    Hydrodynamics and morphodynamics performance assessment of three coastal protection structures

    Coastal areas accommodate a great part of large metropolises as they support a great amount of economic and leisure activities. The attraction of people to coastal zones is contributing to an intense and continuous urbanization of these areas, while the ecosystems are threatened by the increase of natural extreme weather events (e.g., intensity and duration of storms, floods), which interfere with local wave climate and changes in morphological beach characteristics. Protection of coastal zones predisposed to coastline recession, due to the action of high tides, high sediment transport deficit, and high wave energy, may involve various coastal structures to reduce or at least to mitigate coastal erosion problems. Many of the current coastal protections (notably groins, seawalls, and emerged breakwaters) were built with a single purpose, which was to protect at all costs without environmental or economic concerns, especially maintenance costs, or the negative consequences that such structures could cause up to considerable distances along the coast. The current concept of integrated coastal zone management presupposes studies involving other types of concerns and more actors in the decision-making process for the implementation of coastal works. In this context, multifunctional structures emerge and are increasingly frequent, such as the so-called multifunctional artificial reefs (MFARs), with the aim of improving leisure, fishing, diving, and other sporting activities, in addition to coastal protection. MFARs are in fact one of the latest concepts for coastal protection. Behind the search for more efficient and sustainable strategies to deal with coastal retreat, this study focused on a comparison between the performance of two traditional coastal protection solutions (submerged detached breakwater and emerged detached breakwater) and an MFAR on a particular coastal stretch. In order to analyse the hydro- (wave height and wave energy dissipation) and morphodynamics (sediment accumulation and erosion areas, and bed level) of the structures and beach interactions, two numerical models were used: SWAN (Simulation WAves Nearshore) for hydrodynamics and XBeach for hydrodynamics and morphodynamics. In addition, a comparison between SWAN and XBeach hydrodynamic results was also performed. From the simulations conducted by SWAN and XBeach, it can be concluded that amongst all structures, the emerged detached breakwater was the most efficient in reducing significant wave heights at a larger scale due to the fact that it constituted a higher obstacle to the incoming waves, and that, regarding both submerged structures (detached breakwater and the MFAR), the MFAR presented a more substantial shadow zone. Regarding morphodynamics, the obtained results presented favourable tendencies to sediment accretion near the shoreline, as well as at the inward areas for the three structures, especially for the emerged detached breakwater and for the MFAR in both wave directions. However, for the west wave direction, along the shoreline, substantial erosion was observed for both structures with more noticeable values for the emerged detached breakwater. For all the northwest wave direction scenarios, no noticeable erosion areas were visible along the shoreline. Overall, considering the balance of erosion and accretion rates, it can be concluded that for both wave predominance, the submerged detached breakwater and the MFAR presented better solutions regarding morphodynamics. The MFAR storm wave condition performed in XBeach indicated substantial erosion areas located around the structure, which added substantial changes in the bed level.This research was funded by the Portuguese Foundation for Science and Technology (FCT), PhD grant number SFRH/BD/141381/201

    March1-dependent modulation of donor MHC II on CD103+ dendritic cells mitigates alloimmunity.

    In transplantation, donor dendritic cells (do-DCs) initiate the alloimmune response either by direct interaction with host T cells or by transferring intact donor MHC to host DCs. However, how do-DCs can be targeted for improving allograft survival is still unclear. Here we show CD103+ DCs are the major do-DC subset involved in the acute rejection of murine skin transplants. In the absence of CD103+ do-DCs, less donor MHC-II is carried to host lymph nodes, fewer allogenic T cells are primed and allograft survival is prolonged. Incubation of skin grafts with the anti-inflammatory mycobacterial protein DnaK reduces donor MHC-II on CD103+DCs and prolongs graft survival. This effect is mediated through IL-10-induced March1, which ubiquitinates and decreases MHC-II levels. Importantly, in vitro pre-treatment of human DCs with DnaK reduces their ability to prime alloreactive T cells. Our findings demonstrate a novel therapeutic approach to dampen alloimmunity by targeting donor MHC-II on CD103+DCs

    Contribution for new genetic markers of rheumatoid arthritis activity and severity : sequencing of the tumor necrosis factor-alpha gene promoter

Each patient's disease activity was assessed by the disease activity score using 28 joint counts (DAS28) and functional capacity by the Health Assessment Questionnaire (HAQ) score. Systemic manifestations, radiological damage evaluated by the Sharp/van der Heijde (SvdH) score, disease-modifying anti-rheumatic drug use, joint surgeries, and work disability were also assessed. The promoter region of the TNF-alpha gene, between nucleotides -1,318 and +49, was sequenced using an automated platform. Five hundred fifty-four patients were evaluated and genotyped for 10 single-nucleotide polymorphism (SNP) markers, but 5 of these markers were excluded due to failure to fall within Hardy-Weinberg equilibrium or to monomorphism. Patients with more than 10 years of disease duration (DD) presented significant associations between the -857 SNP and systemic manifestations, as well as joint surgeries. Associations were also found between the -308 SNP and work disability in patients with more than 2 years of DD and radiological damage in patients with less than 10 years of DD. A borderline effect was found between the -238 SNP and HAQ score and radiological damage in patients with 2 to 10 years of DD. An association was also found between haplotypes and the SvdH score for those with more than 10 years of DD.     Increased exposure to sodium during pregnancy and lactation changes basal and induced behavioral and neuroendocrine responses in adult male offspring

    Excessive sodium (Na+) intake in modern society has been associated with several chronic disorders such as hypertension. Several studies suggest that early life events can program physiological systems and lead to functional changes in adulthood. Therefore, we investigated behavioral and neuroendocrine responses under basal conditions and after 48 h of water deprivation in adult (60-day-old Wistar rats) male, Wistar rats originating from dams were offered only water or 0.15 mol/L NaCl during pregnancy and lactation. Early life salt exposure induced kidney damage, as shown by a higher number of ED-1 positive cells (macrophages/monocytes), increased daily urinary volume and Na+ excretion, blunted basal water intake and plasma oxytocin levels, and increased plasma corticosterone secretion. When challenged with water deprivation, animals exposed to 0.15 mol/L NaCl during early life showed impaired water intake, reduced salt preference ratio, and vasopressin (AVP) secretion. In summary, our data demonstrate that the perinatal exposure to excessive Na+ intake can induce kidney injury in adult offspring and significantly affect the key mechanisms regulating water balance, fluid intake, and AVP release in response to water deprivation. Collectively, these novel results highlight the impact of perinatal programming on the homeostatic mechanisms regulating fluid and electrolyte balance during exposure to an environmental stress (i.e. dehydration) in later life.Fil: Silva, Marcia S..     Guidelines on the management of valvular heart disease: The Task Force on the Management of Valvular Heart Disease of the European Society of Cardiology.

    80These guidelines focus on valvular heart disease in adults and adolescents, are oriented towards management, and will not deal with endocarditis and congenital valve diseases in adults and adolescents, since recent guidelines have been produced by the ESC on these topics. Although valvular heart disease is less common in industrialized countries than coronary disease, heart failure, or hypertension, guidelines are needed in this field for several reasons: valvular heart disease is common and often requires intervention; substantial advances have been made in the understanding of its pathophysiology; the patient population has changed with a continuous decline of acute rheumatic fever and an increased incidence of degenerative valvular diseases in industrialized countries. The incidence of endocarditis remains stable and other causes of valve disease are rare. Because of the predominance of degenerative valve disease, the two most frequent valve diseases are now calcific aortic stenosis and mitral regurgitation. Aortic regurgitation and mitral stenosis have become less common. Diagnosis is now dominated by echocardiography, which has become the standard to evaluate valve structure and function.     HPV infection and p53 and p16 expression in esophageal cancer: are they prognostic factors?

    Background: Esophageal squamous cell carcinoma (ESCC) is a highly lethal malignant tumor. Currently, Human papillomavirus (HPV) is suggested as a potential risk factor for esophageal cancer (EC) in addition to the classic risk factors, alcohol and tobacco, but this hypothesis still remains contradictory. We sought to investigate wether HPV and well-known biomarkers (p16 and p53) and patient-related factors that may have impact on survival of ESCC. Methods: We conducted a prospective cohort study. By using multiplex PCR, we determined the prevalence of high risk HPV in ESCC, and evaluated the immunohistochemical expression of p16 and p53, molecular markers related to esophageal carcinogenesis in order to verify the potential influence of these variables in patients's survival. Survival rates were estimated using Kaplan-Meier methods. A multivariate confirmatory model was performed using Cox proportional hazards regression. Results: Twelve (13.8%) of 87 patients were HPV-DNA positive. Positive reactions of p16 and p53 were 10.7% and 68.6%, respectively. Kaplan-Meier analysis indicated that men (p = 0.025) had poor specific-cancer survival and a shorter progression-free survival (p = 0.050) as compared to women; III or IV clinical stage (p < 0.019) had poor specific-cancer survival and a shorter progression-free survival (p < 0.001) compared to I and II clinical stage; not submitted to surgery (< 0.001) and not submitted to chemoradiotherapy (p = 0.039) had a poor specific-cancer survival, as well. The multivariate analysis showed that HPV, p16 and p53 status are not predictive parameters of progression-free and specific-cancer survival.
