Stanford type B aortic dissection in a middle aged female in civil hospital Karachi, Pakistan

Aortic dissection is a potentially catastrophic illness that presents very rarely usually in sixth or seventh decade. There are two types of dissection; Stanford type A, which is treated surgically while type B is managed pharmacologically. Mortality is high for both types of aortic dissections.  However, when an aortic dissection is detected early and treated promptly, the chance of survival greatly improves.  It usually presents as a sharp tearing pain radiating to interscapular region but can also present atypically hence a high index of clinical suspicion is needed. We register a case a 50 year old female who was a known case of HTN presented with complain of dyspnea. On further investigation she was found to have aortic dissection involving descending aorta. She was managed conservatively but unfortunately died

mso-hansi-theme-font:minor-latin;} The effect of linguistic is the integral part of the Social and political consideration in the multi-linguistic countries all over the World. South Asian countries are facing multi-linguistic problem and as a result separation movements exist in the region since decades. However, each state/country is confronting this issue according to its environment and local circumstances. This paper will focus the impacts of ethnicity on education and poverty in Sri Lanka. The existence of India, Pakistan and Bangladesh is one of the major reasons of linguistic issue. In case of Sri Lanka, the Tamil movement though could not succeed due to various reasons but originally initiated due to linguistic deprivation of Tamil community. The main reasons are not only language but the deprivation in education, employment and social status like poverty of particular community. If their problems are addressed properly then the linguistic issue cannot be highlighted. It is worth mentioning that all the linguistic based separation movement succeeded if supported by the International community otherwise failed. The good examples are Tamil and Khalistan separation movements. In addition to above, the Sri Lankan government took advantage of 9/11 incident and crushed the movement with force and international community remained silent. As a result this linguistic based separation movement could not succeeded in Sri Lanka despite the fact that these kind of movements succeeded in South Asia.  </p

Frequency of cardiorenal syndrome type-I in hospitalized children with acute heart failure in a tertiary-care hospital

Objective: To determine the frequency of cardiorenal syndrome in hospitalized children with acute heart failure. Study Design: Descriptive study. Place and Duration of Study: Paediatric Intensive Care Unit, The Aga Khan University Hospital, Karachi, from December 2010 to December 2011. Methodology: Sixty eight (68) children with acute heart failure fulfilling the selection criteria were evaluated for worsening of renal function (WRF). Serum creatinine was done at baseline and repeated at 72 hours to see the worsening of renal function. Estimated serum creatinine clearance was calculated by Schwartz formula. Results: Mean age of patients was 43.6 ± 55.2 months. There were 43 (63%) males, 70% were under 57 months of age. Mean weight on admission was 14.7 ± 19.13 kg and mean height was 83 cm (± 31.08 SD). Mean serum creatinine on admission was 0.77 mg/dl (± 1.18 SD). Worsening renal function was noted in 55 (81%) of children, out of those, majority 36 (70.5%) were under 5 years of age. Conclusion: Worsening renal function was found in 81% of children admitted with the diagnosis of acute heart failure. Majority (70.5%) were under 5 years of age indicating a closer observation of renal status in younger age group to reduce, morbidity and mortality

Changes in thyroid function status of suicidal patients

Background This study was carried out at Punjab Institute of Mental Health and Centre for Nuclear Medicine Mayo Hospital, Lahore. It is aimed at the possible association of thyroid malfunctioning with suicide attempts of patients. Objective Determination of thyroid function status of suicidal psychiatric patients and their comparison with psychiatric patients without suicide attempt or ideation. Methods Total 54 patients with either past history of suicide attempt or current suicidal ideation were selected for analysis of their thyroid function status (age 15-55 years). Age matched 50 non-suicide psychiatric patients were included for comparison. Results Two patients with suicide attempt had overt thyroid dysfunction. Remaining patients had serum FT4, FT3 and TSH level within normal range. Suicide attempter patients had lower FT4 but increased FT3 and TSH levels compared to suicidal ideation patients. Serum FT4 and TSH levels in suicidal patients were not different from psychiatric patients. Serum FT3 in suicidal patients was lower than psychiatric patients (3.7 ± 0.8 vs. 4.3 ± 0.5; p < 0.05). Female suicidal patients had lower FT3 levels compared to male patients (3.4 ± 0.6 vs. 3.9 ± 0.8 pmol/L; p < 0.05). Discussion Local suicidal patients have higher incidence of overt thyroid disorder and lower FT3 levels compared to non-suicidal psychiatric patients

Comparison of solvent evaporation and ultrasonicassisted production methods in the development of nimesulide nanosponges and their characterization

Purpose: To compare solvent evaporation and ultrasonic assisted synthesis in preparation of nimesulide nanosponges using polyvinyl-alcohol and Eudragit L100 as a polymer/copolymer and dichloromethane as a cross linker.Methods: Twelve formulations of nimesulide were prepared, six with each method by varying the ratios of both polymer and co-polymer. The resulting nanosponges were evaluated characterized by preformulation studies, production yield (%), differential scanning calorimeter, x-ray diffraction, Fourier transformation infrared spectroscopy, scanning electron microscopy, entrapment efficiency (%), actual drug content (%) and in-vitro dissolution studies.Results: The results revealed that the formulation with high amounts of co-polymer in both methods showed crystalline structures with enhanced dissolution rates in basic media. Drug entrapment was higher for products prepared by solvent evaporation method (74 %) than that prepared by ultrasonic assisted method (61 %). This correlates with the enhanced dissolution rates for products by solvent evaporation method and increased solubility due to drug-polymer complex formation.Conclusion: Formulations made by solvent evaporation method demonstrate higher production yield and drug entrapment. However, both methods exhibit enhanced dissolution rates in basic medium generally as well as other characteristics that are comparable to nanosponges reported in the literature with regard to their comb like structure

PCR-Based Molecular Diagnosis of Hepatitis Virus (HBV and HDV) in HCV Infected Patients and Their Biochemical Study

Seroprevalence of HCV indicates that HCV is found in more than 10% of HBV-or HDV-infected patients worldwide leading to liver disease. Here we show HBV and HDV coinfection association with HCV infected Pakistani patients, study of disease severity, and possible interpretation of associated risk factors in coinfected patients. A total of 730 liver diseased patients were included, out of which 501 were found positive for HCV infection via PCR. 5.1% of patients were coinfected with HBV while 1% were coinfected with HBV and HDV both. LFTs were significantly altered in dually and triply infected patients as compared to single HCV infection. Mean bilirubin, AST, and ALT levels were highest (3.25 mg/dL, 174 IU/L, and 348 IU/L) in patients with triple infection while dual infection LFTs (1.6 mg/dL, 61 IU/L, and 74 IU/L) were not high as in single infection (1.9 mg/dL, 76 IU/L, and 91 IU/L). The most prominent risk factor in case of single (22%) and dual infection (27%) group was &quot;reuse of syringes&quot; while in triple infection it was &quot;intravenous drug users&quot; (60%). It is concluded that HBV and HDV coinfections are strongly associated with HCV infected Pakistani patients and in case of severe liver disease the possibility of double and triple coinfection should be kept in consideration
