    Recursion and Ambiguity: a Linguistic and Computational Perspective

    In this chapter I will be concerned with what characterizes human language and the parser that computes it in real communicative situations. I will start by discussing and dismissing Hauser, Chomsky and Fitch's(2002) (HC&F) disputed claim that the "only uniquely human component of the faculty of language" be "recursion". I will substantiate my rejection of HC&F's claims, with the fact that recursion only appears in mature and literate language - an opinion also shared by some papers in a book on recursion by Harry van der Hulst (2010). I will then present in detail Chomsky's proposal - now part of the Minimalist Theory (MT) - of the architecture of the human parser as being based on Phases. I will accept this part of the theory and compare it with the computational architecture contained in a system for deep text understanding called Getaruns (Delmonte,2007;2009). I will then argue in favour of what I regard the peculiar component of human language faculty that is "the ability to associate meaning to deficient propositions and generic linguistic expressions, which are highly ambiguous in their structure”. And this is also due to the presence of recursion (but not only). I will then speak in favour of a parser that takes "context" into account and strives for a "complete" syntactic representation. As to the problem of ambiguity, I will introduce the use of a computational device, a lookahead mechanism, which is presented in association with the need to specify UG parameters characterizing a given language. I will discuss the use of psychologically viable Parsing Strategies implemented in the parser to overcome ambiguity and prevent Garden Path, whenever possible. This will be highlighted by reference to peculiar features associated to different languages, Italian and English. Eventually, I will present a theory that encompasses all my previous proposals and is called LSLT

    GABA Receptors and the Pharmacology of Sleep

    Current GABAergic sleep-promoting medications were developed pragmatically, without making use of the immense diversity of GABAA receptors. Pharmacogenetic experiments are leading to an understanding of the circuit mechanisms in the hypothalamus by which zolpidem and similar compounds induce sleep at α2βγ2-type GABAA receptors. Drugs acting at more selective receptor types, for example, at receptors containing the α2 and/or α3 subunits expressed in hypothalamic and brain stem areas, could in principle be useful as hypnotics/anxiolytics. A highly promising sleep-promoting drug, gaboxadol, which activates αβδ-type receptors failed in clinical trials. Thus, for the time being, drugs such as zolpidem, which work as positive allosteric modulators at GABAA receptors, continue to be some of the most effective compounds to treat primary insomnia

    Postpartum psychiatric disorders

    Pregnancy is a complex and vulnerable period that presents a number of challenges to women, including the development of postpartum psychiatric disorders (PPDs). These disorders can include postpartum depression and anxiety, which are relatively common, and the rare but more severe postpartum psychosis. In addition, other PPDs can include obsessive–compulsive disorder, post-traumatic stress disorder and eating disorders. The aetiology of PPDs is a complex interaction of psychological, social and biological factors, in addition to genetic and environmental factors. The goals of treating postpartum mental illness are reducing maternal symptoms and supporting maternal–child and family functioning. Women and their families should receive psychoeducation about the illness, including evidence-based discussions about the risks and benefits of each treatment option. Developing effective strategies in global settings that allow the delivery of targeted therapies to women with different clinical phenotypes and severities of PPDs is essential

    A short history of the 5-HT2C receptor: from the choroid plexus to depression, obesity and addiction treatment

    This paper is a personal account on the discovery and characterization of the 5-HT2C receptor (first known as the 5- HT1C receptor) over 30 years ago and how it translated into a number of unsuspected features for a G protein-coupled receptor (GPCR) and a diversity of clinical applications. The 5-HT2C receptor is one of the most intriguing members of the GPCR superfamily. Initially referred to as 5-HT1CR, the 5-HT2CR was discovered while studying the pharmacological features and the distribution of [3H]mesulergine-labelled sites, primarily in the brain using radioligand binding and slice autoradiography. Mesulergine (SDZ CU-085), was, at the time, best defined as a ligand with serotonergic and dopaminergic properties. Autoradiographic studies showed remarkably strong [3H]mesulergine-labelling to the rat choroid plexus. [3H]mesulergine-labelled sites had pharmacological properties different from, at the time, known or purported 5-HT receptors. In spite of similarities with 5-HT2 binding, the new binding site was called 5-HT1C because of its very high affinity for 5-HT itself. Within the following 10 years, the 5-HT1CR (later named 5- HT2C) was extensively characterised pharmacologically, anatomically and functionally: it was one of the first 5-HT receptors to be sequenced and cloned. The 5-HT2CR is a GPCR, with a very complex gene structure. It constitutes a rarity in theGPCR family: many 5-HT2CR variants exist, especially in humans, due to RNA editing, in addition to a few 5-HT2CR splice variants. Intense research led to therapeutically active 5-HT2C receptor ligands, both antagonists (or inverse agonists) and agonists: keeping in mind that a number of antidepressants and antipsychotics are 5- HT2CR antagonists/inverse agonists. Agomelatine, a 5-HT2CR antagonist is registered for the treatment of major depression. The agonist Lorcaserin is registered for the treatment of aspects of obesity and has further potential in addiction, especially nicotine/ smoking. There is good evidence that the 5-HT2CR is involved in spinal cord injury-induced spasms of the lower limbs, which can be treated with 5-HT2CR antagonists/inverse agonists such as cyproheptadine or SB206553. The 5-HT2CR may play a role in schizophrenia and epilepsy. Vabicaserin, a 5-HT2CR agonist has been in development for the treatment of schizophrenia and obesity, but was stopped. As is common, there is potential for further indications for 5-HT2CR ligands, as suggested by a number of preclinical and/or genome-wide association studies (GWAS) on depression, suicide, sexual dysfunction, addictions and obesity. The 5-HT2CR is clearly affected by a number of established antidepressants/antipsychotics and may be one of the culprits in antipsychotic-induced weight gain

    Implementation and validation of telepathology triage at cancer referral center in rural Rwanda

    Purpose Connecting a cancer patient to the appropriate treatment requires the correct diagnosis provided in a timely manner. In resource-limited settings, the anatomic pathology bridge to efficient, accurate, and timely cancer care is often challenging. In this study, we present the first phase of an anatomic telepathology triage system, which was implemented and validated at the Butaro District Hospital in northern rural Rwanda. Methods Select cases over a 9-month period in three segments were evaluated by static image telepathology and were independently evaluated by standard glass slide histology. Each case via telepathology was classified as malignant, benign, infectious/inflammatory, or nondiagnostic and was given an exact histologic diagnosis. Results For cases triaged as appropriate for telepathology, correlation with classification and exact diagnosis demonstrated greater than 95% agreement over the study. Cases in which there was disagreement were analyzed for cause, and the triage process was adjusted to avoid future problems. Conclusion Challenges to obtaining a correct and complete diagnosis with telepathology alone included the need for immunohistochemistry, assessment of the quality of images, and the lack of images representing an entire sample. The next phase of the system will assess the effect of telepathology triage on turnaround time and the value of on-site immunohistochemistry in reducing that metric and the need for evaluation outside of telepathology

    Reanalysis and Limited Repair Parsing: Leaping off the Garden Path

    This chapter develops a theory of reanalysis called limited repair parsing. Repair parsers deal with the problem of local ambiguity in part by modifying previously built structure when the chosen structure later proves to be inconsistent. This modification of existing structure distinguishes repair parsing from parallel or multi-path parsing, least-commitment parsing, backtracking, or reparsing strategies. Parsers with a limited capability for repair are psycholinguistically important because they can potentially explain the contrasts between difficult garden path structures (when repair fails) and unproblematic local ambiguities (when repair is successful or easy). Although the idea of repair has been implicit in some psycholinguistic work (and emerged explicitly in the diagnosis model of Fodor & Inoue, 1994, and the NL-Soar model of Lewis, 1993), there has been no clear formulation of the general class of repair parsers.
