    The association between parity, CVD mortality, and CVD risk factors among Norwegian women and men

    Background Several studies have shown that women and men with two children have lower mortality than the childless, but there is less certainty about mortality, including CVD mortality, at higher parities and meagre knowledge about factors underlying the parity–mortality relationship. Methods The association between parity and CVD mortality was analyzed by estimating discrete-time hazard models for women and men aged 40–80 in 1975–2015. Register data covering the entire Norwegian population were used, and the models included a larger number of relevant sociodemographic control variables than in many previous studies. To analyze the relationship between parity and seven CVD risk factors, logistic models including the same variables as the mortality models were estimated from the CONOR collection of health surveys, linked to the register data. Results Men (but not women) who had four or more children had higher mortality from CVD than those with two, although this excess mortality was not observed for the heart disease sub-group. Overweight, possibly in part a result of less physical activity, seems to play a role in this. All CVD risk factors except smoking and alcohol may contribute to the relatively high CVD mortality among childless. Conclusions Childbearing is related to a number of well-known CVD risk factors, and becoming a parent or having an additional child is, on the whole, associated with lower—or at least not higher—CVD mortality in Norway. However, for men family sizes beyond three children are associated with increased CVD mortality, with risks of overweight one possible pathway

    Differential gene expression in brain tissues of aggressive and non-aggressive dogs

    <p>Abstract</p> <p>Background</p> <p>Canine behavioural problems, in particular aggression, are important reasons for euthanasia of otherwise healthy dogs. Aggressive behaviour in dogs also represents an animal welfare problem and a public threat. Elucidating the genetic background of adverse behaviour can provide valuable information to breeding programs and aid the development of drugs aimed at treating undesirable behaviour. With the intentions of identifying gene-specific expression in particular brain parts and comparing brains of aggressive and non-aggressive dogs, we studied amygdala, frontal cortex, hypothalamus and parietal cortex, as these tissues are reported to be involved in emotional reactions, including aggression. Based on quantitative real-time PCR (qRT-PCR) in 20 brains, obtained from 11 dogs euthanised because of aggressive behaviour and nine non-aggressive dogs, we studied expression of nine genes identified in an initial screening by subtraction hybridisation.</p> <p>Results</p> <p>This study describes differential expression of the <it>UBE2V2 </it>and <it>ZNF227 </it>genes in brains of aggressive and non-aggressive dogs. It also reports differential expression for eight of the studied genes across four different brain tissues (amygdala, frontal cortex, hypothalamus, and parietal cortex). Sex differences in transcription levels were detected for five of the nine studied genes.</p> <p>Conclusions</p> <p>The study showed significant differences in gene expression between brain compartments for most of the investigated genes. Increased expression of two genes was associated with the aggression phenotype. Although the <it>UBE2V2 </it>and <it>ZNF227 </it>genes have no known function in regulation of aggressive behaviour, this study contributes to preliminary data of differential gene expression in the canine brain and provides new information to be further explored.</p

    A comparison of self-reported data on disability pension status with data from a nationwide administrative register

The objective of this study was to estimate the sensitivity and specificity, as well as positive and negative predictive values of self-reported disability pension status using an official administrative register as reference standard. Methods: Data from Cohort of Norway (CONOR) surveys conducted in 2001 in the three Norwegian counties Oslo (HUBRO), Hedmark and Oppland (OPPHED) are included in this study, altogether 17,244 individuals. At the time of investigation, the subjects included in our study-population were aged 30-31, 40-41, 45-47 and 59-61 years. Self-reported data on disability pension status was compared with data from the nationwide population and housing census in Norway (Statistics Norway), performed November 3rd 2001. Data were linked using the unique 11-digit identification number, assigned to all individuals living in Norway. Results: Sensitivity and specificity for self-reported questions on disability pension were 97.6% (95% CI 91.1-94.1) and 96.8% (96.5-97.1). Positive and negative predictive values were 70.1% (67.9-72.3) and 99.8% (99.7-99.9). Validity measures in a subpopulation, those surveyed in October-November 2001, were almost identical as for the total study-population surveyed in 2001. Conclusion: The sensitivity of self-reported disability pension status was high. The positive predictive value was lower which may reflect a low prevalence

    Intimate partner violence and prescription of potentially addictive drugs: prospective cohort study of women in the Oslo Health Study

    Objectives:To investigate the prescription of potentially addictive drugs, including analgesics and central nervous system depressants, to women who had experienced intimate partner violence (IPV). Design: Prospective population-based cohort study. Setting: Information about IPV from the Oslo Health Study 2000/2001 was linked with prescription data from the Norwegian Prescription Database from 1 January 2004 through 31 December 2009. Participants: The study included 6081 women aged 30–60 years. Main outcome measures: Prescription rate ratios (RRs) for potentially addictive drugs derived from negative binomial models, adjusted for age, education, paid employment, marital status, chronic musculoskeletal pain, mental distress and sleep problems. Results: Altogether 819 (13.5%) of 6081 women reported ever experiencing IPV: 454 (7.5%) comprised physical and/or sexual IPV and 365 (6.0%) psychological IPV alone. Prescription rates for potentially addictive drugs were clearly higher among women who had experienced IPV: crude RRs were 3.57 (95% CI 2.89 to 4.40) for physical/sexual IPV and 2.13 (95% CI 1.69 to 2.69) for psychological IPV alone. After full adjustment RRs were 1.83 (1.50 to 2.22) for physical/sexual IPV, and 1.97 (1.59 to 2.45) for psychological IPV alone. Prescription rates were increased both for potentially addictive analgesics and central nervous system depressants. Furthermore, women who reported IPV were more likely to receive potentially addictive drugs from multiple physicians. Conclusions: Women who had experienced IPV, including psychological violence alone, more often received prescriptions for potentially addictive drugs. Researchers and clinicians should address the possible adverse health and psychosocial impact of such prescription and focus on developing evidence-based healthcare for women who have experienced IPV

    Disability pension is associated with the use of benzodiazepines 20 years later: A prospective study

Identification of factors that may impair rehabilitation efforts, such as information on the start of benzodiazepines in DP recipients, may be useful in this context.Thus, the aim of the study was to describe the association between receipt of DP and later prescriptions of benzodiazepines among non-users at baseline. Methods: We followed a cohort reporting non-use of benzodiazepines, 6645 men and 6455 women aged 40—42 years who underwent health surveys in 1985—89 in two Norwegian counties, with respect to subsequent use of benzodiazepines by linkage to the Norwegian Prescription Database for 2004—2006. At baseline, 83 men and 163 women reported receiving DP. Results: In both genders, the proportion started on at least one prescription of a benzodiazepine was approximately doubled among those reporting being on a DP 20 years in the past, 21% of all men and 29% of all women, respectively. Univariate odds ratios for a benzodiazepine prescription for men and women on a DP were 2.6 (95% confidence interval (CI) = 1.5—4.4) and 2.1 (95% CI = 1.5—2.9), respectively, as compared with those not receiving a DP. After adjustment for alcohol use, smoking habits, physical activity, socioeconomic and health variables, the odds ratios were lowered for both genders, being 2.1 (95% CI = 1.2—3.7) (men) and 1.6 (95% CI = 1.1—2.3) (women). Conclusions: For both men and women, the chance of being prescribed benzodiazepines was higher among those reporting being DP recipients 20 years in the past

    Quantifying the contribution of established risk factors to cardiovascular mortality differences between Russia and Norway.

    Surprisingly few attempts have been made to quantify the simultaneous contribution of well-established risk factors to CVD mortality differences between countries. We aimed to develop and critically appraise an approach to doing so, applying it to the substantial CVD mortality gap between Russia and Norway using survey data in three cities and mortality risks from the Emerging Risk Factor Collaboration. We estimated the absolute and relative differences in CVD mortality at ages 40-69 years between countries attributable to the risk factors, under the counterfactual that the age- and sex-specific risk factor profile in Russia was as in Norway, and vice-versa. Under the counterfactual that Russia had the Norwegian risk factor profile, the absolute age-standardized CVD mortality gap would decline by 33.3% (95% CI 25.1-40.1) among men and 22.1% (10.4-31.3) among women. In relative terms, the mortality rate ratio (Russia/Norway) would decline from 9-10 to 7-8. Under the counterfactual that Norway had the Russian risk factor profile, the mortality gap reduced less. Well-established CVD risk factors account for a third of the male and around a quarter of the female CVD mortality gap between Russia and Norway. However, these estimates are based on widely held epidemiological assumptions that deserve further scrutiny

    Treatment outcome of new culture positive pulmonary tuberculosis in Norway

    BACKGROUND: The key elements in tuberculosis (TB) control are to cure the individual patient, interrupt transmission of TB to others and prevent the tubercle bacilli from becoming drug resistant. Incomplete treatment may result in excretion of bacteria that may also acquire drug resistance and cause increased morbidity and mortality. Treatment outcome results serves as a tool to control the quality of TB treatment provided by the health care system. The aims of this study were to evaluate the treatment outcome for new cases of culture positive pulmonary TB registered in Norway during the period 1996–2002 and to identify factors associated with non-successful treatment. METHODS: This was a register-based cohort study. Treatment outcome was assessed according to sex, birthplace, age group, isoniazid (INH) susceptibility, mode of detection and treatment periods (1996–1997, 1998–1999 and 2000–2002). Logistic regression was also used to estimate the odds ratio for treatment success vs. non-success with 95% confidence interval (CI), taking the above variables into account. RESULTS: Among the 655 patients included, the total treatment success rate was 83% (95% CI 80%–86%). The success rates for those born in Norway and abroad were 79% (95% CI 74%–84%) and 86% (95% CI 83%–89%) respectively. There was no difference in success rates by sex and treatment periods. Twenty-two patients (3%) defaulted treatment, 58 (9%) died and 26 (4%) transferred out. The default rate was higher among foreign-born and male patients, whereas almost all who died were born in Norway. The majority of the transferred out group left the country, but seven were expelled from the country. In the multivariate analysis, only high age and initial INH resistance remained as significant risk factors for non-successful treatment. CONCLUSION: Although the TB treatment success rate in Norway has increased compared to previous studies and although it has reached a reasonable target for treatment outcome in low-incidence countries, the total success rate for 1996–2002 was still slightly below the WHO target of success rate of 85%. Early diagnosis of TB in elderly patients to reduce the death rate, abstaining from expulsion of patients on treatment and further measures to prevent default could improve the success rate further
