    Effects of vitamin D and the antimicrobial peptide in asthma

    Background: Vitamin D modulates a variety of processes and regulatory systems including host defense, inflammation, and immunity. A connection between Vitamin D status and asthma has been considered. Vitamin D mediates innate immunity, particularly through enhanced expression of the human cathelicidin antimicrobial peptide (LL-37). Increased levels of high sensitive-C reactive protein (Hs-CRP) were found to be significantly associated with respiratory function impairment. Objective: to evaluate the relation between serum vitamin D, Hs-CRP and LL-37 levels and asthma. Study design: Thirty children (15 males and 15 females) with proven diagnosis of asthma (ages ranged from 3-13 years) were studied; they attended the pediatric department of Zagazig University Hospital in the year 2011. In addition, 30 age and sex matched apparently healthy children served as a control group. All children were subjected to history taking, clinical examination, laboratory investigations (CBC, CRP, ESR), determination of serum level of 25-hydroxyvitamin D (25 OHD) and plasma LL-37. Results: The study revealed a highly significant decrease in 25 OHD, LL-37 and a highly significant increase in Hs-CRP in children with asthma than in control group. There were a highly significant positive correlation between vitamin D and LL37 in patients' and control groups and a significant negative correlation between both 25OHD and LL37 and Hs-CRP in patients' group. High WBC count (specially neutrophils and lymphocytes), Hs-CRP level and low levels of hemoglobin, 25OHD and LL 37 in patient group were considered risk factors of asthma. Conclusion: Inappropriate concentration of vitamin D decreases the ability of the immune system to defend against infection through lowering LL-37 and elevated Hs-CRP which leads to occurrence and precipitation of asthma.Keywords: Vitamin D- LL-37- asthmaEgypt J Pediatr Allergy Immunol 2012;10(2):101-10

    Histopathological pattern of gynaecological malignancies: National Health Laboratory (NHL), Sudan

    Gynaecological malignancies (GM) is a common cancer problem in both developing and developed countries .The burden of GM is more critical in developing countries, this is due to lack or deficient screening and late clinical presentation. The aim of this study is to study the histopathological pattern of GM in the NHL (National Health Laboratory) Sudan. Material &Methods: Clinical data, histopathological reports, stained tissue slides of patients diagnosed histologically as GM were revised from the archive of the department of histopathology and cytology at the NHL .Results: 372 patients diagnosed histologically as GM from 1st Jan 2009-31st Dec to the 1st Jan 2013 were studied. The commonest GM histological type was cervical malignancies, followed by ovarian, endometrial, vaginal, vulvar, choriocarcinoma and tubal cancer respectively .Conclusion: There is an urgent need for screening, early detection and prompt management for GM.Keywords: ,

    Microbial ecology of Rum Jungle II: environmental study of two flooded opencuts and smaller, associated water bodies.

    The microbial status of the flooded Intermediate and White's opencuts of the abandoned uranium mine at Rum Jungle was investigated by sampling the water column and sediments of several areas in each opencut. Smaller water bodies associated with the experimental heap-leach pile were also investigated. Several groups of bacteria were identified and population sizes were estimated using selective media techniques. Various physicochemical parameters of each sample were determined and correlated with the occurence of bacteria. Both opencuts although behaving differently were found to be heavily polluted by sulphuric acid and heavy metals White's more so than Intermediate. White's opencut was found to be stratified into an aerobic zone about five metres deep and a microaerophilic zone below this. Large populations of Thiobacillus ferrooxidans and autotrophic sulphur-oxidising bacteria indicated that degradation of sulphidic minerals in the walls and floors of the opencuts was still occurring. The isolation of T. ferrooxidans from sediments also containing anaerobic bacterial species suggested that T. ferrooxidans was degrading sulphidic minerals either anaerobically or microaerophilically. The smaller water bodies also were found to be heavily polluted by acid and heavy metals from drainage and seepage from the sulphidic heap-leach pile

    Microbial ecology of Rum Jungle III : leaching behaviour of sulphidic waste material under controlled conditions.

    The discharge into river systems of acid and heavy metals generated by leaching of sulphidic waste materials at the abandoned opencut uranium mine at Rum Jungle Northern Territory is causing continuing pollution of the surrounding environment. The maximum effects of acid and microorganisms on samples from the overburden dump material under defined and controlled environmental conditions were assessed using reactor systems. These samples came from the overburden dump resulting from the mining of White's orebody. Similarly the stability of tailings material under conditions of flooding and increasing acidity was determined. At ph 2.5 metals in White's dump material were solubilised by acid attack only whereas at pH 3.5 bacterial activity (principally that of Thiobacillus ferrooxidans) generated acidity and contributed significantly to metal release. Under microaerophilic conditions Thiobacillus ferrooxidans continued to effect metal release from the ore but did not produce further acidity. If White's overburden is returned to the acidic flooded opencuts complete solubilisation of the material will occur. The exclusion of oxygen from the dump will not necessarily stop bacterially catalysed leaching processes. Under highly aerated and agitated flooded conditions the tailings material was not active except for copper release of about 2 g kg -1 ore at pH 4.0. The only deleterious element released by increasing acidity was copper which was 100 per cent solubilised at pH 2.5. Uranium was always lss than 3 mu g kg -1 ore and lead was detected only at pH 2.5.     Dielectric Barrier Plasma Discharge Exsolution of Nanoparticles at Room Temperature and Atmospheric Pressure

    Exsolution of metal nanoparticles (NPs) on perovskite oxides has beendemonstrated as a reliable strategy for producing catalyst-support systems.Conventional exsolution requires high temperatures for long periods of time,limiting the selection of support materials. Plasma direct exsolution isreported at room temperature and atmospheric pressure of Ni NPs from amodel A-site deficient perovskite oxide (La 0.43 Ca 0.37 Ni 0.06 Ti 0.94 O2.955 ). Plasmaexsolution is carried out within minutes (up to 15 min) using a dielectricbarrier discharge configuration both with He-only gas as well as with He/H2gas mixtures, yielding small NPs (<30 nm diameter). To prove the practicalutility of exsolved NPs, various experiments aimed at assessing their catalyticperformance for methanation from synthesis gas, CO, and CH4 oxidation arecarried out. Low-temperature and atmospheric pressure plasma exsolution aresuccessfully demonstrated and suggest that this approach could contribute tothe practical deployment of exsolution-based stable catalyst systems

    The stroke oxygen pilot study: a randomized control trial of the effects of routine oxygen supplementation early after acute stroke--effect on key outcomes at six months

    Introduction: Post-stroke hypoxia is common, and may adversely affect outcome. We have recently shown that oxygen supplementation may improve early neurological recovery. Here, we report the six-month outcomes of this pilot study. Methods: Patients with a clinical diagnosis of acute stroke were randomized within 24 h of admission to oxygen supplementation at 2 or 3 L/min for 72 h or to control treatment (room air). Outcomes (see below) were assessed by postal questionnaire at 6 months. Analysis was by intention-to-treat, and statistical significance was set at p#0.05. Results: Out of 301 patients randomized two refused/withdrew consent and 289 (148 in the oxygen and 141 in the control group) were included in the analysis: males 44%, 51%; mean (SD) age 73 (12), 71 (12); median (IQR) National Institutes of Health Stroke Scale score 6 (3, 10), 5 (3, 10) for the two groups respectively. At six months 22 (15%) patients in the oxygen group and 20 (14%) in the control group had died; mean survival in both groups was 162 days (p= 0.99). Median (IQR) scores for the primary outcome, the modified Rankin Scale, were 3 (1, 5) and 3 (1, 4) for the oxygen and control groups respectively. The covariate-adjusted odds ratio was 1.04 (95% CI 0.67, 1.60), indicating that the odds of a lower (i.e. better) score were non-significantly higher in the oxygen group (p= 0.86). The mean differences in the ability to perform basic (Barthel Index) and extended activities of daily living (NEADL), and quality of life (EuroQol) were also non-significant. Conclusions: None of the key outcomes differed at 6 months between the groups. Although not statistically significant and generally of small magnitude, the effects were predominantly in favour of the oxygen group; a larger trial, powered to show differences in longer-term functional outcomes, is now on-going.
