Place du scanner dans la prise en charge des traumatismes crâniens légers en population pédiatrique. Analyse rétrospective des pratiques au CHU de Rouen

Objectif principal : Évaluer le taux de scanner réalisé pour traumatisme crânien léger en population pédiatrique au CHU de Rouen. Objectifs secondaires : Étudier l’association entre les signes cliniques et la demande d’imagerie, étudier le taux de scanner ayant conduit à une prise en charge neurochirurgicale et tenter d’élaborer un algorithme de prise en charge local intégrant l’IRM. Matériels et méthodes : Cette étude rétrospective monocentrique à été réalisée sur 1274 patients de moins de 16 ans se présentant aux urgences pour traumatisme crânien léger avec un score de Glasgow de 13 ou plus. Les signes cliniques, la demande d’imagerie, les anomalies à l’imagerie et la prise en charge ont été recueillis. La comparaison entre les groupes a été réalisée par le test du Chi 2 ou le test exact de Fisher lorsque le Chi 2 n’était pas applicable. Résultats : Une imagerie a été réalisée chez 79 patients (6,2%) dont 74 scanners (5.8%). 18 patients (1,4%) présentaient au moins une anomalie post-traumatique. Il existait une différence significative (p Conclusion : Le taux d’imagerie réalisée après traumatisme crânien est faible dans notre étude et a mené à une prise en charge chirurgicale chez seulement un patient. L’IRM est plus performante pour la détection des lésions intraparenchymateuses et pourrait avoir sa place en dehors des suspicions de fracture de la base du crâne

ddpcr: an R package and web application for analysis of droplet digital PCR data [version 1; referees: 2 approved]

Droplet digital polymerase chain reaction (ddPCR) is a novel platform for exact quantification of DNA which holds great promise in clinical diagnostics. It is increasingly popular due to its digital nature, which provides more accurate quantification and higher sensitivity than traditional real-time PCR. However, clinical adoption has been slowed in part by the lack of software tools available for analyzing ddPCR data. Here, we present ddpcr – a new R package for ddPCR visualization and analysis. Bilateral hypoglossal nerve stimulation for treatment of adult obstructive sleep apnoea

Background and aim Hypoglossal nerve stimulation (HNS) decreases obstructive sleep apnoea (OSA) severity via genioglossus muscle activation and decreased upper airway collapsibility. This study assessed the safety and effectiveness at 6 months post-implantation of a novel device delivering bilateral HNS via a small implanted electrode activated by a unit worn externally, to treat OSA: the Genio™ system. Methods This prospective, open-label, non-randomised, single-arm treatment study was conducted at eight centres in three countries (Australia, France and the UK). Primary outcomes were incidence of device-related serious adverse events and change in the apnoea–hypopnoea index (AHI). The secondary outcome was the change in the 4% oxygen desaturation index (ODI). Additional outcomes included measures of sleepiness, quality of life, snoring and device use. This trial was registered with ClinicalTrials.gov, number NCT03048604. Results 22 out of 27 implanted participants (63% male, aged 55.9±12.0 years, body mass index (BMI) 27.4±3.0 kg·m−2) completed the protocol. At 6 months BMI was unchanged (p=0.85); AHI decreased from 23.7±12.2 to 12.9±10.1 events·h−1, a mean change of 10.8 events·h−1 (p\u3c0.001); and ODI decreased from 19.1±11.2 to 9.8±6.9 events·h−1, a mean change of 9.3 events·h−1 (p\u3c0.001). Daytime sleepiness (Epworth Sleepiness Scale; p=0.01) and sleep-related quality of life (Functional Outcomes of Sleep Questionnaire-10; p=0.02) both improved significantly. The number of bed partners reporting loud, very intense snoring, or leaving the bedroom due to participant snoring decreased from 96% to 35%. 91% of participants reported device use \u3e5 days per week, and 77% reported use for \u3e5 h per night. No device-related serious adverse events occurred during the 6-month post-implantation period. Conclusions Bilateral HNS using the Genio™ system reduces OSA severity and improves quality of life without device-related complications. The results are comparable with previously published HNS systems despite minimal implanted components and a simple stimulation algorithm

New shapes on the dance floor:influencing ambient sound and vision with computationally augmented poi

This article charts the design, development and build of an interactive performance piece created specifically for use in a club context. At the time of writing it remains work in progress but our aim here is to articulate our creative and research goals and unpack some of the issues surrounding creating hybridized work. The piece takes a traditional set of poi that clubbers often dance with in trance and techno clubs and attempts to augment them with sensor technology in order to allow the shapes, patterns and movements made by the participant to become visualized elsewhere in the club space. Choreographic motifs drawn from the participants' motion in space with the poi add a further layer to the performance, which seeks to set up physical conversations between the dancer, the space, the technology and the club context itself
