Maximus of Tyre on the Zeus of Homer and Plato
The present article discusses the ingenious account of Zeus that was put forward by Maximus of Tyre in his Orations IV and XXVI. When reading into Homer various Platonic and Stoic concepts, Maximus originally amalgamates the notion of Demiurge with that of Providence. As he thus unearths Homer’s latent theology, Maximus not only praises the heritage of Greek culture but also demonstrates the close affinity between poetry and philosophy
Alegoryczna interpretacja historii Abrahama. Analiza III, IV i VI Homilii o Księdze Rodzaju Orygenesa
Origen was one of the most important thinkers of early Christianity. In his works on the Old and New Testament, he frequently employed allegorical interpretation. The purpose of this article is to analyze Origen’s Homilies III, IV and VI on Genesis. Allegorical interpretation is for Origen both a hermeneutical and a philosophical problem. Origen’s manner of interpreting Abraham’s story is compared here with the works of other Judeo-Hellenistic and Christian authors, Philo in particular. Origen’s account of Abraham’s story aims to defend Christianity against the charges raised by both pagans and Jews.Orygenes był jednym z najważniejszych myślicieli wczesnego chrześcijaństwa. W swoich pismach odnoszących się do Starego i Nowego Testamentu bardzo często korzystał z interpretacji alegorycznej. Celem artykułu jest analiza III, IV i VI Homilii o Księdze Rodzaju Orygenesa. Interpretacja alegoryczna jest dla Orygenesa problemem zarówno hermeneutycznym jak i filozoficznym. Sposób interpretowania historii Abrahama przez Orygenesa został tu porównany z utworami innych autorów judeohellenistycznych i chrześcijańskich, głównie Filona. Proponowane przez Orygenesa ujęcie historii Abrahama przez Orygenesa ma na celu obronę chrześcijaństwa przed zarzutami wystosowanymi przez pogan i żydów
Renal denervation in patients with symptomatic chronic heart failure despite resynchronization therapy : a pilot study
Introduction: Renal denervation (RD) has been shown to decrease sympathetic function in patients with hypertension. Its efficacy in symptomatic chronic heart failure (CHF) patients not responding to cardiac resynchronization therapy (CRT) has not been evaluated. Aim: To assess whether a less invasive treatment method – renal denervation – is safe in symptomatic heart failure patients despite optimal medical treatment and resynchronization therapy and whether it is associated with an improvement in clinical status, exercise capacity and hemodynamic parameters. Material and methods: The study was an open-label, randomized, controlled clinical trial. Patients were divided into an intervention (RD) and a control group. Clinical data collection, blood pressure (BP) measurements, echocardiography, 6-minute walk test (6MWT) and laboratory tests were performed before, 6 and 12 months after RD. The patients were followed-up to 24 months. Results: We included 20 patients aged 52.0 to 86.0 years (median age: 71.5 years), 15 males and 5 females with median left ventricular ejection fraction (LVEF) of 32.5%, body mass index 31.3 kg/m2. Renal denervation was safe, no significant adverse effects were registered. There were no significant differences in LVEF, BP, 6MWT and N-terminal prohormone of brain natriuretic peptide (NT-proBNP) concentration 6 and 12 months after RD or control. Conclusions: Our results indicate that RD in CHF patients not responding to CRT is safe and does not worsen exercise capacity and hemodynamic parameters

Fulminant heart failure due to giant cell myocarditis affecting the left ventricle
A 56-year-old woman, previously healthy, was hospitalized after an episode of ventricular tachycardia in the course of infection. In view of the fulminant course of heart failure the patient was connected to an extracorporeal membrane oxygenation (ECMO) system. After 3 weeks of treatment with ECMO the patient received a heart transplant. A histopathological examination of the tissues of the explanted heart revealed giant cell myocarditis. The patient was treated with immunosuppression based on induction therapy followed by a standard regimen with steroids. Currently, the patient remains in good general condition with an left ventricular ejection fraction of 60%
Depressed systemic arterial compliance is associated with the severity of heart failure symptoms in moderate-to-severe aortic stenosis : a cross-sectional retrospective study
Background: Patients with aortic stenosis (AS) may develop heart failure even in the absence of severe valve stenosis. Our aim was to assess the contribution of systemic arterial properties and the global left ventricular afterload to graded heart failure symptoms in AS. Methods: We retrospectively reviewed medical records of 157 consecutive subjects (mean age, 71±10 years; 79 women and 78 men) hospitalized owing to moderate-to-severe degenerative AS. Exclusion criteria included more than mild aortic insufficiency or disease of another valve, atrial fibrillation, coronary artery disease, severe respiratory disease or anemia. Heart failure symptoms were graded by NYHA class at admission. Systemic arterial compliance (SAC) and valvulo-arterial impedance (Zva) were derived from routine echocardiography and blood pressure. Results: Sixty-one patients were asymptomatic, 49 presented mild (NYHA II) and 47 moderate-to-severe (NYHA III-IV) heart failure symptoms. Mild symptoms were associated with lower SAC and transvalvular gradients, while more severe exercise intolerance coincided with older age, lower systolic blood pressure, smaller aortic valve area and depressed ejection fraction. By multiple ordinal logistic regression, the severity of heart failure symptoms was related to older age, depressed ejection fraction and lower SAC. Each decrease in SAC by 0.1 ml/m² per mmHg was associated with an increased adjusted odds ratio (OR) of a patient being in one higher category of heart failure symptoms graded as no symptoms, mild exercise intolerance and advanced exercise intolerance (OR: 1.16 [95% CI, 1.01-1.35], P=0.045). Conclusions: Depressed SAC may enhance exercise intolerance irrespective of stenosis severity or left ventricular systolic function in moderate-to-severe AS. This finding supports the importance of non-valvular factors for symptomatic status in AS
Wpływ leków wazoaktywnych na potencjał antyoksydacyjny i funkcje śródbłonka u chorych z przewlekłym niedokrwieniem kończyn dolnych w stadium IIb według klasyfikacji Fontaine’a
Background. Atherosclerotic lower limb ischemia is the most important cause of intermittent claudication.
Patients with intermittent claudication are at relatively low risk of limb loss. Therefore the predominant
therapeutic approach in this group of patients is conservative treatment. The objective of this study was
a comparison of bencyclane versus pentoxifilline effects on the clinical picture (pain free walking distance
and maximum walking distance) and selected parameters of oxidative stress including blood level of nitric
oxide (NO) and total antioxidant status of the plasma in patients with chronic lower limb ischaemia
(Fontaine IIb).
Material and methods. Adult patients (60 subjects) with claudication distance shorter than 200 m (Fontaine
II b) were divided into three groups: I. 20 patients administered bencyclane 200 mg i.v. for 7 days followed by
2 × 200 mg orally for 21 days; II. 20 patients administered pentoxifilline 300 mg i.v. for 7 days followed by
2 × 600 mg orally for 21 days; III. 20 patients administered placebo i.v. for 7 days followed by 2 × 1 tablet orally
for 21 days. In all patients walking distance until first pain, maximum walking distance and ankle brachial index
(ABI) were measured. In all patients blood samples were taken on days 0, 7, 14 and 28 of the treatment and
blood levels of nitric oxide (NO) and plasma total antioxidant status (TAS) were measured.
Results. Statistically significant increase in pain-free walking distance i.e. 127 ± 37.95 m in patients administered
bencyclane (p < 0.001) and 125.29 ± 41.10 m in patients administered pentoxifilline (p < 0.001)
was observed. Total antioxidant status (TAS) of the plasma and blood levels of nitric oxide also increased in
patients treated with bencyclane and pentoxifilline (p < 0.05). Conclusions. The administration of bencyclane and pentoxifilline is an effective therapeutic approach for
patients with Fontaine II b chronic limb ischaemia. No significant differences were found in efficacy of the two
active treatments in the study.Wstęp. Miażdżycowe niedokrwienie kończyn dolnych jest najważniejszą przyczyną wywołującą chromanie przestankowe.
Chorzy z chromaniem przestankowym są obarczeni stosunkowo niewielkim ryzykiem utraty kończyny. Z tego
powodu dominującym sposobem leczenia tej grupy chorych jest terapia zachowawcza.
Celem pracy było porównanie wpływu bencyklanu i pentoksyfiliny na obraz kliniczny (dystans przejścia bezbólowego
i maksymalnego) i na wybrane parametry stresu oksydacyjnego, czyli stężenie tlenku azotu (NO) we krwi i całkowity
potencjał antyoksydacyjny osocza pacjentów z przewlekłym niedokrwieniem kończyn dolnych (stadium IIb).
Materiał i metody. Badaną grupę stanowili dorośli pacjenci (60 osób) z chromaniem przestankowym poniżej 200 m
(stadium IIb niedokrwienia wg klasyfikacji Fontaine’a) podzieleni na 3 grupy: grupa I - 20 chorych otrzymujących
bencyklan w dawce 200 mg i.v. przez 7 dni i 2 × 200 mg p.o. przez 21 dni; grupa II
- 20 chorych, otrzymujących pentoksyfilinę w dawce 300 mg i.v. przez 7 dni i 2 × 600 mg p.o. przez 21 dni; grupa
III - 20 chorych otrzymujących placebo i.v. przez 7 dni i 2 × 1 tabl. p.o. przez 21 dni.
U wszystkich pacjentów przeprowadzono: badanie dystansu przejścia do pierwszego bólu (PFWD), maksymalnego
(MWD) i zmierzono wskaźnik kostka-ramię (ABI). Od badanych pobrano krew w: 0., 7., 14. i 28. dniu podawania
leków i oznaczono w surowicy krwi stężenie tlenku azotu (NO) i całkowity potencjał antyoksydacyjny osocza (TAS).
Wyniki. Stwierdzono znamienne statystycznie zwiększenie dystansu przejścia bezbólowego, czyli 127,00 ± 37,95 m w grupie chorych przyjmujących bencyklan (p < 0,001) i 125,29 ± 41,10 m u chorych
otrzymujących pentoksyfilinę (p < 0,001). W grupie pacjentów stosujących bencyklan i pentoksyfilinę wzrastała
wartość całkowitego potencjału antyoksydacyjnego osocza i stężenie tlenku azotu (p < 0,05).
Wnioski. Zastosowanie bencyklanu i pentoksyfiliny jest skuteczną formą terapii u chorych z przewlekłym
niedokrwieniem kończyn w klasie IIb według klasyfikacji Fontaine’a. Nie stwierdzono istotnych różnic w skuteczności
działania obu badanych preparatów
