    Effects of adenotonsillectomy on plasma inflammatory biomarkers in obese children with obstructive sleep apnea: A community-based study.

    BackgroundObesity and obstructive sleep apnea syndrome (OSA) are highly prevalent and frequently overlapping conditions in children that lead to systemic inflammation, the latter being implicated in the various end-organ morbidities associated with these conditions.AimTo examine the effects of adenotonsillectomy (T&A) on plasma levels of inflammatory markers in obese children with polysomnographically diagnosed OSA who were prospectively recruited from the community.MethodsObese children prospectively diagnosed with OSA, underwent T&A and a second overnight polysomnogram (PSG) after surgery. Plasma fasting morning samples obtained after each of the two PSGs were assayed for multiple inflammatory and metabolic markers including interleukin (IL)-6, IL-18, plasminogen activator inhibitor-1 (PAI-1), monocyte chemoattractant protein-1 (MCP-1), matrix metalloproteinase-9 (MMP-9), adiponectin, apelin C, leptin and osteocrin.ResultsOut of 122 potential candidates, 100 obese children with OSA completed the study with only one-third exhibiting normalization of their PSG after T&A (that is, apnea-hypopnea index (AHI) ≤1/hour total sleep time). However, overall significant decreases in MCP-1, PAI-1, MMP-9, IL-18 and IL-6, and increases in adropin and osteocrin plasma concentrations occurred after T&A. Several of the T&A-responsive biomarkers exhibited excellent sensitivity and moderate specificity to predict residual OSA (that is, AHI⩾5/hTST).ConclusionsA defined subset of systemic inflammatory and metabolic biomarkers is reversibly altered in the context of OSA among community-based obese children, further reinforcing the concept on the interactive pro-inflammatory effects of sleep disorders such as OSA and obesity contributing to downstream end-organ morbidities

    Virtual acoustics of the Cathedral of Malaga (Spain)

    The Catholic Cathedral of Malaga (southern Spain), located in the historical centre, is the most emblematic building of the city. Its construction began in 1528 and was completed in 1782, suffering several interruptions and modifications at the hands of various architects, and hence various architectural styles are superimposed: the interior is Renaissance and the façade is predominantly Baroque. Rectangular in shape, it has three naves of the same height, with the central nave being the widest. In this work, a 3D model of this ecclesiastical space is created in order to carry out an acoustic simulation of its sound field by using the simulation algorithms of CATT Acoustic software implemented in the new TUCT calculation motor (The Universal Cone Tracer). The virtual model created is calibrated through an iterative process of adjustment of reverberation times simulated in such a way that they differ by no more than 5% from those measured. These measured values were obtained from the impulse responses monitored in situ. Likewise, a comparison of the experimental and simulated results of other acoustic parameters in terms of their just noticeable differences (JND) was also carried out, which supports the reliability of the computational acoustic model implemented.     The multicomponent 2D Toda hierarchy: Discrete flows and string equations

    The multicomponent 2D Toda hierarchy is analyzed through a factorization problem associated to an infinite-dimensional group. A new set of discrete flows is considered and the corresponding Lax and Zakharov--Shabat equations are characterized. Reductions of block Toeplitz and Hankel bi-infinite matrix types are proposed and studied. Orlov--Schulman operators, string equations and additional symmetries (discrete and continuous) are considered. The continuous-discrete Lax equations are shown to be equivalent to a factorization problem as well as to a set of string equations. A congruence method to derive site independent equations is presented and used to derive equations in the discrete multicomponent KP sector (and also for its modification) of the theory as well as dispersive Whitham equations.Comment: 27 pages.     We present a method to characterize and compute the large N formal asymptotics of regular and critical Hermitian matrix models with general even potentials in the one-cut and two-cut cases. Our analysis is based on a method to solve continuum limits of the discrete string equation which uses the resolvent of the Lax operator of the underlying Toda hierarchy. This method also leads to an explicit formulation, in terms of coupling constants and critical parameters, of the members of the Painlev\'e I and Painlev\'e II hierarchies associated with one-cut and two-cut critical models respectively

    Fourier transform mid infrared spectroscopy applications for monitoring the structural plasticity of plant cell walls

    [EN] Fourier transform mid-infrared (FT-MIR) spectroscopy has been extensively used as a potent, fast and non-destructive procedure for analyzing cell wall architectures, with the capacity to provide abundant information about their polymers, functional groups, and in muro entanglement. In conjunction with multivariate analyses, this method has proved to be a valuable tool for tracking alterations in cell walls.     Monitoring Coxiella burnetii Infection in Naturally Infected Dairy Sheep Flocks Throughout Four Lambing Seasons and Investigation of Viable Bacteria.

    Progression of Coxiella burnetii infection in four naturally infected sheep flocks, and in their farm environment, was monitored throughout four lambing seasons. Flocks with an active infection were selected based on the presence of C. burnetii DNA in bulk-tank milk (BTM) and a high seroprevalence in yearlings during the previous milking period (Spring 2015). During four consecutive lambing seasons (2015/16-2018/19), samples were collected within 1 week after each lambing period from animals (vaginal swabs, milk and feces from ewes, and yearlings) and the environment (dust indoor sheep premises). BTM samples and aerosols (outdoors and indoors) were monthly collected between lambing and the end of milking. Real-time PCR analyses showed different trends in C. burnetii shedding in the flocks, with a general progressive decrease in bacterial shedding throughout the years, interrupted in three flocks by peaks of reinfection associated with specific management practices. A significant relationship was found between C. burnetii fecal shedding and the bacterial burden detected in dust, whereas shedding by vaginal route affected the detection of C. burnetii in indoor aerosols. Three genotypes were identified: SNP8 (three flocks, 52.9% of the samples), SNP1 (two flocks, 44.8% samples), and SNP5 (one flock, two environmental samples). Coxiella burnetii viability in dust measured by culture in Vero cells was demonstrated in two of the flocks, even during the fourth lambing season.     Multiscale entropy analysis of unattended oximetric recordings to assist in the screening of paediatric sleep apnoea at home

    Producción CientíficaUntreated paediatric obstructive sleep apnoea syndrome (OSAS) can severely affect the development and quality of life of children. In-hospital polysomnography (PSG) is the gold standard for a definitive diagnosis though it is relatively unavailable and particularly intrusive. Nocturnal portable oximetry has emerged as a reliable technique for OSAS screening. Nevertheless, additional evidences are demanded. Our study is aimed at assessing the usefulness of multiscale entropy (MSE) to characterise oximetric recordings. We hypothesise that MSE could provide relevant information of blood oxygen saturation (SpO2) dynamics in the detection of childhood OSAS. In order to achieve this goal, a dataset composed of unattended SpO2 recordings from 50 children showing clinical suspicion of OSAS was analysed. SpO2 was parameterised by means of MSE and conventional oximetric indices. An optimum feature subset composed of five MSE-derived features and four conventional clinical indices were obtained using automated bidirectional stepwise feature selection. Logistic regression (LR) was used for classification. Our optimum LR model reached 83.5% accuracy (84.5% sensitivity and 83.0% specificity). Our results suggest that MSE provides relevant information from oximetry that is complementary to conventional approaches.
