    O objeto do presente artigo é fomentar a analise de uma perspectiva para o desenvolvimento e implantação de um sistema de emissão de diplomas baseado em certificação digital na, pela inovação na gestão universitária, através do uso da tecnologia de certificação digital. Essa ferramenta de gestão, que necessita de um estudo de análise sociotécnica de viabilidade para implantação, tem por finalidade a otimização de processos e agregar segurança ao processo de emissão de diplomas da UFSC. A proposta de valor desta pesquisa contempla o estudo dos requisitos mínimos, no que tangem a tecnologia, as pessoas e os processos, para auxiliar a administração universitária na tomada de decisão quanto a implantação da certificação digital no processo de emissão de diplomas na UFSC. A busca pela qualidade no diploma está contemplado na proposta de valor supracitada, com melhoria na eficiência do processo, na inovação da gestão pela difusão da tecnologia, e oportunizará agregar confiabilidade, agilidade e segurança aos diplomas emitidos pela UFSC. O diploma com certificação digital, como estratégia para divulgação e difusão do uso da certificação digital na comunidade universitária, é outro ponto relevante na proposta de valor desta pesquisa

    High blood pressure in overweight children and adolescents

    OBJETIVO: verificar a prevalência de pressão arterial elevada em crianças e adolescentes obesos ou com sobrepeso, bem como os fatores associados. MÉTODO: estudo transversal com 200 crianças e adolescentes, entre dois e 18 anos. Foram aferidos circunferência abdominal, pressão arterial, peso e estatura. Tanto a pressão arterial sistólica quanto a diastólica foram consideradas elevadas quando iguais ou superiores ao percentil 90. O valor da pressão arterial nos percentis 25, 50 e 75 foi descrito de acordo com faixa etária, sexo, estado nutricional e circunferência abdominal. Para avaliar a associação da pressão arterial elevada com as variáveis clínicas foi utilizado o teste do qui-quadrado e adotado nível de significância de 5%. RESULTADOS: a maioria das crianças e adolescentes (70,5%) apresentava pressão arterial elevada: 6% mostravam elevação apenas na pressão arterial sistólica, 33% na diastólica e 31,5% em ambas. Valores pressóricos sistólicos mais elevados foram observados nos indivíduos com obesidade grave e circunferência abdominal aumentada, assim como naqueles do sexo masculino e no grupo dos adolescentes. Entre os que tinham pressão arterial elevada, o limite máximo da normalidade da pressão sistólica foi observado no percentil 50 e, da diastólica, no 75. A pressão arterial sistólica elevada esteve associada aos adolescentes com obesidade grave. CONCLUSÃO: a alta prevalência de pressão arterial elevada observada neste estudo mostra a necessidade da implantação de políticas e ações voltadas à prevenção e controle da obesidade e suas co-morbidades, a exemplo da hipertensão arterial sistêmica.OBJECTIVE: to evaluate the prevalence of high blood pressure in fat or overweight children and adolescents and its associated factors. METHODS: Cross-sectional study including 200 children and adolescents between two and 18 years old. It was measured the abdominal circumference, blood pressure, weight and height. The systolic and diastolic blood pressure were considered high when equal or higher than 90th percentile. The value of the blood pressure in the 25th, 50th and 75th percentiles were described according to age groups, sex, nutritional state and abdominal circumference. To check the association between high blood pressure and clinical variables it was used qui-square tests and a significant level of 5%. RESULTS: The majority of children and adolescents (70.5 %) had high blood pressure: 6% showed an increase only in the systolic blood pressure, 33% in diastolic blood pressure and 31.5 % in both. Systolic blood pressure values were higher observed in individuals with severe obesity and increased waist circumference, as well as those of the male sex and in the group of adolescents. Among those who had high blood pressure, the normal maximum limit of the systolic blood pressure was observed in the 50th percentile and for the diastolic in the 75th percentile. The high systolic blood pressure was associated with adolescents with severe obesity. CONCLUSION: The high prevalence of high blood pressure observed in this study indicates the need to implement policies and actions oriented towards the prevention and control of obesity and its co-morbidities, as the case of high blood pressure

    Active and intelligent packaging has been based on a interaction of the pack with food and / or the environment that surrounds it. Are on the market active packaging to reduce lipid oxidation rates, control breathing, moisture content and minimize microbial growth. Such packages contain absorbers and emitting carbon dioxide, odor absorbents, ethylene removers, aroma-emitting and antimicrobial compounds. Intelligent packagings include indicators of time and temperature, ripeness indicators and deterioration, biosensors and radio frequency identification. Due to deliberate interaction with food and / or their environment, migration of substances may pose a food safety concern, should their use be regulated in legislation. This article presents: definitions and history of active and intelligent packaging; the main types of materials developed for food contact; future trends of active and intelligent packaging with a special emphasis on safety, legal aspects and evaluation; and the global market of active and intelligent packaging.

    Cultura de segurança do doente pediátrico num centro hospitalar da zona norte

    Enquadramento: O doente pediátrico, pelas características inerentes ao seu desenvolvimento e crescimento está mais suscetível a sofrer eventos adversos. Torna-se importante que todos os profissionais orientem a sua prática de cuidados no sentido de construir e assegurar uma cultura de segurança do doente, de modo a alcançarem melhores níveis de segurança e de qualidade nos cuidados à criança. Objetivo: Identificar fatores que influenciam a perceção da cultura de segurança do doente pediátrico num centro hospitalar da região Norte. Metodologia: Estudo quantitativo, descritivo-correlacional e transversal, realizado a partir da aplicação da escala Hospital Survey on Patient Safety Culture (Agency for Healthcare Research and Quality, 2014), a uma amostra de 80 profissionais de saúde. São maioritariamente do sexo feminino (88,8%), com idades entre os 25 e os 61 anos, tendo a maioria entre 13 a 20 anos de experiência na prestação de cuidados à criança (30,0%%). Resultados: Do total da amostra, 81,3% não fez qualquer notificação de eventos/ocorrências nos últimos 12 meses. Apenas a dimensão “Trabalho em equipa” se revelou ser um ponto forte, as dimensões “Dotação de profissionais”, “Apoio à segurança do doente pela gestão”, “Resposta ao erro não punitiva” e “Frequência de notificação de eventos”, são aspetos “críticos/problemáticos”. A perceção da cultura de segurança do doente pediátrico difere significativamente quanto às categorias dos fatores: sexo, grupo etário, serviço onde trabalha, formação em segurança e gestão de risco e conhecimento do sistema nacional de notificação de incidentes. Conclusão: A cultura de segurança do doente pediátrico, percecionada pelos participantes no estudo, caracteriza-se como uma cultura de receio de resposta punitiva ao erro e de não notificação de eventos adversos. Consideramos que é importante refletir em conjunto nos aspetos identificados como mais críticos, para implementar medidas de melhoria e desenvolver uma cultura de segurança.Abstract Background: The pediatric patient, due to the characteristics inherent to his/her development and growth is more likely to suffer adverse events. It is important that all professionals guide their practice of care in order to build and ensure a patient safety culture, so that they can achieve better levels of safety and quality in childcare. Objective: To identify factors that influence the perception of the pediatric patient safety culture in a hospital of the North. Methodology: Quantitative, descriptive-correlational and cross-sectional study conducted from the application range Hospital Survey on Patient Safety Culture (Agency for Healthcare Research and Quality, 2014), to a sample of 80 health professionals. They are mostly female (88.8%), aged between 25 and 61 years, and most of them have between 13 and 20 years of experience in providing child care (30.0 %%). Results: Of the total sample, 81.3% hasn’t made any event/occurrences reports in the last 12 months. Only the dimension "Team Work" turned out to be a strength, the dimensions "Professional Allocation", "Support for Patient Safety by Management", "Response Not Punitive to Error" and "Frequency of Event Report", are "critical/problematic" aspects. The perception of the pediatric patient safety culture differs significantly in the categories of factors: gender, age group, service where he/she works, safety training and risk management and knowledge of the national system of incident reporting. Conclusion: The safety culture of the pediatric patient, perceived by the participants in the study, is characterised as a culture of fear of a punishing answer to error and the no report of adverse events. We believe it is important to consider together about the aspects identified as the most critical to implement improvement measures and develop a culture of safety

    Impact of safety-related dose reductions or discontinuations on sustained virologic response in HCV-infected patients: Results from the GUARD-C Cohort

    BACKGROUND: Despite the introduction of direct-acting antiviral agents for chronic hepatitis C virus (HCV) infection, peginterferon alfa/ribavirin remains relevant in many resource-constrained settings. The non-randomized GUARD-C cohort investigated baseline predictors of safety-related dose reductions or discontinuations (sr-RD) and their impact on sustained virologic response (SVR) in patients receiving peginterferon alfa/ribavirin in routine practice. METHODS: A total of 3181 HCV-mono-infected treatment-naive patients were assigned to 24 or 48 weeks of peginterferon alfa/ribavirin by their physician. Patients were categorized by time-to-first sr-RD (Week 4/12). Detailed analyses of the impact of sr-RD on SVR24 (HCV RNA <50 IU/mL) were conducted in 951 Caucasian, noncirrhotic genotype (G)1 patients assigned to peginterferon alfa-2a/ribavirin for 48 weeks. The probability of SVR24 was identified by a baseline scoring system (range: 0-9 points) on which scores of 5 to 9 and <5 represent high and low probability of SVR24, respectively. RESULTS: SVR24 rates were 46.1% (754/1634), 77.1% (279/362), 68.0% (514/756), and 51.3% (203/396), respectively, in G1, 2, 3, and 4 patients. Overall, 16.9% and 21.8% patients experienced 651 sr-RD for peginterferon alfa and ribavirin, respectively. Among Caucasian noncirrhotic G1 patients: female sex, lower body mass index, pre-existing cardiovascular/pulmonary disease, and low hematological indices were prognostic factors of sr-RD; SVR24 was lower in patients with 651 vs. no sr-RD by Week 4 (37.9% vs. 54.4%; P = 0.0046) and Week 12 (41.7% vs. 55.3%; P = 0.0016); sr-RD by Week 4/12 significantly reduced SVR24 in patients with scores <5 but not 655. CONCLUSIONS: In conclusion, sr-RD to peginterferon alfa-2a/ribavirin significantly impacts on SVR24 rates in treatment-naive G1 noncirrhotic Caucasian patients. Baseline characteristics can help select patients with a high probability of SVR24 and a low probability of sr-RD with peginterferon alfa-2a/ribavirin

    Fyn Mediates Leptin Actions in the Thymus of Rodents

    BACKGROUND:Several effects of leptin in the immune system rely on its capacity to modulate cytokine expression and apoptosis in the thymus. Surprisingly, some of these effects are dependent on signal transduction through the IRS1/PI3-kinase, but not on the activation of JAK2. Since all the well known effects of leptin in different cell types and tissues seem to be dependent on JAK2 activation, we hypothesized that, at least for the control of thymic function, another, unknown kinase could mediate the transduction of the leptin signal from the ObR towards the IRS1/PI3-kinase signaling cascade. METHODOLOGY/PRINCIPAL FINDINGS:Here, by employing immunoblot, real-time PCR and flow citometry we show that the tyrosine kinase, Fyn, is constitutively associated with the ObR in thymic cells. Following a leptin stimulus, Fyn undergoes an activating tyrosine phosphorylation and a transient association with IRS1. All these effects are independent of JAK2 activation and, upon Fyn inhibition, the signal transduction towards IRS1/PI3-kinase is abolished. In addition, the inhibition of Fyn significantly modifies the effects of leptin on thymic cytokine expression. CONCLUSION/SIGNIFICANCE:Therefore, in the thymus, Fyn acts as a tyrosine kinase that transduces the leptin signal independently of JAK2 activation, and mediates some of the immunomodulatory effects of leptin in this tissue

    Poor Regenerative Outcome after Skeletal Muscle Necrosis Induced by Bothrops asper Venom: Alterations in Microvasculature and Nerves

A frequent outcome of such local pathology is deficient skeletal muscle regeneration, which causes muscle dysfunction, muscle loss and fibrosis, thus provoking permanent sequelae that greatly affect the quality of life of patients. The causes of such poor regenerative outcome of skeletal muscle after viperid snakebites are not fully understood. Methodology/Principal Findings: A murine model of muscle necrosis and regeneration was adapted to study the effects of the venom and isolated toxins of Bothrops asper, the medically most important snake in Central America. Gastrocnemius muscle was injected with either B. asper venom, a myotoxic phospholipase A2 (Mtx), a hemorrhagic metalloproteinase (SVMP), or saline solution. At various time intervals, during one month, tissue samples were collected and analyzed by histology, and by immunocytochemical and immunohistochemical techniques aimed at detecting muscle fibers, collagen, endothelial cells, myoblasts, myotubes, macrophages, TUNEL-positive nuclei, and axons. A successful regenerative response was observed in muscle injected with Mtx, which induces myonecrosis but does not affect the microvasculature. In contrast, poor regeneration, with fibrosis and atrophic fibers, occurred when muscle was injected with venom or SVMP, both of which provoke necrosis, microvascular damage leading to hemorrhage, and poor axonal regeneration. Conclusions/Significance: The deficient skeletal muscle regeneration after injection of B. asper venom is likely to depend on the widespread damage to the microvasculature, which affects the removal of necrotic debris by phagocytes, and the provision of nutrients and oxygen required for regeneration.
