    La presente investigación evaluó los efectos de entrenar estudiantes universitarios para satisfacer criterios de congruencia en tareas de ajuste lector con base en parámetros de congruencia y de coherencia. El grupo experimental fue entrenado para satisfacer criterios de congruencia y fue expuesto a dos pruebas de evaluación en satisfacción de criterios de congruencia y coherencia. El grupo control solo fue sometido a las pruebas de evaluación. Los resultados revelan que el entrenamiento favoreció la satisfacción de criterios tanto de congruencia como de coherencia. Se discute la relevancia del desempeño efectivo en una situación lectora con criterios de congruencia como condición para el desarrollo de conductas lectoras funcionalmente más complejas

    Chemical and mechanical stability of air annealed cathodic arc evaporated CrAlON coatings

    This study reports the synthesis and characterization of ternary Cr-Al-O and quaternary Cr-Al-O-N coatings deposited by cathodic arc physical vapour deposition, for various nitrogen and oxygen mass flow ratios during the growth process. The composition, microstructure, indentation hardness and modulus of the films have been characterized by scanning electron microscopy, electron probe micro-analysis, X-ray diffraction, and nanoindentation techniques. The evolution of the microstructure and mechanical properties of the coatings after ambient air annealing from 800 °C up to 1100 °C have been investigated. As the oxygen to nitrogen mass flow increases, the as-deposited coatings exhibit lower hardness, higher roughness, lower crystallinity and a more marked columnar structure. At oxygen to nitrogen mass flow ratios bigger than 10/90, the coatings exhibit a stoichiometry of the type (CrAl)2+εO3−ε. Only the coatings with an oxygen to nitrogen mass flow ratio smaller than 10/90 retained nitrogen in their compositions. In all cases, the coatings developed a cubic fcc lattice structure. After annealing at 1100 °C the resulting microstructure showed a clear dependency upon the initial composition of the films. The evolution of the microstructure during the high temperature tests, as well as the analysis of the nanoindentation hardness, composition and thickness also provided valuable information about the combined effects of the thermal stability and the oxidation of the deposited coatings

    Growth of nanocolumnar thin films on patterned substrates at oblique angles

    The influence of one dimensional substrate patterns on the nanocolumnar growth of thin films deposited by magnetron sputtering at oblique angles is theoretically and experimentally studied. A well‐established growth model has been used to study the interplay between the substrate topography and the thin film morphology. A critical thickness has been defined, below which the columnar growth is modulated by the substrate topography, while for thicknesses above, the impact of substrate features is progressively lost in two stages; first columns grown on taller features take over neighboring ones, and later the film morphology evolves independently of substrate features.     Thin film nanostructuring at oblique angles by substrate patterning

    It is demonstrated that, besides classical nanocolumnar arrays, the oblique angle geometry induces the growth of singular structures in the nanoscale when using wisely designed patterned substrates. Well-ordered array of crosses, cylindrical nanorods or hole structures arranged in square or hexagonal regular geometries are reported as examples, among others. The fundamental framework connecting substrate topography and film growth at oblique angles is presented, allowing the use of substrate patterning as a feasible thin film nanostructuring technique. A systematic analysis of the growth of TiO2 thin films on 4 different lithographic patterned substrates in 4 different scale lengths is also presented. A first conclusion is the existence of a height-based selective growth in the initial stages of the deposition, by which the film preferentially develops on top of the tallest substrate features. This behavior is maintained until the film reaches a critical thickness, the so-called Oblivion Thickness, above which the film topography becomes gradually independent of the substrate features.     Brote de adenovirus grave en Colombia: Experiencia de un hospital pediátrico de tercer nivel en 2022

    Introduction. During the SARS-CoV2 pandemic, many countries experienced decreased respiratory virus circulation, followed by an out-of-season outbreak. In a pediatric hospital in Colombia, we observed a surge in severe adenovirus infections, leading to concerns about the impact of eased public health restrictions and immune debt in children under 5 years old.Objective. Describe the clinical characteristics of patients with severe Adenovirus infection in a pediatric hospital in Colombia.Materials and methods. We reviewed 227 patients with severe adenovirus infection in Fundación Hospital Pediátrico la Misericordia (HOMI).Results. The study included 196 cases; the median age of patients was 2 years, and 62% were male. Adenovirus was isolated in all patients; 97% were admitted to the pediatric ICU, 94% required respiratory support, and the in-hospital lethality rate was 11%.Conclusion. In 2022 there was an outbreak of severe Adenovirus infection, affecting mainly children under 5 years, with mortality higher than usual.Introducción. Durante la pandemia por SARS-CoV2, muchos países evidenciaron una disminución en la circulación de virus respiratorios, por seguido por un brote fuera de la temporada esperada. En un hospital en Colombia, observamos un aumento en los casos de infección grave por Adenovirus, generando preocupación sobre el impacto que tuvo la disminución de los cuidados establecidos durante pandemia y la posible deuda inmunológica en niños menores de 5 años.Objetivo. Describir las características clínicas de pacientes con infección grave por Adenovirus en un hospital pediátrico en Colombia. Materiales y métodos. Se revisaron 227 pacientes con infección grave por adenovirus en Fundación HOMI, desde el 1 de enero de 2022 hasta el 31 de diciembre de 2022. Resultados: El estudio incluyó 196 casos, la edad media de los pacientes fue de 2 años, 62% eran hombres. El Adenovirus se aisló en todos los pacientes; 97% ingresaron a la unidad de cuidados intensivos, 94% requirió soporte ventilatorio, con una mortalidad del 11%.Conclusiones. En 2022 hubo un brote de Adenovirus que afectó principalmente a los niños menores de 5 años, con una mortalidad mayor a lo reportado con anterioridad en Colombia

    Nanoestructuración a la Carta de Películas Delgadas en Geometría de Ángulo Oblicuo Mediante el Control Topográfico del Sustrato

    Trabajo presentado en el XVI Congreso Nacional de Materiales CNMAT 2022, celebrado en Ciudad Real (España), del 28 de junio al 1 de julio de 2022La técnica de pulverización catódica operada en geometría de ángulo oblicuo es bien conocida por permitir el crecimiento de películas delgadas nanocolumnares sobre sustratos planos. En otro tipo de sustratos, e.g. rugosos o litografiados, la casuística es variada, apareciendo estructuras complejas y diversas, incluso para espesores del orden de la micra. Estos resultados sugieren la existencia de una fuerte conexión entre el crecimiento de la película y la topografía del sustrato, que podría utilizarse para obtener un mayor control nanoestructural. En esta presentación se analiza teórica y experimentalmente la relación entre ambos aspectos al utilizar la técnica de pulverización catódica a ángulo oblicuo. Se demuestra la posibilidad de crecer estructuras singulares como, por ejemplo, cruces nanopilares o incluso de huecos dentro de una matriz compacta, ordenados de acuerdo a un patrón regular con distancias típicas del orden de cientos de nanómetros (ver Figura 1). Se describe, por lo tanto, el marco conceptual que permitiría que las técnicas actuales de litografía superficial se puedan utilizar como técnicas efectivas de control nanoestructural de películas delgadas. Como resultado, se demuestran varios principios que definen los diferentes estadios de la deposición que servirían como guías para el diseño inteligente del sustrato e inducir crecimientos a la carta. Estos principios se han validado experimentalmente al crecer diferentes películas delgadas sobre un total de 16 sustratos litografiados con diferentes topografías, patrones y escalas típicas por debajo de la micra, así como en otros sustratos con rugosidad intrínseca sometidos a diferentes tipos de pulido

    Clinical validation of risk scoring systems to predict risk of delayed bleeding after EMR of large colorectal lesions

    [Background and Aims]: The Endoscopic Resection Group of the Spanish Society of Endoscopy (GSEED-RE) model and the Australian Colonic Endoscopic Resection (ACER) model were proposed to predict delayed bleeding (DB) after EMR of large superficial colorectal lesions, but neither has been validated. We validated and updated these models.[Methods]: A multicenter cohort study was performed in patients with nonpedunculated lesions ≥20 mm removed by EMR. We assessed the discrimination and calibration of the GSEED-RE and ACER models. Difficulty performing EMR was subjectively categorized as low, medium, or high. We created a new model, including factors associated with DB in 3 cohort studies.[Results]: DB occurred in 45 of 1034 EMRs (4.5%); it was associated with proximal location (odds ratio [OR], 2.84; 95% confidence interval [CI], 1.31-6.16), antiplatelet agents (OR, 2.51; 95% CI, .99-6.34) or anticoagulants (OR, 4.54; 95% CI, 2.14-9.63), difficulty of EMR (OR, 3.23; 95% CI, 1.41-7.40), and comorbidity (OR, 2.11; 95% CI, .99-4.47). The GSEED-RE and ACER models did not accurately predict DB. Re-estimation and recalibration yielded acceptable results (GSEED-RE area under the curve [AUC], .64 [95% CI, .54-.74]; ACER AUC, .65 [95% CI, .57-.73]). We used lesion size, proximal location, comorbidity, and antiplatelet or anticoagulant therapy to generate a new model, the GSEED-RE2, which achieved higher AUC values (.69-.73; 95% CI, .59-.80) and exhibited lower susceptibility to changes among datasets.[Conclusions]: The updated GSEED-RE and ACER models achieved acceptable prediction levels of DB. The GSEED-RE2 model may achieve better prediction results and could be used to guide the management of patients after validation by other external groups.     Healthcare workers hospitalized due to COVID-19 have no higher risk of death than general population. Data from the Spanish SEMI-COVID-19 Registry

    Aim To determine whether healthcare workers (HCW) hospitalized in Spain due to COVID-19 have a worse prognosis than non-healthcare workers (NHCW). Methods Observational cohort study based on the SEMI-COVID-19 Registry, a nationwide registry that collects sociodemographic, clinical, laboratory, and treatment data on patients hospitalised with COVID-19 in Spain. Patients aged 20-65 years were selected. A multivariate logistic regression model was performed to identify factors associated with mortality. Results As of 22 May 2020, 4393 patients were included, of whom 419 (9.5%) were HCW. Median (interquartile range) age of HCW was 52 (15) years and 62.4% were women. Prevalence of comorbidities and severe radiological findings upon admission were less frequent in HCW. There were no difference in need of respiratory support and admission to intensive care unit, but occurrence of sepsis and in-hospital mortality was lower in HCW (1.7% vs. 3.9%; p = 0.024 and 0.7% vs. 4.8%; p<0.001 respectively). Age, male sex and comorbidity, were independently associated with higher in-hospital mortality and healthcare working with lower mortality (OR 0.211, 95%CI 0.067-0.667, p = 0.008). 30-days survival was higher in HCW (0.968 vs. 0.851 p<0.001). Conclusions Hospitalized COVID-19 HCW had fewer comorbidities and a better prognosis than NHCW. Our results suggest that professional exposure to COVID-19 in HCW does not carry more clinical severity nor mortality

    Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.
