    TPU Cloud-Based Generalized U-Net for Eye Fundus Image Segmentation

    Medical images from different clinics are acquired with different instruments and settings. To perform segmentation on these images as a cloud-based service we need to train with multiple datasets to increase the segmentation independency from the source. We also require an ef cient and fast segmentation network. In this work these two problems, which are essential for many practical medical imaging applications, are studied. As a segmentation network, U-Net has been selected. U-Net is a class of deep neural networks which have been shown to be effective for medical image segmentation. Many different U-Net implementations have been proposed.With the recent development of tensor processing units (TPU), the execution times of these algorithms can be drastically reduced. This makes them attractive for cloud services. In this paper, we study, using Google's publicly available colab environment, a generalized fully con gurable Keras U-Net implementation which uses Google TPU processors for training and prediction. As our application problem, we use the segmentation of Optic Disc and Cup, which can be applied to glaucoma detection. To obtain networks with a good performance, independently of the image acquisition source, we combine multiple publicly available datasets (RIM-One V3, DRISHTI and DRIONS). As a result of this study, we have developed a set of functions that allow the implementation of generalized U-Nets adapted to TPU execution and are suitable for cloud-based service implementation.Ministerio de Economía y Competitividad TEC2016-77785-

Dentro de dicha secretaría, existe la Dirección General de Higiene y Seguridad Industrial, la cual tiene la facultad para realizar visitas de inspección y toda clase de investigaciones para análisis y estudio. Las empresas para asegurar la seguridad de los empleados le proporcionan diversos equipos de protección dependiendo del tipo de labor que realicen en la empresa. La empresa donde se realizó la evaluación de seguridad e higiene está ubicada en el Parque Industrial portezuelo, Km. 5 ½ Carretera Norte, semáforo Portezuelo 400 metros al norte; Managua, Nicaragua.     Groundcovers in olive groves in semiarid climates: Are they always beneficial?

    Olive groves on the Mediterranean Basin are usually managed by continuous tillage and low organic matter inputs, leading to several soil degradation processes. Groundcovers (GCs) have come out as an alternative soil management strategy to improve soil's fertility, soil's organic matter quantity, and the sustainability of agro-ecosystems. Nevertheless, farmers are still unwilling to implement GCs or reduce tilling frequency while there are still some uncertainties and lack of global analyses. The purpose of this study was to perform an assessment of the effects of using GC on the soil parameters microbiological biomass, crop yield, and olive oil quality. A field trial was performed on a gypsiferous soil in central Spain with three different GCs: permanent GC of Brachypodium distachyon (BRA); permanent GC of spontaneous vegetation (SVE), and annual GC of Vicia ervilia (BIT) to be compared with conventional tillage (TIL). After three years, numbers of nematodes, fungi, and bacteria were higher on plant roots regardless of GC composition. BRA was the treatment with the biggest impact on soil and yield parameters. BRA increased soil organic carbon 1.03 Mg ha-1 yr-1 at 0-10 cm depth but reduced significantly olive yield because of nitrogen competition. The BIT treatment improved soil organic carbon stocks and soil structure, and did not reduce olive yield significantly regarding TIL.     Tracking changes on soil structure and organic carbon sequestration after 30 years of different tillage and management practices

    Long-term field trials are essential for monitoring the effects of sustainable land management strategies for adaptation and mitigation to climate change. The influence of more than thirty years of different management is analyzed on extensive crops under three tillage systems, conventional tillage (CT), minimum tillage (MT), and no-tillage (NT), and with two crop rotations, monoculture winter-wheat (Triticum aestivum L.) and wheat-vetch (Triticum aestivum L.-Vicia sativa L.), widely present in the center of Spain. The soil under NT experienced the largest change in organic carbon (SOC) sequestration, macroaggregate stability, and bulk density. In the MT and NT treatments, SOC content was still increasing after 32 years, being 26.5 and 32.2 Mg ha−1, respectively, compared to 20.8 Mg ha−1 in CT. The SOC stratification (ratio of SOC at the topsoil/SOC at the layer underneath), an indicator of soil conservation, increased with decreasing tillage intensity (2.32, 1.36, and 1.01 for NT, MT, and CT respectively). Tillage intensity affected the majority of soil parameters, except the water stable aggregates, infiltration, and porosity. The NT treatment increased available water, but only in monocropping. More water was retained at the permanent wilting point in NT treatments, which can be a disadvantage in dry periods of these edaphoclimatic conditions

    Liquid biopsy: a non-invasive approach for Hodgkin lymphoma genotyping

    The Hodgkin lymphoma (HL) genomic landscape is hardly known due to the scarcity of tumour cells in the tissue. Liquid biopsy employing circulating tumour DNA (ctDNA) can emerge as an alternative tool for non-invasive genotyping. By using a custom next generation sequencing (NGS) panel in combination with unique molecule identifiers, we aimed to identify somatic variants in the ctDNA of 60 HL at diagnosis. A total of 277 variants were detected in 36 of the 49 samples (73·5%) with a good quality ctDNA sample. The median number of variants detected per patient was five (range 1–23) with a median variant allele frequency of 4·2% (0·84–28%). Genotyping revealed somatic variants in the following genes: SOCS1 (28%), IGLL5 (26%), TNFAIP3 (23%), GNA13 (23%), STAT6 (21%) and B2M (19%). Moreover, several poor prognosis features (high LDH, low serum albumin, B-symptoms, IPI ≥ 3 or at an advanced stage) were related to significantly higher amounts of ctDNA. Variant detection in ctDNA by NGS is a feasible approach to depict the genetic features of HL patients at diagnosis.     Early detection of anthracycline- and trastuzumab-induced cardiotoxicity: value and optimal timing of serum biomarkers and echocardiographic parameters.

    Aims To evaluate echocardiographic and biomarker changes during chemotherapy, assess their ability to early detect and predict cardiotoxicity and to define the best time for their evaluation. Methods and results Seventy-two women with breast cancer (52 ± 9.8 years) treated with anthracyclines (26 also with trastuzumab), were evaluated for 14 months (6 echocardiograms/12 laboratory tests). We analysed: high-sensitivity cardiac troponin T, NT-proBNP, global longitudinal strain (GLS), left ventricle end-systolic volume (LVESV), left ventricle end-diastolic volume (LVEDV), and left ventricular ejection fraction (LVEF). Cardiotoxicity was defined as a reduction in LVEF>10% compared with baseline with LVEF<53%. High-sensitivity troponin T levels rose gradually reaching a maximum peak at 96 ± 13 days after starting chemotherapy (P < 0.001) and 62.5% of patients presented increased values during treatment. NT-proBNP augmented after each anthracycline cycle (mean pre-cycle levels of 72 ± 68 pg/mL and post-cycle levels of 260 ± 187 pg/mL; P < 0.0001). Cardiotoxicity was detected in 9.7% of patients (mean onset at 5.2 months). In the group with cardiotoxicity, the LVESV was higher compared with those without cardiotoxicity (40 mL vs. 29.5 mL; P = 0.045) at 1 month post-anthracycline treatment and the decline in GLS was more pronounced ( 17.6% vs. 21.4%; P = 0.03). Trastuzumab did not alter serum biomarkers, but it was associated with an increase in LVESV and LVEDV (P < 0.05). While baseline LVEF was an independent predictor of later cardiotoxicity (P = 0.039), LVESV and GLS resulted to be early detectors of cardiotoxicity [odds ratio = 1.12 (1.02–1.24), odds ratio = 0.66 (0.44–0.92), P < 0.05] at 1 month post-anthracycline treatment. Neither high-sensitivity troponin T nor NT-proBNP was capable of predicting subsequent cardiotoxicity. Conclusions One month after completion of anthracycline treatment is the optimal time to detect cardiotoxicity by means of imaging parameters (LVESV and GSL) and to determine maximal troponin rise. Baseline LVEF was a predictor of later cardiotoxicity.     Convergent Validity between Electromyographic Muscle Activity, Ultrasound Muscle Thickness and Dynamometric Force Measurement for Assessing Muscle

    Muscle fatigue is defined as a reversible decline in performance after intensive use, which largely recovers after a resting period. Surface electromyography (EMG), ultrasound imaging (US) and dynamometry are used to assess muscle activity, muscle morphology and isometric force capacity. This study aimed to assess the convergent validity between these three methods for assessing muscle fatigue during a manual prehension maximal voluntary isometric contraction (MVIC). A diagnostic accuracy study was conducted, enrolling 50 healthy participants for the measurement of simultaneous changes in muscle thickness, muscle activity and isometric force using EMG, US and a hand dynamometer, respectively, during a 15 s MVIC. An adjustment line and its variance (R2) were calculated. Muscle activity and thickness were comparable between genders (p > 0.05). However, men exhibited lower force holding capacity (p < 0.05). No side-to-side or dominance differences were found for any variable. Significant correlations were found for the EMG slope with US (r = 0.359; p < 0.01) and dynamometry (r = 0.305; p < 0.01) slopes and between dynamometry and US slopes (r = 0.227; p < 0.05). The sample of this study was characterized by comparable muscle activity and muscle thickness change between genders. In addition, fatigue slopes were not associated with demography or anthropometry.
