    Impact of the Global Crisis on Overseas Workers and the Families-Left-Behind: A Snapshot of the Philippine Case

    The Global Economic Crisis (GEC) of 2008-2009 was seen as negatively affecting the Philippines in a different way. Most countries affected by the GEC suffered export declines and domestic economic slowdown leading to the collapse of certain sectors in the economy. Though the Philippines was affected this way in some aspects, it is more the unique nature of its economy having around 10% of its population abroad working or permanently settled but continuing to send a significant amount of remittances. It was the concern of the Philippines that the crisis affecting many of the destination countries of its migrant workers will lead to massive layoffs and creating a much larger crisis at home. Based on this hypothesis, this study attempts to view how the GEC actually affected the economy by directly looking at the impacts on the overseas Filipino workers (OFWs) and their families-left-behind in the Philippines and how they coped with the perceived effects of the crisis. The study sampled the National Capital Region or Metro Manila. It used a semi-structured questionnaire which profiled the demographic characteristics and determined the coping strategies of the respondents. A focus group discussion with ten (10) of the respondents for the purpose of validating the data gathered by the questionnaire was also done. The results show that a number of affected households indicate decreases in their family income during the crisis period as compared to previous years. However, there are relatively few affected workers who returned home. Majority opted to stay abroad hoping for better opportunities in their affected host countries rather than to return home. The families-left-behind also adjusted to the crisis by implementing coping strategies such as cuts in spending (primarily in recreation, food and utilities), seeking extra jobs/sideline and a decline in allocation for savings

    Elevated Baseline Serum Fibrinogen: Effect on 2-Year Major Adverse Cardiovascular Events Following Percutaneous Coronary Intervention.

    BackgroundElevated fibrinogen is associated with short-term major adverse cardiovascular events (MACE) after percutaneous coronary intervention, but the relation with late MACE is unknown.Methods and resultsBaseline demographics and 2-year MACE were recorded among subjects undergoing nonemergent percutaneous coronary intervention. A total of 332 subjects (66.6±19.5 years, 69.9% male, 25.3% acute coronary syndrome) were enrolled. Two-year MACE (periprocedural myocardial infarction 9.0%, rehospitalization 6.3%, revascularization 12.7%, non-periprocedural myocardial infarction 4.5%, stent thrombosis 0.9%, stroke 1.8%, and death 0.6%) were associated with higher fibrinogen (352.8±123.4 mg/dL versus 301.6±110.8 mg/dL; P<0.001), longer total stent length (40.1±25.3 mm versus 32.1±19.3 mm; P=0.004), acute coronary syndrome indication (38.7% versus 17.8%; P<0.001), number of bare-metal stents (0.5±1.1 versus 0.2±0.5; P=0.002), and stent diameter ≤2.5 mm (55.8% versus 38.4%, P=0.003). No relation between platelet reactivity and 2-year MACE was observed. Fibrinogen ≥280 mg/dL (odds ratio [OR] 3.0, confidence interval [CI], 1.6-5.4, P<0.001), total stent length ≥32 mm (OR 2.2, CI, 1.3-3.8, P<0.001), acute coronary syndrome indication (OR 4.1, CI, 2.3-7.5, P<0.001), any bare-metal stents (OR 3.2, CI, 1.6-6.1, P<0.001), and stent diameter ≤2.5 mm (OR 2.0, CI, 1.2-3.5, P=0.010) were independently associated with 2-year MACE. Following a landmark analysis excluding periprocedural myocardial infarction, fibrinogen ≥280 mg/dL remained strongly associated with 2-year MACE (37.0% versus 17.4%, log-rank P<0.001).ConclusionsElevated baseline fibrinogen level is associated with 2-year MACE after percutaneous coronary intervention. Acute coronary syndrome indication for percutaneous coronary intervention, total stent length implanted, and use of bare-metal stents or smaller-diameter stents are also independently associated with 2-year MACE, while measures of on-thienopyridine platelet reactivity are not

    Analysis of the influence of module construction upon forward osmosis performance

    The potential of a commercial forward osmosis (FO) module to recover water from NEWater brine, an RO retentate, was assessed by taking an innovative approach to obtaining the mass transfer coefficients. The performance comparison of the spiral wound (S-W) FO module with that of the flat sheet laboratory unit suggests that the winding involved in S-W construction can adversely affect performance; the values for the S-W mass transfer coefficients were half of those expected. This first-of-its-kind performance comparison utilised coupons of the membrane and spacers taken from the module. The module was used both in the conventional manner for FO and in the reverse manner with the active layer facing the draw solution. Estimates of membrane parameters and mass transfer coefficients experiments for the two orientations were obtained using pure water, 10 mM and 25 mM NaCl solution on the feed side and 1 M NaCl as draw solution. The fouling potential of NEWater brine per se was found to be low. These are the first results with a S-W module that suggest potential for this niche application; nevertheless the level of the water flux through the S-W module clearly indicates that industrial applications of S-W FO will be constrained to special cases

    Transport across nanogaps using semiclassically consistent boundary conditions

    Charge particle transport across nanogaps is studied theoretically within the Schrodinger-Poisson mean field framework and the existence of limiting current investigated. It is shown that the choice of a first order WKB wavefunction as the transmitted wave leads to self consistent boundary conditions and gives results that are significantly different in the non-classical regime from those obtained using a plane transmitted wave. At zero injection energies, the quantum limiting current density, J_c, is found to obey the local scaling law J_c ~ (V_g)^alpha/(D)^{5-2alpha} with the gap separation D and voltage V_g. The exponent alpha > 1.1 with alpha --> 3/2 in the classical regime of small de Broglie wavelengths. These results are consistent with recent experiments using nanogaps most of which are found to be in a parameter regime where classical space charge limited scaling holds away from the emission dominated regime.Comment: 4 pages, 4 ps figure

    Accelerating cycle expansions by dynamical conjugacy

    Periodic orbit theory provides two important functions---the dynamical zeta function and the spectral determinant for the calculation of dynamical averages in a nonlinear system. Their cycle expansions converge rapidly when the system is uniformly hyperbolic but greatly slowed down in the presence of non-hyperbolicity. We find that the slow convergence can be associated with singularities in the natural measure. A properly designed coordinate transformation may remove these singularities and results in a dynamically conjugate system where fast convergence is restored. The technique is successfully demonstrated on several examples of one-dimensional maps and some remaining challenges are discussed

    Systematic review and network meta-analysis of atrial fibrillation percutaneous catheter ablation technologies using randomized controlled trials

    AIMS: We sought out to make comparisons between all atrial fibrillation (AF) catheter ablation technologies using randomized controlled trial data. Our comparisons were freedom from AF, procedural duration, and fluoroscopy duration. METHODS: Searches were made of EMBASE, MEDLINE, and CENTRAL databases, and studies were selected which had cryoballoon, conventional radiofrequency (RF), multipolar RF catheters, and laser technology as an arm in the study and were identified as randomized controlled trials (RCTs). These studies were analyzed for direct comparisons using conventional meta-analysis and a combination of indirect and direct comparisons via a network meta-analysis (NMA). RESULTS: With respect to freedom from AF both direct comparisons and NMA did not demonstrate any significant difference. However in analysis of procedural and fluoroscopy duration (minutes) for the pulmonary vein ablation catheter (PVAC), both conventional analysis and NMA revealed significantly shorter procedure times, RF vs PVAC (conventional: 61.99 [38.03-85.94], P <.00001; NMA: 54.76 [36.64-72.88], P < .0001) and fluoroscopy times, RF vs PVAC (conventional: 12.96 [6.40-19.53], P = .0001; NMA: 8.89 [3.27-14.51], P < .01). The procedural duration was also shorter for the cryoballoon with NMA, RF vs CRYO (20.56 [3.47-37.65], P = .02). DISCUSSION: Our analysis demonstrated that while there was no difference in the efficacy of the individual catheter technologies, there are significant differences in the procedural duration for the PVAC and the cryoballoon. While they may seem an attractive solution for high-volume centers, further RCTs of next-generation technologies should be examined

    Candidemia from a Urinary Tract Source: Microbiological Aspects and Clinical Significance

    Twenty-six cases of candidemia associated with a well-defined urinary tract source were retrospectively identified and reviewed. Urinary tract abnormalities were present in 23 of 26 patients (88%), 19 (73%) of whom had urinary tract obstruction. Nineteen patients had undergone urinary tract procedures before the onset of candidemia. Episodes of candidemia were brief and low-grade in intensity (median duration, 1 day; median colony count, 1.5 cfu/10 mL of blood). Only eight patients (31%) received ⩾500 mg of amphotericin B. There were five in-hospital deaths (19%); two of these deaths were attributed to candidiasis. No late complications of candidemia were documented for the surviving patients. Patients with urologic pathology and candiduria who undergo surgery or manipulation of the urinary tract are at significant risk for candidemia, and further studies should examine the issue of administration of prophylaxis to this grou

    Moment inversion problem for piecewise D-finite functions

    We consider the problem of exact reconstruction of univariate functions with jump discontinuities at unknown positions from their moments. These functions are assumed to satisfy an a priori unknown linear homogeneous differential equation with polynomial coefficients on each continuity interval. Therefore, they may be specified by a finite amount of information. This reconstruction problem has practical importance in Signal Processing and other applications. It is somewhat of a ``folklore'' that the sequence of the moments of such ``piecewise D-finite''functions satisfies a linear recurrence relation of bounded order and degree. We derive this recurrence relation explicitly. It turns out that the coefficients of the differential operator which annihilates every piece of the function, as well as the locations of the discontinuities, appear in this recurrence in a precisely controlled manner. This leads to the formulation of a generic algorithm for reconstructing a piecewise D-finite function from its moments. We investigate the conditions for solvability of the resulting linear systems in the general case, as well as analyze a few particular examples. We provide results of numerical simulations for several types of signals, which test the sensitivity of the proposed algorithm to noise
