    Macadamia Oil Supplementation Attenuates Inflammation and Adipocyte Hypertrophy in Obese Mice

    Excess of saturated fatty acids in the diet has been associated with obesity, leading to systemic disruption of insulin signaling, glucose intolerance, and inflammation. Macadamia oil administration has been shown to improve lipid profile in humans. We evaluated the effect of macadamia oil supplementation on insulin sensitivity, inflammation, lipid profile, and adipocyte size in high-fat diet (HF) induced obesity in mice. C57BL/6 male mice (8 weeks) were divided into four groups: (a) control diet (CD), (b) HF, (c) CD supplemented with macadamia oil by gavage at 2g/Kg of body weight, three times per week, for 12 weeks (CD + MO), and (d) HF diet supplemented with macadamia oil (HF + MO). CD and HF mice were supplemented with water. HF mice showed hypercholesterolemia and decreased insulin sensitivity as also previously shown. HF induced inflammation in adipose tissue and peritoneal macrophages, as well as adipocyte hypertrophy.     O CASO DOS REFUGIADOS SÍRIOS NO BRASIL E A POLÍTICA INTERNACIONAL CONTEMPORÂNEA

    The conflicts in Syria has generated innumerous problems in the basic elements of State responsibility, as political, ethical and religious crises denote a governmental imbalance scenario and has caused the escape of many Syrians to other countries, as they aim for a better quality of life. In this context, the asylum of Syrians in Brazil has led the ranking of refugees in the country. This article aims to address the Brazilian policy for Syrian refugees and to analyze the role played by the United Nations High Commissioner for Refugees (UNHCR), discussing the recognition of international politics. Therefore, the deductive method is used, by starting from the conflict in Syria and the refugee phenomenon and proceeding to the case of the Syrian refugees in Brazil. It is noticed that the problem goes beyond the boundaries of Brazilian policy as it impacts international organizations, such as the United Nations (UN) and its specialized agency for refugees, the UNHCR. Despite the Brazilian government\u27s efforts, there is still a major deficiency in the ability to provide adequate aid and, even with the financial support of these international organizations, the amount of violations of the Refugees Statute entails inefficiency of the refugee policies and protection.Os conflitos na Síria têm gerado diversos problemas nos quesitos básicos de responsabilidade do Estado, já que crises políticas, étnicas e religiosas marcam um cenário de desequilíbrio governamental e vêm ocasionando a fuga de muitos sírios para outros países, almejando melhor qualidade de vida. Nesse contexto, o acolhimento de sírios no Brasil tem liderado o ranking de refugiados no país. Este artigo pretende abordar a política brasileira para os refugiados Sírios e analisar o papel desenvolvido pelo Alto Comissariado das Nações Unidas para os Refugiados (ACNUR), debatendo o reconhecimento da política internacional. Para tanto, utiliza-se o método dedutivo, pois parte-se dos conflitos na Síria e do fenômeno dos refugiados para o caso dos refugiados sírios no Brasil. Percebe-se que a problemática ultrapassa as fronteiras da política executada pelo Brasil e movimenta organismos internacionais, como a Organização das Nações Unidas (ONU) e sua agência especializada em refugiados, o ACNUR. Apesar do esforço do governo brasileiro, ainda há grande deficiência na sua aptidão para prestar ajuda adequada e, mesmo com o suporte financeiro desses organismos internacionais, as inúmeras violações ao Estatuto dos Refugiados acarretam ineficiência das políticas e do amparo aos refugiados

    Endothelin-1 Predicts Hemodynamically Assessed Pulmonary Arterial Hypertension in HIV Infection.

    BackgroundHIV infection is an independent risk factor for PAH, but the underlying pathogenesis remains unclear. ET-1 is a robust vasoconstrictor and key mediator of pulmonary vascular homeostasis. Higher levels of ET-1 predict disease severity and mortality in other forms of PAH, and endothelin receptor antagonists are central to treatment, including in HIV-associated PAH. The direct relationship between ET-1 and PAH in HIV-infected individuals is not well described.MethodsWe measured ET-1 and estimated pulmonary artery systolic pressure (PASP) with transthoracic echocardiography (TTE) in 106 HIV-infected individuals. Participants with a PASP ≥ 30 mmHg (n = 65) underwent right heart catheterization (RHC) to definitively diagnose PAH. We conducted multivariable analysis to identify factors associated with PAH.ResultsAmong 106 HIV-infected participants, 80% were male, the median age was 52 years and 77% were on antiretroviral therapy. ET-1 was significantly associated with higher values of PASP [14% per 0.1 pg/mL increase in ET-1, p = 0.05] and PASP ≥ 30 mmHg [PR (prevalence ratio) = 1.24, p = 0.012] on TTE after multivariable adjustment for PAH risk factors. Similarly, among the 65 individuals who underwent RHC, ET-1 was significantly associated with higher values of mean pulmonary artery pressure and PAH (34%, p = 0.003 and PR = 2.43, p = 0.032, respectively) in the multivariable analyses.ConclusionsHigher levels of ET-1 are independently associated with HIV-associated PAH as hemodynamically assessed by RHC. Our findings suggest that excessive ET-1 production in the setting of HIV infection impairs pulmonary endothelial function and contributes to the development of PAH

However, the use of expensive and not readily accessible catalysts complicates the process of pharmacological application of these compounds. A plausible way to overcome this issue is developing and using cheaper, more accessible, and equally effective catalysts. First-row transition (3d) metals have shown to be important catalysts in this matter.     Troponin utilization in patients presenting with atrial fibrillation/flutter to the emergency department: retrospective chart review

    Abstract Background There are few recommendations about the use of cardiac markers in the investigation and management of atrial fibrillation/flutter. Currently, it is unknown how many patients with atrial fibrillation/flutter undergo troponin testing, and how positive troponin results are managed in the emergency department. We sought to look at the emergency department troponin utilization patterns. Methods We performed a retrospective chart review of patients with atrial fibrillation/flutter presenting to the emergency department at three centers. Outcome measures included the rates of troponins ordered by emergency doctors, number of positive troponins, and those with positive troponins treated as acute coronary syndrome (ACS) by consulting services. Results Four hundred fifty-one charts were reviewed. A total of 388 (86%) of the patients had troponins ordered, 13.7% had positive results, and 4.9% were treated for ACS. Conclusions Troponin tests are ordered in a high percentage of patients with atrial fibrillation/flutter presenting to emergency departments. Five percent of our total patient cohort was diagnosed as having acute coronary syndrome by consulting services
