The frontier problems of the Heracleian Dynasty with special reference to the second half of the seventh century

The Heracleians were the rulers of the Byzantine Empire during a particularly interesting period. The seventh century was an age of invasions and, in this respect, it did not differ greatly from much which had preceded it. But the Heracleians, in the course of their efforts to defend the empire against the Slavs, the Persians and the Arabs, were forced to introduce many radical changes. The empire which they succeeded in preserving was very different from that which they had inherited from the successors of Justinian. This development is the salient feature of Byzantium in the seventh century and is directly connected with frontier problems of the Heracleians. The reigns of Heraclius himself, of Constans II, of Constantine IV and of Justinian II almost span this century and their work needs to be treated as a whole since it played a special part in Byzantine history. It i s in this light that the available material has been considered, and, while it is not claimed that anything new has been discovered, such an approach has, at some points, led to a somewhat different interpretation of what has been widely used in the past by others.Both the primary and the secondary material for this period are to be found in a variety of languages and scripts (1) with consequent problems in citation, and it has not proved practicable to follow throughout one uniform method. Names of Greek authors and titles of Greek works are, as is the usual custom, given in Latin wherever there is an accepted Latin form, otherwise they are left in the original. Names of Russian authors and titles of Russian works have been transliterated according to the system recommended by the School of Slavonic and East European Studies, University of London. Arabic,Syriac, Armenian, Georgian and Ethiopic texts, however, have had to be read only in translation and the methods of transliteration have therefore normally been those of the editions in which these texts appear. In the case of Arabic, however, an attempt at consistency, based upon the system of Professor P. K. Hitti, has been made possible through the valuable advice of Dr. W. J. Martin, Rankin Lecturer in Hebrew and Ancient Semitic Studies in the University of Liverpool. Names of places that have acquired a customary English spelling have been left in that form. Quotations have been given in English except where a purpose could be served by preserving the original.<p

The effects of rational and irrational coach team talks on the cognitive appraisal and achievement goal orientation of varsity football athletes

The effects of rational and irrational coach team talks on cognitive appraisal and achievement goal orientation were examined. During the half-time interval of a 60-minute football match, 25 male varsity football athletes (Mage = 20.20; SD ± 1.38 years) received a rational (n = 13) or an irrational (n = 12) team talk from a coach. Irrational and rational beliefs were measured before the football match. Task engagement, cognitive appraisal (challenge and threat), and achievement goal orientation (approach and avoidance) regarding second-half football performance were measured following team-talk delivery. Athletes in the rational team talk condition reported significantly lower threat appraisal and avoidance goal orientation than athletes in the irrational team talk condition. No significant between-condition differences emerged for challenge appraisal and approach goal orientation. For coaching practice, data suggest that communicating rational or irrational beliefs to football athletes through a half time team talk will influence appraisal and achievement goal orientation regarding upcoming performance. Bone Marrow B cell Precursor Number after Allogeneic Stem Cell Transplantation and GVHD Development

Patients without chronic graft-versus-host disease (cGVHD) have robust B cell reconstitution and are able to maintain B cell homeostasis after allogeneic hematopoietic stem cell transplantation (HSCT). To determine whether B lymphopoiesis differs before cGVHD develops, we examined bone marrow (BM) biopsies for terminal deoxynucleotidyl transferase (TdT) and PAX5 immunostaining early post-HSCTat day 30 when all patients have been shown to have high B cell activating factor (BAFF) levels. We found significantly greater numbers of BM B cell precursors in patients who did not develop cGVHD compared with those who developed cGVHD (median = 44 vs 2 cells/high powered field [hpf]; respectively; P < .001). Importantly, a significant increase in precursor B cells was maintained when patients receiving high-dose steroid therapy were excluded (median = 49 vs 20 cells/hpf; P =.017). Thus, we demonstrate the association of BM B cell production capacity in human GVHD development. Increased BM precursor B cell number may serve to predict good clinical outcome after HSCT

Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P &lt; 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Design of a Day/Night Lunar Rover

The pair of lunar rovers discussed in this report will return video and state data to various ventures, including theme park and marketing concerns, science agencies, and educational institutions. The greatest challenge accepted by the design team was to enable operations throughout the extremely cold and dark lunar night, an unprecedented goal in planetary exploration. This is achieved through the use of the emerging technology of Alkali Metal Thermal to Electric Converters (AMTEC), provided with heat from a innovative beta-decay heat source, Krypton-85 gas. Although previous space missions have returned still images, our design will convey panoramic video from a ring of cameras around the rover. A six-wheel rocker bogie mechanism is implemented to propel the rover. The rovers will also provide the ability to safeguard their operation to allow untrained members of the general public to drive the vehicle.
