    Proposing a Tool for Supply Chain Configuration: An Application to Customised Production

    The full implementation of collaborative production networks is crucial for companies willing to respond to consumer demand strongly focused on product customisation. This chapter proposes an approach to evaluate the performance of different Supply Chain (SC) configurations in a customised production context. The model is based on discrete-event simulation and is applied to the case of supply chain in the fashion sector to support the comparison between mass and customised production. A prototype web-based interface is also developed and proposed to facilitate the use of the model not only for experts in simulation but for any user in the SC management field

    Olber's Paradox for Superluminal Neutrinos: Constraining Extreme Neutrino Speeds at TeV-ZeV Energies with the Diffuse Neutrino Background

    The only invariant speed in special relativity is c; therefore, if some neutrinos travel at even tiny speeds above c, normal special relativity is incomplete and any superluminal speed may be possible. I derive a limit on superluminal neutrino speeds v >> c at high energies by noting that such speeds would increase the size of the neutrino horizon. The increased volume of the Universe visible leads to a brighter astrophysical neutrino background. The nondetection of "guaranteed" neutrino backgrounds from star-forming galaxies and ultrahigh energy cosmic rays (UHECRs) constrains v/c at TeV--ZeV energies. I find that v/c <= 820 at 60 TeV from the nondetection of neutrinos from star-forming galaxies. The nondetection of neutrinos from UHECRs constrains v/c to be less than 2500 at 0.1 EeV in a pessimistic model and less than 4.6 at 4 EeV in an optimistic model. The UHECR neutrino background nondetection is strongly inconsistent with a naive quadratic extrapolation of the OPERA results to EeV energies. The limits apply subject to some caveats, particularly that the expected pionic neutrino backgrounds exist and that neutrinos travel faster than c when they pass the detector. They could be improved substantially as the expected neutrino backgrounds are better understood and with new experimental neutrino background limits.     Immunoglobulin, glucocorticoid, or combination therapy for multisystem inflammatory syndrome in children: a propensity-weighted cohort study

    Background: Multisystem inflammatory syndrome in children (MIS-C), a hyperinflammatory condition associated with SARS-CoV-2 infection, has emerged as a serious illness in children worldwide. Immunoglobulin or glucocorticoids, or both, are currently recommended treatments. Methods: The Best Available Treatment Study evaluated immunomodulatory treatments for MIS-C in an international observational cohort. Analysis of the first 614 patients was previously reported. In this propensity-weighted cohort study, clinical and outcome data from children with suspected or proven MIS-C were collected onto a web-based Research Electronic Data Capture database. After excluding neonates and incomplete or duplicate records, inverse probability weighting was used to compare primary treatments with intravenous immunoglobulin, intravenous immunoglobulin plus glucocorticoids, or glucocorticoids alone, using intravenous immunoglobulin as the reference treatment. Primary outcomes were a composite of inotropic or ventilator support from the second day after treatment initiation, or death, and time to improvement on an ordinal clinical severity scale. Secondary outcomes included treatment escalation, clinical deterioration, fever, and coronary artery aneurysm occurrence and resolution. This study is registered with the ISRCTN registry, ISRCTN69546370. Findings: We enrolled 2101 children (aged 0 months to 19 years) with clinically diagnosed MIS-C from 39 countries between June 14, 2020, and April 25, 2022, and, following exclusions, 2009 patients were included for analysis (median age 8·0 years [IQR 4·2–11·4], 1191 [59·3%] male and 818 [40·7%] female, and 825 [41·1%] White). 680 (33·8%) patients received primary treatment with intravenous immunoglobulin, 698 (34·7%) with intravenous immunoglobulin plus glucocorticoids, 487 (24·2%) with glucocorticoids alone; 59 (2·9%) patients received other combinations, including biologicals, and 85 (4·2%) patients received no immunomodulators. There were no significant differences between treatments for primary outcomes for the 1586 patients with complete baseline and outcome data that were considered for primary analysis. Adjusted odds ratios for ventilation, inotropic support, or death were 1·09 (95% CI 0·75–1·58; corrected p value=1·00) for intravenous immunoglobulin plus glucocorticoids and 0·93 (0·58–1·47; corrected p value=1·00) for glucocorticoids alone, versus intravenous immunoglobulin alone. Adjusted average hazard ratios for time to improvement were 1·04 (95% CI 0·91–1·20; corrected p value=1·00) for intravenous immunoglobulin plus glucocorticoids, and 0·84 (0·70–1·00; corrected p value=0·22) for glucocorticoids alone, versus intravenous immunoglobulin alone. Treatment escalation was less frequent for intravenous immunoglobulin plus glucocorticoids (OR 0·15 [95% CI 0·11–0·20]; p<0·0001) and glucocorticoids alone (0·68 [0·50–0·93]; p=0·014) versus intravenous immunoglobulin alone. Persistent fever (from day 2 onward) was less common with intravenous immunoglobulin plus glucocorticoids compared with either intravenous immunoglobulin alone (OR 0·50 [95% CI 0·38–0·67]; p<0·0001) or glucocorticoids alone (0·63 [0·45–0·88]; p=0·0058). Coronary artery aneurysm occurrence and resolution did not differ significantly between treatment groups. Interpretation: Recovery rates, including occurrence and resolution of coronary artery aneurysms, were similar for primary treatment with intravenous immunoglobulin when compared to glucocorticoids or intravenous immunoglobulin plus glucocorticoids. Initial treatment with glucocorticoids appears to be a safe alternative to immunoglobulin or combined therapy, and might be advantageous in view of the cost and limited availability of intravenous immunoglobulin in many countries.     Experimental study on hybrid precast tunnel segments reinforced by macro-synthetic fibres and glass fibre reinforced polymer bars

    Increasing the popularity of using fibres in precast structures has brought questions about their usability in segmental tunnel linings as an alternative to the conventional reinforced lining. Most prior studies have already revealed that the replacement of conventional steel reinforcement is possible with steel fibres. However, the usability of macro-synthetic fibres (MSFs) as reinforcements in precast tunnel segments is still unclear and one of the controversial questions to be answered. As such, this study aims to evaluate the structural applicability of using polypropylene (PP) MSFs in precast tunnel segments by means of an experimental program on full-scale specimens. Within this framework, totally fourteen full-scale precast tunnel segments of Mecidiyekoy – Mahmutbey underground railway tunnel in Istanbul/Turkey characterized by four different reinforcement cases were analysed: i) typical conventional steel reinforcement; ii) the combination of MSFs and conventional reinforcement; iii) the combination of MSFs and glass fibre reinforced polymer (GFRP) rebars; and iv) MSFs only. Flexural tests were carried out to compare the flexural behaviour of specimens at the allowable crack opening width, while point load tests were conducted to observe the structural performance of precast tunnel segments under the effect of design thrust forces. The experimental results showed that the combination of MSFs and GFRP could be an innovative solution for precast tunnel segments in case of using a suitable quantity that satisfied the project requirements. Although PP fibres exhibited adequate spalling and splitting stress control, it is observed that they could not overcome high flexure forces without using reinforcement bars at a low volume of fractions. Thus, more comprehensive studies need to perform on GFRP + MSFs segments because of having the advantages of corrosion resistance in the presence of an aggressive surrounding environment

    A comparative study on the strengthening of RC frames

    In order to improve the behavior of buildings and in order to prevent total collapse, necessary amount of strengthening must be provided. The frame of the test specimen (1/3 scaled, 2-story, 3-bay) is detailed such that it has the common deficiencies of existing buildings in Turkey. Two types of strengthening techniques, namely introducing an infill RC wall, and CFRP strengthened hollow clay tile wall, are investigated. The test specimens are subjected to reversed cyclic quasi-static loading. By means of special transducers, axial force, shear force and bending moment at the base of the exterior columns are measured. Strength, stiffness, and story drifts of the test specimen are determined
