Abdominal Compartment Syndrome in Patients with Abdominal Sepsis I. A. Kryvoruchko, M. S. Antonova, O. A. Tonkoglas, S.-K. T. Goni Kharkov National Medical University Ministry of Health of Ukraine Abstract Investigations were carried out in 53 patients who were operated on in 2015 about abdominal sepsis (AS). All patients were assigned according the severity of the condition, which was determined depending on the severity of the systemic inflammatory response and multiple organ failure: I group - patients with AS (28); II group - patients with severe AS (14); III group - patients with septic shock (11). Surgical management of all patients included two major components: control source of infection (source control) and control of organ damaged and the protective mechanisms system (damage control). In the first group, the laparotomy ended by suturing the wounds tightly with traditional drainage; in groups II and III surgical treatment were supplemented by the use of techniques aimed at the prevention and reduction of elevated intra-abdominal pressure. Studies have shown that 100 % of patients with AS showed an increase in intra-abdominal pressure and the development of intra-abdominal hypertension. While ACS developed in 18.9 % of cases (4 patients with severe sepsis and 6 - with septic shock). The highest mortality rate was observed among patients with grade III and IV intra-abdominal hypertension (11 of 25 patients). Key words: intra-abdominal pressure, abdominal sepsis, abdominal compartment syndrome, treatment

The Attractiveness of City as Place to Live: The Case of Yekaterinburg

Within increasing competition between cities for human resources a problem to enhance attractiveness of urban life is coming to the fore. While planning and designing urban environment major focus is given to the needs and interests of different population groups. Hence, a city should become a place of attracting creative people, individuals and groups which possess special knowledge and skills necessary for promoting, developing and improving an area that could become appealing to business, investors, tourists, etc.. With this purpose much attention should be paid to student-age youth as a potential driver of the area development by transforming a place of residence into an attractive urban environment. The purpose of this study is to analyze how student-age young people of Yekaterinburg perceive and assess attractiveness of the city as a place to live. Yekaterinburg is the biggest Russian industrial city that claims to be the "third" capital (after Moscow and Saint Petersburg). In 2017 we worked out an author's questionnaire containing 10 questions and conducted a standardized sociological poll of students of Yekaterinburg. 71 men and 129 women aged 18-25 of different academic fields participated in the survey. 42% of poll participants are local residents of Yekaterinburg (they were born in it and live here on a permanent basis), 58% came here from other places (80% of respondents moved to Yekaterinburg to study in higher educational establishments). Research findings showed that Yekaterinburg as a place to live is attractive for students due to a wide range of malls and shops, extended cultural and leisure facilities, opportunities for higher education and self-actualization. Meanwhile, the students pointed out some problem zones of the city like ecological situation, pavement condition, pollution, corruption and impossibility to take part in solving city problems. The young people consider park areas, transport accessibility (subway availability), extended network of cultural and recreational centers to be major attractive factors of urban life. General results of the poll and students' assessments indicate that a city as a place to live has two facets: A "comfortable" city with parks and gardens, cultural and leisure facilities, conditions for creative self-fulfillment and innovation and an "instrumental" city as a place for career development and material welfare. © Published under licence by IOP Publishing Ltd

Influence of ripening in mother solution on characteristics of magnesium-substituted calcium phosphate powders

    Get PDF
Effect of ripening in the mother liquor on the degree of crystallinity and dispersity of mixed powders of calcium and magnesium phosphates is studied as a function of magnesium content with the elemental composition such that (Ca + Mg)/P = 2. Ripening is found to have a positive effect on crystallinity of the apatite phase of powders. Nanocrystals with lowered tendency to aggregation are formed during the ripening period, which affords the powders with specific surface area as high as 80 m2/g. The morphology of the constituent particles depends on the magnesium content. Crystallization processes are essentially completed by the 21st day of ripenin

Objective. The investigation objective was improvement of the surgical treatment results in patients, suffering аbdominal sepsis, using individualized tactics of treatment, taking into account a possibility for the postoperative complications occurrence and the treatment results prognostication. Маterials and methods. The results of treatment of the adult patients, suffering abdominal sepsis in 2009 – 2019 yrs, excluding an acute cholecystitis cases, were analyzed. The patients were divided retrospectively and prospectively in accordance to the Sepsis–3 classification. Of them 130 have suffered peritoneal sepsis, 33 – intestinal one, and 38 – pancreatogenic abdominal. The results estimation was compared with application of modern systems: APACHE–II, Mannheim іndex of peritonitis (MPI), MODS and SOFA. The indices were estimated on the 1st, 2nd, 3rd and 4th postoperative days before subsequent outcomes of a primary operation. Results. The prognosis systems with a square under the ROC–curve (AUC) > 0.8 have included MPI only for determination of indications for reoperations: the ROC–curves value for the first subgroup (the closed treatment) have constituted 0.73, for the second subgroup (reoperations in accordance to indications) – 0.91, and for the third – 0.84 (the programmed reoperations). But in the patients of first subgroup this index have constituted 0,73, indicating its application restricting the decision making process, concerning the reoperations efficacy in patients with absence of the septic shock signs. Conclusion. Calculations of the occurrence possibility for postoperative complications and mortality in patients, depending on their preoperative state severity permits to select a most rational tactics of treatment. Most optimal approach for the rate reduction of postoperative complications occurrence and mortality in patients with abdominal sepsis and septic shock is performance of reoperations in accordance to indications to control the infection source, if it is possible

The article studies the impact of flagship enterprises on the level of socio-economic development of territories, the inflow of human capital and the intensification of entrepreneurial activity on the example of municipalities of three  subjects of the Siberian Federal district by constructing a regression model. The paper reveals the significant level  of differentiation of socio-economic development indicators for municipalities located within the same region. The  study determines the high influence of the growth of the population incomes in the absence of a significant increase in the population on the aggregate indicator of territorial development, the gross municipal product in  municipalities with flagship enterprises. The authors have identified the average impact of the flagship enterprises, in terms of stimulating the development of entrepreneurial activity, as well as revenue growth of companies  operating in these enterprises

Periodic Bursts of Coherent Radio Emission from an Ultracool Dwarf

We report the detection of periodic (p = 1.96 hours) bursts of extremely bright, 100% circularly polarized, coherent radio emission from the M9 dwarf TVLM 513-46546. Simultaneous photometric monitoring observations have established this periodicity to be the rotation period of the dwarf. These bursts, which were not present in previous observations of this target, confirm that ultracool dwarfs can generate persistent levels of broadband, coherent radio emission, associated with the presence of kG magnetic fields in a large-scale, stable configuration. Compact sources located at the magnetic polar regions produce highly beamed emission generated by the electron cyclotron maser instability, the same mechanism known to generate planetary coherent radio emission in our solar system. The narrow beams of radiation pass our line of sight as the dwarf rotates, producing the associated periodic bursts. The resulting radio light curves are analogous to the periodic light curves associated with pulsar radio emission highlighting TVLM 513-46546 as the prototype of a new class of transient radio source.Comment: 12 pages, 3 figures, accepted for publication in ApJ Letter

    Potato resistance to quarantine diseases

The casual agent of potato wart Synchytrium endobioticum (Schilb.) Perc. and potato golden nematode (PGN) Globodera rostochiensis (Wollenweber) Behrens are the quarantine species causing the most widespread and destructive diseases of potato in the Russian Federation and other countries of the world. The potato pale nematode Globodera pallida (Stone) Behrens is not found in Russia, although in the European Union it is found everywhere. The review provides information on the harmfulness of S. endobioticum and PGN. To date, 43 pathotypes of S. endobioticum and 5 pathotypes of PGN have been revealed in the world. In the Russian Federation, only the first (D1) pathotype of potato wart and pathotype Ro1 of PGN have been found. Modern sets of differentials for S. endobioticum and PGN and methods of pathotype composition determination, including efforts to develop molecular markers (SSR) to determine the race of S. endobioticum, are presented. Data on the resistance of commercial potato cultivars to these quarantine diseases and methods for resistance determination are reviewed. Modern data on the genetics of potato resistance to S. endobioticum, G. rostochiensis and G. pallida, including mapping and cloning of R-genes, are presented. Available literature data on molecular markers of R-genes for marker assisted selection and the evaluation of their effectiveness are presented. The use of multiplex systems allows the presence of several genes for resistance to one or more pathogens to be analyzed at once. Mechanisms of potato quantitative resistance to S. endobioticum and PGN and adaptation processes in pathogens populations resulting in overcoming resistance of host are discussed. Cultivation of cultivars poorly susceptible to PGN can stimulate the adaptive variability of the pathogen and induce virulent pathotypes for 2–3 pathogen generations

Background. The harmful organisms affecting potato cause great economic damage in all areas where the crop is grown. One of the most economically important pathogens is the potato late blight (agent Phytophthora infestans Mont. (de Bary)). Also, Potato Virus Y (PVY) and potato cyst nematode (PCN) of the Ro1 pathotype, the only one that occurs in the Russian Federation, cause significant yield losses. Materials and methods. Phytopathological and molecular screening was carried out on original interspecific potato hybrids obtained in 11 crossing combinations, seven of which were studied for the first time. Hybrids resulted from crosses that involved Mexican and South American potato species as sources of late blight and PVY resistance. Field observations of late blight resistance were conducted during three seasons under high infection pressure. A part of plant material was screened in laboratory tests. PCR tests employed 9 DNA markers of genes associated with resistance to Ph. infestans, PVY and PCN of Ro1 pathotype used in our previous study. Results. In field evaluation, hybrid clones mostly expressed high, and a part of them moderate late blight resistance. Of the 6 markers of the used Rpi genes, at least two were detected in most clones, while some had 3 or 4 markers. Some clones were found to have markers of PCN resistance genes and of several genes for resistance to PVY. The cytoplasm type was determined for all plant material. Conclutions. Within a small but pedigree-diverse set of interspeсific hybrids, the markers of resistance genes to all three pests were found. A part of clones obtained by using the sources of PVY and late blight resistance which we had identified, were found to contain two or more resistance genes to these pathogens. According to the results of molecular tests, the clones were found to carry several markers of late blight and PVY resistance genes. Original interspecific potato hybrids with determined late blight resistance levels, cytoplasm type and availability of markers of genes for resistance to three pests can be used in further hybridization
