    Diagnosis and management of iridocorneal endothelial syndrome

    The iridocorneal endothelial (ICE) syndrome is a rare ocular disorder that includes a group of conditions characterized by structural and proliferative abnormalities of the corneal endothelium, the anterior chamber angle, and the iris. Common clinical features include corneal edema, secondary glaucoma, iris atrophy, and pupillary anomalies, ranging from distortion to polycoria. The main subtypes of this syndrome are the progressive iris atrophy, the Cogan-Reese syndrome, and the Chandler syndrome. ICE syndrome is usually diagnosed in women in the adult age. Clinical history and complete eye examination including tonometry and gonioscopy are necessary to reach a diagnosis. Imaging techniques, such as in vivo confocal microscopy and ultrasound biomicroscopy, are used to confirm the diagnosis by revealing the presence of "ICE-cells" on the corneal endothelium and the structural changes of the anterior chamber angle. An early diagnosis is helpful to better manage the most challenging complications such as secondary glaucoma and corneal edema. Treatment of ICE-related glaucoma often requires glaucoma filtering surgery with antifibrotic agents and the use of glaucoma drainage implants should be considered early in the management of these patients. Visual impairment and pain associated with corneal edema can be successfully managed with endothelial keratoplasty

    Spatially inhomogeneous evolutionary games

    We introduce and study a mean-field model for a system of spatially distributed players interacting through an evolutionary game driven by a replicator dynamics. Strategies evolve by a replicator dynamics influenced by the position and the interaction between different players and return a feedback on the velocity field guiding their motion. One of the main novelties of our approach concerns the description of the whole system, which can be represent-dimensional state space (pairs (x, σ) of position and distribution of strategies). We provide a Lagrangian and a Eulerian description of the evolution, and we prove their equivalence, together with existence, uniqueness, and stability of the solution. As a byproduct of the stability result, we also obtain convergence of the finite agents model to our mean-field formulation, when the number N of the players goes to infinity, and the initial discrete distribution of positions and strategies converge. To this aim we develop some basic functional analytic tools to deal with interaction dynamics and continuity equations in Banach spaces that could be of independent interest. © 2021 The Authors.     Lavender Autonomous Navigation with Semantic Segmentation at the Edge

    Achieving success in agricultural activities heavily relies on precise navigation in row crop fields. Recently, segmentation-based navigation has emerged as a reliable technique when GPS-based localization is unavailable or higher accuracy is needed due to vegetation or unfavorable weather conditions. It also comes in handy when plants are growing rapidly and require an online adaptation of the navigation algorithm. This work applies a segmentation-based visual agnostic navigation algorithm to lavender fields, considering both simulation and real-world scenarios. The effectiveness of this approach is validated through a wide set of experimental tests, which show the capability of the proposed solution to generalize over different scenarios and provide highly-reliable results

    Enhancing Navigation Benchmarking and Perception Data Generation for Row-based Crops in Simulation

    Service robotics is recently enhancing precision agriculture enabling many automated processes based on efficient autonomous navigation solutions. However, data generation and infield validation campaigns hinder the progress of large-scale autonomous platforms. Simulated environments and deep visual perception are spreading as successful tools to speed up the development of robust navigation with low-cost RGB-D cameras. In this context, the contribution of this work is twofold: a synthetic dataset to train deep semantic segmentation networks together with a collection of virtual scenarios for a fast evaluation of navigation algorithms. Moreover, an automatic parametric approach is developed to explore different field geometries and features. The simulation framework and the dataset have been evaluated by training a deep segmentation network on different crops and benchmarking the resulting navigation.Comment: Accepted at the 14th European Conference on Precision Agriculture (ECPA) 202

    Malignant epithelioid neoplasm of the ileum with ACTB-GLI1 fusion mimicking an adnexal mass

    Background: Malignant epithelioid neoplasm with ACTB-GLI1 fusion are considered different from the more common pericytic lesions, such myopericytoma, because they have a spectrum of different genetic abnormalities. They appear to pursue a benign clinical course in young adults, although in sporadic cases lymph node metastasis were described. The categorization of this new type of tumor may also lead to new therapeutic strategies, because they might be sensitive to SHH pathway inhibitors. Case presentation: The case involves a 72-years-old multiparous woman who accessed our department after an incidental finding of a right adnexal mass of 43 mm with contrast-enhancement on a control computed tomography scan made for suspected diverticulitis. Our intervention was a detailed ultrasound description of the suspected neoplasm; a diagnostic laparoscopy and the contextual laparotomic removal of abdominal mass; its histological and immunohistochemical analysis. Our main outcome measure is the definition and future recognition of new pathologic entity called malignant epithelioid neoplasm with ACTB-GLI1 fusion. Conclusions: We described for the first time the ultrasound characteristic of this type of lesion using standardized terminology and we believe that it may be the first step to improve the acknowledgement of this novel pathologic entity defined as malignant epithelioid neoplasm with GLI-1 fusions

    Fertility-Sparing Treatment for Early-Stage Cervical Cancer ≥ 2 cm: A Problem with a Thousand Nuances—A Systematic Review of Oncological Outcomes.

    BACKGROUND: Fertility-sparing treatments (FSTs) have played a crucial role in the management of early-stage cervical cancer (ECC). The guidelines have recognized various approaches, depending on the tumor stage and other risk factors such as histotype and lymphovascular positivity. Much more debate has centered around the boundary within which these treatments should be considered. Indeed, these are methods to be reserved for ECC, but tumor size may represent the most significant limitation. In particular, there is no consensus on the strategy to be adopted in the case of ECC ≥ 2 cm. Therefore, this systematic review was to collect the literature evidence regarding the management of these patients. METHODS: Following the recommendations in the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement, we systematically searched the Pubmed and Scopus databases was conducted in April 2022, from the date of the first publication. We made no limitation on the country. We included all studies containing data on disease-free survival, overall survival, recurrence rate (RR), or complete response rate (CRR) to chemotherapy. RESULTS: Twenty-six studies fulfilled the inclusion criteria, and 691 patients were analyzed regarding FST. Surgery-based FST showed an RR of between 0 and 42.9%, which drops to 12.9% after excluding the vaginal or minimally invasive approaches. Furthermore, papers regarding FST based on the neoadjuvant chemotherapy (NACT) approach showed a CRR of between 21.4 and 84.5%, and an RR of between 0 and 22.2% CONCLUSION: This paper focused on the significant heterogeneity present in the clinical management of FST of ECC ≥ 2 cm. Nevertheless, from an oncological point of view, approaches limited to the minimally invasive or vaginal techniques showed the highest RR. Vice versa, the lack of standardization of NACT schemes and the wealth of confounders to be attributed to the histological features of the tumor make it difficult, if not impossible, to set a standard of treatment.
