    What is the psychological impact of mammographic screening on younger women with a family history of breast cancer? Findings from a prospective cohort study (PIMMS)

    PURPOSE: Studies are underway to establish the clinical effectiveness of annual mammographic screening in women younger than 50 years with a family history of breast cancer. This study investigated both the positive and negative psychological effects of screening on these women. PATIENTS AND METHODS: Women who received an immediate all-clear result after mammography (n = 1,174) and women who were recalled for additional tests before receiving an all-clear result (false positive; n = 112) completed questionnaires: 1 month before mammography, and 1 and 6 months after receiving final results. The questionnaires included measures of cancer worry, psychological consequences, and perceived benefits of breast screening. RESULTS: Women who received an immediate all-clear result experienced a decrease in cancer worry and negative psychological consequences immediately after the result, whereas women who were recalled for additional tests did not. By 6 months this cancer-specific distress had reduced significantly in both groups. Changes in levels of distress were significantly different between the two groups, but in absolute terms the differences were not large. Recalled women reported significantly greater positive psychological consequences of screening immediately after the result, and were also more positive about the benefits of screening compared with women who received an immediate all-clear result. CONCLUSION: For women receiving an immediate all-clear result, participating in annual mammographic screening is psychologically beneficial. Furthermore, women who are recalled for additional tests do not appear to be harmed by screening: these women's positive views about mammography suggest that they view any distress caused by recall as an acceptable part of screening

    Migratory corridors and foraging hotspots: Critical habitats identified for Mediterranean green turtles

We provide a comprehensive overview of adult green turtle (Chelonia mydas) distribution during nesting, migration and foraging phases, highlighting transitory as well as residential areas of high use to facilitate adequate protection for this long-lived, migratory species. Location: Mediterranean Sea. Methods: Thirty-four females were satellite tracked from breeding grounds in the four countries with major nesting (Cyprus, Turkey, Israel and Syria) for a total of 8521 (mean: 251) tracking days in a collaborative effort to summarize the most comprehensive set of distribution data thus far assembled for this species in the Mediterranean. Results: Ten foraging grounds are identified, with two major hotspots in Libya accounting for >50% of turtles tracked to conclusive endpoints. The coastlines of Egypt and Libya contain high densities of migrating turtles following the nesting season, particularly July-September, and likely also pre-nesting (April-June). A high-use seasonal pelagic corridor running south-west from Turkey and Cyprus to Egypt is also evident, used by >50% of all tracked turtles. Main conclusions: Bycatch levels and mortality rates for the key foraging areas and high-density seasonal pathways identified here are largely unknown and should be investigated as a priority. We recommend that the Gulf of Sirte in Libya be explored as a potential biodiversity hotspot and considered for proposal as a marine protected area (MPA). Green turtle fidelity to nesting beaches, foraging areas and migratory pathways renders them vulnerable to localized threats but enables targeted mitigation measures and protection

    Interstellar scintillation as the origin of rapid radio variability in the quasar J1819+3845

    Quasars shine brightly due to the liberation of gravitational energy as matter falls onto a supermassive black hole in the centre of a galaxy. Variations in the radiation received from active galactic nuclei (AGN) are studied at all wavelengths, revealing the tiny dimensions of the region and the processes of fuelling the black hole. Some AGN are variable at optical and shorter wavelengths, and display radio outbursts over years and decades. These AGN often also show faster variations at radio wavelengths (intraday variability, IDV) which have been the subject of much debate. The simplest explanation, supported by a correlation in some sources between the optical (intrinsic) and faster radio variations, is that the rapid radio variations are intrinsic. However, this explanation implies physically difficult brightness temperatures, suggesting that the variations may be due to scattering of the incident radiation in the interstellar medium of our Galaxy. Here we present results which show unambiguously that the variations in one extreme case are due to interstellar scintillation. We also measure the transverse velocity of the scattering material, revealing a surprising high velocity plasma close to the Solar System

    Mediterranean sea turtles: Current knowledge and priorities for conservation and research

Available on open access from Inter Research via the DOI in this recordThe available information regarding the 2 sea turtle species breeding in the Mediterranean (loggerhead turtle Caretta caretta and green turtle Chelonia mydas) is reviewed, including biometrics and morphology, identification of breeding and foraging areas, ecology and behaviour, abundance and trends, population structure and dynamics, anthropogenic threats and conservation measures. Although a large body of knowledge has been generated, research efforts have been inconsistently allocated across geographic areas, species and topics. Significant gaps still exist, ranging from the most fundamental aspects, such as the distribution of major nesting sites and the total number of clutches laid annually in the region, to more specific topics like age at maturity, survival rates and behavioural ecology, especially for certain areas (e.g. south-eastern Mediterranean). These gaps are particularly marked for the green turtle. The recent positive trends of nest counts at some nesting sites may be the result of the cessation of past exploitation and decades of conservation measures on land, both in the form of national regulations and of continued active protection of clutches. Therefore, the current status should be considered as dependent on such ongoing conservation efforts. Mitigation of incidental catch in fisheries, the main anthropogenic threat at sea, is still in its infancy.     Comparative Population Genetics of the Immunity Gene, Relish: Is Adaptive Evolution Idiosyncratic?

    The frequency of adaptive evolution acting on common loci in distant lineages remains an outstanding question in evolutionary biology. We asked whether the immunity factor, Relish, a gene with a history of directional selection in Drosophila simulans, shows evidence of a similar selective history in other Drosophila species. We found only weak evidence of recurrent adaptive protein evolution at the Relish locus in three sister species pairs, suggesting that this key component of the insect immune system has an idiosyncratic evolutionary history in Drosophila

    May Measurement Month 2019: an analysis of blood pressure screening results from the United Kingdom and Republic of Ireland

    In the UK, heart and circulatory diseases account for 29% of all deaths (14% through coronary heart disease and 8% through stroke). In 2015, the prevalence of hypertension was 20% in the UK and 23% in the Republic of Ireland. In 2019, 14% of people registered with a UK general practice had hypertension and yet it was the attributable risk factor for around half of all deaths from coronary heart disease or stroke. We participated in May Measurement Month 2019 to increase awareness of blood pressure (BP) measurement, and to identify the proportion of undiagnosed hypertension and degree of uncontrolled hypertension in the community. The 2019 campaign set up screening sites within the community at places of worship, supermarkets, GP surgeries, workplaces, charity events, community pharmacies, gyms, and various other public places. We screened 10194 participants (mean age 51 ± 18 years, 60% women) and found that 1013 (9.9%) were on antihypertensive treatment, while 3408 (33.4%) had hypertension. Of the 3408 participants with hypertension, only 33.5% were aware of their condition despite 98.8% having previous BP measurements. In those on antihypertensive medication, only 38.2% had controlled BP (<140 and <90 mmHg). Our UK and Republic of Ireland data demonstrate concerning levels of undiagnosed hypertension and sub-optimal BP control in many individuals with a diagnosis. This evidence supports a critical need for better systematic community and primary care screening initiatives

    Breakdown of the adiabatic limit in low dimensional gapless systems

    It is generally believed that a generic system can be reversibly transformed from one state into another by sufficiently slow change of parameters. A standard argument favoring this assertion is based on a possibility to expand the energy or the entropy of the system into the Taylor series in the ramp speed. Here we show that this argumentation is only valid in high enough dimensions and can break down in low-dimensional gapless systems. We identify three generic regimes of a system response to a slow ramp: (A) mean-field, (B) non-analytic, and (C) non-adiabatic. In the last regime the limits of the ramp speed going to zero and the system size going to infinity do not commute and the adiabatic process does not exist in the thermodynamic limit. We support our results by numerical simulations.     The potential of unmanned aerial systems for sea turtle research and conservation: A review and future directions

Available on open access from Inter Research via the DOI in this recordThe use of satellite systems and manned aircraft surveys for remote data collection has been shown to be transformative for sea turtle conservation and research by enabling the collection of data on turtles and their habitats over larger areas than can be achieved by surveys on foot or by boat. Unmanned aerial vehicles (UAVs) or drones are increasingly being adopted to gather data, at previously unprecedented spatial and temporal resolutions in diverse geographic locations. This easily accessible, low-cost tool is improving existing research methods and enabling novel approaches in marine turtle ecology and conservation. Here we review the diverse ways in which incorporating inexpensive UAVs may reduce costs and field time while improving safety and data quality and quantity over existing methods for studies on turtle nesting, at-sea distribution and behaviour surveys, as well as expanding into new avenues such as surveillance against illegal take. Furthermore, we highlight the impact that high-quality aerial imagery captured by UAVs can have for public outreach and engagement. This technology does not come without challenges. We discuss the potential constraints of these systems within the ethical and legal frameworks which researchers must operate and the difficulties that can result with regard to storage and analysis of large amounts of imagery. We then suggest areas where technological development could further expand the utility of UAVs as data-gathering tools; for example, functioning as downloading nodes for data collected by sensors placed on turtles. Development of methods for the use of UAVs in sea turtle research will serve as case studies for use with other marine and terrestrial taxa
