    Fetus in fetu : a case report

    Fetus-in-fetu (FIF), gebelik esnasındaki anormal embriyogenezis sonrası gelişen nadir bir anomalidir. Malforme fetüs ikiz eşinin vücudu içinde büyümekte ve sıklıkla abdominal bir kitle olarak tespit edilmektedir. Klinikte FIF ve fetiform teratom (FT) tanıları arasında kesin bir sınır yoktur. Kliniğimize karında kitle nedeniyle başvuran yedi yaşındaki erkek hasta değerlendirildi. Bu nadir görülen olgu nedeniyle, FIF ile FT ayırıcı tanısındaki gerekli kriterleri tartıştık.Fetus in fetu (FIF) is a rare anomaly due to an abnormal embriogenesis of fetus during pregnancy. Malformed fetus grows in the body of co-twin and its frequently detected as an abdominal mass. There is no clear distinction between the term of FIF and fetiform teratoma (FT). A seven years old boy who admitted to our clinic with an abdominal mass was examined. In the light of this case, we discussed the criteria's of differential diagnosis between FIF and FT

    Acute effect of phosphodiesterase type 5 inhibitor on serum oxidative status and prolidase activities in men with erectile dysfunction

    OBJECTIVES: To investigate the acute effect of phosphodiesterase type 5 (PDE5) inhibitor on erectile dysfunction by evaluating serum oxidative status and prolidase activity. METHODS: Serum samples of 36 patients with erectile dysfunction and 30 control cases were analyzed for total antioxidant status, total oxidant status, and prolidase activity, before and after the administration of tadalafil citrate. RESULTS: Before and after tadalafil citrate administration, serum total antioxidant status, total oxidant status, and prolidase were 1.1+0.0 vs. 1.6 + 0.0 umol H2O2 Eq/L, 10.3+1.1 vs. 6.9 + 1.2 umol H2O2 Eq/L, and 236.4+19.5 vs. 228.2 + 19.2 U/L, respectively (

Preoperatif ve postoperatif sperm sayısı ve motilitesi karşılaştırıldı. Her iki grupta da bu parametrelerdeki postoperatif dönemde belirgin iyileşme gözlemlendi. Alçak ligasyon tekniğinin düşük nüks hızı ve lokal anestestezi altında yapılabilir olması ve dolayısı ile düşük maliyeti nedeni ile retroperitoneal yüksek ligasyon yöntemine göre üstün ve önerilebilir olduğunu öne sürüyoruz. 2

    Is there any effect of insulin resistance on male reproductive system?

    Objectives: To investigate the possible effect of insulin resistance (IR) on male reproductive system via evaluation of semen analysis, male sex hormones and serum lipid profiles, and testicular volumes. Methods: After the exclusions, a total of 80 male patients were enrolled in this prospective study. Body Mass Index (BMI), Testicular volume, semen samples, serum hormone/lipid profiles, high sensitive C-Reactive Protein (hsCRP) were obtained from all the subjects. Results: The patients were divided into two groups as study and control according to the presence of IR. There were no statistical differences in terms of age, marriage period, testicular volume, serum levels of hormone and lipid profiles and BMI between the groups. There were no relationship between homeostasis model assessment of insulin resistance (HOMA-IR) and semen volume (r = -0.10, p = 0.37), total sperm count (r = -0.09, p = 0.39), motility (r = -0.15, p = 0.16) and morphology (r = -0.14, p = 0.19). However, HOMA-IR was closely associated with hsCRP levels (r = 0.94, p < 0.0001). Conclusions: Despite of the documented strong inverse relationships between Diabetes Mellitus (DM) and male/female fertility, and also between IR and female infertility via ovarian functions as in polycystic ovary syndrome, to our knowledge, there is no report about any influence of IR on male fertility. DM and metabolic syndrome (MetS) have negative influence on fertility. Thus, IR may be accused of causing detrimental effect on male infertility due to hyperinsulinemic state and being one of the components for MetS. Interestingly, due to our preliminary results, we do not found any inverse correlation between IR and male reproductive functions

    Introduction: Although Mustafa Kemal Ataturk (1881-1938) was a national hero with his intrepid and enlightened attempts to establish modern Turkey from the remnants of Ottoman heritage, he had been suffering from lifelong "kidney disease" that appeared with intermittent flank pain and fever without an identified source. However, we think that this physical pain that he endured only increased his motivation to focus on his military and political aims. Methods & Results: In this historical review article, we have focused on his personal medical life and specifically his "kidneys" from the beginning of the complaint till his death through European medical and political history with geographic locations and speculated upon it via past, near past, and recent medical literature. Conclusion: Mustafa Kemal Ataturk, the great military and political leader for his country, had always suffered from uro/nephrological problems throughout his life. We think that this was one of the reasons that urology has been privileged and thus to be the oldest separated medical surgical branch in Turkey and to some significant extent with European urological history

    What is the correct staging and treatment strategy for locally advanced prostate cancer extending to the bladder?

    In locally advanced prostate cancer with bladder invasion, frequently encountered problems such as bleeding, urinary retention, hydronephrosis, and pain create distress for the patients. Therefore patients’ quality of life is disrupted and duration of hospitalization is prolonged. Relevant literature about accurate staging and treatment of locally advanced prostate cancer with bladder invasion was investigated. Locally advanced prostate cancer can present as a large-volume aggressive tumor extending beyond boundaries of prostate gland, and involving neighboring structures which can be involved as recurrence(s) following initial local therapy. Survival times of these patients can range between 5 and 8 years. Their common characteristics are adverse and severe local symptoms unfavorably affecting quality of life Control of local symptoms and their effective palliation are independent clinical targets influencing survival outcomes of these patients. The treatment outcomes of locally advanced prostate cancer into the bladder are currently debatable. Although in the current TNM classification, it is defined in T4a, we think that this may be categorized as a subgroup of T3 and thus encourage surgeons for the indication of radical surgeries (radical prostatectomy, radical cystoprostatectomy) in selected patient populations after discussing issues concerning consequences of the treatment alternatives, and expectations with the patients. Cystoprostatectomy followed by immediate androgen deprivation therapy may be a feasible option for selected patients with previously untreated prostate cancer involving the bladder neck because of excellent local control and long term survival

News about the discovery of fantastic archeological findings is not so surprising for local citizens because they were regarded as ordinary events. It was recently announced by an archeological society that cave art depicting human beings dated back to nearly 8,000 years ago had been discovered at the southern-most point of Anatolia.     We report a case of isolated distal ureter tuberculosis who presented with irritative voiding symptoms treated with sole medical therapy and discuss the clinical, imaging, diagnostic and therapeutical features. In case of irritative voiding symptoms and radiological presentation of ureteral pathologies, genitourinary tuberculosis should be considered in the differential diagnosis. We believe that medical therapy should be the main option before the invasive procedures
