Five diseases, one vaccine : a boost for emerging livestock farmers in South Africa
Livestock are essential to the economic, nutritional and social wellbeing of African farmers. Besides providing food, clothing and other products, they are a measure of wealth and social standing; they are used for barter, as lobola (bride price) at traditional weddings, and also as a ‘bank’, whereby animals can be sold to pay for emergency needs, such as funerals. We thank Drs Giral and colleagues for their interest in our work.1 They raise the important query of whether our findings would still persist after adjustment for γ-glutamyltransferase (GGT), given that GGT activity hydrolyzes glutathione (GSH) to produce glutamate+cysteinylglycine. This point, however, is not relevant to our description of GSH/cystine as a useful biomarker of cardiovascular disease, because our samples were all collected with a preservation solution containing a GGT
A Novel Biomarker of Oxidative Stress is Associated with Risk of Death in Patients with Coronary Artery Disease
Background—Free radical scavengers have failed to improve patient outcomes promoting the concept that clinically important oxidative stress (OS) may be mediated by alternative mechanisms. We sought to examine the association of emerging aminothiol markers of non-free radical mediated oxidative stress with clinical outcomes. Methods and Results—Plasma levels of reduced (cysteine and glutathione) and oxidized (cystine and glutathione disulphide) aminothiols were quantified by high performance liquid chromatography in 1411 patients undergoing coronary angiography (mean age 63 years, male 66%). All patients were followed for a mean of 4.7±2.1 years for the primary outcome of all-cause death (n=247). Levels of cystine (oxidized) and glutathione (reduced) were associated with risk of death (p\u3c0.001 both) before and after adjustment for covariates. High cystine and low glutathione levels (\u3e+1 SD & \u3c-1 SD respectively) were associated with higher mortality (adjusted HR 1.63 (95% CI 1.19-2.21; HR 2.19 (95% CI 1.50-3.19), respectively) compared to those outside these thresholds. Furthermore, the ratio of cystine/glutathione was also significantly associated with mortality (adjusted HR 1.92 (95% CI 1.39-2.64) and was independent of and additive to hs-CRP level. Similar associations were found for other outcomes of cardiovascular death and combined death and myocardial infarction. Conclusions—A high burden of OS, quantified by the plasma aminothiols, cystine, glutathione and their ratio is associated with mortality in patients with CAD, a finding that is independent of and additive to the inflammatory burden. Importantly, this data supports the emerging role of non-free radical biology in driving clinically important oxidative stress
Peste des petits ruminants virus tissue tropism and pathogenesis in sheep and goats following experimental infection
Peste des petits ruminants (PPR) is a viral disease which primarily affects small ruminants, causing significant economic
losses for the livestock industry in developing countries. It is endemic in Saharan and sub-Saharan Africa, the Middle East
and the Indian sub-continent. The primary hosts for peste des petits ruminants virus (PPRV) are goats and sheep; however
recent models studying the pathology, disease progression and viremia of PPRV have focused primarily on goat models.
This study evaluates the tissue tropism and pathogenesis of PPR following experimental infection of sheep and goats using
a quantitative time-course study. Upon infection with a virulent strain of PPRV, both sheep and goats developed clinical
signs and lesions typical of PPR, although sheep displayed milder clinical disease compared to goats. Tissue tropism of PPRV
was evaluated by real-time RT-PCR and immunohistochemistry. Lymph nodes, lymphoid tissue and digestive tract organs
were the predominant sites of virus replication. The results presented in this study provide models for the comparative
evaluation of PPRV pathogenesis and tissue tropism in both sheep and goats. These models are suitable for the
establishment of experimental parameters necessary for the evaluation of vaccines, as well as further studies into PPRV-host

Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries
Abstract
Background
Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres.
Methods
This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries.
Results
In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia.
Conclusion
This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries
New livestock vaccines - a boost for emerging farmers in Africa
This work was carried out with the aid of a grant from Canada’s International Development Research Centre (IDRC), and with financial support from the Government of Canada, provided through Global Affairs Canada (GAC)Although commercial farmers generally make good use of livestock vaccines, emerging livestock farmers face major challenges in effective vaccine acquisition and use. Given the diverse uses of livestock and their socio-economic importance in farming communities, the loss of even a single animal has significant and sometimes crippling effects on a family. To ensure the relevance of newly developed vaccines to emerging farmers, this project uses an integrative and gender responsive approach, linking vaccine development with education, economics and social science. The researchers believe that the new vaccines will have the potential to control six important African livestock diseases
The Development of a Multivalent Capripoxvirus-Vectored Vaccine Candidate to Protect against Sheeppox, Goatpox, Peste des Petits Ruminants, and Rift Valley Fever
Capripoxviruses are the causative agents of sheeppox, goatpox, and lumpy skin disease (LSD) in cattle, which cause economic losses to the livestock industry in Africa and Asia. Capripoxviruses are currently controlled using several live attenuated vaccines. It was previously demonstrated that a lumpy skin disease virus (LSDV) field isolate from Warmbaths (WB) South Africa, ORF 005 (IL-10) gene-deleted virus (LSDV WB005KO), was able to protect sheep and goats against sheeppox and goatpox. Subsequently, genes encoding the protective antigens for peste des petits ruminants (PPR) and Rift Valley fever (RVF) viruses have been inserted in the LSDV WB005KO construct in three different antigen forms (native, secreted, and fusion). These three multivalent vaccine candidates were evaluated for protection against PPR using a single immunization of 104 TCID50 in sheep. The vaccine candidates with the native and secreted antigens protected sheep against PPR clinical disease and decreased viral shedding, as detected using real-time RT-PCR in oral and nasal swabs. An anamnestic antibody response, measured using PPR virus-neutralizing antibody response production, was observed in sheep following infection. The vaccine candidates with the antigens expressed in their native form were evaluated for protection against RVF using a single immunization with doses of 104 or 105 TCID50 in sheep and goats. Following RVF virus infection, sheep and goats were protected against clinical disease and no viremia was detected in serum compared to control animals, where viremia was detected one day following infection. Sheep and goats developed RVFV-neutralizing antibodies prior to infection, and the antibody responses increased following infection. These results demonstrate that an LSD virus-vectored vaccine candidate can be used in sheep and goats to protect against multiple viral infections
