    Enclavijado de Ender en las fracturas del macizo trocantéreo: análisis retrospectivo de la ocupación del canal medular

    Se revisaron las historias clínicas de 226 pacientes afectos de fracturas del macizo trocantérico, tratadas mediante enclavijado de Ender en un periodo de 9 años en el Hospital del Insalud de Barbastro. La estabilidad de las fracturas se determinó mediante la clasificación de Ramadier. Se valoró el porcentaje de ocupación de la cavidad medular femoral por los clavos de Ender y la posible relación de este parámetro con la migración del material de osteosíntesis. Se evaluó también la utilidad del bloqueo distal de los clavos de Ender con agujas de Kirschner, en la prevención de su migración distal. Se comprobó una relación significativa entre el porcentaje de ocupación del canal medular y la migración distal del material en las fracturas inestables. El bloqueo distal con aguja de Kirschner no se acompañó de una menor incidencia en la migración distal de los clavos.A retrospective study was done on 226 trochanteric fractures managed with Ender nails during a 9 years period at the Insalud Hospital of Barbastro. Stability of these fractures was classified following Ramadier's criteria. Filling of the medular cavity by Ender nails and its relation with osteosyntehsis migration was analysed. Distal locking of Ender nails by Kirschner pins was evaluated to prevent its distal mobilization. A significative relationship was observed between a high filling rate of the medular cavity and lower distal migration rate in unestable trochanteric fractures.     The evaluation of surface diffusion coefficients of gold and platinum atoms at electrochemical interfaces from combined STM-SEM imaging and electrochemical techniques

    A simple method is presented for measuring the surface diffusion coefficients of Au and Pt atoms at electrodispersed electrodes of the same metals in contact with 0.5M H2SO4. The technique is based upon the time dependence of the surface roughness factor of electrodispersed metal overlayers. The method requires a model for the surface roughness of the metal structure. The model is deduced from microscopic measurements by a STM integrated into a conventional SEM microscope. This allows the relationship between the roughness factor and the area of the surface structure to be obtained.     Cyprus is established as a summer destination. To aid the destination in developing its winter season as well, this research uses a qualitative inductive approach to explore the tourists’ current image of the island and their motivations of visiting it. The research indicates that the current image, which essentially portrays Cyprus as a sun-and-sea destination is thought to dissuade tourists from perceiving the island as a year-round destination. Nonetheless, increasing the pull factors of the destination through the development of unique special interest products can help in extending the tourism season as well as broaden its narrow image

    New speleothem data from Molinos and Ejulve caves reveal Holocene hydrological variability in northeast Iberia

    New speleothem records from northeastern Iberian caves provide data to explore the climatic patterns during the Holocene. We present delta C-13 and Mg/Ca from three speleothems from two different caves located in the Iberian Range allowing replication of the climatic signal for several millennia. Through the integration of those stalagmites covering since the Holocene onset to 2 ka, the early Holocene (11.7-8.5 ka) appears as the wettest interval. A marked change towards aridity is observed during the middle Holocene (8.5-4.8 ka) and an increase of humidity afterwards (4.8-2 ka). This three-part pattern, contrasting with other Iberian sequences, seems to be associated with the different role that seasonality has played in the response of different proxies (or records) to changes in water availability. Interpreting our speleothem records as changes in winter-spring precipitation along the Holocene allows reconciling previous data on hydrological variability from the western Mediterranean borderlands

    Health effects of the Federal Bureau of Prisons tobacco ban

    BACKGROUND: Tobacco smoking remains the leading cause of preventable death in America, claiming 450,000 lives annually. Chronic Obstructive Pulmonary Disease, caused by smoking in the vast majority of cases, became the third leading cause of death in the U.S. in 2008. The burden of asthma, often exacerbated by tobacco exposure, has widespread clinical and public health impact. Despite this considerable harm, we know relatively little about the natural history of lung disease and respiratory impairment in adults, especially after smoking cessation. METHODS/DESIGN: Our paper describes the design and rationale for using the 2004 Federal Bureau of Prisons tobacco ban to obtain insights into the natural history of respiratory diseases in adult men and women of different races/ethnicities who are imprisoned in federal medical facilities. We have developed a longitudinal study of new prison arrivals, with data to be collected from each participant over the course of several years, through the use of standardized questionnaires, medical chart reviews, lung function tests, six-minute walk tests, and stored serum for the analysis of present and future biomarkers. Our endpoints include illness exacerbations, medication and health services utilization, lung function, serum biomarkers, and participants\u27 experience with their health and nicotine addiction. DISCUSSION: We believe the proposed longitudinal study will make a substantial contribution to the understanding and treatment of respiratory disease and tobacco addiction

    Measurement of the beam-helicity asymmetry II^{\odot} in the photoproduction of π0π±\pi^0\pi^{\pm}-pairs off protons and off neutrons

    Beam-helicity asymmetries have been measured at the MAMI accelerator in Mainz for the photoproduction of mixed-charge pion pairs in the reactions γpnπ0π+\boldsymbol{\gamma}p\rightarrow n\pi^0\pi^+ off free protons and γd(p)pπ0π\boldsymbol{\gamma}d\rightarrow (p)p\pi^0\pi^- and γd(n)nπ0π+\boldsymbol{\gamma}d\rightarrow (n)n\pi^0\pi^+ off quasi-free nucleons bound in the deuteron for incident photon energies up to 1.4 GeV. Circularly polarized photons were produced from bremsstrahlung of longitudinally polarized electrons and tagged with the Glasgow-Mainz magnetic spectrometer. The charged pions, recoil protons, recoil neutrons, and decay photons from π0\pi^0 mesons were detected in the 4π\pi electromagnetic calorimeter composed of the Crystal Ball and TAPS detectors. Using a complete kinematic reconstruction of the final state, excellent agreement was found between the results for free and quasi-free protons, suggesting that the quasi-free neutron results are also a close approximation of the free-neutron asymmetries. A comparison of the results to the predictions of the Two-Pion-MAID reaction model shows that the reaction mechanisms are still not well understood, in particular at low incident photon energies in the second nucleon-resonance region.Comment: accepted for publication in Eur. phys.
