    Spaceship Earth. Space-driven technologies and systems for sustainability on ground

    As awareness towards the problem is growing, eco-friendliness is today a paramount requirement for all space activities and in particular for the ground segment, fully comparable to other industrial sectors. The present work focuses on the assessment and the sustainable development enhancement of a ground-based space facility, the European Astronaut Centre (EAC), located in Germany. The project is framed within the European Space Agency development of an environmental outlook, which aims not only at the full compliance with the legislation and at assessing the impact of its activities, but also at laying the foundation for future evolution through innovation. Indeed, ESA promotes the sustainable use of space as a necessity and duty for Europe. As history teaches us, technical knowledge emerged within the space sector serves as innovation driver in other industrial branches: the goal of the project is to transform the EAC building into a spaceship integrated with the territory through the conscious management of this spontaneous process, fostering the combination between the space sector and the architecture and civil engineering fields. The work explores the potential of space technologies, processes and systems applied on ground and presents a range of space-driven innovative concepts which may improve the sustainability of the EAC building, focusing on different aspects of its resource demand – energy, water and waste management – and defining the integration with the pre-existing compound, the limitation of the impact on the surrounding landscape and the participation of the local community as additional fundamental requirements. Indeed, the project embraces the full concept of sustainability, which considers not only eco-friendliness but also its balance with economic and social aspects. Two factors – a certain urgency for action, which leaves little space for research and experimentation, and a call for ground-breaking solutions – guided the design activity: taking advantage of these conflicting requirements, a comparison between standard technologies and innovative space-related concepts was performed. When dealing with complex and uncertain scenarios, decision among the possible solutions is not straightforward and needs to be supported by appropriate methodologies: a multi-criteria and quantitative decision-making tool, able to concentrate on the main goal while considering all other relevant aspects – environmental, economic, social sustainability – was therefore developed. Furthermore, the project promotes local community participation in the decisional process, as a way to enhance knowledge, generate understanding and promote towards the EAC redesign, space activities and their potential innovative impact on sustainability

    Synthesis, molecular modeling and biological evaluation of two new chicoric acid analogs

    Two conformationally constrained compounds similar to chicoric acid but lacking the catechol and carboxyl groups were prepared. In these analogues, the single bond between the two caffeoyl fragments has been replaced with a chiral oxirane ring and both aromatic residues modified protecting completely or partially the catechol moiety as methyl ether. Preliminary molecular modelling studies carried out on the two analogues showed interactions near the active site of HIV integrase; however, in comparison with raltegravir, the biological evaluation confirmed that CAA-1 and CAA-2 were unable to inhibit infection at lower concentration

    Is rejection a diffuse or localized process in small-bowel transplantation?

    Utilization of endoscopy to both visualize and selectively biopsy an intestinal allograft has become the standard for early recognition and treatment of intestinal allograft rejection. Despite the widespread acceptance of the need for selective mucosal biopsies, it has not been shown that the histological features of intestinal allograft rejection are either localized or occur as part of a more diffuse phenomenon within a tubular allograft. To resolve these issues, 88 ileoscopies were performed in 12 small-bowel allograft recipients and mucosal biopsy samples were obtained at 5, 10, and 15 cm, respectively, from the ileal stoma. Each mucosal biopsy was labeled, processed, and evaluated individually for the presence and severity of any evidence for allograft rejection. The data obtained suggest that intestinal allograft rejection is a diffuse process, and biopsies obtained randomly from an ileal graft are likely to demonstrate evidence of allograft rejection when such is present. © 1994 Springer-Verlag New York Inc

    Passive Intermodulation Due to Conductor Surface Roughness

    The physical mechanism of the experimentally observed dependence of passive intermodulation (PIM) in printed circuits on conductor surface roughness is studied. It is shown that electrothermal (ET) nonlinearity, arising due to heating of imperfect conductors by high-power carriers in a multicarrier system, is correlated with conductor surface roughness and has a unique signature. Carriers modulate the conductor resistivity, skin depth, and surface impedance which generate PIM products. The detailed analysis demonstrates that ET-PIM depends on the conductor resistivity, shape, and roughness profile and also on the electric and thermal properties of the substrate. Their effects on PIM are illustrated by examples of uniform microstrip lines with different conductor and substrate materials, and periodically perturbed and meandered microstrip lines

    Robotic versus open oncological gastric surgery in the elderly. a propensity score-matched analysis

    Although there is no agreement on a definition of elderly, commonly an age cutoff of ≥ 65 or 75 years is used. Even if robot-assisted surgery is a validated option for the elderly population, there are no specific indications for its application in the surgical treatment of gastric cancer. The aim of this study is to evaluate the safety and feasibility of robot-assisted gastrectomy and to compare the short and long-term outcomes of robot-assisted (RG) versus open gastrectomy (OG). Patients aged ≥ 70 years old undergoing surgery for gastric cancer at the Department of Surgery of San Donato Hospital in Arezzo, between September 2012 and March 2017 were enrolled. A 1:1 propensity score matching was performed according to the following variables: age, Sex, BMI, ASA score, comorbidity, T stage and type of resection performed. 43 OG were matched to 43 RG. The mean operative time was significantly longer in the RG group (273.8 vs. 193.5 min, p < 0.01). No differences were observed in terms of intraoperative blood loss, an average number of lymph nodes removed, mean hospital stay, morbidity and mortality. OG had higher rate of major complications (6.9 vs. 16.3%, OR 2.592, 95% CI 0.623–10.785, p = 0.313) and a significantly higher postoperative pain (0.95 vs. 1.24, p = 0.042). Overall survival (p = 0.263) and disease-free survival (p = 0.474) were comparable between groups. Robotic-assisted surgery for oncological gastrectomy in elderly patients is safe and effective showing non-inferiority comparing to the open technique in terms of perioperative outcomes and overall 5-year survival

    Lack of Correlation between Liver Tests Abnormalities and Trabectedin Efficacy in the Treatment of Soft Tissue Sarcoma: A Retrospective Study

    Elevation in liver transaminases is common in patients treated with the marine antitumor agent trabectedin. However, the impact of trabectedin-related transaminase elevations on treatment outcomes is unclear. This retrospective study investigated the correlation between liver tests abnormalities and treatment outcomes in patients with unresectable advanced or metastatic soft tissue sarcomas (STS) treated with trabectedin 1.5 mg/m2 once every 3 weeks at three reference centers in Italy. The effect of grade 3/4 elevations in alanine aminotransferase (ALT) or aspartate aminotransferase (AST) during the first two cycles and at any time during trabectedin treatment on progression-free survival (PFS) and overall survival (OS) were analyzed. Liver tests abnormalities during the first two cycles of chemotherapy or at any time during trabectedin treatment did not significantly affect PFS or OS. Nor were survival outcomes significantly different in the subgroups of patients with or without ALT/AST increases or with ALT/AST elevations ≥15 × the upper limit of normal (ULN) versus those with ALT/AST elevation <15 × ULN. Although liver tests abnormalities are common in patients treated with trabectedin, elevations in ALT and AST are usually transient, occur during the first two cycles of treatment, and do not appear to affect survival
