    Calcifying fibrous pseudotumour of maxilla: A rare entity mimicking malignancy: A case report

    Occurrence of inflammatory pseudotumour in head and neck region or nose and paranasal sinuses is rare. However, when they do occur, they could be quite aggressive. Etiologically, they are believed to be reactive than neoplastic, and calcification may suggest end-stage. Their clinical presentation and radiologic features may resemble a malignancy. Grossly, they are not encapsulated, but multilobulated and can be circumscribed or infiltrative. Histologically, they constitute of bland spindle cells with scant cytoplasm and occasional mitotic figures. Scattered lymphocytic and plasma cell infiltrates with abundant dense hyalinized collagenous stroma and focal small calcifications are seen. Presence of atypia, DNA aneuploidy, and abnormal p53 expression may suggest malignant potential. Though not known to metastasize, they can lead to local complications, causing destruction of bone and surrounding tissues. Management is mainly by surgical excision though adjunct corticosteroids have been advocated. We report such a rare case of calcifying fibrous pseudotumour of maxilla

    The perceptions of the safety of cannabis use among pregnant women

    Cannabis or marijuana is one of the earliest cultivated plants by man. It was used for a variety of reasons for over five millennia, until the mid-20th century, where the US government illegalized it and lobbied other countries in the West to do the same. However, in the last four decades there has been a revived interest in using cannabis for its medicinal and recreational properties with a little less than half of the states in the union having moved to legalize the plant for both medicinal and recreational purposes. While this move has a variety of benefits, we have reached a point where legalization has given people the perception that there is no harm in ingesting cannabis, including women of reproductive age. Pregnant women increasingly feel that cannabis use is a good treatment for anxiety, stress, and nausea. However, a fetus is extremely fragile throughout pregnancy, especially in the first trimester. Furthermore, studies show that 51% of pregnancies are a result of unintended pregnancy making it likely that women use cannabis unintentionally at a point where they may be doing their child more harm than they realize. This study hopes to contribute to the body of literature surrounding perceptions around cannabis use during reproductive age and pregnancy. These group of women in the US are at risk of receiving misinformation putting either themselves, their child, or both at risk for negative effects of cannabis use. Understanding the perceptions and beliefs of these women – what is driving them to use and how we can better inform them – is vital for future public health efforts

    Delivering an enabling environment and multiple benefits for land degradation neutrality:Stakeholder perceptions and progress

    Achieving land degradation neutrality (LDN) was adopted by countries in 2015 as one of the targets of the global Sustainable Development Goals (SDGs). As LDN is a relatively new concept there is an increasing need for evidence on the potential socio-economic and environmental benefits of LDN as well as how an enabling environment for implementing LDN measures can be developed. This paper summarises the results from a global survey of LDN stakeholders, and a review of national progress in target setting that was commissioned by the United Nations Convention to Combat Desertification (UNCCD) in 2018. The study presents the perceptions of relevant stakeholders on the key components of an enabling environment for achieving and maintaining LDN (institutional, financial, policy/regulatory, and science-policy) as well as expectations of multiple benefits from its implementation. We also highlight key challenges and gaps in progress to date that are emerging from ongoing national target setting programs to implement LDN. The study finds that progress in implementing LDN has been widespread across countries. However there remains a lack of awareness of LDN and its key concepts along with high-level political buy-in. This may be impeding the integration of LDN into national development planning and budgeting processes where progress was assessed as limited. National capacities for securing land tenure and governance arrangements and integrated land use planning were perceived as comparatively low, further hampering the implementation of LDN. Despite these gaps, most stakeholders (>90 %) who participated in the global survey expected LDN to deliver a broad range of multiple benefits for human wellbeing, livelihoods and the natural environment. We argue that greater efforts are needed to raise awareness of LDN, educate core stakeholders in its concepts, enablers and benefits, raise its political profile, and provide evidence on national measures that will support implementation of LDN

    A social-ecological systems approach is necessary to achieve land degradation neutrality

    Viewing humans as drivers of change operating outside the natural environment is unhelpful for defining interventions that effectively manage change and complexity. Indeed, there is now broad agreement that en-vironmental governance needs to consider integrated social-ecological systems (SES) in order to tackle theworld’s grand challenges of land degradation. This requires a more differentiated, innovative approach that considers how changes in SES shape the functioning of land systems as a whole, and the synergies and trade-off these changes may produce. In this study, we identify and discuss some of the ways SES science and practice can inspire progress towards land degradation neutrality (LDN) outcomes in an integrated manner, through synthesis of literature and relevant documents related to the United Nations Convention to Combat Desertification(UNCCD). We do these by considering: (i) how LDN has been approached to date and the challenges likely to undermine progress towards achieving it; and (ii) an SES-based LDN approach relevant to the neutrality agenda, in particular, by describing how LDN might be thought of differently through an SES lens. We argue that an SES approach focusing on: (i)“people as part of nature”, not“people and nature”; and (ii) the frame of reference against which neutrality can be assessed across temporal and spatial dimensions, is necessary to both inform policy and guide actions of the different groups involved in avoiding and combating land degradation. Such an(integrated) approach adds a dimension (to achieving neutrality goals) not previously explored in sustainable land management and LDN research. Important next steps in operationalising the SES-based LDN approach involve empirical and field case studies, requiring interdisciplinary, mixed method technique

    Radio-frequency microplasmas with energies suited to in situ selective cleaning of surface adsorbates in ion microtraps

    We have demonstrated a capacitively-coupled, radio-frequency (RF) microplasma inside the 3D electrode structure of an ion microtrap device. For this work, devices with an inter-electrode distance of 340 μm were used. The microplasmas were operated at Ω RF /2π = 23 MHz, in both He and He:N2 gas mixtures, over a range of RF amplitudes (140–220 V) and pressures (250–910 mbar). Spectroscopic analysis of the He I 667 nm and Hα 656 nm emission lines yielded the gas temperature and electron density, which enabled calculation of the mean ion bombardment energy. For the range of operating parameters studied, we calculated mean He+ energies to be between 0.3 and 4.1 eV. While these energies are less than the threshold for He sputtering of hydrocarbon adsorbates on Au, we calculate that the high energy tail of the distribution should remove adsorbate monolayers in as little as 1 min of processing. We also calculate that the distribution is insufficiently energetic to have any significant effect on the Au electrode surface within that duration. Our results suggest that the microplasma technique is suited to in situ selective removal of surface adsorbates from ion microtrap electrodes

    SLC11A1 polymorphisms and host susceptibility to cutaneous leishmaniasis in Pakistan.

    Background:The vector-borne cutaneous leishmaniasis (CL) is endemic in several regions of Pakistan mainly affecting poor populations. Host genetic factors, particularly SLC11A1 (solute carrier transmembrane protein) within macrophages, play a crucial role in disease pathology and susceptibility. Association of SLC11A1 with cutaneous leishmaniasis, a neglected tropical disease, is not well established. Inconsistencies have been observed within different populations worldwide with respect to genetic susceptibility. This study was designed to investigate genetic variation(s) in SLC11A1 and to assess possible association with cutaneous leishmaniasis in Pakistan. Results: Eight polymorphisms (rs2276631, rs3731864, rs2290708, rs2695342, rs201565523, rs17215556, rs17235409, rs17235416) were genotyped across SLC11A1 in 274 patients and 119 healthy controls. Six polymorphisms were studied by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) and sequencing. Two single nucleotide polymorphisms were analyzed with newly designed semi-nested PCR assays. Case-control analysis showed no association between selected polymorphisms in SLC11A1 and cutaneous leishmaniasis. No significant difference was observed in the distribution of alleles between leishmaniasis patients and healthy individuals. Strong pairwise linkage disequilibrium was observed between rs2276631 and rs2290708 (r 2 = 64); and rs17235409 and rs17235416 (r 2 = 78). Conclusions: This study shows that genetic variations in the candidate gene SLC11A1 do not affect susceptibility to cutaneous leishmaniasis in the sample population from Pakistan

    Threats to the Soil Resource Base of Food Security in China and Europe. A report from the Sino-EU Panel on Land and Soil

    To secure adequate food supply is the major challenge for humanity in the 21st century. Growing world population and its urbanization put pressure on this basic need, which is further threatened by the constant loss of fertile land. The assessment of sustainability of food supply under increasing pressure on land resources has been selected as one of the most important priority topics of the activities of Sino-EU Panel on Land and Soil (SEPLS). The Panel has performed a number of related researches and discussed the results on a scientific seminar in January 2012 in Nanjing, China. This report is an output of this seminar with a summary of the structured discussions on the below issues. 1. Urban and peri-urban development (soil sealing and loss of land functions) Urbanization and the linked spread of infrastructural development mean sealing of soil surfaces. Soil sealing is the most rapidly growing limitation for soil functions (including biomass production function) both in China and Europe. Soil sealing in China has been taking dramatic degree in the last two decades and the process is estimated to continue in the coming period as well. While urban and peri-urban development is looked as a necessity for social development, its negative effect on natural resources are inevitable. 2. Land degradation Despite the widely recognized importance of land degradation in the unsustainability of economic development and implementation of various policies to halt degradation (e.g. green for grain programme in China; cross-compliance measures in the EU), loss of land productivity by degradation is an ongoing process both in China and the EU. Major forms of soil degradation (erosion, desertification, landslides etc.) are similar in both regions. Assessment of the causes and consequences of soil degradation processes in relation to policy actions is highlighted among the priorities of the SEPLS. 3. Intensive agriculture and multi-function management of land resources Intensification and extensification in agriculture can be considered as the main changes in land use in rural areas in both EU and China. While agricultural intensification is one of the greatest threats to the soil and environment and then hampers the sustainable development of agriculture and food security. To meet this challenge, sustainable management of multi-functionality of land resources is undoubtedly an effective strategy, in which the EU has a good expertise.     Activation of EGFR/ERBB2 via Pathways Involving ERK1/2, P38 MAPK, AKT and FOXO Enhances Recovery of Diabetic Hearts from Ischemia-Reperfusion Injury

    This study characterized the effects of diabetes and/or ischemia on epidermal growth factor receptor, EGFR, and/or erbB2 signaling pathways on cardiac function. Isolated heart perfusion model of global ischemia was used to study the effect of chronic inhibition or acute activation of EGFR/erbB2 signaling on cardiac function in a rat model of type-1 diabetes. Induction of diabetes with streptozotocin impaired recovery of cardiac function (cardiac contractility and hemodynamics) following 40 minutes of global ischemia in isolated hearts. Chronic treatment with AG825 or AG1478, selective inhibitors of erbB2 and EGFR respectively, did not affect hyperglycemia but led to an exacerbation whereas acute administration of the EGFR ligand, epidermal growth factor (EGF), led to an improvement in cardiac recovery in diabetic hearts. Diabetes led to attenuated dimerization and phosphorylation of cardiac erbB2 and EGFR receptors that was associated with reduced signaling via extracellular-signal-regulated kinase 1/2 (ERK1/2), p38 mitogen activated protein (MAP) kinase and AKT (protein kinase B). Ischemia was also associated with reduced cardiac signaling via these molecules whereas EGF-treatment opposed diabetes and/or ischemia induced changes in ERK1/2, p38 MAP kinase, and AKT-FOXO signaling. Losartan treatment improved cardiac function in diabetes but also impaired EGFR phosphorylation in diabetic heart. Co-administration of EGF rescued Losartan-mediated reduction in EGFR phosphorylation and significantly improved cardiac recovery more than with either agent alone. EGFR/erbB2 signaling is an important cardiac survival pathway whose activation, particularly in diabetes, ischemia or following treatment with drugs that inhibit this cascade, significantly improves cardiac function. These findings may have clinical relevance particularly in the treatment of diabetes-induced cardiac dysfunction
