Classical information deficit and monotonicity on local operations
We investigate classical information deficit: a candidate for measure of
classical correlations emerging from thermodynamical approach initiated in
[Phys. Rev. Lett 89, 180402]. It is defined as a difference between amount of
information that can be concentrated by use of LOCC and the information
contained in subsystems. We show nonintuitive fact, that one way version of
this quantity can increase under local operation, hence it does not possess
property required for a good measure of classical correlations. Recently it was
shown by Igor Devetak, that regularised version of this quantity is monotonic
under LO. In this context, our result implies that regularization plays a role
of "monotoniser".Comment: 6 pages, revte
Population pharmacokinetic modelling of intravenous paracetamol in fit older people displays extensive unexplained variability
Aims Paracetamol is the analgesic most used by older people. The physiological changes occurring with ageing influence the pharmacokinetics (PK) of paracetamol and its variability. We performed a population PK-analysis to describe the PK of intravenous (IV) paracetamol in fit older people. Simulations were performed to illustrate target attainment and variability of paracetamol exposure following current dosing regimens (1000 mg every 6 h, every 8 h) using steady-state concentration (Css-mean) of 10 mg l(-1) as target for effective analgesia. Methods A population PK-analysis, using NONMEM 7.2, was performed based on 601 concentrations of paracetamol from 30 fit older people (median age 77.3 years, range [61.8-88.5], body weight 79 kg [60-107]). All had received an IV paracetamol dose of 1000 mg (over 15 min) after elective knee surgery. Results A two-compartment PK-model best described the data. Volume of distribution of paracetamol increased exponentially with body weight. Clearance was not influenced by any covariate. Simulations of the standardized dosing regimens resulted in a C-ss of 9.2 mg l(-1) and 7.2 mg l(-1), for every 6 h and every 8 h respectively. Variability in paracetamol PK resulted in C-ss above 5.4 and 4.1 mg l(-1), respectively, in 90% of the population and above 15.5 and 11.7, respectively, in 10% at these dosing regimens. Conclusions The target concentration was achieved in the average patient with 1000 mg every 6 h, while every 8 h resulted in underdosing for the majority of the population. Furthermore, due to a large (unexplained) interindividual variability in paracetamol PK a relevant proportion of the fit older people remained either under- or over exposed.Peer reviewe
Neutrino tomography - Learning about the Earth's interior using the propagation of neutrinos
Because the propagation of neutrinos is affected by the presence of Earth
matter, it opens new possibilities to probe the Earth's interior. Different
approaches range from techniques based upon the interaction of high energy
(above TeV) neutrinos with Earth matter, to methods using the MSW effect on the
neutrino oscillations of low energy (MeV to GeV) neutrinos. In principle,
neutrinos from many different sources (sun, atmosphere, supernovae, beams etc.)
can be used. In this talk, we summarize and compare different approaches with
an emphasis on more recent developments. In addition, we point out other
geophysical aspects relevant for neutrino oscillations.Comment: 22 pages, 9 figures. Proceedings of ``Neutrino sciences 2005:

New developments in adult congenital heart disease
Contains fulltext :
225482.pdf (Publisherâs version ) (Open Access)Congenital heart disease (CHD) affects 0.8% of live births and over the past decades technical improvements and large-scale repair has led to increased survival into adulthood of over 95% of the new-born. A new group of patients, those who survived their congenital heart defect, has emerged but late complications including heart failure, pulmonary hypertension (PH), arrhythmias, aneurysms and endocarditis appeared numerous, with a huge impact on mortality and morbidity. However, innovations over the past years have changed the landscape of adult CHD dramatically. In the diagnostic process important improvements have been made in the use of MRI, biomarkers, eâhealth concepts and 3D visualisation of anatomy. Care is now concentrated in specialised centres, with a continuous emphasis on education and the introduction of weekly multidisciplinary consultations on diagnosis and intervention. Surgery and percutaneous intervention have been refined and new concepts applied, further reducing the burden of the congenital malformations. Research has matured from case series to global networks. Currently, adults with CHD are still facing high risks of early mortality and morbidity. By global collaboration and continuous education and development and innovation of our diagnostic and therapeutic arsenal, we will improve the perspectives of these young patients

Stellar Pollution in the Solar Neighborhood
We study spectroscopically determined iron abundances of 642 solar-type stars
to search for the signature of accreted iron-rich material. We find that the
metallicity [Fe/H] of a subset of 466 main sequence stars, when plotted as a
function of stellar mass, mimics the pattern seen in lithium abundances in open
clusters. Using Monte Carlo models we find that, on average, these stars have
accreted about 0.4 Earth masses of iron while on the main sequence. A much
smaller sample of 19 stars in the Hertzsprung gap, which are slightly evolved
and whose convection zones are significantly more massive, have lower average
[Fe/H], and their metallicity shows no clear variation with stellar mass. These
findings suggest that terrestrial-type material is common around solar type
stars.Comment: 33 pages, 11 figures. Submitted to Ap
Dynamics of gelling liquids: a short survey
The dynamics of randomly crosslinked liquids is addressed via a Rouse- and a
Zimm-type model with crosslink statistics taken either from bond percolation or
Erdoes-Renyi random graphs. While the Rouse-type model isolates the effects of
the random connectivity on the dynamics of molecular clusters, the Zimm-type
model also accounts for hydrodynamic interactions on a preaveraged level. The
incoherent intermediate scattering function is computed in thermal equilibrium,
its critical behaviour near the sol-gel transition is analysed and related to
the scaling of cluster diffusion constants at the critical point. Second,
non-equilibrium dynamics is studied by looking at stress relaxation in a simple
shear flow. Anomalous stress relaxation and critical rheological properties are
derived. Some of the results contradict long-standing scaling arguments, which
are shown to be flawed by inconsistencies.Comment: 21 pages, 3 figures; Dedicated to Lothar Schaefer on the occasion of

Low Thrust Propelled Close Approach Maneuvers
The study of orbital maneuvers in space missions is a very important problem in astrodynamics. One of the options is the use of a âgravity assistedâ maneuver, which is a technique where a spacecraft passes close to a celestial body and uses the gravity of this body to change its trajectory. This approach trajectory has symmetry with respect to the periapsis line when observed from a reference frame fixed in the approached body. There is also a more complex maneuver, when the passage by the celestial body is combined with the application of propulsion, either to give extra energy to the spacecraft or to help to satisfy other constraints required by the mission, like passing by a giving point or region in space. The main object of this study is to measure the efficiency of the application of a continuous thrust combined with the âgravity assistedâ maneuver. The effect of this combination is analyzed using maps that give the energy variation of the spacecraft as function of the parameters related to the maneuver. This analysis is made from the point of view of the variation of energy of the spacecraft with respect to the main body of the system. The continuous thrust is applied in the different regions of the trajectory to evaluate the effects of the locations of the thrusting arcs in the variations of energy. The effects of the variations of the direction of the thrust are also studied. This type of combined maneuver may be used to give extra energy to the spacecraft and keeping control of the trajectory during the close approach to better position the spacecraft to complete the mission
Efficacy of a loading dose of IV salbutamol in children with severe acute asthma admitted to a PICU:a randomized controlled trial
The optimal dose regimen for intravenous (IV) treatment in children with severe acute asthma (SAA) is still a matter of debate. We assessed the efficacy of adding a salbutamol loading dose to continuous infusion with salbutamol in children admitted to a pediatric intensive care unit (PICU) with SAA. This multicentre, placebo-controlled randomized trial in the PICUs of four tertiary care childrenâs hospitals included children (2â18 years) with SAA admitted between 2017 and 2019. Children were randomized to receive either a loading dose IV salbutamol (15 mcg/kg, max. 750 mcg) or normal saline while on continuous salbutamol infusion. The primary outcome was the asthma score (Qureshi) 1 h after the intervention. Analysis of covariance models was used to evaluate sensitivity to change in asthma scores. Serum concentrations of salbutamol were obtained. Fifty-eight children were included (29 in the intervention group). Median baseline asthma score was 12 (IQR 10â13) in the intervention group and 11 (9â12) in the control group (p = 0.032). The asthma score 1 h after the intervention did not differ significantly between the groups (p = 0.508, β-coefficient = 0.283). The median increase in salbutamol plasma levels 10 min after the intervention was 13 Οg/L (IQR 5â24) in the intervention group and 4 Οg/L (IQR 0â7) in the control group (p = 0.001). Side effects were comparable between both groups. Conclusion: We found no clinical benefit of adding a loading dose IV salbutamol to continuous infusion of salbutamol, in children admitted to the PICU with SAA. Clinically significant side effects from the loading dose were not encountered. What is Known:â˘Â Pediatric asthma guidelines struggle with an evidence-based approach for the treatment of SAA beyond the initial steps of oxygen suppletion, repetitive administration of inhaled β2-agonists, and systemic steroids.⢠During an SAA episode, effective delivery of inhaled drugs is unpredictable due to severe airway obstruction. What is New:â˘Â This study found no beneficial effect of an additional loading dose IV salbutamol in children admitted to the PICU.⢠This study found no clinically significant side effects from the loading dose
