Ambient Temperature and Biomarkers of Heart Failure: A Repeated Measures Analysis
Background: Extreme temperatures have been associated with hospitalization and death among individuals with heart failure, but few studies have explored the underlying mechanisms. Objectives: We hypothesized that outdoor temperature in the Boston, Massachusetts, area (1- to 4-day moving averages) would be associated with higher levels of biomarkers of inflammation and myocyte injury in a repeated-measures study of individuals with stable heart failure. Methods: We analyzed data from a completed clinical trial that randomized 100 patients to 12 weeks of tai chi classes or to time-matched education control. B-type natriuretic peptide (BNP), C-reactive protein (CRP), and tumor necrosis factor (TNF) were measured at baseline, 6 weeks, and 12 weeks. Endothelin-1 was measured at baseline and 12 weeks. We used fixed effects models to evaluate associations with measures of temperature that were adjusted for time-varying covariates. Results: Higher apparent temperature was associated with higher levels of BNP beginning with 2-day moving averages and reached statistical significance for 3- and 4-day moving averages. CRP results followed a similar pattern but were delayed by 1 day. A 5°C change in 3- and 4-day moving averages of apparent temperature was associated with 11.3% [95% confidence interval (CI): 1.1, 22.5; :p = 0.03) and 11.4% (95% CI: 1.2, 22.5; p = 0.03) higher BNP. A 5°C change in the 4-day moving average of apparent temperature was associated with 21.6% (95% CI: 2.5, 44.2; p = 0.03) higher CRP. No clear associations with TNF or endothelin-1 were observed. Conclusions: Among patients undergoing treatment for heart failure, we observed positive associations between temperature and both BNP and CRP—predictors of heart failure prognosis and severity
High-resolution x-ray telescopes
High-energy astrophysics is a relatively young scientific field, made
possible by space-borne telescopes. During the half-century history of x-ray
astronomy, the sensitivity of focusing x-ray telescopes-through finer angular
resolution and increased effective area-has improved by a factor of a 100
million. This technological advance has enabled numerous exciting discoveries
and increasingly detailed study of the high-energy universe-including accreting
(stellar-mass and super-massive) black holes, accreting and isolated neutron
stars, pulsar-wind nebulae, shocked plasma in supernova remnants, and hot
thermal plasma in clusters of galaxies. As the largest structures in the
universe, galaxy clusters constitute a unique laboratory for measuring the
gravitational effects of dark matter and of dark energy. Here, we review the
history of high-resolution x-ray telescopes and highlight some of the
scientific results enabled by these telescopes. Next, we describe the planned
next-generation x-ray-astronomy facility-the International X-ray Observatory
(IXO). We conclude with an overview of a concept for the next next-generation
facility-Generation X. The scientific objectives of such a mission will require
very large areas (about 10000 m2) of highly-nested lightweight
grazing-incidence mirrors with exceptional (about 0.1-arcsecond) angular
resolution. Achieving this angular resolution with lightweight mirrors will

Socioeconomic Position and Health-Seeking Behavior for Hearing Loss Among Older Adults in England
Objective. To examine whether socioeconomic position (SEP) is associated with progression in the health-seeking process for hearing loss. Method. Logistic regression of data from a cross-sectional survey representative of noninstitutionalized, 50 years and older population of England (ELSA wave 2, 2004). Using self-reported hearing difficulty as starting point, we examined the association between SEP and health-seeking behaviors in 6 stages leading to hearing aid acquisition and use.
Results. Higher SEP was associated with lower odds of self-reported hearing difficulty, adjusted odds ratio [OR] = 0.87 (95% confidence interval [CI] 0.83–0.91, p < .001). There was marginal negative association between higher SEP and receiving hearing aid recommendation (adjusted OR = 0.88, 95% CI 0.78–0.99, p = .05). SEP was not associated with any other stage of health-seeking behavior. Discussion. Among the noninstitutionalized older population of England, SEP-related inequalities exist in the prevalence of self-reported hearing loss. However, SEP is not strongly associated with progression in the remaining stages of health-seeking process during and after an individual’s contact with the health system
The Simulated HI Sky at low redshift
Observations of intergalactic neutral hydrogen can provide a wealth of
information about structure and galaxy formation, potentially tracing accretion
and feedback processes on Mpc scales. Below a column density of NHI ~ 10^19
cm-2, the "edge" or typical observational limit for HI emission from galaxies,
simulations predict a cosmic web of extended emission and filamentary
structures. We study the distribution of neutral hydrogen and its 21cm emission
properties in a cosmological hydrodynamic simulation, to gain more insights
into the distribution of HI below NHI ~ 10^19 cm-2. Such Lyman Limit systems
are expected to trace out the cosmic web, and are relatively unexplored.
Beginning with a 32 h^-1 Mpc simulation, we extract the neutral hydrogen
component by determining the neutral fraction, including a post-processed
correction for self-shielding based on the thermal pressure. We take into
account molecular hydrogen, assuming an average density ratio Omega_H2 /
Omega_HI = 0.3 at z = 0. The statistical properties of the HI emission are
compared with observations, to assess the reliability of the simulation. The
simulated HI distribution robustly describes the full column density range
between NHI ~ 10^14 and NHI ~ 10^21 cm-2 and agrees very well with available
measurements from observations. Furthermore there is good correspondence in the
statistics when looking at the two-point correlation function and the HI mass
function. The reconstructed maps are used to simulate observations of existing
and future telescopes by adding noise and taking account of the sensitivity of
the telescopes. The general agreement in statistical properties of HI suggests
that neutral hydrogen as modeled in this hydrodynamic simulation is a fair

Projections of the current and future disease burden of hepatitis C virus infection in Malaysia
The prevalence of hepatitis C virus (HCV) infection in Malaysia has been estimated at 2.5% of the adult population. Our objective, satisfying one of the directives of the WHO Framework for Global Action on Viral Hepatitis, was to forecast the HCV disease burden in Malaysia using modelling methods.An age-structured multi-state Markov model was developed to simulate the natural history of HCV infection. We tested three historical incidence scenarios that would give rise to the estimated prevalence in 2009, and calculated the incidence of cirrhosis, end-stage liver disease, and death, and disability-adjusted life-years (DALYs) under each scenario, to the year 2039. In the baseline scenario, current antiviral treatment levels were extended from 2014 to the end of the simulation period. To estimate the disease burden averted under current sustained virological response rates and treatment levels, the baseline scenario was compared to a counterfactual scenario in which no past or future treatment is assumed.In the baseline scenario, the projected disease burden for the year 2039 is 94,900 DALYs/year (95% credible interval (CrI): 77,100 to 124,500), with 2,002 (95% CrI: 1340 to 3040) and 540 (95% CrI: 251 to 1,030) individuals predicted to develop decompensated cirrhosis and hepatocellular carcinoma, respectively, in that year. Although current treatment practice is estimated to avert a cumulative total of 2,200 deaths from DC or HCC, a cumulative total of 63,900 HCV-related deaths is projected by 2039.The HCV-related disease burden is already high and is forecast to rise steeply over the coming decades under current levels of antiviral treatment. Increased governmental resources to improve HCV screening and treatment rates and to reduce transmission are essential to address the high projected HCV disease burden in Malaysia

This essay develops a framework for thinking about the moral basis for the commnodification of human reproductive nmaterials. It argues that selling and buyinlg gametes and genes is morally acceptable although there should not be a market for zygotes, embryos, or genomes. Also a market in gametes and genes shouild be regutlated in order to address concerns about the adverse social consequences of conmmodification. Economic factors influencing zoonotic disease dynamics: demand for poultry meat and seasonal transmission of avian influenza in Vietnam
While climate is often presented as a key factor influencing the seasonality of diseases, the importance of anthropogenic factors is less commonly evaluated. Using a combination of methods-wavelet analysis, economic analysis, statistical and disease transmission modelling-we aimed to explore the influence of climatic and economic factors on the seasonality of H5N1 Highly Pathogenic Avian Influenza in the domestic poultry population of Vietnam. We found that while climatic variables are associated with seasonal variation in the incidence of avian influenza outbreaks in the North of the country, this is not the case in the Centre and the South. In contrast, temporal patterns of H5N1 incidence are similar across these 3 regions: periods of high H5N1 incidence coincide with Lunar New Year festival, occurring in January-February, in the 3 climatic regions for 5 out of the 8 study years. Yet, daily poultry meat consumption drastically increases during Lunar New Year festival throughout the country. To meet this rise in demand, poultry production and trade are expected to peak around the festival period, promoting viral spread, which we demonstrated using a stochastic disease transmission model. This study illustrates the way in which economic factors may influence the dynamics of livestock pathogens

Hexatic Order and Surface Ripples in Spherical Geometries
In flat geometries, two dimensional hexatic order has only a minor effect on
capillary waves on a liquid substrate and on undulation modes in lipid
bilayers. However, extended bond orientational order alters the long wavelength
spectrum of these ripples in spherical geometries. We calculate this frequency
shift and suggest that it might be detectable in lipid bilayer vesicles, at the
surface of liquid metals and in multielectron bubbles in liquid helium at low
temperatures. Hexatic order also leads to a shift in the threshold for the
fission instability induced in the later two systems by an excess of electric
