The present contribution deals with heat transfer in an agitated vessel containing non-Newtonian liquid. The impellers used are six-blade Turbine (TPD) and a Propeller (TPI).     A cohort study of duplex Doppler examinations of the carotid artery in primary open angle glaucoma

    Objectives To explore the possibility of pathological change in the common carotid artery at the bifurcation and in the internal carotid artery beyond the bifurcation which could contribute to a reduced diastolic pressure as observed in primary open angle glaucoma. Design Duplex ultrasonic examinations of carotid bifurcations were conducted on 80 patients. Carotid artery defects were allocated into three types: no demonstrable flow defects, internal carotid artery abnormalities and disease in the carotid bulb. Setting Bristol Royal Infirmary Vascular Laboratory. Participants Eighty patients (mean age 69.6 years) providing a total of 160 sides to the analysis. Main outcome measures An estimated central retinal artery pressure, intraocular pressure and field loss were recorded for each side measured. Results Doppler investigations revealed significant levels of pathological change in the internal carotid distinct from changes at the carotid bulb. The disease revealed in the internal carotid artery was significantly associated with intraocular pressure (p = 0.032), with an effect small to medium in magnitude. The Q2 measure, derived from mean arterial pressure and intraocular pressure, was also substantively associated with disease in the internal carotid artery. Both intraocular pressure and the Q2 measure effectively discriminated between groups, with field loss providing rather less discriminating capability. There was a strong trend towards a higher intraocular pressures and a greater visual field loss with internal carotid artery disease. Conclusions Pathological changes in the extra cranial carotid artery in primary open angle glaucoma exceed those in the arteries classified as normal. The presence of disease specifically in the internal carotid artery emphasised the need for a mechanism for the evaluation of the internal carotid apart from the carotid bulb. A basis for clarifying the presence of an ischaemic zone is proposed

    Optimal management of malignant left-sided large bowel obstruction: do international guidelines agree?

    Background Approximately 20% of patients diagnosed with colorectal cancer will present with left-sided large bowel obstruction. The optimal management of this cohort of patients remains unclear. We aimed to review international guidelines to see if there was a consensus on the treatment of this surgical emergency. Methods The PubMed and Medline databases were searched for guidelines on the management of left-sided, malignant large bowel obstruction (MBO) between 2010 and 2018. Results Nineteen guidelines were identified spanning a range of continents. There was no clear consensus on the management of potentially resectable disease. Eight guidelines (42%) suggested primary surgery, two guidelines (11%) suggested stenting as a bridge to surgery and nine guidelines (47%) suggested surgery or stenting could be performed. Primary resection with or without anastomosis was the most frequently recommended procedure (n = 6 35%), but over a third of guidelines gave no operative recommendations. There was very limited detail on the stenting procedure and how long elective surgery should be deferred. In the palliative situation, there was general agreement that stents should be offered in preference to surgery. Conclusion International guidelines offer limited and contrasting recommendations on the management of left-sided MBO. There is a lack of high-quality evidence to support whether emergency surgery or stenting as a bridge to surgery is the optimal procedure in terms of morbidity, mortality and long-term oncological outcome

In many ways, technology it has seen as an enabler to both the teaching and learning processes. There is a need for collaboration of all who are directly and indirectly involved in teaching process. Without sufficient training and opportunities to learn and relearn new technology. This study is intended and wondering how to elicit and investigate the challenges and problems on the Educational Technological Tools (ETT) Usage in Jordan private secondary schools? This research aimed to examine the effect of gender, experience, and qualification on the use of (ETT) by secondary English language teachers from the perspective of the educational supervisors. This study was design, using a mix-method case-study approach, to explore Educational Supervisors’ (ES) perspectives on the use of Educational Technological Tools (ETT) for Teaching and Learning of English (TLE) focusing on the data collected from all the relevant stakeholders. The respondents of the study were educational supervisors and teachers affiliated to secondary schools located in Amman. The findings revealed that there is a lack of agreement in ETT Usage in the teaching process surfaced as the data presented. A concern that in both the teachers’ and educational supervisors’ views, strongly affected the using technology and teaching process. Low motivation, and job satisfaction, as well as the shortage of ETT in the Jordanian schools. This current study could make a significant contribution to this area particularly in filling in the gaps connecting the current trends on the ETT Usage in the field of English language education in Jordan private secondary schools
