    A GEO-REFERENCED DATABASE TO MANAGE THE LANDSCAPE AREAS UNDER PRESERVATION IN LOMBARDY REGION

    Abstract. One of the main goals of the Lombardy Region Landscape Plan (Piano Paesaggistico Regionale – PPR) review was to set up a detailed investigation of landscape binding actions that are issued according to article 136 of the Code of Cultural Heritage and Landscape (D.Lgs. 42/2004).To achieve this goal, a survey was launched and a geo-referenced database was constructed, considering all the data included in the 888 landscape binding actions currently in force in Lombardy.The geo-referenced database takes into account also other architectural preservation measures and natural landscape protections, because a large quantity areas in Lombardy are safeguarded as Regional Parks and Natural Monuments, ecological corridors, geosites and various zones with natural significance are issued as ZPS and SIC by UE.Databases from other agencies and institutions were collected, examined and updated, to provide a complete tool, usable by public administrations and professionals.Such database can also be integrated by multi-temporal geoinformation availability of applications implemented to manage and share georeferenced historical maps (i.e. GEOPAN ATL@S APP), boosting the utility of these data sets in the daily professional activity.</p

    General practitioners' accounts of negotiating antibiotic prescribing decisions with patients: a qualitative study on what influences antibiotic prescribing in low, medium and high prescribing practices

    Background Antimicrobial resistance (AMR) is high on the UK public health policy agenda, and poses challenges to patient safety and the provision of health services. Widespread prescribing of antibiotics is thought to increase AMR, and mostly takes place in primary medical care. However, prescribing rates vary substantially between general practices. The aim of this study was to understand contextual factors related to general practitioners’ (GPs) antibiotic prescribing behaviour in low, high, and around the mean (medium) prescribing primary care practices. Methods Qualitative semi-structured interviews were conducted with 41 GPs working in North-West England. Participants were purposively sampled from practices with low, medium, and high antibiotic prescribing rates adjusted for the number and characteristics of patients registered in a practice. The interviews were analysed thematically. Results This study found that optimizing antibiotic prescribing creates tensions for GPs, particularly in doctor-patient communication during a consultation. GPs balanced patient expectations and their own decision-making in their communication. When not prescribing antibiotics, GPs reported the need for supportive mechanisms, such as regular practice meetings, within the practice, and in the wider healthcare system (e.g. longer consultation times). In low prescribing practices, GPs reported that increasing dialogue with colleagues, having consistent patterns of prescribing within the practice, supportive practice policies, and enough resources such as consultation time were important supports when not prescribing antibiotics. Conclusions Insight into GPs’ negotiations with patient and public health demands, and consistent and supportive practice-level policies can help support prudent antibiotic prescribing among primary care practices

    Elemental composition of soils and sediments by using portable X-ray fluorescence (pXRF) spectroscopy: a preliminary study

    Portable X-ray fluorescence spectrometry (pXRF) provides rapid and low-cost analysis in laboratory and field conditions for a wide range of environmental applications. Its successful application in soils requires a good instrument calibration and the possibility to implement custom calibrations and methods using known samples as well as reference materials. In the present study two set of samples, one from pedogenized sediments of a basin in Marradi (Italy; n=13) and the other from agricultural land in Meuse valley (Belgium; n=10), were analyzed by using a portable X-ray fluorescence (pXRF TRACER 5i made by Bruker) to detect and quantify the element species (Al, Si, Ca, Fe, Mg, K, Mn, P, S, Ba, Cd, Co, Cr, Cu, Mo, Pb, Sb, Sn, Sr and Zn). Element concentrations results were compared with analytical results from aqua regia extractions (AQ) followed by ICP-OES analysis. The aim of this work was to verify and find the best instrument setup for the specific matrix, elements and their concentration range with the purpose of using the pXRF technique directly insitu for the screening of sites potentially polluted. A standard factory calibration for soil applications was used for samples from Meuse valley, while a custom calibration for P, S, Ca and Fe quantification with the coefficient correction method was developed and used for pedogenized sediments from Marradi, as well as for Cu and Cr quantification in both sites. Comparing portable XRF and AQ results, elements could be grouped in three classes: 1) elements where the content determined by both methods is not correlated (p>0.05; Al, Si, and K); 2) elements strongly correlated (0.840.60), carbonates (Ca, r2>0.55) and oxy(hydr)oxides (Fe and Mn) have instead comparable concentrations between pXRF and AQ. Preliminary results showed that portable XRF can quickly provide data for some elucidations of important pedogenic processes involving many elements (class 2) in the studied soils. Alternative methods offer results after expensive and time-consuming analytical procedures. Moreover for environmental and human health purposes, the pXRF should be a valid tool for a rapid screening of heavy metals, such as Cu, Cr, Zn and Pb. A larger set of soil samples and further custom calibration will be tested

    Posterior cortical atrophy and Alzheimer’s disease : a meta-analytic review of neuropsychological and brain morphometry studies

    This paper presents the first systematic review and meta-analysis of neuropsychological and brain morphometry studies comparing posterior cortical atrophy (PCA) to typical Alzheimer's disease (tAD). Literature searches were conducted for brain morphometry and neuropsychological studies including a PCA and a tAD group. Compared to healthy controls (HC), PCA patients exhibited significant decreases in temporal, occipital and parietal gray matter (GM) volumes, whereas tAD patients showed extensive left temporal atrophy. Compared to tAD patients, participants with PCA showed greater GM volume reduction in the right occipital gyrus extending to the posterior lobule. In addition, PCA patients showed less GM volume loss in the left parahippocampal gyrus and left hippocampus than tAD patients. PCA patients exhibit significantly greater impairment in Immediate Visuospatial Memory as well as Visuoperceptual and Visuospatial Abilities than patients with tAD. However, tAD patients showed greater impairment in Delayed Auditory/Verbal Memory than patients with PCA.     [Preliminary results in the use of polydimethylsiloxane textured elastomers (Bioplastique) in the treatment of leakage around tracheoesophageal voice prosthesis].

    Leakage around the prosthesis is one of the worst problems afflicting patients fitted with tracheoesophageal (TE) voice prosthesis. Removing the prosthesis for a few days can reduce the size of the fistula, or the leakage may be eliminated simply by replacing the prosthesis. In cases that are refractory to such treatment, the perifistular injection of different types of autologous, heterologous or alloplastic substances has been proposed. A new kind of silicone, previously available abroad, has recently appeared on the Italian market as well; constituted by polydimethylsiloxane textured elastomers (Bioplastique) for soft tissue implants, it appears to guarantee excellent standards of long-term safety and stability. Three patients fitted with Provox2 tracheoesophageal voice protheses who had been experiencing leakage for about 4 months underwent Bioplastique implant procedures. The correction of the fistula size was easy to accomplish and well tolerated by the patients. There was no sign of inflammation, formation of granulomas or other pathological modifications, and the size of the fistula, together with its fluid-tightness, remained stable in time. From our point of view, the Bioplastique implant appears to provide valid and effective treatment of gaping TE fistulas

    The present study is an evaluation of a designated driver intervention. The objectives were: (1) to verify the intervention assumption, which is that offering incentives, linked to a positive result of the breathalyser test (&lt;0.5 g/l) when leaving the club, leads to a decrease in the percentage of drivers leaving the club with a BAC above the legal limit; (2) to understand the reason why drivers take part in the intervention and assess whether the operation was effective in lowering the amount of alcohol consumed during the night; (3) to verify the assumption that there is a bias in the selection of designated drivers
