    The Evolution of State Higher Education Public Funding Policy

    In the past several decades, public colleges and universities have increasingly struggled to fund all of the programs demanded of them as states slowly drain funding from their coffers. This discussion will examine different policies and frameworks that have been used by states to determine funding for their public institutions. Different factors that will be considered include political relationships, governance structures, and accountability models. The political trend of “free college” will also be considered. Then, the discussion will narrow to South Carolina funding policy, how it has changed over the years, and what the current challenges are. A policy proposal for South Carolina will conclude the argument

    MULTILINGUALISM IN EDUCATION: THE ROLE OF FIRST LANGUAGE

    Abstract: This paper discusses the possibilities and effects of different language policies in schools around the world, based on recent research on second language learning and on the relationship between language and cultural identity. With reference to some of the latest findings from brain imaging in learners who speak one, two or more languages, it looks at the physical effects of bilingualism and multilingualism, and how these may be applied in teaching and in school policy to improve education. With reference to a recent survey of students in international secondary schools in Indonesia, it also discusses the cultural aspects of language use in schools and the role of language in identity. It examines the effects of learning more than one language at a young age, and of learning subjects through a language which is not the learner's first language. A large number of schools worldwide currently teach English to speakers of other languages from an early age. This paper discusses the degree to which such early foreign or second language teaching is actually useful or effective. In many parts of the world, it has also become popular to teach mainstream or even all curriculum subjects in English. Some, however, have found this to be detrimental in several ways and have therefore reverted to teaching subjects in their national language. Many are concerned about the possibility that languages with a small number of speakers are doomed to being lost and replaced by a national language, or that their national language could eventually be replaced by English. This could have important repercussions both on local cultures and on international relations, given the emotive aspects of one's first language. Vital decisions on language are frequently made based on the need for English in careers. However, it may be that the current language teaching situation is not actually succeeding in improving students' use of English or their learning in general. Moreover, there is a great risk of   producing one or more generations of school leavers and graduates who cannot function beyond everyday conversation in their first language. It may be impossible for English to continue to dominate education without raising affective barriers to learning by giving a low status to local languages and cultures.     SILENCE NO MORE: A TRANSFORMATIVE TRANSCENDENTAL PHENOMENOLOGICAL STUDY INVESTIGATING THE EXPERIENCES OF TEEN MOTHERS WHO GO TO COLLEGE IN THE RURAL SOUTHEAST

    According to ethnographer Kristin Luker (1996) \u27most poignantly, in the vast majority of cases, giving birth while still a teenager is a pledge of hope, an acted-out wish that the lives of the next generation will be better than those of the current generation, that this young mother can give her child something that she never had.\u27 Unfortunately, teen pregnancy prevention rhetoric, which often perpetuates the negative socially constructed image of teenage mothers, frequently focuses on the economic costs that teen pregnancy is reported to have. Not enough research has been devoted to the individual experiences of teen mothers, in particular teen mothers who go to college. This dissertation will use a transformative transcendental phenomenological approach to examine the experiences of teenaged mothers who go to college in the rural Southeastern United States, It will attempt to answer the question, \u27What does it mean to be a teen mother who goes to college in a rural setting?\u27 The author will identify themes that emerged from phenomenological interviews in which teen mothers who went on to two and four-year colleges share their experiences

    Drawing Encounters: A practice-led investigation into collaborative drawing as a means of revealing tacit elements of one-to-one social encounter.

    What might we discover by drawing the spaces between us? Situated in the field of drawing research, the thesis begins to articulate the term dialogic drawing through the development of the Drawing Encounter. Building on David Bohm and Martin Buber’s work on dialogue and encounter, the Drawing Encounter (using drawing rather than speech) is a new method to elicit tacit elements of one-to-one social interaction. The research is largely situated in the public arena where the author met strangers using collaborative drawing as a means of interaction. The drawings themselves prompted conversations about meeting in this way. Locations for the meetings included several train journeys, an arts therapy centre, an international conference, a drawing exhibition and an art college. Throughout the inquiry drawing was employed to collate, analyse and synthesise the data generated by all the research activities. The thesis presents significant encounters and drawings with participants’ commentaries and references to Bohm’s model of dialogue and Buber’s notion of the between. The drawing strategies that emerged during the inquiry are discussed in methodological terms in the context of naturalistic inquiry. The findings propose that by providing a novel means to facilitate and interrogate one-to-one interaction the Drawing Encounter method can enhance existing personal and professional relationships. Findings from researcher facilitated encounters indicate that the method has potential for applications in educational and professional situations where intra- and inter-personal issues are relevant. The thesis expands our understanding of visual arts processes as methods of creative inquiry by articulating the role of drawing as a research strategy and a materialising practice. It suggests that visual analogy can address pressing questions of human co-existence, by showing how what we do in the spaces between us can help us remain individuals yet still feel connected

    Addition of a dairy fraction rich in milk fat globule membrane to a high-saturated fat meal reduces the postprandial insulinaemic and inflammatory response in overweight and obese adults.

    Meals high in SFA, particularly palmitate, are associated with postprandial inflammation and insulin resistance. Milk fat globule membrane (MFGM) has anti-inflammatory properties that may attenuate the negative effects of SFA-rich meals. Our objective was to examine the postprandial metabolic and inflammatory response to a high-fat meal composed of palm oil (PO) compared with PO with an added dairy fraction rich in MFGM (PO+MFGM) in overweight and obese men and women (n 36) in a randomised, double-blinded, cross-over trial. Participants consumed two isoenergetic high-fat meals composed of a smoothie enriched with PO with v. without a cream-derived complex milk lipid fraction ( dairy fraction rich in MFGM) separated by a washout of 1-2 weeks. Serum cytokines, adhesion molecules, cortisol and markers of inflammation were measured at fasting, and at 1, 3 and 6 h postprandially. Glucose, insulin and lipid profiles were analysed in plasma. Consumption of the PO + MFGM v. PO meal resulted in lower total cholesterol (P = 0·021), LDL-cholesterol (P = 0·046), soluble intracellular adhesion molecule (P = 0·005) and insulin (P = 0·005) incremental AUC, and increased IL-10 (P = 0·013). Individuals with high baseline C-reactive protein (CRP) concentrations (≄3 mg/l, n 17) had higher (P = 0·030) insulin at 1 h after the PO meal than individuals with CRP concentrations <3 mg/l (n 19). The addition of MFGM attenuated this difference between CRP groups. The addition of a dairy fraction rich in MFGM attenuated the negative effects of a high-SFA meal by reducing postprandial cholesterol, inflammatory markers and insulin response in overweight and obese individuals, particularly in those with elevated CRP

    Dietary recommendations suggest decreased consumption of SFA to minimise CVD risk; however, not all foods rich in SFA are equivalent. To evaluate the effects of SFA in a dairy food matrix, as Cheddar cheese, v. SFA from a vegan-alternative test meal on postprandial inflammatory markers, a randomised controlled cross-over trial was conducted in twenty overweight or obese adults with metabolic abnormalities. Individuals consumed two isoenergetic high-fat mixed meals separated by a 1- to 2-week washout period. Serum was collected at baseline, and at 1, 3 and 6 h postprandially and analysed for inflammatory markers (IL-6, IL-8, IL-10, IL-17, IL-18, TNFα, monocyte chemotactic protein-1 (MCP-1)), acute-phase proteins C-reactive protein (CRP) and serum amyloid-A (SAA), cellular adhesion molecules and blood lipids, glucose and insulin. Following both high-fat test meals, postprandial TAG concentrations rose steadily (P < 0·05) without a decrease by 6 h. The incremental AUC (iAUC) for CRP was significantly lower (P < 0·05) in response to the cheese compared with the vegan-alternative test meal. A treatment effect was not observed for any other inflammatory markers; however, for both test meals, multiple markers significantly changed from baseline over the 6 h postprandial period (IL-6, IL-8, IL-18, TNFα, MCP-1, SAA). Saturated fat in the form of a cheese matrix reduced the iAUC for CRP compared with a vegan-alternative test meal during the postprandial 6 h period.     Cluster randomised trials of prescribing policy:an ethical approach to generating drug safety evidence? A discussion of the ethical application of a new research method

    For most chronic medical conditions, multiple medications are available and prescribers often have limited evidence about which therapy is likely to be the most effective and safe for an individual patient. As many patients are exposed every day to medicines that may be less effective than available alternatives, this is of public health importance. Cluster randomised trials of prescribing policy offer an opportunity to rapidly obtain evidence of comparative effectiveness and safety. These trials can pose a low risk to patients and cause minimal disruption to usual care. Despite the potential scientific value of this approach, there remain valid concerns about consent, medication switching and the use of routinely collected data in research. We discuss these concerns with reference to an ongoing pilot study (Evaluating Diuretics in Normal Care (EVIDENCE) - a cluster randomised evaluation of hypertension prescribing policy, ISRCTN 46635087, registered 11 August 2017)

    Changing trends in the microvascular reconstruction and oral rehabilitation following maxillary cancer

    PurposeThe maxillectomy defect is complex and the best means to achieve optimal reconstruction, and dental rehabilitation is a source of debate. The refinements in zygomatic implant techniques have altered the means and speed by which rehabilitation can be achieved and has also influenced the choice regarding ideal flap reconstruction. The aim of this study is to report on how the method of reconstruction and oral rehabilitation of the maxilla has changed since 1994 in our Institution, and to reflect on case mix and survival.MethodsConsecutive head and neck oncology cases involving maxillary resections over a 27-year period between January 1994 and November 2020 were identified from hospital records and previous studies. Case note review focussed on clinical characteristics, reconstruction, prosthetic rehabilitation, and survival.ResultsThere were 186 patients and the tumour sites were: alveolus for 56% (104), hard palate for 19% (35), maxillary sinus for 18% (34) and nasal for 7% (13). 52% (97) were Brown class 2 defects. Forty-five patients were managed by obturation and 78% (142/183) had free tissue transfer. The main flaps used were radial (52), anterolateral thigh (27), DCIA (22), scapula (13) and fibula (11). There were significant changes over time regarding reconstruction type, use of primary implants, type of dental restoration, and length of hospital stay. Overall survival after 24 months was 64% (SE 4%) and after 60 months was 42% (SE 4%).ConclusionThese data reflect a shift in the reconstruction of the maxillary defect afforded by the utilisation of zygomatic implants
