89) of the patients. Use of antivirals was associated with liver injury (P = 0.041; OR, 3.36; 95% CI, 1.05-10.78), while continued immunosuppression during COVID-19 was associated with a lower rate of liver injury (P = 0.009; OR, 0.26; 95% CI, 0.09-0.71). The rates of severe COVID-19 (15.5% versus 20.2%, P = 0.231) and all-cause mortality (10% versus 11.5%, P = 0.852) were not different between AIH and non-AIH CLD. Cirrhosis was an independent predictor of severe COVID-19 in patients with AIH (P < 0.001; OR, 17.46; 95% CI, 4.22-72.13). Continuation of immunosuppression or presence of liver injury during COVID-19 was not associated with severe COVID-19. Conclusions: This international, multicenter study reveals that patients with AIH were not at risk for worse outcomes with COVID-19 than other causes of CLD. Cirrhosis was the strongest predictor for severe COVID-19 in patients with AIH. Maintenance of immunosuppression during COVID-19 was not associated with increased risk for severe COVID-19 but did lower the risk for new-onset liver injury during COVID-19.Fil: Efe, Cumali. Effects of immunosuppressive drugs on COVID-19 severity in patients with autoimmune hepatitis
Background: We investigated associations between baseline use of immunosuppressive drugs and severity of Coronavirus Disease 2019 (COVID-19) in autoimmune hepatitis (AIH). Patients and methods: Data of AIH patients with laboratory confirmed COVID-19 were retrospectively collected from 15 countries. The outcomes of AIH patients who were on immunosuppression at the time of COVID-19 were compared to patients who were not on AIH medication. The clinical courses of COVID-19 were classified as (i)-no hospitalization, (ii)-hospitalization without oxygen supplementation, (iii)-hospitalization with oxygen supplementation by nasal cannula or mask, (iv)-intensive care unit (ICU) admission with non-invasive mechanical ventilation, (v)-ICU admission with invasive mechanical ventilation or (vi)-death and analysed using ordinal logistic regression. Results: We included 254 AIH patients (79.5%, female) with a median age of 50 (range, 17-85) years. At the onset of COVID-19, 234 patients (92.1%) were on treatment with glucocorticoids (n = 156), thiopurines (n = 151), mycophenolate mofetil (n = 22) or tacrolimus (n = 16), alone or in combinations. Overall, 94 (37%) patients were hospitalized and 18 (7.1%) patients died. Use of systemic glucocorticoids (adjusted odds ratio [aOR] 4.73, 95% CI 1.12-25.89) and thiopurines (aOR 4.78, 95% CI 1.33-23.50) for AIH was associated with worse COVID-19 severity, after adjusting for age-sex, comorbidities and presence of cirrhosis. Baseline treatment with mycophenolate mofetil (aOR 3.56, 95% CI 0.76-20.56) and tacrolimus (aOR 4.09, 95% CI 0.69-27.00) were also associated with more severe COVID-19 courses in a smaller subset of treated patients. Conclusion: Baseline treatment with systemic glucocorticoids or thiopurines prior to the onset of COVID-19 was significantly associated with COVID-19 severity in patients with AIH.Fil: Efe, Cumali. A key problem and challenge for hepatology: Obesity-related metabolic liver diseases
With the arrival of the new millennium, gastroenterologists have been faced with the problem of metabolic liver diseases associated with obesity. The active role of the liver in metabolism and inflammation make it a key organ in the war against the rapidly-spreading world-wide epidemic of obesity. Many lives and much money could be saved if the work of hepatologists led to the development of effective diagnostic and therapeutic strategies against this growing leader of cirrhosis
Gastric cancer prevention from the point of helicobacter
WOS: 000345025900038PubMed ID: 25254545When the first cancer estimates of world were made in 1975, gastric cancer (GC) was the most common neoplasm, making 70% of the total. Although its rate decreased to 6.8% in 2012, it is still the fifth most common malignancy after cancers of the lung, breast, colo-rectum, and prostate and the third leading cause of cancer death in both sexes worldwide (8.8%, 723,000 deaths) (1). Proximal (cardia) and distal (non-cardia) gastric adenocarcinomas have different epidemiological and clinical features. Although there is an increase in proximal GCs, most of the GCs are still distally located, and intestinal-type. H. pylori is an established trigger of gastric carcinogenesis; reversibility of precancerous conditions, including intestinal metaplasia (IM), after eradication treatment is a hot topic for research. Therefore, we read with great interest the study by Galiatsatos P et al. (2) on the sensitivity of gastric biopsy for H. pylori detection in the presence of IM. The data have once again emphasized the importance of using non-invasive tests and histopathology together in the presence of gastric IM
Cultural ethymology of animal names in the Dede Korkut Book
Dede Korkut Kitabı Türk toplumunun kültürel belleğine izlerini bırakmış çok önemli bir kaynaktır. Bünyesinde barındırdığı Oğuzların tarihi, kültürel ve dilbilimsel pek çok unsur bu eserin önemini daha da arttırmaktadır. Oğuzların toplumsal yaşamında bulundukları coğrafyada olumlu veya olumsuz karşılaştıkları tüm olaylar kültürel kimliklerine kazandırdıkları olgular dil aracılığı ile günümüze kadar ulaşmıştır. Bu anlamda dil hem bu kültürel olguların taşıyıcısı hem de en somut belgesidir. Günümüzde gerek Türk halkbilimi gerekse dilbilimi alanında sayısız çalışmaya kaynaklık Dede Korkut Kitabı'nın sınırları sonsuzdur. Kültürel derinliğin tespit ve değerlendirmesinde ilk başvurulacak kaynak Dede Korkut Kitabı olacaktır. Dede Korkut Kitabı'nda hayvanlar Oğuzların inanç ve düşünce sistemleri, sosyo-psikolojik ve kültürel yapısını anlamlandırmaya yardımcı olur. Dolayısıyla hem maddi bir gereklilik hem de manevi değerler sisteminin anlamsal bütünlüğünü ifade etmenin, açıklamanın, yorumlamanın aracı olmuştur. Bu kapsamda Dede Korkut Kitabı'nda ismi geçen hayvanların etimolojisinden yola çıkarak kültürel izleri takip ve tespit etmek amaçlanmaktadır.Dede Korkut's book is a very important resource that has left its traces in the cultural memory of Turkish society. Many historical, cultural and linguistic elements of the Oghuz that it contains further increase the importance of this work. All the events that Oghuz people encounter positively or negatively in their social life in the geography in which they are located, the facts that they have acquired their cultural identity have survived to the present day through language. In this sense, language is both the bearer of these cultural facts and the most concrete document. Currently, the boundaries of The Book of Dede Korkut, which is the source of numerous studies in both Turkish folklore and linguistics, are endless. Dede Korkut book will be the first source to be applied in the determination and evaluation of cultural depth. In the book of Dede Korkut, animals help to make sense of the belief and thought systems, socio-psychological and cultural structure of the Oghuz. 360 Derece Geribildirim Sistemine Eleştirel Bir Bakış: Bir Örnek Olay
Performans değerlendirme önemi giderek artan ve diğer insan kaynakları fonksiyonlarına veri sağlayan bir özellik taşımaktadır. Performans değerlendirme sistemlerinin tamamlayıcısı niteliği taşıyan 360 derece geri bildirim sistemi, çalışanlarının performanslarının değerlendirilmesinde onlarla ilişkide olan tüm taraflardan geribildirim sağlamaktadır. İnsan kaynaklarının ve işletmenin gelişimi için yararlı ve etkinolduğu iddia edilen bu çağdaş ve çok yönlü sistemin başarısı birçok önşarta bağlanmıştır. Bu nedenle 360 derece geribildirim esasında eleştirel bir bakış açısıyla değerlendirilmelidir. Bu çalışma ile incelenen örnek olay bağlamında bu eleştirel bakış açısının detaylandırılması amaçlanmıştır
Acute temozolomide induced liver injury: Mixed type hepatocellular and cholestatic toxicity
Temozolomide (TMZ) is an oral imidazotetrazine methylating agent which is used for the treatment of glioblastoma multiforme (GBM). We report a case of acute hepatotoxicity in a 53-year old male patient after administration of TMZ for GBM. He had fatigue, nausea, anorexia and jaundice. His laboratory analysis showed alanine aminotransferase(ALT): 632 IU/L (normal range 0-40); aspartate aminotransferase(AST): 554 IU/L (normal range 5-34); alkaline phosphatase(ALP): 1143 IU/L (normal range 40- 150); -glutamyl transpeptidase(GGT): 514 IU/L (normal range 9-64 IU/L); total bilirubin: 15.1 mg/dL (normal range 0-1.2); direct bilirubin: 13.2 mg/dL and prothrombin time(PT): 13.5 s, with international normalized ratio (INR): 1.1 (normal range 0.8-1.2). His liver biopsy specimen showed mixed-type (both hepatocellular and cholestatic) hepatic injury, compatible with a diagnosis of drug-induced hepatitis. An objective causality assessment using the Naranjo probability scale suggested that TMZ was the probable cause of the acute hepatitis. His liver function tests gradually normalized in 2 months after discontinuation of the drug. In susceptible individuals, TMZ use may lead to acute mixed type liver toxicity. Complete recovery may be possible if the drug is discontinued before severe liver injury is established
