An in-line coaxial-to-waveguide transition for q-band single-feed-per-beam antenna systems
An in-line transition between a coaxial cable and rectangular waveguide operating in Q-band (33â50 GHz) is presented. The aim of the work is to minimize the modifications in the waveguide to the strictly necessary to overcome the manufacturing issues due to the high frequencies involved. In addition, the transition is compact and it does not increase the space occupation on the transverse section, this suggests its application in horn antennas clusters arrangement. The operating principle consists of both a modal conversion and an impedance matching between the devices. The modal conversion is realized in an intermediate region, where the coaxial penetrates in the waveguide: the device geometry is designed so that the electric field in the transition is a trade-off between the TEM mode of the coaxial and the TE10 of the guide. A shaped waveguide backshort and a reactive air gap in the coaxial cable co-participate to achieve the matching. An optimized Chebyshev stepped transformer completes the transition to fulfil the impedance mismatch with the full waveguide. The design issues and technological aspects are considered. The influences of the feeding pin misalignment, the presence of groove is included in the analysis and these practical aspects are discussed and numerically validated via the scattering parameters analysis of the proposed design. The return loss is higher than 25 dB over the whole Q-band
A sharp incisor tool for predator house mice back to the wild
The house mouse (Mus musculus domesticus), as a successful invasive species worldwide, has to forage a variety of resources. Subantarctic mice display among the most notable diet shift from the usual omnivorousâgranivorous diet, relying on a larger proportion of terrestrial animal prey. In agreement, a recent study of their mandible morphology evidenced an evolution of their mandible shape to optimize incisor biting and hence seize preys. Here, the incisors themselves are the focus of a morphometric analysis combined with a 3D study of their internal structure, aiming at a comparison between subantarctic populations (Guillou island, Kerguelen archipelago) with a range of western European continental, commensal mice. The predatory foraging behavior of Guillou mice was indeed associated with a sharper bevel of the lower incisor, which appears as an efficient morphology for piercing prey. The incisor of these mice also displays a reduced pulp cavity, suggesting slower eruption counterbalancing a reduced abrasion on such soft food material. The dynamics of the everâgrowing incisor may thus allow adaptive incisor sculpting and participate to the success of mice in foraging diverse resources
Migraine and gastrointestinal disorders in middle and old age: A UK Biobank study
Introduction: Migraine is a prevalent condition causing a substantial level of disability worldwide. Despite this, the pathophysiological mechanisms are not fully understood. Migraine often co-occurs with gastrointestinal disorders, but the direction of a potential causal link is unclear. The aim of this project was to investigate the associations between migraine and several gastrointestinal disorders in the same cohort in order to determine the relative strengths of these associations. Methods: This cross-sectional study examined whether migraine is associated with irritable bowel syndrome (IBS), peptic ulcers, Helicobacter pylori (HP) infections, celiac disease, Crohn's disease and ulcerative colitis. Baseline data covering 489,753 UK Biobank participants (migraine group: n = 14,180) were analyzed using Pearson's chi-square tests and adjusted binary logistic regression models. Results: Migraine was significantly associated with IBS (odds ratio [OR] 2.24, 95% confidence interval [CI] 2.08â2.40, p <.001) and peptic ulcers (OR 1.55, 95% CI 1.35â1.77, p <.001). Migraine was not associated with HP infection (OR 1.34, 95% CI 1.04â1.73, p =.024), celiac disease (OR 1.29, 95% CI 1.04â1.60, p =.023), Crohn's disease (OR 1.08, 95% CI 0.80â1.45, p =.617) or ulcerative colitis (OR 1.00, 95% CI 0.79â1.27, p =.979) after adjusting for multiple testing. Conclusions: Migraine was associated with IBS and peptic ulcers in this large population-based cohort. The associations with HP infection, celiac disease, Crohn's disease, and ulcerative colitis did not reach significance, suggesting a weaker link between migraine and autoimmune gastrointestinal conditions or HP infection

In recent years, several studies have shed light on the pathogenesis of many novel immune defects associated with narrow susceptibility to bacterial, viral and fungal infections. Genetically determined forms of susceptibility to Candida species infection, which had been elusive for a long time, have been recognized. Refractory or recurrent infections of skin, nails and the mucous membranes are clinical signs of chronic mucocutaneous candidiasis, frequently associated with immunological defects in the Th17 subset, which is crucial for host response against infections at mucosal and epithelial sites. Unravelling the functional and technological potential of soy milk based microencapsulated Lactobacillus crispatus and Lactobacillus gasseri
The main aim of this study was to evaluate the potential of microencapsulation with soy milk as a carrier, conducted by spray-drying, on three mixed cultures of functional vaginal lactobacilli, Lactobacillus crispatus (BC1, BC3, and BC4) and Lactobacillus gasseri BC9, that could be further exploited as an adjunct in health foods. The i) physicochemical properties (using scanning electronic microscopy), ii) viability of encapsulated bacteria over time, and iii) functional features of powders containing the encapsulated bacteria were characterised considering different storage conditions. All microbial mixtures exhibited high viability for 90 d of storage, regardless of the storage conditions. All microencapsulated microbial combinations, excluding L. crispatus BC3 + L. gasseri BC9, exhibited hydrophobicity values exceeding 60%. Moreover, regardless of the considered combination, the in vitro digestion results exhibited a more significant decrease in cell viability for non-encapsulated microbial combinations than that of the encapsulated strains
WSN hardware for automotive applications: Preliminary results for the case of public transportation
The ubiquitous nature and great potential of Wireless Sensors Network has not yet been fully exploited in automotive applications. This work deals with the choice of the cost-effective hardware required to face the challenges and issues proposed by the new trend in the development of intelligent transportation systems. With this aim, a preliminary WSN architecture is proposed. Several commercially available open-source platforms are compared and the Raspberry Pi stood out as a suitable and viable solution. The sensing layer is designed with two goals. Firstly, accelerometric, temperature, and relative humidity sensors were integrated on a dedicated PCB to test if mechanical or environmental stresses during bus rides could be harmful to the device or to its performances. The physical quantities are monitored automatically to alert the driver, thus improving the quality of service. Then, the rationale and functioning of the management and service layer is presented. The proposed cost-effective WSN node was employed and tested to transmit messages and videos, while investigating if any quantitative relationship exists between these operations and the environmental and operative conditions experienced by the hardware
Translation, cross-cultural adaptation, and validation of the Italian version of the anterior cruciate ligamentâreturn to sport after injury (ACL-RSI) scale and its integration into the K-STARTS test
Background: The timing of a return to sport (RTS) after anterior cruciate ligament reconstruction (ACLR) represents a major subject of debate in sports medicine practice. Recently, the Knee Santy Athletic Return to Sport (KâSTARTS) composite test was validated. This consists of a battery of physical tests and a psychological evaluation using the anterior cruciate ligamentâreturn to sport after injury scale (ACLâRSI). This study aimed to translate the ACLâRSI and KâSTARTS from English to Italian and determine the scaleâs reliability and validity in an Italian context.
Methods: The translation and cultural adaptation process was performed according to the guidelines for the crossâ cultural adaptation of selfâreport measures. The patients were asked to fill an anonymized online form created for this purpose that included the KOOS, the Lysholm, the IKDCâSKF, and the Italian translation of the ACLâRSI (ACLâRSIâIt). After 1 week, the attendees were asked to repeat the ACLâRSIâIt to investigate the testâretest reliability.
Results: The final study population comprised 115 patients who underwent ACLR, with a mean followâup of 37.37 ± 26.56 months. The ACLâRSIâIt showed axcellent internal consistency (Cronbachâs α = 0.963), reliability (testâ retest ICC = 0.966), and good construct validity (positive correlations with the other scales were above 75%).
Conclusions: The ACLâRSIâIt is valid, reliable, and comparable to the original English version of the questionnaire for Italianâspeaking patients. It can be used to assess the psychological readiness of patients for a RTS after primary and unilateral ACLR, and can be integrated into the Italian KâSTARTS test
A pharmacogenetic risk score for the evaluation of major depression severity under treatment with antidepressants
The severity of symptoms as well as efficacy of antidepressants in major depressive disorder (MDD) is modified by single nucleotide polymorphisms (SNPs) in different genes, which may contribute in an additive or synergistic fashion. We aimed to investigate depression severity in participants with MDD under treatment with antidepressants in relation to the combinatory effect of selected genetic variants combined using a genetic risk score (GRS). The sample included 150 MDD patients on regular AD therapy from the population-based Swiss PsyCoLaus cohort. We investigated 44 SNPs previously associated with antidepressant response by ranking them with regard to their association to the Center for Epidemiologic Studies Short Depression Scale (CES-D) score using random forest. The three top scoring SNPs (rs12248560, rs878567, rs17710780) were subsequently combined into an unweighted GRS, which was included in linear and logistic regression models using the CES-D score, occurrence of a major depressive episode (MDE) during follow-up and regular antidepressant treatment during the 6 months preceding follow-up assessment as outcomes. The GRS was associated with MDE occurrence (p =.02) and ln CES-D score (p =.001). The HTR1A rs878567 variant was associated with ln CES-D after adjustment for demographic and clinical variables [p =.02, lower scores for minor allele (G) carriers]. Additionally, rs12248560 (CYP2C19) CC homozygotes showed a six-fold higher likelihood of regular AD therapy at follow-up compared to minor allele homozygotes [TT; ultrarapid metabolizers (p =.03)]. Our study suggests that the cumulative consideration of pharmacogenetic risk variants more reliably reflects the impact of the genetic background on depression severity than individual SNPs
A wireless sensors network for monitoring the Carasau bread manufacturing process
This work copes with the design and implementation of a wireless sensors network architecture to automatically and continuously monitor, for the first time, the manufacturing process of Sardinian Carasau bread. The case of a traditional bakery company facing the challenge of the Food-Industry 4.0 competitiveness is investigated. The process was analyzed to identify the most relevant variables to be monitored during the product manufacturing. Then, a heterogeneous, multi-tier wireless sensors network was designed and realized to allow the real-time control and the data collection during the critical steps of dough production, sheeting, cutting and leavening. Commercial on-the-shelf and cost-effective integrated electronics were employed, making the proposed approach of interest for many practical cases. Finally, a user-friendly interface was provided to enhance the understanding, control and to favor the process monitoring. With the wireless senors network (WSN) we designed, it is possible to monitor environmental parameters (temperature, relative humidity, gas concentrations); cinematic quantities of the belts; and, through a dedicated image processing system, the morphological characteristics of the bread before the baking. The functioning of the WSN was demonstrated and a statistical analysis was performed on the variables monitored during different seasons
Costâutility analysis of pharmacogenetic testing based on CYP2C19 or CYP2D6 in major depressive disorder: assessing the drivers of different cost-effectiveness levels from an Italian societal perspective
Background and Objectives Major depressive disorder (MDD) is a common and severe psychiatric disorder that has enor- mous economical and societal costs. As pharmacogenetics is one of the key tools of precision psychiatry, we analyze the costâutility of test screening of CYP2C19 and CYP2D6 for patients suffering from major depressive disorder (MDD) and try to understand the main drivers that influence the costâutility.
Methods We developed two pharmacoeconomic nonhomogeneous Markov models to test the costâutility, from an Ital- ian societal perspective, of pharmacogenetic testing genetic to characterize the metabolizing profiles of cytochrome P450 (CYP) 2C19 and CYP2D6 in a hypothetical case study of patients suffering from major depressive disorder (MDD). The model considers different scenarios of adjustment of antidepressant treatment according to the patientâs metabolizing profile or treatment over a period of 18 weeks. The uncertainty of model parameters is tested through both a probabilistic sensitivity analysis and a one-way deterministic sensitivity analysis, and these results are used in a post-hoc analysis to understand the main drivers of three alternative cost-effectiveness levels (âpoor,â âstandard,â and âhighâ). These drivers are first evaluated from an exploratory multidimensional perspective and next from a predictive perspective as the probability that a patient belongs to a specific cost-effectiveness level is estimated on the basis of a restricted set of parameters used in the original pharmacoeconomic model.
Results The models for CYP2C19 and CYP2D6 indicate that screening has an incremental cost-effectiveness ratio of 60,000⏠and 47,000⏠per quality-adjusted life year (QALY), respectively. The probabilistic sensitivity analysis shows that the treat- ments are cost-effective for a 75,000⏠willingness to pay (WTP) threshold in 58% and 63% of the Monte Carlo replications, respectively. The post-hoc analysis highlights the factors that allow us to clearly discriminates poor cost-effectiveness from high cost-effectiveness scenarios and demonstrates that it is possible to predict with reasonable accuracy the cost-effectiveness of a genetic test and the associated therapeutic pattern.
Conclusions Our findings suggest that screenings for both CYP2C19 and CYP2D6 enzymes for patients with MDD are cost-effective for a WTP threshold of 75,000⏠per QALY, and provide relevant suggestions about the most important aspects to be further explored in clinical studies aimed at addressing the cost-effectiveness of genetic testing for patients diagnosed with MDD
