    CAGIRE: a wide-field NIR imager for the COLIBRI 1.3 meter robotic telescope

    The use of high energy transients such as Gamma Ray Bursts (GRBs) as probes of the distant universe relies on the close collaboration between space and ground facilities. In this context, the Sino-French mission SVOM has been designed to combine a space and a ground segment and to make the most of their synergy. On the ground, the 1.3 meter robotic telescope COLIBRI, jointly developed by France and Mexico, will quickly point the sources detected by the space hard X-ray imager ECLAIRs, in order to detect and localise their visible/NIR counterpart and alert large telescopes in minutes. COLIBRI is equipped with two visible cameras, called DDRAGO-blue and DDRAGO-red, and an infrared camera, called CAGIRE, designed for the study of high redshift GRBs candidates. Being a low-noise NIR camera mounted at the focus of an alt-azimutal robotic telescope imposes specific requirements on CAGIRE. We describe here the main characteristics of the camera: its optical, mechanical and electronics architecture, the ALFA detector, and the operation of the camera on the telescope. The instrument description is completed by three sections presenting the calibration strategy, an image simulator incorporating known detector effects, and the automatic reduction software for the ramps acquired by the detector.     Familial hypocalciuric hypercalcemia associated with crystal deposition disease.

    International audienceChondrocalcinosis is a common disease occasionally associated with hypercalcemia in case of primary hyperparathyroidism. Familial Hypocalciuric Hypercalcemia (FHH) is a rare and almost always asymptomatic condition, due to an autosomal dominant mutation of the calcium-sensing receptor gene. We report the case of a 61-year-old female with chronic hypercalcemia and joint pain. Clinical and biological data revealed chondrocalcinosis associated with FHH. Since primary hyperparathyroidism may mimic FHH, calcium to creatinine clearance ratio should be calculated in every case to avoid a wrong diagnosis and useless parathyroid surgery. The paucity of FFH complications, including chondrocalcinosis, makes their study difficult: additional studies are needed to clearly evaluate the link between FHH and chondrocalcinosis

    The port governance is still a fascinating theme that has been evolving for 3 millennia according to influences that can be reformative, conservative, dogmatic or even liberal. The purpose of this research is to investigate port governance through the use of the key concept of compromise by questioning the capacity of public, private and civil stakeholders to develop managerial tools in a collaborative perspective. The theory of social regulation and pragmatic sociology enable to consolidate our conceptual framework. The Port Authority of Le Havre – France and the Port Authority of Abidjan – Ivory Coast are the two research fields that were investigated through semi-directive interviews. The main findings of the research are consolidated in a unique managerial and strategic toolbox with five key steps for operationalizing the practice of port governance by using the compromise.La gouvernance portuaire évolue depuis trois millénaires au gré d’influences qui peuvent être réformatrices, conservatrices, dogmatiques ou encore libérales. L’objet de cette recherche est d’analyser la gouvernance des ports au moyen du concept clé de compromis. Il s’agira d’interroger la capacité des parties prenantes publique, privée et civile à développer des outils managériaux dans une perspective participative. La théorie de la régulation sociale et la sociologique pragmatique ont permis de consolider les fondements conceptuels de la démarche. Les ports du Havre en France et d’Abidjan en Côte d’Ivoire constituent les deux terrains de recherche qui ont été investis. Des entretiens semi-directifs ont été conduits auprès des acteurs leaders. Les principaux résultats du travail d’analyse sont synthétisés dans une boîte à outils managériale et stratégique. Celle-ci identifie cinq étapes clés qui mobilisent le compromis dans l’opérationnalisation de la gouvernance portuaire

    Starvation ketoacidosis during prolonged fasting of 26 days

    International audienceKetosis is a metabolic situation involving an increase in blood and urine concentrations of ketones that, when prolonged, leads to acidosis. Moderate ketosis usually appears after a fast of a few hours, but its prolongation exposes to hyperketosis. Observation A 25-year-old woman presented to the emergency department for cohercitive vomiting. She was fasting for a long time in a spiritual setting and had a restricted diet limited to water and vitamin supplements. Clinical and biological assessment was in favour of fasting ketoacidosis. Evolution was favorable with intravenous hydration, poly-ionic and micronutrient supplementation and a gradual resumption of oral feeding. Conclusion We report the case of a patient with fasting ketoacidosis. Besides consequences of this ketoacidosis, the challenge was also in resuming oral feeding in order to avoid a potentially fatal inappropriate renutrition syndrome

    Pharmacist-led medication reconciliation at patient discharge: a tool to reduce healthcare utilization? an observational study in patients 65 years or older

    International audienceAbstract Background Older patients often experience adverse drug events (ADEs) after discharge that may lead to unplanned readmission. Medication Reconciliation (MR) reduces medication errors that lead to ADEs, but results on healthcare utilization are still controversial. This study aimed to assess the effect of MR at discharge (MRd) provided to patients aged over 65 on their unplanned rehospitalization within 30 days and on both patients’ experience of discharge and their knowledge of their medication. Methods An observational multicenter prospective study was conducted in 5 hospitals in Brittany, France. Results Patients who received both MR on admission (MRa) and MRd did not have significantly fewer deaths, unplanned rehospitalizations and/or emergency visits related to ADEs (OR = 1.6 [0.7 to 3.6]) or whatever the cause ( p = 0.960) 30 days after discharge than patients receiving MRa alone. However, patients receiving both MRa and MRd were more likely to feel that their discharge from the hospital was well organized ( p = 0.003) and reported more frequently that their community pharmacist received information about their hospital stay ( p = 0.036). Conclusions This study found no effect of MRd on healthcare utilization 30 days after discharge in patients over 65, but the process improved patients’ experiences of care continuity. Further studies are needed to better understand this positive impact on their drug care pathway in order to improve patients’ ownership of their drugs, which is still insufficient. Improving both the interview step between pharmacist and patient before discharge and the transmission of information from the hospital to primary care professionals is needed to enhance MR effectiveness.
