Peningkatan Hasil Belajar Siswa Melalui Pembelajaran Kontekstual Konsep Reproduksi Manusia Siswa MTs Negeri 1 Matangglumpangdua
Teacher is one important component in improving teaching quality. One way to improve the teaching quality is by applying various innovative strategies. One of the innovative teaching strategies is contextual learning. Contextual teaching strategy is a teaching strategy which emphasize on studentsâ activity in learning, so that students can memorize the material for longer time and can get better achievement. This research aimed to figure out the improvement of studentsâ achievement in Human Reproduction Concept at MTs Negeri 1 Matangglumpangdua. This research used two cycles with four components: planning, acting, observing and reflecting. Test was conducted at the end of each cycle. Data was analyzed qualitatively by using descriptive statistic. The result shows that the average of cycle 1 increased from 70,00 to 76,09 at cycle 2. Students give positive responses 72,28%; 75, 82%; 73,39% and 71,16% to attention indicator, interest indicator, possitive belief indicator and satisfaction indicator (agreement scale) respectively. Finally, it is recommended that contextual learning is suitable to be used in Human Reproduction Concept. Nevertheless, contextual learning strategy shouldnât be applied continously to different materials to avoid studentâs boredom

Spectrogram Based Window Selection for the Detection of Voltage Variation
This paper presents the application of spectrogram with K-nearest neighbors (KNN) and Support Vector Machine (SVM) for window selection and voltage variation classification. The voltage variation signals such as voltage sag, swell and interruption are simulated in Matlab and analyzed in spectrogram with different windows which are 256, 512 and 1024. The variations analyzed by spectrogram are displayed in time-frequency representation (TFR) and voltage per unit (PU) graphs. The parameters are calculated from the TFR obtained and be used as inputs for KNN and SVM classifiers. The signals obtained are then added with noise (0SNR and 20SNR) and used in classification. The tested data contain voltage variation signals obtained using the mathematical models simulated in Matlab and the signals added with noise. Classification accuracy of each window by each classifier is obtained and compared along with the TFR and voltage PU graphs to select the best window to be used to analyze the best window to be used to analyze the voltage variation signals in spectrogram. The results showed window 1024 is more suitable to be used
Eligibility criteria for different platinum-based chemotherapy regimens in metastatic urothelial carcinoma
Introduction
Bladder cancer is one of the most prevalent cancers worldwide, with significant morbidity and mortality rates. Treatment options for metastatic urothelial carcinoma (mUC) primarily include platinum-based chemotherapy. Cisplatin-based chemotherapy is conventionally used for treating mUC, but many patients are ineligible due to various factors such as poor performance status, creatinine clearance, neuropathy, and cardiac function. Carboplatin-based therapy is another alternative, which typically yields less favorable outcomes. Some centers use split-dose cisplatin for treating patients with comorbidities and impaired renal function, broadening cisplatin's spectrum. While eligibility criteria for full-dose cisplatin are well-established, those for split-dose cisplatin and carboplatin lack strong evidence. This study aims to assess the recommended criteria for full-dose cisplatin, split-dose cisplatin, and carboplatin regimens in real-world settings, including hematological parameters for patients with mUC.
MethodsÂ
A cross-sectional web-based survey was conducted among 136 oncologists from 21 countries, assessing criteria such as creatinine clearance, Eastern Cooperative Oncology Group (ECOG) performance status (PS), neurotoxicity, hearing loss, heart failure classification, and hematological parameters.
Results
The survey revealed diverse preferences among 113 oncologists treating mUC, regarding the eligibility criteria for each chemotherapy regimen with 81% prioritizing full-dose cisplatin, 21% split-dose cisplatin, and 14% carboplatin regimens. Criteria for all three regimens included specific thresholds. For full-dose cisplatin, the preferred criteria included creatinine clearance â„60 mL/min, ECOG PS â€1, grade 1 neuropathy, grade 1 deafness, New York Heart Association (NYHA) heart failure â€class II with â„50% cardiac ejection fraction, and normal blood parameters. Split-dose cisplatin criteria were creatinine clearance â„40 mL/min, ECOG PS â€2, grade 1 neuropathy, grade 1 deafness, NYHA heart failure â€class II with â„50% cardiac ejection fraction, and normal blood parameters. Carboplatin eligibility criteria were creatinine clearance â„30, ECOG PS â€2, grade â€2 neuropathy, grade â€2 deafness, NYHA heart failure â€class II with â„50% cardiac ejection fraction, and normal blood parameters. Hematological parameters were deemed crucial for all regimens, particularly stringent for carboplatin-based chemotherapy.
Conclusion
The study underscores the importance of renal function and hematological parameters in determining chemotherapy eligibility for patients with mUC. It highlights the importance of precise treatment criteria in mUC management, with hematological factors playing a significant role. Standardized criteria and further research are warranted to optimize treatment outcomes and minimize adverse events associated with chemotherapy regimens. Understanding the preferences of oncologists globally can facilitate tailored treatment approaches and improve patient care in the management of mUC
Genetic pleiotropy underpinning adiposity and inflammation in self-identified Hispanic/Latino populations
Background: Concurrent variation in adiposity and inflammation suggests potential shared functional pathways and pleiotropic disease underpinning. Yet, exploration of pleiotropy in the context of adiposity-inflammation has been scarce, and none has included self-identified Hispanic/Latino populations. Given the high level of ancestral diversity in Hispanic American population, genetic studies may reveal variants that are infrequent/monomorphic in more homogeneous populations. Methods: Using multi-trait Adaptive Sum of Powered Score (aSPU) method, we examined individual and shared genetic effects underlying inflammatory (CRP) and adiposity-related traits (Body Mass Index [BMI]), and central adiposity (Waist to Hip Ratio [WHR]) in HLA participating in the Population Architecture Using Genomics and Epidemiology (PAGE) cohort (N = 35,871) with replication of effects in the Cameron County Hispanic Cohort (CCHC) which consists of Mexican American individuals. Results: Of the > 16 million SNPs tested, variants representing 7 independent loci were found to illustrate significant association with multiple traits. Two out of 7 variants were replicated at statistically significant level in multi-trait analyses in CCHC. The lead variant on APOE (rs439401) and rs11208712 were found to harbor multi-trait associations with adiposity and inflammation. Conclusions: Results from this study demonstrate the importance of considering pleiotropy for improving our understanding of the etiology of the various metabolic pathways that regulate cardiovascular disease development
EPIdemiology of Surgery-Associated Acute Kidney Injury (EPIS-AKI) : Study protocol for a multicentre, observational trial
More than 300 million surgical procedures are performed each year. Acute kidney injury (AKI) is a common complication after major surgery and is associated with adverse short-term and long-term outcomes. However, there is a large variation in the incidence of reported AKI rates. The establishment of an accurate epidemiology of surgery-associated AKI is important for healthcare policy, quality initiatives, clinical trials, as well as for improving guidelines. The objective of the Epidemiology of Surgery-associated Acute Kidney Injury (EPIS-AKI) trial is to prospectively evaluate the epidemiology of AKI after major surgery using the latest Kidney Disease: Improving Global Outcomes (KDIGO) consensus definition of AKI. EPIS-AKI is an international prospective, observational, multicentre cohort study including 10 000 patients undergoing major surgery who are subsequently admitted to the ICU or a similar high dependency unit. The primary endpoint is the incidence of AKI within 72 hours after surgery according to the KDIGO criteria. Secondary endpoints include use of renal replacement therapy (RRT), mortality during ICU and hospital stay, length of ICU and hospital stay and major adverse kidney events (combined endpoint consisting of persistent renal dysfunction, RRT and mortality) at day 90. Further, we will evaluate preoperative and intraoperative risk factors affecting the incidence of postoperative AKI. In an add-on analysis, we will assess urinary biomarkers for early detection of AKI. EPIS-AKI has been approved by the leading Ethics Committee of the Medical Council North Rhine-Westphalia, of the Westphalian Wilhelms-University MĂŒnster and the corresponding Ethics Committee at each participating site. Results will be disseminated widely and published in peer-reviewed journals, presented at conferences and used to design further AKI-related trials. Trial registration number NCT04165369
