    Process of Patient Education and Orem,s Self Care Theory, An Integrative Model Curriculum: A Qualitative Study

    Introduction: The development of nursing curriculum to develop the skills and the professional tasks by using the proper training is necessary for planning. Learning based on nursing models and theories is an original strategy in nursing education. This study aimed to clarify participants' experiences about patient education curriculum. Method: This study was part of a qualitative study using Glasser grounded theory approach in 2014. To explore experiences and understanding of nursing students, lecturers, and clinical tutors, in-depth and semi-structural interviews about patient education was used to gather the data. Data analysis conducted simultaneously by data gathering. The extracted codes were categorized into three main themes with several subthemes. Results: Three main themes were extracted including ‘the role of professional patient education’, ‘application of nursing theory in clinical practice’, and ‘integrated curriculum course’. Conclusion: This study recommends the application of Orem nursing model for managing and organizing the patient education curriculum.     The relationship between quality of life and demographic variables in hemodialysis patients

    Introduction: Quality of life is an important criterion in evaluation of medical treatment and status of health and well-being, contributing to the prediction of mortality rate in hospitals. QOL may be affected by diseases and their related complications. This study aims to investigate the relationship between the quality of life (QOL) of hemodialysis patients and demographic variables. Materials and Methods: In this descriptive analytic study, SF36 questionnaire was used to examine the QOL of 60 hemodialysis patients in Kerman. Results: The mean score of the QOL in hemodialysis patients was 49.83 ± 17.56. The maximum of score was related to the physical activity domain (57.09) and minimum score was given to the vitality domain (43.85). The relationship between quality of life and age, job, literacy and marriage was significant. Conclusion: Our results revealed that the QOF in hemodialysis patients was less that in than normal people. It was shown that these patients need to live a happy life. Therefore, appropriate mental support and instruction on compatibility methods can increase their QOF

    Patient’s Perception of Safety in the Hospital Settings: A Qualitative Systematic Review

    Introduction: Patient safety is a key indicator and element in securing quality healthcare and this goal is multi-step, systemic, and multidisciplinary. The aim of the study was to investigate the meanings and definitions of “feeling of safety” in patient’s view and experiences during their hospital stay and to identify the antecedents and consequences of the concept.Methods: This qualitative systematic review was carried out based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2009. Six databases (PubMed, Web of Science, Scopus, ProQuest, Embase, Cochrane) were searched up to 2019 with no time and language limit. Two authors individually evaluated the study quality using Joanna Briggs Institute Qualitative Assessment and Review Instrument (JBI-QARI). Data from studies meeting the inclusion criteria were analyzed with concept analysis using of Walker and Avant approach (2011).Results: Twenty-five papers were included in this study. Data analysis resulted finally in eleven main categories: “receiving safe care”, “appropriate physical environment”, “resorting to spirituality”, “having previous negative experiences”, and “presence of family and friends”, “Feeling of Protection in a Safe Place”, “Emotional Enrichment and Confidence”, “Comfort and Tranquility”, “Feeling of Control on the Situation”, “Optimism towards Life” and “Coping”.Conclusions: Safe care is one of the fundamental needs in creating feeling of safety in patients. Focusing on this type of patient-centered care may promote quality care and improve the treatments provided in the hospital setting

    Spiritual development in Iranian nurses

    Background: Spiritual development is one of the most important aspects of socialization that has attracted the attention of researchers. It is needed to train nursing student and novice nurses to provide high-quality care for patients. There is ambiguity in the definition of spiritual development and its relations, especially in the eastern countries. Research objectives: To explore the concept of spiritual development in Iranian nurses. Research design: Qualitative content analysis approach. Data were gathered from semi-structured interviews. Participants and research context: The participants were 17 Iranian Muslim nurses selected using a purposeful sampling. The place of interviews was on their choice. Ethical considerations: Based on the principles of the Helsinki declaration, the focus was on preserving the participants’ autonomy, confidentiality, and anonymity. The participants were told the study purposes and trends, and their rights were emphasized; they were then asked to sign written consent forms. Formal research approval was obtained from Kerman University of Medical Sciences. Ethical approval was granted by the University Ethics Committee before the study was conducted (K/92 etc). Findings: Three themes for spiritual development were defined: obligation to religion, commitment to ethics, and commitment to law. From the results, factors such as connection to the limitless divine power, personal and society-oriented ethical codes, and commitment to the law are proposed. Discussion: There are some differences between these findings and previous study, especially in the relation of the spirituality, religion, and law. Some studies, mostly Iranian, support these findings partially. Conclusion: The results suggest that it is better to teach nursing education based on humanistic principles, ethics, and law to the new generation of nurses to improve community health and development. More studies are needed to examine the relation between these themes

    Intraorbital Steroid Injection for Active Thyroid Ophthalmopathy

    Purpose: To evaluate the effect of orbital steroid injections in patients with active thyroid ophthalmopathy resistant to or dependent on systemic steroids, or with complications related to systemic steroid use. Methods: This prospective non-comparative case series includes 31 eyes of 17 patients with active thyroid ophthalmopathy and clinical activity score (CAS) of 3 or more, without compressive optic neuropathy or overt exposure keratopathy. All subjects had a history of previous systemic steroid use (with steroid resistance or dependence) or had developed complications related to steroids. A combination of steroids including triamcinolone acetonide 20 mg and dexamethasone 4 mg was injected in the upper and lower retroseptal orbital spaces three or four times at one-month intervals. The patients were examined periodically after each injection and at least three months after the last injection. Results: Mean pre-injection CAS was 5.2 ± 1.3 which was improved to 1.6 ± 1 after the fourth injection (P < 0.001). Upper and lower lid retraction improved in 100% and 68.2% of the affected eyes, respectively. Strabismus completely resolved in one of five affected patients and the most significant improvement was observed in supraduction. Mean improvement in exophthalmos was 1.2 ± 1.1 mm. Visual acuity did not significantly change after the injections. Eyelid ecchymosis and/or subconjunctival hemorrhage was observed in 7.1% and intraocular pressure rise occurred in 8.8% of eyes. Conclusion: Orbital steroid injections can be used for the treatment of active thyroid ophthalmopathy when the patient is resistant to or dependent on systemic steroids or has developed complications of systemic steroids

    Survey of risk factors status in patients with 3VD candidate for coronary artery bypass graft surgery (CABGs)

    Cardiovascular diseases are one of the most common health problems that threaten life. coronary artery disease (CAD) is major cause of morbidity and mortality in human communities.CAD have many mental، physical and social consequences for sufferer and its risk factors divided to two major groups: modifiable and no modifiable that modifiable risk factors will control with change in life style The main purpose of this study was to determine the coronary risk factors status among patient with 3VD after angiography that candidate for CABG. The study investigated 1000 coronary patients with 3VD (660 males and 340 females) aged 54.8±9.1 years. Outcome variable included demographic and coronary risk factors parameters were collected. We used of a questionnaire for collecting data from patient. Using descriptive statistics performed data analysis 38% of patient had history of diabetes mellitus، 32% had hypertension and 40% had history of cigarette smoking.11% had BMI≥30، % 45.6 of patient had O+ (blood group).87% of patient had total cholestrol≥200، 81% had TG≥200 and about 70% had LDL>100.there were statistically significant differences between sex with cigarette smoking، HTN، DM (P<0.05) and age with TG (P<0.05). Coronary risk factors such as inappropriate clinical status and undesirable lipid profile status threaten population health and CAD patient with 3VD، therefore more control and preventive strategies should be considered.     An Overview of Prescription in the Nursing

    Introduction: The role which nurses are playing in medication management is seriously changing over time. Nurses are authorized to prescribe medications in an ever-increasing number of countries. The main objective of the present review study is to introduce nurse prescribing as well as different types of prescriptions and their corresponding advantages, disadvantages, and barriers in the leading countries of nursing. Methods: The present study was performed by searching over the articles in information databases such as “PubMed”, “Google Scholar”, “Embase” , “Proquest”, “Ovid”, “Web of Science” and “Science Direct” using the English keywords such as “independent prescribing”, “supplementary nurse prescribing” and “prescribing”. As a result, 250 relevant papers were collected, out of which 38 ones were used in the present study. Results: The results of searching the sources and articles related to nurse prescribing were presented as the definition of nurse prescribing as well as different types of prescribing and their advantages and obstacles in the leading countries in nurse medication prescribing. Conclusions: Although there are many advantages for nurse prescribing such as cost-effectiveness of this method, the official prescribing by nurses is not authorized in many Asian countries such as Iran. Considering the positive role of nurse prescribing in the nursing professional improvement and the patient's satisfaction, it is also hoped to be applied in all countries

    Exploring Strategies Adopted by Adolescent Girls for Continuing Sports Activities

    Background: Physical activity maintenance is an active process that requires active use of strategies and techniques for its maintenance. The present study was designed to understand these strategies from the viewpoints of adolescent girls, who are often significantly less physically active than their male counterparts. Methods: This study was a qualitative content analysis conducted through semi-structured interviews with 25 participants (16 adolescent girls, 7 mothers and two fathers). Data gathering was continued until data saturation. All the interviews were recorded, transcribed, and finally were analyzed by conventional content analysis using OneNote. Results: The Strategy of "strive for perfection", with categories of reviewing experiences and thoughtful decision making and activity, was the dominant strategy used by adolescent girls to continue their sport activities. Conclusion: Adolescent girls in the recent study facilitated the continuation of their sports activities through using cognitive and behavioral strategies appropriate to their conditions. Therefore, it seems that paying attention to creating conditions for adolescents to have their own choices, decisions and strategies is useful in the continuation of sports activities

    Empowerment of Support Staff of Kerman University of Medical Sciences, Iran

    Introduction: Nowadays staff development is more a culture than a process. In this culture, organizations’ staff are considered the most crucial resource. They evaluate the existing problems actively and creatively while at the same time they try to solve them in teams. Performing the process of staff development in different organizations especially health organizations has its own fragile points depending upon many different conditions. This process has been done in the central supporting department of the Kerman University of Medical Sciences and its results have been analyzed. Method: Staff development process has been done based on David Coghlan’s Model and it is of action-research type. Results: The findings of this research justify the positive impacts of the intervention on staff development. These interventions include: raising the participation proportion, performing team work, communication freedom, access to information, and clear goals, feedback, and organization learning. Conclusion: The results of this research show the positive impacts of the intervention on development of those in charge of the qualitative cycle. In addition to these there are some barriers in the development process, the omission of which can lead to the improvement of this process.
