CARACTERIZACIÓN DEL CAMBIO DE GUARDIA ENTRE EL CENTRO QUIRÚRGICO Y LA UNIDAD DE TERAPIA INTENSIVA
A passagem de plantão deve conter todas as informações indispensáveis para continuidade da assistência. Falhas nesse processo podem prejudicar a segurança do paciente, levando a um tratamento inadequado e com potencial perigo. O objetivo deste estudo foi caracterizar a passagem de plantão entre o Centro Cirúrgico e a Unidade de Terapia Intensiva. Estudo descritivo e transversal, realizado na Unidade de Terapia Intensiva adulto de um hospital escola no município de Goiânia – Goiás, no período de maio a agosto de 2014. Os dados foram coletados por meio de um checklist dividido em quatro categorias contendo informações referentes à identificação, procedimento realizado, condições clínicas e cuidados pós-operatórios. A maioria das informações do checklist não foi repassada para a equipe da Unidade de Terapia Intensiva. Espera-se que os dados apresentados contribuam para criação de estratégias seguras e fortaleçam o comprometimento dos profissionais com a qualidade das informações transmitidas durante a passagem de plantão.The handover process must contain all the necessary information for continuing care. Ineffective handover process, with loss of information can be a risk to patient safety. The present study aimed to characterize the handover process from the Surgical Center and the Intensive Care Unit. Descriptive cross-sectional study conducted at an adult Intensive Care Unit of a teaching hospital in the city of de Goiânia – Goiás, from May to August 2014. Data was collected through a checklist divided into four categories containing information related to patient identification, procedure performed, clinical status and postoperative care. A great deal of information of the checklist was not transferred to the Intensive Care Unit staff. We expect that the data provided here contribute to the creation of safe strategies and strengthen the commitment of health professionals with the quality of the information transmitted during the handover process.El cambio de guardia debe contener todas las informaciones indispensables para continuidad de la asistencia. Errores en ese proceso pueden perjudicar la seguridad del paciente, llevando a un tratamiento inadecuado y con potencial peligro. El objetivo de este estudio fue caracterizar cambio de guardia entre el Centro Quirúrgico y la Unidad de Terapia Intensiva. Estudio descriptivo y transversal, realizado en la Unidad de Terapia Intensiva de adultos de un hospital escuela en municipio de Goiânia – Goiás, en el periodo de mayo a agosto de 2014. Los datos fueron obtenidos por medio de un checklist dividido en cuatro categorías que contienen informaciones referentes a identificación, procedimiento realizado, condiciones clínicas y cuidatos posoperatorios. La mayoría de las informaciones del checklist no fue comunicada al equipo de la Unidad de Terapia Intensiva. Es esencial que los datos presentados puedan contribuir para la creación de estrategias seguras y fortalezcan el comprometimiento de los profesionales con la cualidad de las informaciones comunicadas durante el cambio de guardia
Torocotomia ressuscitativa de emergência: uma medida para melhoria da sobrevida: Emergency resuscitative Torocotomy: a measure to improve survival
Método: O presente trabalho aplicou a metodologia de revisão sistemática, com acesso a base de dados BVS, PubMed e SciELO, entre o período de 2013 a 2022. Objetivo: de entender a indicação para realização da toracotomia ressuscitativa e a evolução do paciente frente a sobrevida. Resultado: Obteve-se como análise que a toracotomia ressuscitativa de emergência pode ser utilizada como método emergencial em pacientes com trauma torácico, e que possui maior benefício em paciente não idosos devido o período de recuperação após realização do procedimento. Conclusão: Dado os estudos analisados, entende-se que há um aumento da sobrevida a longo prazo em pacientes de meia idade, mas há um aumento do tempo de internação hospitalar, assim como maior tempo de recuperação da realização das atividades de vida diária. 
Ultrassom FAST- aplicabilidade em pacientes hemodinamicamente instáveis na emergência: FAST ultrasound - applicability in hemodynamically unstable patients in emergency
Objetivo: Compreender a importância do uso do FAST e eFAST como meio diagnóstico e conduta no paciente hemodinamicamente instável vítima de trauma. Método: Trata-se de uma revisão sistemática realizada através da pesquisa no banco de dados BVS com acesso ao PubMed, Medline e SciElo, entre o período de 2009 a 2022. Resultados: Foram selecionados 11 artigos que abordavam de forma detalhada o tema proposto. Desta analise pode-se entender a facilidade da ultrassonografia beira leito como método diagnóstico de líquido livre intra-abdominal e achados de trauma torácico em pacientes instáveis hemodinamicamente, com alta sensibilidade e especificidade, evitando-se procedimentos invasivos desnecessários. Conclusão: Entende-se por fim que a utilização do FAST e o eFAST auxiliam no diagnóstico inicial do paciente vítima de trauma e de forma mais rápida e barata que a utilização da tomografia computadorizada

Global burden of 288 causes of death and life expectancy decomposition in 204 countries and territories and 811 subnational locations, 1990–2021: a systematic analysis for the Global Burden of Disease Study 2021
BACKGROUND Regular, detailed reporting on population health by underlying cause of death is fundamental for public health decision making. Cause-specific estimates of mortality and the subsequent effects on life expectancy worldwide are valuable metrics to gauge progress in reducing mortality rates. These estimates are particularly important following large-scale mortality spikes, such as the COVID-19 pandemic. When systematically analysed, mortality rates and life expectancy allow comparisons of the consequences of causes of death globally and over time, providing a nuanced understanding of the effect of these causes on global populations. METHODS The Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021 cause-of-death analysis estimated mortality and years of life lost (YLLs) from 288 causes of death by age-sex-location-year in 204 countries and territories and 811 subnational locations for each year from 1990 until 2021. The analysis used 56 604 data sources, including data from vital registration and verbal autopsy as well as surveys, censuses, surveillance systems, and cancer registries, among others. As with previous GBD rounds, cause-specific death rates for most causes were estimated using the Cause of Death Ensemble model-a modelling tool developed for GBD to assess the out-of-sample predictive validity of different statistical models and covariate permutations and combine those results to produce cause-specific mortality estimates-with alternative strategies adapted to model causes with insufficient data, substantial changes in reporting over the study period, or unusual epidemiology. YLLs were computed as the product of the number of deaths for each cause-age-sex-location-year and the standard life expectancy at each age. As part of the modelling process, uncertainty intervals (UIs) were generated using the 2·5th and 97·5th percentiles from a 1000-draw distribution for each metric. We decomposed life expectancy by cause of death, location, and year to show cause-specific effects on life expectancy from 1990 to 2021. We also used the coefficient of variation and the fraction of population affected by 90% of deaths to highlight concentrations of mortality. Findings are reported in counts and age-standardised rates. Methodological improvements for cause-of-death estimates in GBD 2021 include the expansion of under-5-years age group to include four new age groups, enhanced methods to account for stochastic variation of sparse data, and the inclusion of COVID-19 and other pandemic-related mortality-which includes excess mortality associated with the pandemic, excluding COVID-19, lower respiratory infections, measles, malaria, and pertussis. For this analysis, 199 new country-years of vital registration cause-of-death data, 5 country-years of surveillance data, 21 country-years of verbal autopsy data, and 94 country-years of other data types were added to those used in previous GBD rounds. FINDINGS The leading causes of age-standardised deaths globally were the same in 2019 as they were in 1990; in descending order, these were, ischaemic heart disease, stroke, chronic obstructive pulmonary disease, and lower respiratory infections. In 2021, however, COVID-19 replaced stroke as the second-leading age-standardised cause of death, with 94·0 deaths (95% UI 89·2-100·0) per 100 000 population. The COVID-19 pandemic shifted the rankings of the leading five causes, lowering stroke to the third-leading and chronic obstructive pulmonary disease to the fourth-leading position. In 2021, the highest age-standardised death rates from COVID-19 occurred in sub-Saharan Africa (271·0 deaths [250·1-290·7] per 100 000 population) and Latin America and the Caribbean (195·4 deaths [182·1-211·4] per 100 000 population). The lowest age-standardised death rates from COVID-19 were in the high-income super-region (48·1 deaths [47·4-48·8] per 100 000 population) and southeast Asia, east Asia, and Oceania (23·2 deaths [16·3-37·2] per 100 000 population). Globally, life expectancy steadily improved between 1990 and 2019 for 18 of the 22 investigated causes. Decomposition of global and regional life expectancy showed the positive effect that reductions in deaths from enteric infections, lower respiratory infections, stroke, and neonatal deaths, among others have contributed to improved survival over the study period. However, a net reduction of 1·6 years occurred in global life expectancy between 2019 and 2021, primarily due to increased death rates from COVID-19 and other pandemic-related mortality. Life expectancy was highly variable between super-regions over the study period, with southeast Asia, east Asia, and Oceania gaining 8·3 years (6·7-9·9) overall, while having the smallest reduction in life expectancy due to COVID-19 (0·4 years). The largest reduction in life expectancy due to COVID-19 occurred in Latin America and the Caribbean (3·6 years). Additionally, 53 of the 288 causes of death were highly concentrated in locations with less than 50% of the global population as of 2021, and these causes of death became progressively more concentrated since 1990, when only 44 causes showed this pattern. The concentration phenomenon is discussed heuristically with respect to enteric and lower respiratory infections, malaria, HIV/AIDS, neonatal disorders, tuberculosis, and measles. INTERPRETATION Long-standing gains in life expectancy and reductions in many of the leading causes of death have been disrupted by the COVID-19 pandemic, the adverse effects of which were spread unevenly among populations. Despite the pandemic, there has been continued progress in combatting several notable causes of death, leading to improved global life expectancy over the study period. Each of the seven GBD super-regions showed an overall improvement from 1990 and 2021, obscuring the negative effect in the years of the pandemic. Additionally, our findings regarding regional variation in causes of death driving increases in life expectancy hold clear policy utility. Analyses of shifting mortality trends reveal that several causes, once widespread globally, are now increasingly concentrated geographically. These changes in mortality concentration, alongside further investigation of changing risks, interventions, and relevant policy, present an important opportunity to deepen our understanding of mortality-reduction strategies. Examining patterns in mortality concentration might reveal areas where successful public health interventions have been implemented. Translating these successes to locations where certain causes of death remain entrenched can inform policies that work to improve life expectancy for people everywhere. Pythium insidiosum em equino: Relato de caso
A pitiose é uma afecção granulomatosa crônica que acomete equinos e outras espécies como cães, bovinos, caprinos, felinos, animais silvestres e seres humanos. Ocorre em regiões de climas tropicais, subtropicais ou temperados, e em áreas com acúmulo de água, banhados e lagoas. O Brasil está entre os países tropicais como local endêmico de pitiose equina. O agente etiológico em mamíferos é um oomiceto do Reino Stramenopila, Filo Oomycota, Classe Oomycetes, Ordem Pythiales, Família Pyhtiaceae, Gênero Pythium, Espécie Pythium insidiosum. É um parasita de plantas aquáticas, que através de reprodução assexuada produz zoósporos que podem infectar os animais. Causa grandes prejuízos na equinocultura e o prognóstico da doença depende da extensão das lesões e comprometimento de estruturas como tendões, articulações e tecidos ósseos. É uma doença que provoca um quadro infeccioso na pele e região subcutânea, pode apresentar quadros sistêmicos, pulmonares, gastrointestinais, oculares, entre outros. A enfermidade em equinos caracteriza-se pela formação de granulomas eosinofílicos, com a presença de massas necróticas denominadas de “kunkers”. Os “kunkers” histologicamente apresentam-se como concreções eosinofílicas de tamanho variado, forma circular, contornos irregulares, compostas de hifas, colágeno, arteríolas e células inflamatórias. Muitos protocolos para tratamento têm sido utilizados: químicos (antifúngicos), cirúrgicos e de imunoterapia. O tratamento mais indicado para a cura da pitiose em equinos é a remoção cirúrgica do granuloma combinada com a imunoterapia. Porém, o tratamento é complicado devido a características singulares do agente que difere dos fungos verdadeiros na produção de zoósporos móveis e na composição de sua parede celular. Fungos verdadeiros possuem quitina em sua parede, enquanto o Pythium contém celulose e β-glucanas, o que dificulta a penetração dos fármacos. Neste trabalho, propõe-se relatar um caso de pitiose diagnosticado em um equino atendido no Hospital de Clínicas Veterinárias (HCV) da UFRGS (Universidade Federal do Rio Grande do Sul). Observou-se um tratamento longo, dispendioso, com complicações e reações adversas no local de aplicação do imunoterápico. Contudo, a terapia foi eficiente na diminuição da lesão, melhora do quadro infeccioso da pele e região subcutânea
Comprehension of the education handout and health literacy of pacemaker users
<div><p>ABSTRACT Objective: To verify the comprehension of the education handout and the level of Functional Health Literacy of individuals with cardiac pacemaker (PM) and whether there is correlation between the comprehension and Functional Health Literacy (FHL). Method: Cross-sectional study with 63 individuals with PM who answered to comprehension tests of the handout, literacy assessment (SAHLPA-50) and cognition (MMSE). Measurements of dispersion, Pearson correlation and multiple linear regression were calculated. Results: Most women, study time ≤ 9 years, 66.21 (average age) presented no cognitive changes. An adequate literacy level was evidenced in 50.8% individuals with PM and satisfactory comprehension of the handout. No correlation was identified between FHL, handout comprehension, age, years of study and cognition. Conclusion: The handout comprehension assessed by individuals with appropriate FHL indicated that it can be a printed material suitable for use, aiming to improve care process and knowledge of individuals with PM.</p></div
