Contrastación de hipótesis en diseños multivariados split-plot con matrices de dispersión arbitrarias
El presente trabajo examina diversos procedimientos para contrastar hipótesis nulas globales, correspondientes a datos obtenidos mediante diseños multivariados split-plot cuando se incumple el supuesto de homogeneidad de las matrices de dispersión. Un examen de estos procedimientos para un amplio número de variables confirma, por un lado, la robustez del procedimiento multivariado de Welch-James dado por Johansen (1980) para probar el efecto principal de los ensayos y, por otro, la robustez de la generalización multivariada del procedimiento de Brown-Forsythe (1974) para probar la interacción de los grupos x los ensayos. Nuestros resultados también ponen de relieve que las diferencias de potencia eran pequeñas en aquellas condiciones en que tanto el procedimiento de Welch-James como de Brown-Forsythe controlaban las tasas de error de Tipo I
EPIC-DB: a proteomics database for studying Apicomplexan organisms
<p>Abstract</p> <p>Background</p> <p>High throughput proteomics experiments are useful for analyzing the protein expression of an organism, identifying the correct gene structure of a genome, or locating possible post-translational modifications within proteins. High throughput methods necessitate publicly accessible and easily queried databases for efficiently and logically storing, displaying, and analyzing the large volume of data.</p> <p>Description</p> <p>EPICDB is a publicly accessible, queryable, relational database that organizes and displays experimental, high throughput proteomics data for <it>Toxoplasma gondii </it>and <it>Cryptosporidium parvum</it>. Along with detailed information on mass spectrometry experiments, the database also provides antibody experimental results and analysis of functional annotations, comparative genomics, and aligned expressed sequence tag (EST) and genomic open reading frame (ORF) sequences. The database contains all available alternative gene datasets for each organism, which comprises a complete theoretical proteome for the respective organism, and all data is referenced to these sequences. The database is structured around clusters of protein sequences, which allows for the evaluation of redundancy, protein prediction discrepancies, and possible splice variants. The database can be expanded to include genomes of other organisms for which proteome-wide experimental data are available.</p> <p>Conclusion</p> <p>EPICDB is a comprehensive database of genome-wide <it>T. gondii </it>and <it>C. parvum </it>proteomics data and incorporates many features that allow for the analysis of the entire proteomes and/or annotation of specific protein sequences. EPICDB is complementary to other -genomics- databases of these organisms by offering complete mass spectrometry analysis on a comprehensive set of all available protein sequences.</p
Quality check: concordance between two monitoring systems for postoperative organ/space-surgical site infections in rectal cancer surgery. Linkage of data from the Catalan Cancer Plan and the VINCat infection surveillance programme
Background The Catalan Cancer Plan (CCP) undertakes periodic audits of cancer treatment outcomes, including organ/space surgical site infections (O/S-SSI) rates, while the Catalan Healthcare-associated Infections Surveillance Programme (VINCat) carries out standardized prospective surveillance of surgical site infections (SSI) in colorectal surgery. This cohort study aimed to assess the concordance between these two monitoring systems for O/S-SSI following primary rectal cancer surgery. Methods The study compared O/S-SSI incidence data from CCP clinical audits versus the VINCat Programme in patients undergoing surgery for primary rectal cancer, in 2011-12 and 2015-16, in publicly funded centres in Spain. The main outcome variable was the incidence of O/S-SSI in the first 30 days after surgery. Concordance between the two registers was analysed using Cohen's kappa. Discordant cases were reviewed by an expert, and the main reasons for discrepancies evaluated. Results Pooling data from both databases generated a sample of 2867 patients. Of these, O/S-SSI was detected in 414 patients-235 were common to both registry systems, with satisfactory concordance (kappa = 0.69, 95% confidence interval 0.65-0.73). The rate of discordance from the CCP (positive cases in VINCat and negative in CCP) was 2.7%, and from VINCat (positive in CCP and negative in VINCat) was 3.6%. External review confirmed O/S-SSI in 66.2% of the cases in the CCP registry and 52.9% in VINCat. Conclusions This type of synergy shows the potential of pooling data from two different information sources with a satisfactory level of agreement as a means to improving O/S-SSI detection. A propensity-score-matched analysis of laparoscopic vs open surgery for rectal cancer in a population-based study
Aim: the oncological risk/benefit trade-off for laparoscopy in rectal cancer is controversial. Our aim was to compare laparoscopic vs open surgery for resection of rectal cancer, using unselected data from the public healthcare system of Catalonia. Methods: this was a multicentre retrospective cohort study of all patients who had surgery with curative intent for primary rectal cancer at Catalonian public hospitals from 2011 to 2012. We obtained follow-up data for up to 5 years. To minimize the differences between the two groups, we performed propensity score matching on baseline patient characteristics. We used multivariate Cox proportional hazards regression analyses to assess locoregional relapse at 2 years and death at 2 and 5 years. Results: of 1513 patients with Stage I-III rectal cancer, 933 (61.7%) had laparoscopy (conversion rate 13.2%). After applying our propensity score matching strategy (2:1), 842 laparoscopy patients were matched to 517 open surgery patients. Multivariate Cox analysis of death at 2 years [hazard ratio (HR) 0.65, 95% CI 0.48, 0.87; P = 0.004] and 5 years (HR 0.61, 95% CI 0.5, 0.75; P < 0.001) and of local relapse at 2 years (HR 0.44, 95% CI 0.27, 0.72; P = 0.001) showed laparoscopy to be an independent protective factor compared with open surgery. Cconclusions: laparoscopy results in lower locoregional relapse and long-term mortality in rectal cancer in unselected patients with all-risk groups included. Studies using long-term follow-up of cohorts and unselected data can provide information on clinically relevant outcomes to supplement randomized controlled trials
Clinical Audit of the Radiotherapy Process in Rectal Cancer: Clinical Practice guidelines and Quality Certification Do Not Avert Variability in Clinical Practice
Background: The therapeutic approach to cancer is complex and multidisciplinary. Radiotherapy is among the essential treatments, whether used alone or in conjunction with other therapies. This study reports a clinical audit of the radiotherapy process to assess the process of care, evaluate adherence to agreed protocols and measure the variability to improve therapeutic quality for rectal cancer. Methods: Multicentre retrospective cohort study in a representative sample of patients diagnosed with rectal cancer in the Institut Català d'Oncologia, a comprehensive cancer centre with three different settings. We developed a set of indicators to assess the key areas of the radiotherapy process. The clinical audit consisted of a review of a random sample of 40 clinical histories for each centre. Results: The demographic profile, histology and staging of patients were similar between centres. The MRI reports did not include the distance from tumour to mesorectal fascia (rCRM) in 38.3% of the cases. 96.7% of patients received the planned dose, and 57.4% received it at the planned time. Surgery followed neoadjuvant treatment in 96.7% of the patients. Among this group, postoperative CRM was recorded in 65.5% of the cases and was negative in 93.4% of these. With regard to the 34.5% (n = 40) of cases where no CRM value was stated, there were differences between the centres. Mean follow-up was 3.4 (SD 0.6) years, and overall survival at four years was 81.7%. Conclusions: The audit revealed a suboptimal degree of adherence to clinical practice guidelines. Socioeconomic Status and Distance to Reference Centers for Complex Cancer Diseases: A Source of Health Inequalities? A Population Cohort Study Based on Catalonia (Spain)
The centralization of complex surgical procedures for cancer in Catalonia may have led to geographical and socioeconomic inequities. In this population-based cohort study, we assessed the impacts of these two factors on 5-year survival and quality of care in patients undergoing surgery for rectal cancer (2011-12) and pancreatic cancer (2012-15) in public centers, adjusting for age, comorbidity, and tumor stage. We used data on the geographical distance between the patients' homes and their reference centers, clinical patient and treatment data, income category, and data from the patients' district hospitals. A composite 'textbook outcome' was created from five subindicators of hospitalization. We included 646 cases of pancreatic cancer (12 centers) and 1416 of rectal cancer (26 centers). Distance had no impact on survival for pancreatic cancer patients and was not related to worse survival in rectal cancer. Compared to patients with medium-high income, the risk of death was higher in low-income patients with pancreatic cancer (hazard ratio (HR) 1.46, 95% confidence interval (CI) 1.15-1.86) and very-low-income patients with rectal cancer (HR 5.14, 95% CI 3.51-7.52). Centralization was not associated with worse health outcomes in geographically dispersed patients, including for survival. However, income level remained a significant determinant of survival
Tracking kelp-type seaweed fuel in the archaeological record through Raman spectroscopy of charred particles: examples from the Atacama Desert coast
The use of seaweed as fuel has been mentioned in ethnographic and historical sources of different coastal regions. Nevertheless, the archaeological record of seaweed burning is still limited to contexts where preservation is exceptional and macroscopic discrimination of charred remains is possible. Here, we evaluate the effectiveness of Raman spectroscopy in discriminating seaweed vs. plant/wood char. Our dataset (N = 92) consists of modern and archaeological seaweed and plant/wood charred remains, including specimens of unknown origin from the Atacama Desert coast, Northern Chile. The charred samples were processed to obtain 13 parameters which were then fed into five supervised machine learning models. The models, built on samples of known origin (seaweed and plant/wood), performed remarkably well in terms of accuracy, kappa, sensitivity, and specificity. The models were used for final predictions on 10 non-identified archaeological charcoals. Our results suggest that Raman spectroscopy combined with machine learning techniques is a robust methodology for discriminating seaweed and plant/wood charred remains in the archaeological record. The predictions on unknown samples confirm that seaweed was used as fuel in a specific funerary ritual in the southern Atacama Desert coast around 5000 cal BP. Furthermore, charred specimens of Lessonia spp. recovered from combustion features in other northern Chile coastal settlements, suggest that seaweed pyrotechnology developed by Atacama Desert coast people is likely a long-term process. We study deviation of ergodic averages for dynamical systems given by
self-similar tilings on the plane and in higher dimensions. The main object of
our paper is a special family of finitely-additive measures for our systems. An
asymptotic formula is given for ergodic integrals in terms of these
finitely-additive measures, and, as a corollary, limit theorems are obtained
