    Examining the effect of on-the-job training on employees’ occupational performance in Sepah Bank, Qazvin Province, Iran

    On-the-job training is considered as one of the most effective management methods for adapting employees to environmental changes and it can guarantee desired performance, particularly in services and governmental organizations. This purpose of study is to investigate the impact of employees’ on-the-job- training on their occupational performance in Sepah Bank of Qazvin, Iran. To do this, the opinions of 155 employees working in Sepah Bank of Qazvin Province, and opinions their direct managers towards the impact of on-the-job- training on occupational performance were collected through questionnaires. The results of this study showed a significant relationship between on-the-job- training and performance yield increase, revealing that the training increased employees’ knowledge and occupational skills as well as their cooperation morale. Yet, no significant relationship was found between increased performance after following on-the-job training and employees’ accurate decisions and judgments

    Hearing loss etiology in patients referring to Isfahan cochlear implantation center

    Introduction: Hearing loss is the most common congenital disorders occurring among newborn. Identifying the factors affecting it would reduce the incidence of this disorder. Therefore, the aim of this study was to examine the etiology of congenital hearing loss in patients referring to Isfahan Cochlear implantation center. Materials and Method: This study was performed on 689 patients with cochlear implantation. Demographic data and relative frequency of different causes of congenital hearing loss (acquired and genetic) were determined and recorded. T-test, Chi-Square and Mann-Whitney tests were used to compare the variables studied. Results: Our findings showed that 50 patients (7.7%) had history of drug use, 9 (1.3%) had history of taking ototoxic drugs, 99 (14.3%) had history of kernicterus, 157 patients (22.8%) had a history of hyperbilirubinemia, 15 (2.1%) history of meningitis, 57 (8.3%) had a history of seizure, one (0.1%) had a birth weight less than 1500 grams, 4 cases (0.06%) had history of hypoxia, 18 cases (2.6%) had history of trauma, one (0.1%) had CMV history, and one (0.1%) hearing loss due to syndromicity. On the other hand, it was determined that the probable causes of hearing loss were not related to the severity of hearing loss and age of the patients (p <0.05) Conclusion: Although the prevalence of congenital hearing loss is low, it is important to identify and screen for postnatal congenital hearing loss, especially in people with risk factors known in this study, including patients with kernicterus.

    Hemiballismus, Hyperphagia, and Behavioral Changes following Subthalamic Infarct

    The function of subthalamic nucleus (STN) which is a part of the basal ganglia system is not clear, but it is hypothesized that this component might be involved in action selection. Unilateral damage to STN, which can commonly occur due to the small vessel stroke mainly, causes hemiballismus and sometimes hemichorea-hemiballismus. This paper deals with a 60-year-old patient with sudden onset of abnormal movements in his right limbs. He had increased appetite and hyperphagia and also developed mood and behavioral changes (aggressiveness, irritability, anxiety, and sometimes obscene speech). The magnetic resonance imaging revealed infarct area in left subthalamus. In our case, hemiballismus is caused by infarction in left subthalamic area. Occurrence of irritability, anxiety, and some behavioral changes such as aggressiveness and obscene speech can be explained by impairment of STN role in nonmotor behavior and cognitive function as a result of infarct

    Ziv-aflibercept in Diabetic Macular Edema: Relation of Subfoveal Choroidal Thickness with Visual and Anatomical Outcomes

    Purpose: To evaluate the effects of intravitreal ziv-aflibercept injections (IVZ) on subfoveal choroidal thickness (SCT) as well as on central macular thickness (CMT) and on best corrected visual acuity (BCVA) changes in eyes with center-involved diabetic macular edema (CI-DME). Methods: Fifty-seven eyes of 36 patients with CI-DME were included in this prospective interventional case series. Structural optical coherence tomography (OCT) and enhanced depth imaging OCT were performed at baseline followed by three monthly 1.25 mg IVZ injections. Changes of SCT, CMT, and BCVA at each follow-up session were assessed. The association between baseline SCT and its monthly changes with final visual and anatomical outcomes were also assessed. Results: CMT at baseline, and at the first, second, and third month follow-up sessions were 396 ± 119, 344 ± 115, 305 ± 89, and 296 ± 101 Όm, respectively (P-value < 0.001). SCT at baseline, and at months one, two, and three were 236 ± 47, 245 ± 56, 254 ± 54, and 241 ± 54 Όm, respectively (Pvalue > 0.99). Corresponding figures for BCVA were 0.58 ± 0.29, 0.47 ± 0.31, 0.4 ± 0.24, and 0.37 ± 0.23 LogMAR, respectively (P-value < 0.001). There was a statistically significant positive correlation between BCVA and CMT changes following IVZ injections (P-value < 0.001). However, there were no significant correlations between SCT changes and visual acuity (VA) and CMT changes following IVZ injections. Conclusion: IVZ improved visual outcomes and macular thickness profiles in patients with CI-DME. However, IVZ had no significant effect on SCT. Baseline SCT and its monthly changes had no association with visual and anatomical outcomes
