    Prevalence, clinical investigation, and management of gallbladder disease in Rett syndrome

    AIM: This study determined the prevalence of cholelithiasis and/or cholecystectomy in Rett syndrome, described gallbladder function in a clinical cohort, and identified recommendations for assessment and management of gallbladder disease. METHOD The incidence of cholelithiasis/cholecystectomy was estimated from data describing 270 and 681 individuals with a pathogenic MECP2 mutation in the Australian Rett Syndrome Database and the International Rett Syndrome Phenotype Database respectively. Gallbladder function in 25 females (mean age 16y 5mo, SD 20y 7mo, range 3y 5mo–47y 10mo) with Rett syndrome (RTT) was evaluated with clinical assessment and ultrasound of the gallbladder. The Delphi technique was used to develop assessment and treatment recommendations. RESULTS: The incidence rate for cholelithiasis and/or cholecystectomy was 2.3 (95% confidence interval [CI] 1.1–4.2) and 1.8 (95% CI 1.0–3.0) per 1000 person-years in the Australian and International Databases respectively. The mean contractility index of the gallbladder for the clinical sample was 46.5% (SD 38.3%), smaller than for healthy individuals but similar to children with Down syndrome, despite no clinical symptoms. After excluding gastroesophageal reflux, gallbladder disease should be considered as a cause of abdominal pain in RTT and cholecystectomy recommended if symptomatic. INTERPRETATION: Gallbladder disease is relatively common in RTT and should be considered in the differential diagnosis of abdominal pain in RTT

Long-term prognosis in RA was greatly improved following the introduction of highly effective medications such as methotrexate (MTX). Despite the importance of this drug in RA, 8%–16% of patients must discontinue the treatment because of adverse effects. Last decade, we developed a promising new nanocarrier as a drug-delivery system, lipid-core nanocapsules.Objective: The aim of the investigation reported here was to evaluate if methotrexate-loaded lipid-core nanocapsules (MTX-LNC) reduce proinflammatory and T-cell-derived cytokines in activated mononuclear cells derived from RA patients and even in functional MTX-resistant conditions. We also aimed to find out if MTX-LNC would reduce inflammation in experimentally inflammatory arthritis at lower doses than MTX solution.Methods: Formulations were prepared by self-assembling methodology. The adjuvant arthritis was induced in Lewis rats (AIA) and the effect on edema formation, TNF-a levels, and interleukin-1 beta levels after treatment was evaluated. Mononuclear cells obtained from the synovial fluid of RA patients during articular infiltration procedures were treated with MTX solution and MTX-LNC. For in vitro experiments, the same dose of MTX was used in comparing MTX and MTX-LNC, while the dose of MTX in the MTX-LNC was 75% lower than the drug in solution in in vivo experiments.Results: Formulations presented nanometric and unimodal size distribution profiles, with D[4.3] of 175±17 nm and span of 1.6±0.2. Experimental results showed that MTX-LNC had the same effect as MTX on arthritis inhibition on day 28 of the experiment (P<0.0001); however, this effect was achieved earlier, on day 21 (P<0.0001), by MTX-LNC, and this formulation had reduced both TNF-α (P=0.001) and IL-1α (P=0.0002) serum levels by the last day of the experiment. Further, the MTX-LNC were more effective at reducing the cytokine production from mononuclear synovial cells than MTX.Conclusion: The MTX-LNC were better than the MTX solution at reducing proinflammatory cytokines and T-cell-derived cytokines such as interferon-gamma and interleukin-17A. This result, combined with the reduction in the dose required for therapy, shows that MTX-LNC are a very promising system for the treatment of RA.Keywords: drug delivery, drug targeting, arthritis, cytokines, TNF-α, IL-6, IL-1, IL-17A, IFN-&gamma

    Interações medicamentosas: fundamentos para a pratica clínica da enfermagem Drugs interactions: fundamental aspects for clinical practice nursing

    O fenômeno das interações medicamentosas constitui na atualidade um dos temas mais importantes da farmacologia, para a prática clínica dos profissionais da saúde. O uso concomitante de vários medicamentos, enquanto estratégia terapêutica, e o crescente número destes agentes no mercado são alguns dos fatores que contribuem para ampliar os efeitos benéficos da terapia, mas que também possibilitam a interferência mútua de ações farmacológicas podendo resultar em alterações dos efeitos desejados. Este artigo, de revisão, tem por objetivos rever os princípios farmacológicos relacionados aos mecanismos das interações medicamentosas; descrever as classes dos medicamentos interativos, os grupos de pacientes expostos ao risco e sugerir medidas práticas para a equipe de enfermagem, no intuito de prevenir a ocorrência de reações adversas decorrentes de interações fortuitas.<br>Nowadays drugs interactions constitute one the most important subjects of pharmacological for clinical practive of health professionals. The concomitant use of many drugs as therapeutic strategy and the growing number of agents available contribute to enlarge the benefical effects of therapy. Besides, these factors can also permit the mutual interference of pharmacological actions that result in alteractions of therapeutics effects. The aim of this article is to review the principles pharmacological related to the mechanisms of drugs interactions, to describe the types of interactive drugs; the groups of risk patients and to suggest specific nursing interventions aiming the prevention of occurrence of adverse reactions from accidental interactions

    The objective of this study was to estimate the association between levels of air pollutants and respiratory symptoms in children. An ecological time-series study was conducted between April 2002 and March 2003 with daily data on PM10, SO2, CO, NO2, and O3 and paediatric care in emergency rooms due to symptoms indicating bronchial obstruction. The potential confounding factors considered were: time trend, seasonality, meteorological variables, respiratory infections and the effects of weekends and holidays. A semi-parametric Poisson regression was used to model the time series. Splines (data smoothing functions), indicator variables and cubic polynomials were used to adjust the effects of the confounding variables. A 5% significance level was adopted for the study. A statistically significant increase of 6.7% in paediatric visits of children of less than 2 years of age was associated with PM10. A 3% positive association with O3 showed borderline significance (p < 0.06) in this age bracket. Our findings highlight the existence of an acknowledged public health problem in Rio de Janeiro, Brazil, and also emphasize the need to identify the principal sources of air pollutants.<br>O objetivo deste trabalho foi estimar a associação entre os níveis dos poluentes atmosféricos e sintomas respiratórios em crianças. Foi realizado um estudo ecológico de séries temporais, entre abril de 2002 e março de 2003, com dados diários de PM10, SO2, CO, NO2 e O3 e dos atendimentos pediátricos de emergência ocasionados por sintomas de obstrução brônquica. As variáveis de confusão foram: tendência temporal, sazonalidade, fatores meteorológicos, infecções respiratórias e os efeitos dos finais de semana e feriados. Regressão de Poisson semiparamétrica foi usada para modelar as séries temporais. Curvas suaves, splines, variáveis indicadoras e polinômios cúbicos foram empregadas para corrigir os efeitos dos fatores de confundimento. Foi adotado o nível de significância de 5%. Um incremento estatisticamente significativo dos atendimentos, de 6,7%, esteve associado com PM10 nas crianças menores do que dois anos. Associação positiva com o O3, em torno de 3%, nessa faixa etária, teve significado estatístico limítrofe (p < 0,06). Esses resultados evidenciam um problema de saúde pública não reconhecido no Rio de Janeiro, Brasil, que reforça a necessidade de identificação das principais fontes de poluição

    No Amazonas, o acidente ofídico é um problema de saúde pública pouco conhecido. Por este motivo, foi realizado um estudo descritivo dos acidentes ofídicos atendidos nas Unidades de Saúde de 34 municípios, um distrito e dois pelotões de fronteira do Estado do Amazonas. As características mais comuns encontradas dentre os pacientes foram: agricultor (50,4%), do sexo masculino (81,3%), em idade produtiva (72,1%), picado no membro inferior (88,5%), por jararaca (48,6%) ou surucucu (46,8%), na zona rural de seu município (70,2%) e que só recebeu atendimento médico em tempo superior a seis horas, após acidente (57,3%). As manifestações locais mais freqüentes foram: edema (76,9%), dor (68,7%), eritema (10,2%) e hemorragia (9,3%). Hemorragia (18,8%) foi a manifestação sistêmica mais freqüente. O antiveneno foi administrado em apenas 65,9% dos pacientes. A via mais utilizada foi a endovenosa (52,3%), sendo relevante o uso de vias não mais recomendadas (47,7%). O antiveneno administrado, na maioria dos pacientes, foi o antibotrópico (66,7%). As complicações mais freqüentes foram abcesso (13,7%), necrose (12,3%), infecção secundária (8,3%), insuficiência renal (2,5%) e gangrena (2,5%). Os procedimentos médicos mais usados para o tratamento das complicações foram: drenagem (52,6%), debridamento (28,9%), amputação (10,5%), limpeza cirúrgica (5,3%) e diálise peritoneal (2,6%). A letalidade foi de 1%.<br>In the State of Amazonas, accidents with snakes are a public health problem. For this reasons, the objective of this work was to carry out a descriptive study of the snake accidents attended in the health units of 34 municipalities, one district and two border platoons in the State of Amazonas. The characteristics most commonly observed among those involved in snake accidents were: farmers (50.4%), male (81.3%), belonging to the working age-group (72.1%), bitten on an upper limb (88.5%) by a "jararaca" (48.6%) or a "surucucu" (46.8%) in the rural part of the municipality (70.2%). The local signs and symptoms most frequently observed in those who received medical care more than 6 hours after the accident (57.3%) were edema (76.9%), pain (68.7%), erithema (10.2%) and hemorrhage (9.3%). The systemic manifestation most frequently observed was hemorrhage (18.8%). Serotherapy was administered in only 65.9% of patients, the intravenous route being the route most commonly used to administer the antivenin (52.3%), while other non- recommended routes were widely used. In the majority of patients the antivenin given was antibotropic. The most frequent complications were: abscess 13.7%, necrosis 12.3%, secondary infection 8.3%, renal insufficiency 2.5% and gangrene 2.5%. The medical procedures most used in the treatment of these complications were drainage 52.6%, debridement 28.9%, amputation 10.5%, surgical cleaning 5.3% and peritoneal dialysis 2.6%.
